치조열환자에서 치조골 이식술의 예후 (Prognosis of Alveolar Bone Graft Alveolus in Cleft Patients : the preliminary report)

  • The case records of 24 patients in Seoul National University Hospital who had bone grafting of 29 alveolar clefts between 2001 and 2004 were examined. Details were recorded of age, sex, preoperative orthodontictreatment, the time of bone grafting, the type of donor site, cleft width, functional load applicationand the success of grafting as established by lowest marginal bone levels. Using this results and review of literature, we concluded that secondary bone graft with iliac bone before canine eruption with root development of 1/2 to 1/3 provide more favorable results and the functional load introduced to the grafted bone lower the resorption rates.

구순구개열 환아에서의 치조골이식 (TOOTH MOVEMENTS TO THE SITE OF ALVEOLAR BONE GRAFT)

  • 치조파열 및 구개열과 같은 선천성 기형은 이환된 환자에게 기능적, 심미적으로 많은 문제점을 야기하므로 정상으로 회복시켜 주는 것은 중요한 문제이다. 장기간 방치될 경우 영양장애, 구강위생 불량, 호흡기 감염, 언어 장애, 악안면 변형, 그리고 정신적인 문제 등이 복합적으로 발생할 수 있다. 따라서 구순구개열 환자의 치료는 여러 전문 치료 분야의 복합적 인 접근이 필요하다. 골이식은 구순구개열 환자에 있어 중요한 치과치료 단계이다. 치아의 맹출과 치열의 안정화를 위해 치열궁은 골결손이 없이 완전해져야 한다. 또한 치열궁의 파열이 있는 부위로는 정상적인 교정적 치아이동이 곤란하다. 따라서 구순구개열 환자에 있어 골이식은 광범위하게 적용되고 있는 외과적 술식이다. 치조골을 이식함으로써 치조열은 안정화되고, 견치 또는 절치가 이식부위로 이동할 수 있게 된다. 그리고 골 이식 후, 교정을 통해 치조열 부위의 공간을 폐쇄함으로써 보철치료 없이 치열을 재형성 할 수 있다. 골이식술에는 다양한 이식재료가 사용되었다. 자가골을 이식할 경우 장골이 가장 선호되며, 그밖에 경골, 늑골, 두개골, 하악골을 이용하기도 한다. 그리고 골이식은 골이식 시기에 따라 일차골이식, 조기 이차골이식, 이차골이식, 만기 이차골이식으로 구분할 수 있다. 이차골이식은 혼합치열기 말경에 시행되는 것을 말하며 가장 좋은 것으로 받아들여지고 있다. 영구 견치가 맹출되기 전에 이차골이식을 시행하면 인접치아의 맹출과 보존에 도움이 된다. 본 증례에서는 치조구개파열 환자의 장골에서 골을 채취하여 골 이식을 시행한 후, 골이 채워진 이식 부위에 인접 치아가 성공적으로 이동하거나 치축이 개선되고 교정치료를 통해 치열궁 배열의 만족스러운 결과를 얻었기에 이를 보고하는 바이다.

Current Methods for the Treatment of Alveolar Cleft

  • Alveolar cleft is a tornado-shaped bone defect in the maxillary arch. The treatment goals for alveolar cleft are stabilization and provision of bone continuity to the maxillary arch, permitting support for tooth eruption, eliminating oronasal fistulas, providing an improved esthetic result, and improving speech. Treatment protocols vary in terms of the operative time, surgical techniques, and graft materials. Early approaches including boneless bone grafting (gingivoperiosteoplasty) and primary bone graft fell into disfavor because they impaired facial growth, and they remain controversial. Secondary bone graft (SBG) is not the most perfect method, but long-term follow-up has shown that the graft is absorbed to a lesser extent, does not impede facial growth, and supports other teeth. Accordingly, SBG in the mixed dentition phase (6-11 years) has become the preferred method of treatment. The most commonly used graft material is cancellous bone from the iliac crest. Recently, many researchers have investigated the use of allogeneic bone, artificial bone, and recombinant human bone morphogenetic protein, along with growth factors because of their ability to decrease donor-site morbidity. Further investigations of bone substitutes and additives will continue to be needed to increase their effectiveness and to reduce complications.

  • The fracture of facial bone usually accompanies alveolar bone fracture and dislocation or fracture of teeth. Thus, aside from the reduction of fracture, the reconstruction of occlusion through the rehabilitation of lost teeth should be considered. The dislocation of tooth after trauma accompanying alveolar bone fracture needs bone grafting in case of implant treatment. Although autogenous bone graft shows good prognosis, it has the disadvantage of requiring a secondary surgery. This is a case of a mandibular condyle head fracture accompanied by alveolar bone fracture. The condylar head fragment removed during open reduction was grafted to the alveolar bone fracture site, thereby foregoing the need for secondary surgery.

양측성 치조열의 재건을 위한 협부 점막피판의 사용:2증례 (The Use of Bilateral Buccal Mucosal Flap for the Repair of Bilateral Cleft Alveolus : 2 Case Reports)

  • In alveolar deformity of cleft patient, the flap design is very important to make the functional and esthetic outcome. Especially in bilateral cleft alveolus with wide defect, deficiency of covering tissue is a greatest problem. Wound dehiscence may develop oronasal fistula of palatal and labial region and loss of the bone graft. We report 2 cases with bilateral cleft alveolus. In both case, bilateral buccal mucosal flap was used for closure of bilateral cleft alveolus with wide defect. The one was operated with iliac bone graft according to secondary grafting method, the other was closed without bone grafting. The patient was 3 years old. So, secondary alveolar bone graft will be required some years later for the establishment of bony continuity and esthetic advantage. In both cases, we found the entire soft tissue closure without the lack of covering flap. In these case, the closure of alveolus defect was accomplished successfully by the use of bilateral buccal mucosal flap. There was no complication, secondary fistula. The most important thing is the tension-free closure of the bilateral buccal mucosal flap. So, we report these cases with literatures.

동맥류성 골낭종으로 오인된 거골에 발생한 연골모세포종: 증례 보고 (Chondroblastoma of the Talus Mimicking an Aneurysmal Bone Cyst: A Case Report)

  • Chondroblastoma is a rare benign tumor that produces giant cells and cartilage matrix. The tumor occurs in people between 10 and 25 years with slightly higher incidence in males. The condition occurs in the proximal epiphysis of the tibia and humerus, distal epiphysis of the femur, but its occurrence in the talus is relatively rare, accounting for 4% of the total number of chondroblastoma cases. Chondroblastoma is often misdiagnosed as a primary aneurysmal bone cyst, giant cell tumor, chondromyxoid, and lesion of a secondary aneurysmal bone cyst by fibrous dysplasia. The most commonly used surgical method for chondroblastoma is broad curettage with bone grafting. In general, an aneurysmal bone cyst is associated with a second degree chondroblastoma, which is approximately 20%. Chondroblastoma of the talus and secondary aneurysmal bone cysts can be misdiagnosed as primary aneurysmal bone cysts. This paper reports a case of a young male patient with chondroblastoma of the talus, which was initially misdiagnosed as an aneurysmal bone cyst with involvement of the talo-navicular joint.

Speech Outcomes after Delayed Hard Palate Closure and Synchronous Secondary Alveolar Bone Grafting in Patients with Cleft Lip, Alveolus and Palate

  • Background The best timing of closure of the hard palate in individuals with cleft lip, alveolus, and palate (CLAP) to reach the optimal speech outcomes and maxillary growth is still a subject of debate. This study evaluates changes in compensatory articulatory patterns and resonance in patients with unilateral and bilateral CLAP who underwent simultaneous closure of the hard palate and secondary alveolar bone grafting (ABG). Methods A retrospective study of patients with nonsyndromic unilateral and bilateral CLAP who underwent delayed hard palate closure (DHPC) simultaneously with ABG at 9 to 12 years of age from 2013 to 2018. The articulatory patterns, nasality, degree of hypernasality, facial grimacing, and speech intelligibility were assessed pre- and postoperatively. Results Forty-eight patients were included. DHPC and ABG were performed at the mean age of 10.5 years. Postoperatively hypernasal speech was still present in 54% of patients; however, the degree of hypernasality decreased in 67% (p < 0.001). Grimacing decreased in 27% (p = 0.015). Articulation disorders remained present in 85% (p = 0.375). Intelligible speech (grade 1 or 2) was observed in 71 compared with 35% of patients preoperatively (p < 0.001). Conclusion This study showed an improved resonance and intelligibility following DHPC at the mean age of 10.5 years, however compensatory articulation errors persisted. Sequential treatments such as speech therapy play a key role in improvement of speech and may reduce remaining compensatory mechanisms following DHPC.

Oronasal fistula reconstruction using tongue flap with simultaneous iliac bone graft: a case report

  • The ultimate goal of cleft palate repair is to achieve an intact palate with the separation of the oral and nasal cavities. However, some patients develop an oronasal fistula in the secondary palate after palatoplasty. Postoperatively, a secondary palatal oronasal fistula may develop, leading to functional problems. In this study, we describe a patient with recurrent oronasal fistula and alveolar cleft with multiple failed previous reconstructions at another clinic. The oronasal fistula and alveolar cleft were repaired using a tongue flap and an iliac bone graft, respectively. The patient demonstrated excellent clinical progress with no recurrence of the oronasal fistula at the 1-year follow-up.

  • 저자 등은 하악골에 발생한 범랑아 세포종 1례에서 금속망을 이용하여 장골능으로 부터 채취된 PMCB와 피질골을 혼합하여 장착시키는 방법으로 이차적 하악골 재건술을 시행하여 하악골의 기능 및 심미적 측면에서 양호한 결과를 얻었다. 그러나 향후 금속망의 제거가 필요하며, 골수와 함께 이용된 피질골을 효과에 대한 실험적 연구가 요구된다 하겠다.
