Clinical Outcomes of Coil Embolization for Unruptured Intracranial Aneurysms Categorized by Region and Hospital Size : A Nationwide Cohort Study in Korea

  • Objective : To analyze the outcomes of coil embolization (CE) for unruptured intracranial aneurysm (UIA) according to region and hospital size based on National Health Insurance Service data in South Korea. Methods : The incidence of complications, including intracranial hemorrhage (ICRH) and cerebral infarction (CI), occurring within 3 months and the 1-year mortality rates in UIA patients who underwent CE in 2018 were analyzed. Hospitals were classified as tertiary referral general hospitals (TRGHs), general hospitals (GHs) or semigeneral hospitals (sGHs) according to their size, and the administrative districts of South Korea were divided into 15 regions. Results : In 2018, 8425 (TRGHs, 4438; GHs, 3617; sGHs, 370) CEs were performed for UIAs. Complications occurred in 5.69% of patients seen at TRGHs, 13.48% at GHs, and 20.45% at sGHs. The complication rate in TRGHs was significantly lower than that in GHs (p=0.039) or sGHs (p=0.005), and that in GHs was significantly lower than that in sGHs (p=0.030). The mortality rates in TRGHs, GHs, and sGHs were 0.81%, 2.16%, and 3.92%, respectively, with no significant difference. Despite no significant difference in the mortality rates, the complication rate significantly increased as the number of CE procedures per hospital decreased (p=0.001; rho=-0.635). Among the hospitals where more than 30 CEs were performed for UIAs, the incidence of CIs (p=0.096, rho=-0.205) and the mortality rates (3 months, p=0.048, rho=-0.243; 1 year, p=0.009, rho=-0.315) significantly decreased as the number of CEs that were performed increased and no significant difference in the incidence of post-CE ICRH was observed. Conclusion : The complication rate in patients who underwent CE for UIA increased as the hospital size and physicians' experience in conducting CEs decreased. We recommend nationwide quality control policies CEs for UIAs.

A Review of the Time Cheongju Sanesa Buddhist Temple Relics Were Hoarded (청주 사뇌사 유물 퇴장(退藏) 시기에 대한 검토)

  • In 1993, a large number of relics was found in Cheongju Sanesa Buddhist Temple. They show superiority as bronze-based products, and are especially important in since they provide much information about the time of their creation. However, there are many opinions about the time they were hoarded: in the middle of the 13th century, late in the 13th century, early in the 14th century, etc. This study estimates the time they were hoarded to be some time in April 1291 during the invasion of Kădīn (哈丹) in the Yuan Dynasty. Kădīn's troops invaded the Goryeo Dynasty, then went through Yangpyeong and Wonju in January 1291 and appeared in Yeongi-hyeon on May 1 of that year. Based on records, this study verified that the troops passed through Cheongju on their way from Chungju to Yeongi-hyeon (currently Sejong-si) and pointed out that the invasion route of the troops was the background for the hoarding of Sanesa relics. The estimation that the Sanesa relics were hoarded in 1291 when Kădīn's invasion was going on makes it possible to reasonably clear up the era of the relics in which the Heavenly Stems called gānzhī (干支) Muo (戊午), Gyeongshin (庚申), Giyu(己酉) etc. were written. That is, Giyu Geumgo is presumed to be the year 1249, Muo Hyangro 1258, Gyeongshin Hyangwan 1260, etc.

Clinical Results of Drug-Coated Balloon Treatment in a Large-Scale Multicenter Korean Registry Study

  • Background and Objectives: The aim of this study was to demonstrate the efficacy and safety of treatment with drug-coated balloon (DCB) in a large real-world population. Methods: Patients treated with DCBs were included in a multicenter observational registry that enrolled patients from 18 hospitals in Korea between January 2009 and December 2017. The primary outcome was target lesion failure (TLF) defined as a composite of cardiovascular death, target vessel myocardial infarction, and clinically indicated target lesion revascularization at 12 months. Results: The study included 2,509 patients with 2,666 DCB-treated coronary artery lesions (1,688 [63.3%] with in-stent restenosis [ISR] lesions vs. 978 [36.7%] with de novo lesions). The mean age with standard deviation was 65.7±11.3 years; 65.7% of the patients were men. At 12 months, the primary outcome, TLF, occurred in 179 (6.7%), 151 (8.9%), 28 (2.9%) patients among the total, ISR, and de novo lesion populations, respectively. A history of hypertension, diabetes, acute coronary syndrome, previous coronary artery bypass graft, reduced left ventricular ejection fraction, B2C lesion and ISR lesion were independent predictors of 12 months TLF in the overall study population. Conclusions: This large multicenter DCB registry study revealed the favorable clinical outcome of DCB treatment in real-world practice in patient with ISR lesion as well as small de novo coronary lesion.

Preparation of Silica Nanoparticles via Recycling of Silicon Sludge from Semiconductor Dicing Process and Electro-responsive Smart Fluid Application (반도체 다이싱 공정에서 발생하는 실리콘 슬러지를 재활용한 실리카 나노입자의 제조 및 전기감응형 유체로의 응용)

  • In this study, silicon sludge from semiconductor dicing process is recycled to fabricate silica nanoparticles, which are applied as dispersing materials for electro-responsive (ER) smart fluid. In specific, metal impurities are removed from silicon sludge by acid washing to obtain the high-purity silicon powder. And then, silica nanoparticles are synthesized by facile hydrothermal method employing the silicon powder as reactant material. To control the size of silica nanoparticles, the reaction time of hydrothermal method is varied as 8, 15, 20, and 30 hours are applied to control the size of silica nanoparticles. Sizes of silica nanoparticles are increased proportionally to the reaction time owing to the increased numbers of hydrolysis and condensation reactions. As-synthesized silica nanoparticles are prepared as electro-responsive smart fluids by dispersing into silicon oil. Silica nanoparticles synthesized by 30 hours of hydrothermal reaction (SiO2-H30) exhibit the highest shear stress of 21.4 Pa under an applied electric field strength of 3.0kV mm-1. Such enhancement in ER performance of SiO2-H30 among various silica nanoparticles are attribute to the reinforcing effect originated from the mixed particle size, which allowing the formation of rigid chain-like structures. Accordingly, this study successfully propose a recycling method of silicon sludge to synthesize silica nanoparticles and their derived ER fluids, which may suggest new possibility to ESG management emphasizing the eco-friendliness.

Effects of Autoclave and Microwave Treatments on Quality of Cookies (가압가열 및 Microwave 처리가 쿠키의 품질에 미치는 영향)

  • This study was conducted in order to determine the effect of autoclave and microwave treatments on cookies. Wheat dough was treated in an autoclave (30 min), a microwave (1 min), and both autoclave and microwave (30 min/1 min). The treated dough was baked or non-baked and cookies quality was assessed. The pH, moisture content, color, and texture were measured, and sensory evaluation was performed. The pH showed a slight increase when dough was treated in a microwave (before baking) and decreased in both the autoclave and autoclave/microwave (after baking) treatments. The moisture content showed a significant increase, compared to the control, in autoclave treatments. Regarding surface color of cookies, lightness was decreased in all treatments and redness was higher compared with the control, except for microwave (before baking), and yellowness was lower compared to the control, except for microwave treatment (after baking). Hardness, gumminess, chewiness, and shear force showed a significant increase when dough was treated in an autoclave/microwave (after baking) compared to the control. In the sensory evaluation, color, taste, texture, shape, and overall preference of cookies treated with an autoclave/microwave (after baking) showed a low score. These results suggest that autoclaving and microwaving may reduce the quality of bread and that preparation of countermeasures is necessary.

Effects of Tropical Night and Light Pollution on Cicadas Calls in Urban Areas (도심지 열대야 및 빛공해에 의한 매미 울음 영향)

  • Environmental factors that affect the singing of cicadas have not been studied extensively, especially those affecting the cicadas' singing during the nighttime. Therefore, the objective of this study is to identify the effects of tropical night and light pollution on the cicadas' singing in a downtown area. The study sites were an apartment complex in Seocho-gu, Seoul, and the Chiaksan National Park in Wonju-si. The study subjects were Hyalessa fuscata and Cryptotympana atrata, which are the dominant species in Korea during summer. Cicada songs were recorded 24 hours a day, every day. The recording period was between July and August, lasting 25 days at the Seoul site and 14 days at the Chiaksan National Park. Temperature, precipitation, humidity, and amount of sunshine were selected as the environmental factors that potentially affect the cicadas' singing. Statistical analyses included correlations of meteorological factors with the cicadas' singing per hour, per 24 hours, and at nighttime (21:00~04:00). The results showed that: 1) H. fuscata began singing during the dawn hours, and the singing increased in intensity early in the morning. C. atrata's singing reached its peak in the morning and afternoon, ceased during sunset hours, thereby exhibiting a difference in the singing pattern of the two species. 2) The frequency of singing by H. fuscata decreased when C. atrata began to sing intensively in numbers, thereby exhibiting interspecific influence. 3) The results of the correlation analysis between meteorological factors and the singing of H. fuscata and C. atrata showed that both species tended to sing more when the temperature was higher and sang less on rainy days. 4) When limited to nighttime only, C. atrata showed a tendency of singing when the nighttime temperature was high ($24-30^{\circ}C$, average $27^{\circ}C$), whereas H. fuscata did not show a correlation with meteorological factors. However, since H. fuscata sang during the night in areas with artificial lighting, it was concluded that its singing was due to light pollution.

Effects of Autoclave and Microwave Treatments for Quality of Wet Noodles (가압가열 및 Microwave 처리가 생면의 품질에 미치는 영향)

  • This study was conducted to determine the effects of physical treatments for quality of wet noodles. Noodles were being tried with a microwave (for 1 min), an autoclave (for 30 or 50 min), and both autoclave and microwave (for 30/1 min or 50/1 min). The results showed that the pH levels were slightly decreased after treatments of autoclave and autoclave/microwave. The moisture contents were considerably decreased as compared to the control except autoclave (50 min). After all treatments, the lightness was decreased in all samples, but, redness was increased (except microwave) and the yellowness was increased after autoclave (50 min) and autoclave/microwave (50/1 min). Texture was increased as compared to the control except microwave. In the sensory evaluation, the noodles treated with microwave, autoclave (50 min), and autoclave/ microwave (50/1 min) showed a high score in overall preference. From these results, both the autoclave and microwave methods can be applied to the wet noodles without diminishing its quality to a great extent.
