Second Primary Malignant Neoplasms: A Clinicopathological Analysis from a Cancer Centre in India

  • Context: Patients diagnosed with a cancer have a life time risk of developing another de novo malignancy depending on various inherited, environmental and iatrogenic risk factors. Of late the detection of new primary has increased mainly due to refinement in both diagnostic and treatment modalities. Cancer victims are surviving longer and thus are more likely to develop a new metachronous malignancy. Aims: To report our observed trend of increase in prevalence of both synchronous and metachronous second malignant neoplasms among cancer victims and to review the relevant literature. Settings and Design: A hospital based retrospective collection of prospective data of patients diagnosed with second denovo malignancy. Materials and Method: The study was conducted over a 5 year period from July 2008 to June 2012. All patients diagnosed with a histologically proven second malignancy as per Warren Gate's criteria were included. Various details regarding sex, age at presentation, synchronous or metachronous, treatment and outcome were recorded. Conclusions: The occurrence of multiple primary malignancies is not rare. Awareness of the possibility alerts the clinician in evaluation of patients with a known malignancy presenting with unusual sites of metastasis. Individualizing the treatment according to the stages of the primaries will result in durable cancer control particularly in synchronous double malignancy.

Metachronous second primary malignancy in head and neck cancer patients: is five years of follow-up sufficient?

  • Objectives: The aim of this study was to determine the incidence and characteristics of second primary malignancy (SPM) in patients with head and neck squamous cell carcinoma treated at a tertiary care hospital. Materials and Methods: We retrospectively reviewed the medical records of 221 patients who underwent surgery with or without adjuvant treatment for head and neck cancer from 2000 to 2002. Data of age, sex, risk factors, sites of primary and SPM, TNM stage of primary tumor, incidence of SPM, and survival were collected from medical charts. Results: Eighteen patients developed SPM during a median follow-up of 67 months, with an overall incidence of 8.14%. In addition, 77.7% of SPMs occurred in the oral cavity, followed by 11% in the lungs. The 5-year overall survival after the diagnosis of SPM in the head or neck was 70%, compared to 30% for SPM in other body regions. Conclusion: Considering a high incidence of SPM, i.e., 8.14%, in a mean follow-up period of 67 months suggests the need for long-term follow-up. Since treatment of SPM has shown an acceptable survival rate, early detection and curative therapy should be emphasized.

두경부 이차암의 임상적 고찰 (A Clinical Analysis of Second Primary Malignancy in Head and Neck Cancer Patients)

  • Objectives: Minimal improvement in the long-term survival of head and neck cancer(HNC) patients has occurred despite a multitude of advances in the control of loco regional disease and a second primary malignancy(SPM) contribute to the continued poor prognosis for the HNC patients. This study was performed in order to identify the clinical characteristics of SPM in the HNC patients. Materials and Methods: The medical records of 354 patients of head and neck squamous cell carcinoma that were followed up after initial treatment during the period of 1987 through 1994 were reviewed. This study examines the medical records of 354 patients with squamous cell carcinoma of the head and neck, of whom 26 subsequently developed a second neoplasm. Results: The actuarial SPM rate was 7.3%, and median time to presentation for the SPM was 26.8 months. The SPM were more likely to occur in male patients who had oral cavity index tumors. Patient whose index tumor was small at diagnosis had a greater chance of developing a second tumor as did those with no cervical lymph node metastases to the neck. Initial treatment modality was not associated with an increased risk of developing a second tumor. The commonest sites for the SPM were the lung and other head and neck area. The 3-year survival for patients who developed a secondary tumor from the time of its diagnosis was 27.8%. Conclusion: The SPM in the head and neck cancer patients are not uncommon and early detection of the SPM will contribute to increase the long-term survival of HNC patients.

Exocrine pancreatic cancer as a second primary malignancy: A population-based study

  • Backgrounds/Aims: Although cancer survivors are at higher risk of developing second primary malignancies, cancer surveillance strategies for them have not yet been established. This study aimed to identify first primary cancers that had high risks of developing second primary exocrine pancreatic cancer (EPC). Methods: Data on individuals diagnosed with primary cancers between 1993 and 2017 were obtained from the Korea Central Cancer Registry. The standardized incidence ratios (SIRs) of second primary EPCs were analyzed according to the primary tumor sites and follow-up periods. Results: Among the 3,205,840 eligible individuals, 4,836 (0.15%) had second primary EPCs, which accounted for 5.8% of the total EPC patients in Korea. Between 1 and 5 years after the diagnosis of first primary cancers, SIRs of second primary EPCs were increased in patients whose first primary cancers were in the bile duct (males 2.99; females 5.03) in both sexes, and in the small intestine (3.43), gallbladder (3.21), and breast (1.26) in females. Among those who survived 5 or more years after the diagnosis of first primary cancers, SIRs of second primary EPCs were elevated in patients whose first primary cancers were in the bile duct (males 2.61; females 2.33), gallbladder (males 2.29; females 2.22), and kidney (males 1.39; females 1.73) in both sexes, and ovary (1.66) and breast (1.38) in females. Conclusions: Survivors of first primary bile duct, gallbladder, kidney, ovary, and female breast cancer should be closely monitored for the occurrence of second primary EPCs, even after 5 years of follow-up.

조기 위암과 동반된 비장 변연부 림프종 1예 (The Synchronous Occurrence of Splenic Marginal Zone Lymphoma in a Patient with Early Gastric Cancer: A Case Report)

  • 비장 변연부 림프종(Splenic marginal zone lymphoma)은 드문 종양이며, 주로 노년층에서 발견되며, 절반 이상이 10년 이상 생존할 만큼 양호한 경과를 보인다. 본원에서는 이전에 보고된 바가 없는 비장 변연부 림프종과 조기위암이 동시에 발견된 환자의 수술적 치료를 경험하였기에 보고하는 바이다. 74 세 남자환자로 5년 전 알코올성 간경화를 진단 받은 이후 추적관찰 중 시행한 위내시경 검사에서 유문부에 조기위암이 발견되었다. 복부 전산화 단층 촬영 결과, 경미한 간경화와 비장비대, 복강 내 림프절병증이 관찰되었고, 근치적 절제를 위해 위 아전절제술을 시행하였다. 수술 당시 경미한 간경화와 비장이 매우 커져 있는 것과 더불어 다수의 복부 림프절비대가 관찰되었다. 이후 조직 검사 결과에서 조기위암과 림프절에서 비장 변연부 B 세포 림프종으로 진단되었다. 환자는 회복 후 골수검사와 PET 검사 등을 시행하였으며, 조직검사와 같은 결과를 얻었다. 이후 환자는 항암치료 계획 중이다. 비장 변연부 림프종은 Schmid 등에 의해 처음 명명되었으며, 비호치킨성 림프종의 1~2%를 차지한다. 이는 진단 당시 평균 나이가 65세이고, 드문 질환이지만 완만한 경과로 긴 생존 기간을 가지므로 추가로 악성종양이 발생될 수 있다. 대부분 진단 당시 무증상이며, 심각한 혈구 감소증이 없거나, 중등도의 비장비대를 동반한 무증상의 환자는 경과를 관찰하는 것이 타당하다. 본 환자는, 조기 위암 수술 중 발견되어 비장 변연부 림프종으로 진단된 경우로, 동시에 발견된 경우는 보고된 바가 없다. 드물기는 하지만, 비장 변연부 림프종을 가진 환자의 주의 깊은 추적 관찰로 이차로 발생되는 악성 종양을 조기에 발견하는 것이 중요하다.

구강암 환자에서 발생하는 이차암의 임상적 특징 분석 (CLINICAL CHARACTERISTICS OF SECOND PRIMARY CANCER IN ORAL CANCER PATIENTS)

  • Objective: Second primary malignancy (SPM) that occur in various period and region are important factors that deteriorate long-term survival rate in patients who recovered from oral cancer. Researches such as chemoprevention are being tried to reduce occurrence of SPMs. Only if analysis of clinical features of patients who develop SPM such as period, region and factors precedes, adequate prevention and treatment of SPM is possible. But, there are few researches about clinical features of SPMs that have primary lesion in oral cavity. In this study, we analysis that occurrence rates, regions that happen, risk factors and effect to survival rates of 2nd primary malignancies in oral cancer patients. From this survey, we willing to collect basic data for prevention and early diagnosis of SPMs. Methods: The medical records of 139 patients of oral oncology clinic of National Cancer Center who had up to 2-years follow up records after surgical or radiological treatment due to squamus cell carcinoma of oral cavity were reviewed. In these patients, survey of occurrence rate of SPMs, duration, survival rate and risk factors about occurrence of SPMs such as history of smoking, body mass index, age, sex, stage of primary lesion and history of radiologic treatment were achieved. Results: There are 15 patients who developed SPM in 139 cases. The actual occurrence rate of SPM was 10.79% and SPM were more likely to occur in male patients with 11 male Vs 4 female patients. Median age of these patient is 61.47 within 32 to 74 range. The regions that develop SPM are oral cavities (2 cases), stomach (4 cases), esophagus (2 cases), lung (2 cases) and others (1 case each breast, larynx, cervix, liver and kidney). In addition, metachronous cancers were 11 cases which happened more common than 4cases of synchronous cancers. Surveys of risk factors that relate to development of SPMs, such as sex, age, history of radiologic treatment, body mass index, history of smoking and stage of primary disease were done. Among them, factor of sex is only appear statistically significant (P=0.001), but rest are not significant in statistically. Conclusion: Occurrence rates of SPMs were reported from 10% to 20% by precede study. In this study, occurrence rate of SPMs is 10.79% that is similar to results of precede research. In comparison of 5-yr survival rates of groups that develop SPMs or not, there is statistically significance between two groups. Present treatment modalities of SPMs are surgical operation, radiotherapy, chemotherapy and combination of these modalities. In choosing the treatment modality, we must consider the first treatment modality, region of primary disease, region of SPMs and general conditions of patient. Because development of SPMs have big effect on prognosis, prevention of SPMs must regard to important objective of treatments in patients of SCCa in oral cavity.

유방암 환자의 MRI에서 발견된 병변의 악성 예측을 위한 점수체계: 진단적 능력과 이차 초음파 결정에 미치는 영향 (Scoring System to Predict Malignancy for MRI-Detected Lesions in Breast Cancer Patients: Diagnostic Performance and Effect on Second-Look Ultrasonography)

  • 목적 유방암 환자의 MRI에서 발견된 추가적 병변의 악성 예측을 위한 점수체계를 설계하고자 하였다. 대상과 방법 68명 유방암 환자의 86개 MRI 발견 병변(64 양성, 22 악성)이 후향적으로 연구되었다. 스튜던트 t 검정, Fisher 정확검정, 로짓 회귀분석을 이용해 영상적 소견과 조직학적 결과의 상관관계를 분석했다. 의미 있는 악성 시사 소견을 기반으로 한 점수체계를 설계하고 그 것의 진단적 능력을 Breast Imaging-Reporting and Data System (이하 BI-RADS) category와 비교하였다. 결과 병변 크기 ≥ 8 mm (p < 0.001), 주 병소와 동일한 사분면에 위치(p = 0.005), 지연기의 고원형 조영 증강(p = 0.010), T2 등신호(p = 0.034) 혹은 저신호 강도(p = 0.024), 불규칙한 종괴 모양(p = 0.028)이 악성과 관련 있었다. 이 소견들과 의심스러운 비종괴 내부 조영 양상을 바탕으로 한 점수체계는 BI-RADS의 진단적 능력을 향상시켰고(area under the curve, 0.918 vs. 0.727), 3개의 위음성 케이스를 방지할 수 있었다. 또한, 22개의 불필요한 2차 초음파 검사(22/66, 33.3%)를 줄일 수 있었다. 결론 병변 크기, 주 병소와의 상대적 위치, 지연기의 조영 증강 양상, T2 신호강도, 종괴의 모양 및 비종괴 내부 조영 양상을 기반으로 한 점수체계는 유방암 환자의 MRI 발견 병소를 평가하는데 있어 정확도를 높여 줄 수 있다.

10년 관찰기간 중 다수의 국소 재발을 보인 설암 환자 (Locoregional Recurrence of a Tongue Cancer Patient with 10 Year Follow-up)

  • Locoregional recurrence of tongue cancer is higher than that of other sites of the oral cavity. Locoregional control has shown improvement over the past 20 years, however, a high rate of recurrence and second primary tumor occurrence is still frequently reported. Leukoplakia is a clinical term, which describes a whitish lesion of the oral cavity. Clinicopathologic features may range from hyperkeratosis to malignancy. Because of its diverse pathologic characteristics, management of this lesion for diagnosis, treatment planning, establishment of a clear surgical margin, and periodic follow-up is difficult. We report on a case of successfully treated tongue cancer which developed from leukoplakia over 10 years. Periodic follow-up strategy and surgical planning are most important to management of locoregional recurrence.

Discovery of Anticancer Activity of Amentoflavone on Esophageal Squamous Cell Carcinoma: Bioinformatics, Structure-Based Virtual Screening, and Biological Evaluation

  • Esophageal squamous cell carcinoma (ESCC) is the most common primary esophageal malignancy with poor prognosis. Here, due to the necessity for exploring potential therapies against ESCC, we obtained the gene expression data on ESCC from the TCGA and GEO databases. Venn diagram analysis was applied to identify common targets. The protein-protein interaction network was constructed by Cytoscape software, and the hub targets were extracted from the network via cytoHubba. The potential hub nodes as drug targets were found by pharmacophore-based virtual screening and molecular modeling, and the antitumor activity was evaluated through in vitro studies. A total of 364 differentially expressed genes (DEGs) in ESCC were identified. Pathway enrichment analyses suggested that most DEGs were mainly involved in the cell cycle. Three hub targets were retrieved, including CENPF, CCNA2 (cyclin A), and CCNB1 (cyclin B1), which were highly expressed in esophageal cancer and associated with prognosis. Moreover, amentoflavone, a promising drug candidate found by pharmacophore-based virtual screening, showed antiproliferative and proapoptotic effects and induced G1 in esophageal squamous carcinoma cells. Taken together, our findings suggested that amentoflavone could be a potential cell cycle inhibitor targeting cyclin B1, and is therefore expected to serve as a great therapeutic agent for treating esophageal squamous cell carcinoma.

대퇴부 지방육종 수술 후 발생한 반대측 근위 경골의 유잉육종/원시 신경 외배엽 세포종 (Double Primary Presentation of Liposarcoma and Ewing's Sarcoma: A Case Report)

  • 악성 연부조직 종양 수술 후 이시성(metachronous)으로 발생하는 악성 골졸양은 매우 드물다. 이시성 종양은 두 경부 또는 복부에 주로 발생하며, 사지에 발생하는 경우는 더욱더 드물다. 저자들은 48세 남자환자에서 근위 대퇴부 악성 지방 육종의 광범위 변연 절제술 후 발생한 근위 경골 유잉육종/원시 신경 외배엽 세포종 1예를 경험하여 문헌 고찰과 함께 보고하는 바이다. 육종 환자들에게서는 이와 같이 이차적으로 악성 종양이 발생할 확률이 높아지기 때문에 추시 기간 동안 잠복 종양(occult tumor)에 대한 검사가 필요하겠다.
