Comparing Effects of Tai-Chi Self-help Program and Tai-Chi Exercise in Osteoarthritis Patients (골관절염 환자를 위한 타이치 운동그룹과 타이치 자조관리 그룹의 효과 비교분석)

  • Purpose: The purpose of the study was to compare the effects of Tai Chi self-help program and Tai Chi exercise in patients with osteoarthritis. Methods: quasi experimental design was utilized for the study to compare the effects after implementing tai chi self-help program and Tai Chi exercise for 6 weeks. The subjects of the study was osteoarthritis patients who visited to two Public Health Centers in Gyunggi-do from August to December, 2005. Total of 25 OA patients agreed to participate in the study, and 10 tai chi self-help program group and 9 Tai Chi exercise group were included for the final analysis after 6 weeks. Tai Chi self-help program was provided twice a week, 2 hours per session (one hour for tai chi exercise, one hour for self help program) for 6 weeks, and Tai Chi exercise group was provided once a week, 2 hours per session for 6 weeks. Outcome measures include flexibility, 6 m walking, and balance. Mann-Whitney U test of SPSS WIN 11.0 was utilized to compare the group differences. Results: The subjects of both program participants were not improved in their arm flexibility, and the difference was not significant. Tai chi self-help group improved more in their 6 m walking ability than Tai Chi exercise group, but the difference was not significant (F=.000, p>.05). Tai chi self-help group improved more in their balance than Tai Chi exercise group, but the difference was not significant (Rt: U=.24, p>.05; Lt: F=2.60, p>.05). Conclusion: The provision of both Tai Chi self-help program and Tai Chi exercise can lead improvement of 6 m walking, and balance, but the group difference was not significant. A rigorous research design should be further utilized to identify the potential benefits for this population.

  • Purpose: This study was to examine the effects of the hypertension self-help program on knowledge, self-efficacy, self-management compliance, and physiological parameters for workers with hypertension. Methods: The subjects of study were 54 patients with hypertension, divided into 28 of experimental group and 26 of control group, working in a general work place located in K city from June to September, 2009. Experiment treatment was a 12-week self-care program given for two hours, once a week. Results: After conducting a self-help program, the experimental group showed significant increase in knowledge level, self-efficacy, self-management compliance, and physiological parameters including blood pressure, BMI, total cholesterol level, neutral fat, and LDL-cholesterol of blood lipids compared to the control group. Conclusion: This program is greatly recommended for workshops.

Effect of East-West Self-help Group Program for Rehabilitation of Post Stroke Clients (뇌졸중 후 대상자의 동서재활 자조관리 프로그램의 적용 효과)

  • Purpose: The purpose of this study was to evaluate the effect of an East-West Self-help program for Rehabilitation of post-stroke patients, Method: A quasi-experimental design was used. The subjects were 75 post stroke clients(Exp. group : 38, Cont. group:37), The subjects of the experimental group participated in the Self-help group program of six sessions, twice a week, during 6 weeks. The program consisted of health education of stroke, exercise, oriental nursing interventions, and therapeutic recreation. The obtained data were analyzed by using the repeated measure ANOVA of SPSS. Result: 1) The score of rehabilitation self-efficacy increased significantly in the experimental group as compared to the control group. 2) The score of BADL, IADL, amount of use & quality of movement of the affected U/E, and grip power increased significantly in the experimental group as compared to the control group. 3)The level of blood cholesterol decreased significantly in the experimental group as compared to the control group. Conclusion: Considering these research results, the program is effective in improving functional abilities and self-management ability. Therefore this program could be implemented as a community based self-help group program for post stroke clients.

The long-term Effects after Self-help Program for Fibromyalgia (섬유조직염환자를 위한 자조관리과정 지속효과에 관한 연구)

  • Purpose: The purpose of this study was to find out long-term effects of the self-help program including the stretching exercise of patient with fibromyalgia, Our findings will have the way for the nursing intervention for reduction of fibromyalgia symptoms and physical disability. Method: The research design was pre-post test design of an experimental group. 41 subjects with fibromyalgia were subjected to our study. Participants of this program were participated with a group of 8 to 12 members with duration of two and half hours for two years(March. 1998 to January. 2000). The Self-efficacy was measured by the Self-efficacy scale. Symptom was converted to scores based on visual analog scale. Physical disability was measured with questionnaires prepared by researchers of this study. Data were collected by the structured interview using questionnaires after self-help program and after 8 months. Data were analyzed by frequency, $x^2$-test and repeated t-test using SPSS 11.0 for windows. Result: Daily activities were not changed but self-described symptom was changed after 8 months. After 8 months, exercise performance along with taking drugs showed significantly lowering trend with respect to self-help program. Relieved symptom of patient with fibromyalgia has continued until 8months after 6weeks program of self-help. Also Promoted self-efficacy of patient with fibromyalgia continued after 8months. After 8 months, in self-described physical disability has showed significantly worse off than after self-help program. Conclusion: In this study, the long-term effects after self-help program for fibromyalgia have showed all symptoms and self-efficacy except exercise performance and physical disability. Therefore, it will be desirable that application of the follow up program with peer group meeting.

The Effect of Self-help Tai Chi Over 16 Weeks in Community Program for Older Adults Korean American Women (한국계-미국인 여성을 위한 16주간의 자조 타이치 효과)

  • Purpose: The purpose of this study was to examine the effects of a 16-week Self-help Tai Chi for Arthritis (SHTC) combined with health education for Korean American older women. Methods: This research was a designed quasiexperimental pre-posttest. Forty one women aged 55~79 were recruited 20 in SHTC group 21 in control group and, but twelve in SHTC group and thirteen in control group were left after 16 weeks. SHTC group was educated for 1 hour health education and 1hour TCA, once a week during 16 wks. Measurements for comparison were taken three times, at baseline, 8 wks and 16wks. The effect were evaluated with self-efficacy, shoulder flexibility, back flexibility, both hand grip strength and standing balance with closed eyes. Results: All variables except for left hand grip strength at baseline had significant homogeneity between both groups. After 16 weeks intervention, there was a significant interaction effect of time and group on right hand grip strength by repeated measure of ANOVA (F=3.398, p=.044). No significant interaction effects were found on self-efficacy, shoulder and back flexibility, left hand grip strength and standing balance with closed eyes. Conclusion: I can suggest this self-help Tai Chi program may be effective partially, but further research is needed to establish the best times and periods to intervene for a better effect.

Comparing Effects of Tai Chi Exercise, Tai Chi Self-help Program, and Self-help Management Program for Osteoarthritis Patients (골관절염 환자를 위한 타이치, 타이치-자조관리 및 자조관리과정의 효과 비교)

  • Purpose: The purpose of the study was to compare the effects of Tai Chi exercise, Tai Chi self help program, and self help management program on arthritic pain, stiffness, fatigue and difficulties in performing daily activities in patients with osteoarthritis. Method: The pre-post experimental design was utilized with convenient sampling at Public Health Centers. Total of 38 subjects agreed to participate in the study with the inclusion criteria of being diagnosed as osteoarthritis and no previous regular exercise. The 12 subjects were assigned to Tai Chi exercise group, 13 subjects to Tai Chi exercise combined with self help management program, and the other 13 to self help management program. Tai Chi exercise group performed exercise twice a week for one hour per each session. Tai Chi self help management group came to the class twice a week, one hour for Tai Chi exercise, and another hour for self help management program. The third group came to the class once a week for one hour to participate self help management program. At the completion of 6 week programs, 10 subjects from Tai Chi exercise, 9 from Tai Chi self help, another 10 from self help management groups completed the post-test measures. The data were analyzed by SPSSWIN 11.0. The homogeneity tests for three experimental groups were conducted by $X^2-test$, Fisher's exact test, Kruskal-Wallis test. The final analysis of the post-test measures was conducted by ANCOVA with fatigue, pain, stiffness, and difficulties in daily activities of the pretest measures as covariates. Results: The results were as follows. There were no significant differences in fatigue, pain, and difficulties of daily activities among three groups for the pretest measures. The score of arthritic stiffness was significantly different among groups after controlled for covariates, but the post-hoc tests showed no group differences. Conclusion: The nursing interventions applied for osteoarthritis patients have been previously tested to show the cost effectiveness of symptoms management of this population.

  • Purpose: The purpose of this study was to examine the effects of self-help management program on knowledge about stroke, self-efficacy, self-esteem and family support in people with stroke. Methods: With a quasi-experimental design, 44 persons with stroke were assigned into an experimental group (n=21) or a control group (n=23). The knowledge about stroke, self-efficacy and self-esteem were measured at both before and after the self-help management program. Date were analyzed with SPSS/WIN using descriptive statistics, t-test, and $x^2$-test. Results: There was a significant increase in knowledge about stroke, self-efficacy and self-esteem in the experimental group. Conclusion: Self-help management program can be an effective nursing intervention to improve the knowledge about stroke, self-efficacy and self-esteem for patients with stroke.

Effects of Short Term Selp-help Education on Pain, Depression, Self-efficacy, and Quality of life in Patients having Chronic Arthritis (단기 자조관리교육이 만성관절염 환자의 통증, 우울, 자기효능감 및 삶의 질에 미치는 영향)

  • The purpose of this study was to explore the effect of short term selp-help education on pain, depression, self-efficacy, and quality of life in patients having chronic arthritis. This program was carried out 3 hrs per week for 4 weeks by 3 small groups among 10 patients. Research design was a quasi-experimental study of nonequivalent control group pretest-posttest design. The subject of this study were 10 of experimental group and 10 of control group. Before and after 4 weeks program, quality of life, self-efficacy, depression, pain, ADL were measured. As a result, it was confirmed the self-help education was effective to increase quality of life, but self-efficacy, depression, pain, ADL were not improved. This suggests the replicate study to examine the effect of self-help education with the large samples of subjects having chronic arthritis.

Effects of Tai Chi or Self-help Program on Balance, Flexibility, Oxygen Consumption, and Muscle Strength in Women with Osteoarthritis (골관절염을 가진 여성의 균형감, 유연성, 심폐기능 및 근력에 대한 타이치 운동과 자조관리 프로그램의 효과비교)

  • Purpose: This randomized study was done to compare the effects of a 6 month Tai Chi exercise and a self-help program on balance, flexibility and muscle strength in women with osteoarthritis (OA). Method: In this experimental study, 82 women with OA recruited from outpatient clinics or community health centers were randomly assigned either to a Tai Chi group or a self-help group. Thirty women (mean age = 62 years) in the Tai Chi group and 39 (mean age = 59 years) in the self-help group completed posttest measures (balance, flexibility, oxygen consumption, abdominal muscle strength, back muscle strength, and grip strength) at 6 months. Results: After the 6 month, Tai Chi participants had significantly greater balance (mean difference = 2.9 vs. 0.9 for the self-help), grip strength (mean difference = 4.6 vs. 0.9 for the self-help), and back muscle strength (mean difference = 4.1 vs. -0.3 for the self-help). However, flexibility, oxygen consumption, and abdominal muscle strength were not significantly different between the groups. Conclusion: Tai Chi increased balance, grip strength and back muscle strength in older women with osteoarthritis compared to the self-help program. Whether these changes improve physical functioning and fall prevention requires further study.

  • Purpose: The purpose of this study is to examine effects of a self-help management program at public health centers on self-efficacy, self-esteem, knowledge of stroke and family supports in stroke patients. Methods: Based on a quasi-experimental design, 44 persons with stroke were assigned to the experimental group (n=21) or the control group (n=23). Data were analyzed through the descriptive statistics, t-test, $x^2$-test and Mann-Whitney test with the SPSS/WIN 18.0 program. Results: The experimental group showed significant improvement in its members' self-efficacy, self-esteem, knowledge of stroke and family supports. Conclusion: The self-help management program can be an effective nursing intervention to help stroke patients improve their self-efficacy, self-esteem, knowledge of stroke and family supports. That program is also meaningful in that it can contribute to more effective implementation of established programs for stroke patients in public health centers.
