生物活性 スクリニングによる天然物資源からの 抗腫瘍活性物質 (Isolation and Structural Determination of Antitumor Substanes from Natural Products using Bio-active Screening Tests)

  • Many plants collected at Japan, China, Korea, Imdonesia and South America were applied to antitumor and / or cytotoxic screening tests against Sarcoma 180 ascites in mice and / or V-79, KB, P388 cultured cells. On the course of these screening tests, alcoholic extracts of Forsythia viridissima (Oleaceae), Eurycoma longifolia(Simaroubaceae), Rubia cordifolia and R. akane(Rubiaceae), Cissampelos pareira and Abuta concolor (Menispermaceae), Nardostachys chinensis (Valerianacese), Mansoa alliaceae (Bignoniaceae), Casearia sylvestris (Flacourtiacear), Maytenus ilicifolia (Celastraceae), Hedychium coronarium (Zingiberaceae), Croton palanostigma(Euphorbiaceae), Cocculus trilobus(Menispermaceae), Ginkgo biloba(Ginkgoaceae), Alpinia galanga and Cucculus zanthorrhiza(Zingiberaceae), Evodia rutaecarpa(Rutaceae), and Periploca sepium(Asclepiadaceae) showed significant activity and their active principles were clarified. In this paper, a few antitumor substances in above plants are introduced.

  • The in vitro screening tests against the in vitro cultivated $L_3$ of Ascaris suum (in vitro $L_3$), which were cultivated from the embryonated egg to third-stage larva on 7 days in culture(DIC) and the in vivo rat's lung-derived $L_3$ of Ascaris suum (in vivo $L_3$), which were recovered from the lungs of rat on 7 days after infection, carried out in order to compare the anthelmintic efficacy of in vitro $L_3$ and that of in vivo $L_3$ in RPMI medium 1640 with 5% bovine calf serum. And also a screening test of efficacy against adult worms of Trichuris suis performed. The efficacies of screening tests were as follows : 1. The screening efficacies of abamectin and ivermectin against the in vitro $L_3$ were all 100% at the 10ppm concentration in RPMI medium 1640 on 5 DIC. 2. The screening efficacies of abamectin and ivermectin against the in vivo $L_3$ were all 100% at the 20ppm on 5 DIC or at 40ppm on 3 DIC. 3. The screening efficacies of abamectin and ivermectin against the adult worms of Trichuris suis were all 100% at 20ppm on 4 DIC. And therefore, the in vitro cultivated $L_3$ of Ascaris suum were used in the screening test as well as the in vivo rat's lung-derived $L_3$ of Ascaris suum. And also the adult worms such as Trichuris suis and filaroids which is small size and difficult to cultivate to vitro, were used in the screening test in vitro.

Screening for Colorectal Neoplasias with Fecal Occult Blood Tests: False-positive Impact of Non-Dietary Restriction

  • Objective: Screening for colorectal cancer using guaiac-based fecal occult blood tests (gFOBT) is well established in Western populations, but is hampered by poor patient compliance due to the imposed dietary restrictions. Fecal immunochemical tests (FIT) do not require dietary restriction, but are more expensive than gFOBT and therefore restrict its use in developing countries in Asia. However, Asian diets being low in meat content may not require diet restriction for gFOBT to achieve equivalent results. The objective of this study was to evaluate and compare the validity and suitability of gFOBT and FIT or a combination of the two in screening for colorectal neoplasias without prior dietary restriction in an Asian population. Methods: Patients referred to the Endoscopic Unit for colonoscopy were recruited for the study. Stool samples were collected prior to bowel preparation, and tested for occult blood with both gFOBT and FIT. Dietary restriction was not imposed. To assess the validity of either tests or in combination to detect a neoplasm or cancer in the colon, their false positive rates, their sensitivity (true positive rate) and the specificity (true negative rate) were analyzed and compared. Results: One hundred and three patients were analysed. The sensitivity for picking up any neoplasia was 53% for FIT, 40% for gFOBT and 23.3% for the combination. The sensitivities for picking up only carcinoma were 77.8%, 66.7% and 55.5%, respectively. The specificity for excluding any neoplasia was 91.7% for FIT, 74% for gFOBT and 94.5% for a combination, whereas for excluding only carcinomas they were 84%, 73.4% and 93.6%. Of the 69 with normal colonoscopic findings, FOBT was positive in 4.3%, 23.2 %and 2.9% for FIT, gFOBT, or combination of tests respectively. Conclusion: FIT is the recommended method if we are to dispense with dietary restriction in our patients because of its relatively low-false positivity and better sensitivity and specificity rates.

응급실에서 시행한 소변 독성 검사의 유용성: 마약을 중심으로 (Availability of urine toxicologic screening tests in the emergency department: focused on illegal drugs)

  • Purpose: In Korea, it is predicted that the proportion of drug abusers among patients visiting the emergency room will soon increase. Several emergency medical institutions in Korea are conducting field urine screening tests for poisoning. In this study, we investigated the characteristics and usefulness of urine toxicology screening tests. Methods: The medical records of patients with positive results for tetrahydrocannabinol and methamphetamine from urine toxicology screening tests at a tertiary university hospital from August 2016 to August 2019 were reviewed retrospectively. The subjects were classified into positive and false-positive groups, and their clinical characteristics were compared and analyzed. Results: Of the 2,026 patients surveyed, 823 patients (40.6%) tested positive for one or more drugs. Among them, 12 cases (0.6%) were positive for methamphetamine and 40 cases (2.0%) were positive for tetrahydrocannabinol. The positive and the false-positive rates for methamphetamine were 66.7% and 33.3%, respectively. The positive and the false-positive rates for tetrahydrocannabinol were 2.5% and 97.5%, respectively. Conclusion: Methamphetamine showed a relatively low false-positive rate in our study. Therefore, this test seemed to assist in diagnosing methamphetamine poisoning when considered together with the present illness and physical examination results. On the other hand, the high false-positive rate for tetrahydrocannabinol tests indicates that this test was unlikely to assist in diagnosing tetrahydrocannabinol poisoning. However, considering the growing trend of illegal drug abusers in Korea, it may still be useful as a diagnostic tool for identifying drug users.

Yeungnam University type drive-through (YU-Thru) coronavirus disease 2019 (COVID-19) screening system: a rapid and safe screening system

  • Active and prompt scale-up screening tests are essential to efficiently control the coronavirus disease 2019 (COVID-19) outbreak. The goal of this work was to identify shortcomings in the conventional screening system (CSS) implemented in the beginning of the outbreak. To overcome these shortcomings, we then introduced a novel, independently developed system called the Yeungnam University type drive-through (YU-Thru), and distributed it nationwide in Korea. This system is similar to the drive-throughs utilized by fast food restaurants. YU-Thru system has shortened the time taken to test a single person to 2-4 minutes, by completely eliminating the time required to clean and ventilate the specimen collection room. This time requirement was a major drawback of the CSS. YU-Thru system also reduced the risk of subjects and medical staff infecting one another by using a separate and closed examination system. On average, 50 to 60 tests were conducted per day when using the CSS, while now up to 350 tests per day are conducted with the YU-Thru system. We believe that the YU-Thru system has made an important contribution to the rapid detection of COVID-19 in Daegu, South Korea. Here, we will describe the YU-Thru system in detail so that other countries experiencing COVID-19 outbreaks can take advantage of this system.

Attitudes Towards Colorectal Cancer (CRC) and CRC Screening Tests among Elderly Malay Patients

  • Background: Colorectal cancer (CRC) is the third most common malignancy in Malaysia, where data are limited regarding knowledge and barriers in regard to CRC and screening tests. The aim of the study was to assess these parameters among Malaysians. Materials and Methods: The questionnaires were distributed in the Umra Private Hospital in Selangor. The questionnaire had four parts and covered social-demographic questions, respondent knowledge about CRC and colorectal tests, attitude towards CRC and respondentaction regarding CRC. More than half of Malay participants (total n=187) were female (57.2%) and 36.9% of them were working as professionals. Results: The majority of the participants (93.6%) never had a CRC screening test. The study found that only 10.2% of the study participants did not consider that their chances of getting CRC were high. A high percentage of the participants (43.3%) believed that they would have good chance of survival if the cancer would be found early. About one third of the respondents did not want to do screening because of fear of cancer, and concerns of embarrassment during the procedure adversely affected attitude to CRC screening as well. Age, gender, income, family history of CRC, vegetable intake and physical activity were found to be significant determinants of knowledge on CRC. Conclusions: The major barriers identified towards CRC screening identified in our study were fear of pain and embarrassment. The findings have implications for understanding of similarities and differences in attitude to CRC amongst elderly patients in other cultural/geographic regions.

경기일부지역의 적혈구 항체선별검사의 실태조사 (Investigation of Red Cell Antiobody Screening Tests Gyeonggi Areas)

  • 적혈구 동종면역은 수혈자와 공여자간의 적혈구 항원의 차이이다. 수혈을 위해서는 혈구형과 혈청형이 일치하지 않으면 적혈구 항체 선별 검사가 필요하며 불일치의 원인을 해명하는 것이 필수적이다. 적혈구 항체 선별검사는 임상적으로 유의한 항체를 검출하고, 신속 정확하며, 신뢰할 수 있는 방법으로 수혈에 앞서 우선적으로 하는 것을 권장한다. 본 연구자들은 다빈도로 검출된 E, D, M, E+c, C+e 항체에 대한 선별검사를 보고하였다. 따라서 이러한 결과로 항-D, 항-E의 항체가 신생아 용혈성 질환, 지연형 용혈성 수혈부작용 반응의 위험요인으로 인식하고자 한다. 또한 혈액안전관리를 적용하기 위해 적합한 항체선별검사가 요구되고, 수혈 위험요인의 선별에 있어 더 나은 효율성을 제공할 것이며 향후에 각 국가에 따라 실질적인 검출빈도를 찾는 연구가 필요할 것으로 사료된다.

Improving Participation in Colorectal Cancer Screening: a Randomised Controlled Trial of Sequential Offers of Faecal then Blood Based Non-Invasive Tests

  • Background: Poor participation rates are often observed in colorectal cancer (CRC) screening programs utilising faecal occult blood tests. This may be from dislike of faecal sampling, or having benign bleeding conditions that can interfere with test results. These barriers may be circumvented by offering a blood-based DNA test for screening. The aim was to determine if program participation could be increased by offering a blood test following faecal immunochemical test (FIT) non-participation. Materials and Methods: People were invited into a CRC screening study through their General Practice and randomised into control or intervention (n=600/group). Both groups were mailed a FIT (matching conventional screening programs). Participation was defined as FIT completion within 12wk. Intervention group non-participants were offered a screening blood test (methylated BCAT1/IKZF1). Overall participation was compared between the groups. Results: After 12wk, FIT participation was 82% and 81% in the control and intervention groups. In the intervention 96 FIT nonparticipants were offered the blood test - 22 completed this test and 19 completed the FIT instead. Total screening in the intervention group was greater than the control (88% vs 82%, p<0.01). Of 12 invitees who indicated that FIT was inappropriate for them (mainly due to bleeding conditions), 10 completed the blood test (83%). Conclusions: Offering a blood test to FIT non-participants increased overall screening participation compared to a conventional FIT program. Blood test participation was particularly high in invitees who considered FIT to be inappropriate for them. A blood test may be a useful adjunct test within a FIT program.

부산시 일부지역 중년남녀의 암 조기검진 수검 실태 조사 (A survey on cancer screening among the middle-aged in Pusan area)

  • Objectives: Cancer is the most frequent cause of death in Korea. Cancer screenings can save lives through early detection and their effect can be enhanced by regular repeat adherence rather than one-time screening. The aim of this study was to investigate major cancer screening rates and the reasons for not having screening for providing the basic data required. Materials and Methods: The study sample were recruited from the parents of students in 3 different middle and high schools in Pusan. 428 participants(l93 of males, 214 of females) completed a structured self-administered questionnaire from Dec. 21 to 31, 2001 and the response rate was 73.8%. Data were analyzed using descriptive statistics with SPSS Win 10.0. Results: The cancer screening rates of the subjects(male and female respectively) who have had one or more in their life-time were about 36.3% and 34.6% in gastroendoscopy for stomach cancer, about 11.1 % and 8.5% in stool hemocult test and colonoscopy for colon cancer, 13.5% and 9.3% in prostate-antigen test and rectal digit exam for prostate cancer, 36.4% in mammograpy for breast cancer, and 59.3% in Pap smear test for cervical cancer. And the higher proportions of having regular screening were 36.0% in Pap smear test for cervical cancer and 11.7% in mammograpy for breast cancer. The reasons related to not having screening tests were found that ‘seem to be healthy’ was 44.8%∼58.9% and the most common reason and the following was ‘not having opportunity for check-up’. The most common reason related to not having regular screening tests were ‘for the finding of previous check-up was normal’.

Factors Associated with Underscreening for Cervical Cancer among Women in Canada

  • Background: Cervical cancer is the second most common cancer among women worldwide. Failure to prevent cervical cancer is partly due to non-participation in regular screening. It is important to plan and develop screening programs directed towards underscreened women. In order to identify the factors associated with underscreening for cervical cancer among women, this study examined Pap test participation and factors associated with not having a time-appropriate (within 3 years) Pap test among a representative sample of women in Ontario, Canada using Canadian Community Health Survey (CCHS) data. Materials and Methods: Univariate analyses, cross-tabulations, and logistic regression modeling were conducted using cross-sectional data from the 2007-2008 CCHS. Analyses were restricted to 13,549 sexually active women aged 18-69 years old living in Ontario, with no history of hysterectomy. Results: Almost 17% of women reported they had not had a time-appropriate Pap test. Not having a time-appropriate Pap test was associated with being 40-69 years old, single, having low education and income, not having a regular doctor, being of Asian (Chinese, South Asian, other Asian) cultural background, less than excellent health, and being a recent immigrant. Conclusions: Results indicate that disparities still exist in terms of who is participating in cervical cancer screening. It is crucial to develop and implement cervical cancer screening programs that not only target the general population, but also those who are less likely to obtain a Pap tests.
