An exploratory study on practice-oriented reconceptualization of self-sufficiency : Service providers' reflections on their own experiences from the field (현장의 시각으로부터 구조화된 자활 개념 탐색 연구 : 자활사업 실무자의 이해를 중심으로)

  • A self-sufficiency service has worked as a typical workfare policy combined with public assistance in Korea since 2000. Despite of its long history, three core pillars in administrating the self-sufficiency service, policy, research, and practice, have respectively understood the meaning of self-sufficiency in terms of their own interests. As a result, the self-sufficiency service has recently faced with its own identity issues by showing failures to its environmental changes. The current situation makes it necessary to reconceptualize the definition of self-sufficiency by exploring its in-depth understanding perceived by service providers. Specifically, we analyzed practical reflections on 35 service providers' experiences which were collected via focus group interviews for two hours. The study findings presented that service providers had two antithetical approaches towards self-sufficiency. While a dominant approach to self-sufficiency has been concentrated on improving clients' economic outcomes such as employment, job retention, the escape from welfare trap, and increasing earnings and assets, the other approach has been extended to empower clients and achieve their well-being and quality of life. Yet, these contrary perspectives have led to suffer from their role confusions and identity crisis between the work-ready process and the employment-oriented outcomes. Specifically, they described self-sufficiency in terms of psychological, social, and integrated aspects. The psychological aspect included a process of developing inner strengths, intensifying job motivation, and coping with barriers of employment. The social aspect meant a path toward social integration through recovering human relationships. The integrated aspect covered more comprehensive support for their recovery of daily life and autonomy to make a decision for their own life. In conclusion, the study findings suggest that self-sufficiency should be more extensively considered as a stepwise process towards work-ready preparations beyond ultimate economic outcomes. Such an extended concept of self-sufficiency could contribute to restructuring the whole practice of self-sufficiency including organizational and program changes in the fields.

The Relationship of Uncertainty, Hope and Quality of Life in Patients with Breast Cancer (유방암 환자의 불확실성, 희망 및 삶의 질과의 관계)

  • Purpose: The purpose of this study was to explore the relationship of uncertainty, hope and quality of life in patients with breast cancer. Method: The subjects of the study were 113 breast cancer patients treated at 3 university hospitals in Daegu and Busan from June 1 to September 30, 2003. Uncertainty was measured by the Mishel Uncertainty in Il1ness Scale (MUIS), Hope was measured by Nowotny's Hope Scale, and quality of life by Ro's Korean Quality of Life(QOL). Data was analyzed with the SPSS program by t-test, ANOVA, Spearman's rank correlation and Multiple Regression Analysis. Result: The scores of uncertainty were negatively correlated with hope and QOL(all p's<0.05). Uncertainty revealed that there was a negative correlation among subitems of hope such as confidence, relationship with others, possibility of future, and inner motivation(r=-.333, p=.000; r=-.230, p=.015; r=-.260, p=.006; r=-.291, p=.003). Uncertainty also showed a negative correlation among subitems of QOL such as emotional status, self esteem, and relationship with family(r=-.29, p=.004; r=-.326, p=.000; r=-.197, p=.044), whereas hope revealed a positive correlation among subitems of QOL such as emotional status, self esteem, relationship with neighbor, and relationship with family(r=.243, p=.011; r=.487, p=.000; r=.29, p=.001; r=.29, p=.001). The score of uncertainty was low in graduate school students and subjects using problem solving as a coping method. However, the score of hope was high in college students and subjects using a problem solving method, and the score of QOL was high in professionals. Conclusion: Based on the study results, breast cancer patients experienced living with uncertainty and maintaining hope. Thus the study's findings can be useful in directing a comprehensive nursing care plan to improve QOL of breast cancer patients.

Reliability and Validity of the Measurement for Assessing Resourcefulness (자원동원성 측정도구의 신뢰도와 타당도)

  • The purpose of this study was to verify the reliability and validity of the Rosenbaum self-control schedule (SCS) for assessing resourcefulness in Korea and to explore the simplified scale. The study subjects consisted of 787 adults in a community. The data was collected during the period from Oct. to Dec., 1995 and analyzed as Cronbach α, item correlation with total, Pearson correlation and factor analysis with varimax rotation using SAS. Results were as follows : The mean SCS score for this sample was 17.2 and there were statistical differences for gender(men, 13.1 ; women, 20.0) on the SCS scores. The cronbachα of SCS with 36 items was .74 and when simplified with 30 items, it's coefficient alpha was .78. The translated content of the SCS was validated by two nursing faculty members and one professor of psychology. Factor analysis revealed the most parsimonious structure was obtained when six factors were extracted and subsequently rotated via the varimax criterion. There was 40.2% of total communality variance in the SCS with 36 items. The total communality variance was slightly increased to 43.4% with 30 items of the SCS. In order to reduce from 36 items to 30 items, the process excepted 6 items having low item correlation with total and low MSA(means of sampling adequacy) of factor analysis. According to factor analysis, there are six factors such as emotion control, impulse control, self-efficacy, coping with problems, pain control and satisfaction control, The SCS was found to have low, but statistically significant, correlations with social desirability and helplessness. From the above results, it can be concluded that the reliability of the SCS(inherent and simplified) was a acceptable level and its validity was reasonable when comparing it with other validity studies. To determine the usefulness the simplified scale, further study is necessary to simultaneously compare and analyze both scales for stability.

Factors Influencing Mental Health among Nursing students (간호대학생의 정신건강에 영향을 주는 요인)

  • The purpose of this study was to investigate the degree of mental health among nursing students in Korea and identify factors influencing the tendency to mental health. A self-report survey was conducted with South Korean nursing students who were from 19 to 23 years old. 269 nursing students were included in the study. The instruments utilized in the study were Symptom Checklist-90-Revision, Stress and multidimensional coping. Data were analyzed using descriptive statistics, Pearson correlation and Simultaneous multiple regression with SPSS WIN 20.0. The average mental health score of the participants was 0.57. Significant predictors for mental health 'College-level stress', 'Self-criticism', 'Passive withdrawal', 'Nursing satisfaction' and 'Health state'. The study findings suggest that nursing students require special concern regarding the risk of mental health. Multi and interdisciplinary mental health promotion program will enhance the mental health of nursing students.

Model Development of Illness Perception and Consequences in Breast Cancer Patients

  • Breast cancer is the first or second malignancy among women worldwide. Illness perception (IP) and quality of life (QoL) are major issues considering breast cancer management. An attempt was here made to inspect the predictive variables influencing IP and their impact on QoL in cancer patients. The key predictors adapted from previous studies including life satisfaction, perceived social support, self-esteem, hope, optimism, and spiritual well being were taken into account. Our sample included 200 female volunteers suffering from breast cancer applying exclusion criteria. The data were collected using various questionnaires and statistically analyzed by means of path analysis and structural equation modeling. The results revealed that of the six predictors, spiritual well being and social support had direct effects on QoL and IP. The only path with significant indirect correlation with IP was social support. Spiritual well being had the second significant direct effect on IP. Self-esteem occupied the third rank in direct effects on both QoL and IP. In conclusion, here in Iran and most likely in other strongly religious communities, spiritual intervention is an effective strategy to raise QoL. Also social support helps women suffering from breast cancer to experience better conception and coping strategies.

Development of Group Counseling Program to Support for Woman Scientists and Technicians with Career Breaks: A Preliminary Study (경력단절 여성과학기술인 지원을 위한 집단상담 프로그램 개발: 예비연구)

  • The purpose of this preliminary study is to help put the women with career breaks in Science and Technique back to work and keep on working. For this purpose, we developed a 8-session group counseling for woman scientists and technicians with career break: Self-Growth group and Interpersonal Conflict Resolution Group. A total of 14 people participated in Group counseling. The most of participants reported that they have experienced lowered self-esteem, difficulties in relationship with family and colleagues, and psychological pressure and distress. The participants in the Interpersonal Conflict Resolution Group Counseling showed positive changes at stress coping strategies. From this study, the women with career breaks in Science and Technique will be expected to overcome the difficulties of career breaks wisely, demonstrate and confirm their competence, and restore from stress and psychological burnout. Based on the results of this study, implications and limitations of this study and future research were discussed.

  • Using a qualitative multimethod approach, this study was to discover integrated factors to construct a substantive theory about occupational socialization of librarians. Through theoretical sampling, 42 participants were in-deathly interviewed at three studies, i. e phenomenological, ethnographic, and grounded theory approach. These data were analyzed and finally integrated using grounded theory approach. Emerged integrated factors were: Sharing values of the profession in education, Striving to overcome the peripheral perception, Lack of understanding about the profession, Inadequate conditions, Shared work values, Social changes, Perception about users, Organizational culture of library, Human relations in organization, Continuing education and exchange, Seeking the professional, Strategies for work, Strategies for human relations, Strategies for surviving slump and crisis, Strategies for enhancing oneself, Coping strategies for lack of understanding, Self congruence with the profession, Non-self congruence with the profession.

A Voltage Regulation System for Independent Load Operation of Stand Alone Self-Excited Induction Generators

  • In recent years, some converter structures and analyzing methods for the voltage regulation of stand-alone self-excited induction generators (SEIGs) have been introduced. However, all of them are concerned with the three-phase voltage control of three-phase SEIGs or the single-phase voltage control of single-phase SEIGs for the operation of these machines under balanced load conditions. In this paper, each phase voltage is controlled separately through separated converters, which consist of a full-bridge diode rectifier and one-IGBT. For this purpose, the principle of the electronic load controllers supported by fuzzy logic is employed in the two-different proposed converter structures. While changing single phase consumer loads that are independent from each other, the output voltages of the generator are controlled independently by three-number of separated electronic load controllers (SELCs) in two different mode operations. The aim is to obtain a rated power from the SEIG via the switching of the dump loads to be the complement of consumer load variations. The transient and steady state behaviors of the whole system are investigated by simulation studies from the point of getting the design parameters, and experiments are carried out for validation of the results. The results illustrate that the proposed SELC system is capable of coping with independent consumer load variations to keep output voltage at a desired value for each phase. It is also available for unbalanced consumer load conditions. In addition, it is concluded that the proposed converter without a filter capacitor has less harmonics on the currents.

Study on the Development of Home Care Nursing Intervention Protocol for Stroke Patients (뇌졸중 환자의 가정간호중재 프로토콜 개발)

  • Stroke patient needs rehabilitation after receiving an acute treatment in a hospital. When stroke patient gets involved in an early discharge program, home care nurse plays a pivotal role to make them to gain a full strength and to come back to his/her prior life before he/she is sick. In spite of the importance of home care nursing intervention protocol for home care nurses to perform home care nursing autonomously, home care nursing intervention protocol for stroke patient is rarely developed. Therefore this study was conducted to develop home care nursing protocol that is applicable for stroke patients in home care nursing area. 41 home care nursing charts for stroke patients registered in home care nursing agencies from December 1st 1994 to August 31st 1999 at Y hospitals in Seoul and Won-Ju city were analyzed. 44 home care nurses who were having over three years' experience on stroke patients were participated in this study as a user validity validation group. The results of this study are as follows. 1. 28 nursing diagnoses were selected on the basis of evaluation of nursing diagnoses of stroke patients presented in a previous literature and case studies on home care nursing. 2. 17 nursing diagnoses were classified through the frequency analysis of home care nursing charts for 41 stroke patients who had received home care nursing. The order of sequence was like these: impaired skin integrity, risk for infection, nutritional deficit, impaired physical mobility, constipation, knowledge deficit, ineffective airway clearance, anxiety in family members, risk for aspiration, self care deficit, altered urinary elimination, ineffective individual coping, social isolation, risk for injury, self-esteem disturbance, impaired verbal communication, fatigue of family caregiver. 3. Based on validation on expert and user validities, 44 nursing interventions which were above ICV=.80 were chosen. 4. Nursing intervention protocols which showed above ICV=.90 were developed and were like these; pressure ulcer care, position change, preventive care for circulatory dysfunction, tube care : catheter, vital sign monitor, constipation/impaction management, artificial airway management, suction of airway secretion, environmental management : safety, and fall prevention.

Adjustment Process of Hemodialysis Patients : A Grounded Theory Approach (혈액투석환자의 적응과정 경험)

  • Purpose : This research is aimed at developing a substantive theory related to the experience of adjustmented hemodialysis patients by identifying and analyzing the nature, process, and types of adjustment experienced by them. For this purpose, this study takes a grounded theory approach. Method : Data were collected from April, 2002 through September, 2002 through in-depth interviews and close observations of eleven hemodialysis patients who have experienced adjustment. With their consent, the interviews were recorded by audio tapes and later transcribed. Observation memos were also prepared on the subjects' behavior during the interviews. Data collection continued until saturated. The data were analyzed into concepts, subcategories, and categories with the open coding process. The axial coding was done to identify the relationships of the concepts and categories. And the selective coding was done to develop a core category, which is the central phenomenon of the hemodialysis patients who experienced adjustment. Result : This process resulted in 88 concepts, which may be grouped into 24 subcategories and 6 core categories. The 6 categories, in fact, depict the process of changes the patients experience from the sense of crisis, self-control, new life meaning, support system, coping ability, and quality of life improvement. And its core is self-control, which encompasses the dynamic stages from recognition, change attempts, belief solidification, and empowerment. Conclusion : The results of the research offer the following implications. First, research on adjustment is a formative stage in nursing, it is imperative for nursing researchers to develop them, which may be more relevant to nursing. Second, it is important to develop nursing intervention techniques that may be most effective in adjustment of hemodialysis patients and at the same time for each stage of changes taking place in adjustmented hemodialysis patients.
