한국판 목소리 자기조절 척도의 신뢰도 및 타당도 예비연구 (Validation of the Korean Voice Perceived Present Control Scale: A Pilot Study)

  • Background and Objectives The Voice Perceived Present Control scale (VPPC) has been developed to provide better insight into patients' perceived control over their thoughts or behaviors related to voice disorders. The objective of the present study was to validate the Korean VPPC (K-VPPC) by evaluating its internal consistency and reliability. Materials and Method All items of the English VPPC were translated into Korean. Content validity was analyzed through three Delphi survey rounds by an expert panel (n=44) with active clinical and research experience in treating dysphonic patients. Twenty-three patients with a heterogeneous diagnosis of dysphonia and twenty-three gender-matched vocally normal controls (total n=46) were asked to complete the K-VPPC and the Korean Voice Handicap Index-10 (KVHI-10). Psychometric properties including internal consistency and reliability were evaluated to examine the appropriateness of cross-cultural use of K-VPPC. Results Cronbach's alpha coefficient of K-VPPC was 0.89 for dysphonic patients, indicating good internal consistency in clinical samples. Furthermore, patients with dysphonia scored significantly lower on the total score of K-VPPC and higher on voice handicap than the vocally normal controls. Spearman's correlation coefficients indicated an inverse and moderate association between the K-VPPC and all domains of KVHI-10 (Spearman's r=-0.44- -0.68). Conclusion The findings of the current study indicated that the K-VPPC is a valid and reliable tool for the assessment of perceived control in Korean patients with dysphonia. Therefore, the K-VPPC could be a useful and complementary tool for the comprehensive evaluation of dysphonia, thereby improving care in Korean patients with voice disorders.

Translation, Adaptation and Cross-Cultural Validation of Hearing Handicap Inventory for Adult in Malay Language

  • Background and Objectives: Sine a self-reported questionnaire for hearing-impaired listeners is not available by Malay language yet, it is important to develop or translate any available existing questionnaires. The aim of this study was to translate, adapt and validate the Hearing Handicap Inventory for Adult (HHIA) to be used by the audiologist among the hearing-impaired population in Malaysia. Subjects and Methods: The HHIAs was translated to Malay language using forward-backward translation techniques by four-panellists (two for each level). The translated HHIA was then reconciled and harmonized for cultural aspects and content of the questionnaire by the researchers and two expert panels before being pilot-tested among 10 hearing-impaired patients. Questionnaire validation was conducted among 80 adults with a hearing loss to calculate for Cronbach's α (internal reliability), Spearman's correlation (inter-item correlation) and factor analysis. Results: None of the translated items were removed from the scale. The overall Cronbach's α was 0.964; 0.927 and 0.934 for both social and emotional subscales, respectively. The factor analysis (force-concept inventory) demonstrated a two-structure with a strong correlation between all items in either component 1 or 2, that resembled the original scale. The Mann-Whitney test revealed significantly higher scores for those adults with a hearing loss than those adults with normal hearing. Conclusions: The Malay HHIA has been successfully translated and validated for the purpose of determining the psychosocial aspects of adults with hearing loss in the local population.

행동장애 입원환아의 간호문제 (NURSING PROBLEMS OF THE INPATIENTS WITH CONDUCT DISORDER)

  • 1985년 9월 30일부터 1990년 12월 31일 사이에 서울대병원 소아정신과에 입원하여 행동장애로 진단받은 어린이 및 청소년 총 48명을 대상으로 병록고찰을 통해 간호문제를 분석해보았다. 그 결과 미성숙한 자아발달, 낮은 자아정체감, 역기능적 부모-자녀관계, 가족내 상황적 위기, 정신지체, 간질 등 원인적 요인에 따르는 개인의 비효율적 대처기전, 사회적 상호작용장애, 자기개념장애, 폭행잠재성, 부모역할장애, 성장 ${\cdot}$ 발달장애 같은 문제가 입원초기부터 중기이후까지 지속되었고 자기간호결핍, 수면장애, 영양장애, 불안, 고체온증 등 입원중기 이후에는 해결된 일시적 문제들을 보였다. 따라서 행동장애 입원환아들을 위한 간호접근은 보다 근본적이고 지속적인 문제에 따른 장기적 계획을 통해 환아들의 자아기능 증진, 자아정체감 확립, 대인관계개선을 도와야 하고 환아 뿐 아니라 가족을 위한 지지적, 교육적 접근에 중점을 두어야 함을 보았다.

  • 이동이 불편한 장애자에게 있어서 자동차는 필수 불가결한 존재이다. 특히 장애인의 재활을 통한 사회 참여라는 측면에서 볼 때, 정상인들과 동등한 이동의 자유는 성숙사회에 있어서 질적인 삶을 영위하기 위한 중요한 요소이다. 이 연구는 이동수단을 통한 장애인의 적극적 사회참여와 휠체어 사용환경의 확대 추구를 목적으로, 휠체어 사용자들에게 있어서 트랜스포테이션에 대한 제반적인 문제요소를 조사하고, 이들 문제요소의 해결을 위하여 장애인 승차의 특성을 고려한 자동차를 스타일링 측면에서 접근하였다. 이를 위하여 먼저 기존의 양산 차에 장애자를 위한 편의 시설을 보강하고, 구조 변경된 차량들의 조형적인 부분을 제시하였다. 또한 국내에서 생산되는 차종을 선정하여 휠체어 탑승이 가능하도록 내부구조를 제시하고, 스케치를 통해 변경된 차량의 모델을 제시하는 방법으로 연구를 진행하였다.

요양병원 입원 노인환자의 구강건강인식도와 구강건강관련 삶의 질에 관한 연구 (A study on oral health perception and oral health-related quality of life of the elderly patients in a geriatric hospital)

  • Objectives: The purpose of the study was to investigate the relationship between oral health perception and oral health-related quality of life-14(OHIP-14) in a geriatric hospital. Methods: A self-reported questionnaire was completed by 230 elderly people in a long-term care institution. The questionnaire was consisted of items on general characteristics, oral health perception and OHIP-14. Except incomplete answers, 226 data were analyzed statistically with one-way ANOVA, t-test, Pearson's correlation coefficient and post hoc Tukey test and ANOVA when significance was set at 0.05. Results: The OHIP-14 showed a negative linear relationship(p<0.01) of r=-0.272 with the oral health perception. Among the subgroups of the OHIP-14, quantitative linear relationship(p<0.01) was shown in functional limitation(r=0.822), physical pain(r=0.825), psychological discomfort(r=0.568), physical disability(r=0.895), psychological disability(r=0.652), social disability(r=0.804) and handicap(r=0.818). Conclusions: In order to improve the oral health perception and OHIP-14 in a geriatric hospital, it is necessary to develop continuously a variety of oral health education and systematic oral health promotion program.

뇌성마비아 어머니의 스트레스와 대처양상에 관한 연구 (A Study on the Stress and Coping Patterns of Mothers with Cerebral Palsy Children)

  • Mothers with cerebral palsy children have much stress that is related to the care of children with cerebral palsy and to other household duties, and this state of the mother has an effect on the cerebral palsy child and on other household member. Mothers in such stressful situations use various coping patterns. The purpose of this study was as follows : to develop instruments that can be used for measuring the stress and coping patterns of mothers with cerebral palsy children, and to test a hypothetical model on the relationship between the mother's stress, her coping patterns and the variable affecting the stress and coping patterns. The results of this study can be summarized as follows : 1. The stress scale was composed of 44 items and Cronbach's ${\alpha}$ was .94, and the coping pattern scale was composed of 19 items and Cronbach's ${\alpha}$ was .80. The mean score of stress scale was 136.12 out of a total of 220, and the mean score of the coping scale was 72.87 in a total of 95. 2. In test of the hypothetical model, it was found that extra-care demand, the support of the husband, the degree of handicap, health status and self-esteem had statistically significant influence on the mother's stress(r=.285, -.262, -.133, -.126). And the support of the husband, formal support, informal support, and economic status were found to have statitically significant influence on the mother's coping patterns(r=.412, .178, .178, .138).

Depression in family caregivers of elderly people with dementia

  • The caregivers of elderly people with dementia suffer an affectation in the psychological and social order due to the low probability of total rehabilitation of the elderly and the progressive course of this pathology. In the municipality of Guisa, Granma Province there is a health problem regarding this problem since family caregivers of the elderly with dementia constantly go to the Psychology and Psychiatry consultations, in search of specialized help because they report feeling depressed. For this reason, this research was carried out with the objective of determining the main manifestations of depression in these caregivers. The results emanating from the triangulation of methods (scientific observation) and techniques (questionnaire, interviews and inventory Beck), employees at three stages of work where it was found that the main manifestations of depression of these caregivers are: poss of interest or the ability to enjoy activities that were previously pleasurable, loss of emotional reactivity to pleasant environmental events and circumstances, sleep disturbances, marked loss of appetite, weight loss, irritability, muscular tension, feelings of sadness unmotivated that produces discomfort and sufferings, ansiety, entities of handicap, blame, loss of self-confidence and feelings of inferiority, gloomy perspective of the future.

The Impact of Senior Mobility Funding on Hearing Aid Acquisition and Compliance to Hearing Aid Use in a Singapore Hospital

  • Background and Objectives: In 2013, the Singapore government reviewed and expanded the Senior Mobility Fund (SMF) to provide subsidy for assistive devices, including hearing aids (HA). While SMF has improved accessibility to HA, its impact on HA acquisition has not been determined. The study aims to elucidate the influence of SMF on HA acquisition and the relationship between financial funding and compliance to HA use. Subjects and Methods: Retrospective review of 643 patients seen between January 2017 to January 2018 at the earnose and throat specialist outpatient clinic, who were referred for a hearing aid evaluation. Of the 643 patients, 109 patients with baseline hearing handicap (HH) scores recorded, and no formal diagnosis of cognitive impairment were included. The patients were grouped according to SMF eligibility and clinical data were obtained. Results: The odds ratio for acquiring HA was significantly higher with SMF, regardless of HH scores. When looking at actual degree of hearing loss (DHL), HA uptake was significantly higher in the least severe of DHL categories. There is no relationship between SMF and compliance to HA use. However, HH score is positively correlated with data log in regression analyses. Conclusions: SMF appears to influence HA acquisition. Especially in financially funded patients with low self-perceived HH, the benefits of HA may be underappreciated given the few hours of HA use. Further studies are warranted to investigate the impact of financial funding on outcomes of HA users to help policy planners and clinicians be prudent in the utilization of SMF.
