Screening in the Era of Economic Crisis: Misperceptions and Misuse from a Longitudinal Study on Greek Women Undergoing Benign Vacuum-assisted Breast Biopsy

  • Background: To evaluate knowledge about screening tests and tests without proven screening value in a Greek Breast Unit population undergoing benign vacuum-assisted breast biopsy (VABB). Materials and Methods: This study included 81 patients. Three knowledge-oriented items (recommended or not, screening frequency, age of onset) were assessed. Regarding screening tests two levels of knowledge were evaluated: i). crude knowledge (CK), i.e. knowledge that the test is recommended and ii). advanced knowledge (AK), i.e. correct response to all three knowledge-oriented items. Solely CK was evaluated for tests without proven screening value. Risk factors for lack of knowledge were assessed with multivariate logistic regression. A second questionnaire was administered 18 months after VABB to assess its impact on the performance of tests. Results: Concerning screening tests considerable lack of AK was noted (mammogram, 60.5%; Pap smear, 59.3%; fecal occult blood testing, 93.8%; sigmoidoscopy, 95.1%). Similarly lack of CK was documented regarding tests without proven screening value (breast self-examination, 92.6%; breast MRI, 60.5%; abdominal ultrasound, 71.6%; barium meal, 48.1%; urine analysis, 90.1%; chest X-Ray, 69.1%; electrocardiogram, 74.1%; cardiac ultrasound, 75.3%). Risk factors for lack of AK were: place of residence (mammogram), age (Pap smear), personal income (sigmoidoscopy); risk factors for lack of CK included number of offspring (breast MRI, chest X-Ray), BMI (abdominal ultrasound), marital status (urine analysis), current smoking status (electrocardiogram). VABB's only effect was improvement in mammogram rates. Conclusions: A considerable lack of knowledge concerning screening tests and misperceptions regarding those without proven value was documented.

일개 중소 도시의 종합병원에서 검사자에 따른 신생아청각선별검사의 비교 (Comparison of Newborn Hearing Screening Tests Depending on the Examiners in a General Hospital in a City)

  • Objectives: To review the status of newborn hearing screening (NHS) and to investigate the effect of the examiners on NHS tests to help the quality control of NHS at a general hospital in a city. Methods: The charts of newborns from January 2015 to March 2016 and from August 2016 to October 2017 were retrospectively reviewed. We compared the results of tests performed by several examiners(group 1) with those performed by one audiologist (group 2) using the same automated auditory brainstem response test. Results: The screening rate and referral rate were not significantly different between group 1 and group 2. The confirmatory test rate was higher in the group 2, but it was not significant. In group 1, the number of tests performed 3 or more times in one ear at one time was significantly higher. The number of tests performed in only one ear at one time was higher in group 2. The screening rate within one month after birth was 64.21%, referral rate was 7.32%, confirmatory test rate within 3 months after birth was 21.74%, and the prevalence of hearing loss was 1.46%. Conclusions: There was no significant difference of results depending on the examiners. In order to make proper screening test, it is necessary to periodically educate the examiner and to instruct the examiner by the supervisor doctors.

  • Introduction: The aim of the study was to determine the breast, cervical, and colorectal cancer screening rates and the influencing factors in a group of Turkish females. Methods: This descriptive study was conducted in a School of Nursing. The study sample consisted of 603 females who were the mothers/neighbors or relatives of the nursing students. Data collection forms were developed by the investigators after the relevant literature was screened and were used to collect the data. Results: Of the women aged 30 and over, 32.8% had undergone a pap smear test at least once in their life. Of those aged 50 and over, 48.2% had undergone mammography at least once and FOBT had been performed in 12% of these women in their life. Having heard of the screening tests before, knowing why they are done, and having information on the national cancer screening program were important factors influencing the rates of women having these tests done. Discussion: The results of this study show that the rates of women participating in national cervical, breast, and colorectal cancer screening programs are not at the desired levels. It is suggested that written and visual campaigns to promote the service should be used to educate a larger population, thus increasing the participation rates for cancer screening programs.

Noninvasive Testing for Colorectal Cancer Screening: Where Are We Now?

  • Colorectal cancer (CRC) is one of the most prevalent cancers and is the leading cause of cancer-related mortality worldwide. Based on the current screening guidelines by the American Cancer Society and Korean multi-society expert committee, CRC screening is recommended in asymptomatic adults starting at the age of 45 years. Fecal immunochemical test-based screening programs reduce the development of CRC and related mortality in the general population. However, this most popular CRC screening strategy demonstrates a crucial limitation due to modest diagnostic accuracy. Colonoscopy may be considered as an alternative primary method for CRC screening; however, its implementation can still be challenging due to concerns regarding invasiveness, low adherence, cost-effectiveness, and quality assurance. To overcome the limitations of current screening tests, innovative noninvasive tests for CRC screening have been developed with advances in molecular biology, genetics, epigenetics, and microbiomics for detecting CRC, which may enhance the approach to CRC screening and diagnosis in clinical practice in the near future. This review explores the emerging screening methods and discusses their potential for integration into current practice.

  • To an increasing extent, case-control studies are being undertaken to determine if use of early detection procedures is associated with reduced mortality from cancer. The authors recommend that in such studies the analysis focus on screening activity in cases that occurs during an interval prior to diagnosis in which the cancer is believed to be detectable and still curable and to a corresponding time period in controls. This approach places a heavy burden on the investigator to estimate accurately the period during which the tumor ought to be detectable using the test in question and to sort out reliably tests done in response to signs or symptoms of the cancer from screening tests per se. Nonetheless, the authors feel that it offers the greatest ability to discern a true benefit of screening, while minimizing the numerous potential biases that can be present in this type of study.

Opportunistic Screening for Cervical Cancer in a Tertiary Hospital in Karnataka, India

  • The incidence and mortality of cervical cancer remains high in India even after sixty years of introduction of the Pap smear (cervical cytology) which is an effective means of identifying preinvasive lesions of carcinoma cervix. The morbidity and mortality due to cervical cancer has come down drastically in countries with well established screening programmes at national level. This study aims at screening women for cervical cancer opportunistically during their visit to hospital and to study various types of neoplastic and non-neoplastic lesions of the cervix by cervical smear study (Pap smear study). In the present study, a total of 350 cervical smears were studied. The age of patients ranged from 19 years to 80 years with mean age being 37.5 years. Out of 350 cases, the diagnosis of neoplasia was given in 43 cases and 258 cases were diagnosed as inflammatory smears. Forty-cases were normal and 9 cases were inadequate to evaluate. Forty-three patients who were found to have neoplastic lesions on cytology were referred for further investigations like colposcopy and biopsy to confirm the diagnosis and avail proper treatment. Limitation of the present study was small sample size as all female patients aged between 20 and 60 years visiting hospital were not included in the screening, other screening tests like VIA (visual inspection with acetic acid test) and HPV DNA (human papilloma virus) tests were not done. Until the time centrally organised screening programmes for cervical cancer are established in India, arrangements should be made for hospital based opportunistic screening for all women attending hospital. The cost effectiveness of different screening tests for cervical cancer should be evaluated.

종합 건강검진 과정에서 나타난 일부 중년기 여성의 건강문제 (Health Problem of the Middle-Aged Women)

  • This study was carried out to offer the basic data for more qualitative lives of the middle-aged women through their health maintenance and control. So, the researcher reviewed and analyzed the health problems of the middle-aged women, the differences of main symptoms in each subject, positive ratcs in screening tests, the difference of screening tests with age and the correlation of obesity and blood pressure with screening tests. All subjects were 218 women(40-59years) who had a check-up at a comprehensive health check-up center in K University Hospital in Seoul from July 1, 1998 to August 31. 1998. Questionaires were developed to get subjects, general characteristics. main symptoms, medical examination and diagnosed disease by the researcher referring through the questionaires of comprehensive health check-up center and the medical recorders were used as a tool of study. The researcher analyzed the data with SPSS PC+ ; the distribution of the subjects, general characteristics and main symptoms with percentage : the difference between main symptoms and screening tests with X$^2$-test, ANOVA. The results are as follows. 1) Among the age distribution, most were 40-45 years old and unemployed. 2) The most common complaints were indigestion in digestive system, cough in respiratory system, heart beat in cardiovascular system, general sweat in endocrine system. edema in urinary system, easy bruise in hematologic system, backache in musculoskeletal system, headache in psychoneurologic system, lochia in gynecology, itch in dermatology system, eye ball pain in visual system and tinnitus in auditory system. 3) In main symptoms according to age, the 40-45 age group had a higher rate of complaints in digestive system and gynecology than other age group ; the 46-49 age group in endocrine system. 4) The main symptoms according to diagnosis had no difference statistically. 5) The screening tests which showed high rates of abnormality were mammography(the highest), abdominal ultrasonography, upper gastrointestinal series/gastrofiberscopy, PFT. In all age group the highest rate of abnormality was seen in mammography. 6) In screening tests according to age. the 46-49 age group showed higher positive rates of pap smear, blood sugar test, urine test than any other age group ; the 50-55 age group showerd higher positive rates of obesity, BP, chest X-ray, abdominal ultrasonography, ESR, SGOT, CRP and urine micro. 7) In correlation of obesity with screening test, the obesity group showed higher positive rates of blood pressure, chest X-ray, abdominal ultrasonography, HCT and CRP than the normal weight group. 8) In correlation of blood pressure with screening test, the hypertension group showed higher positive rate of obesity, chest X-ray, abdominal ultrasonography, pap smear and blood sugar than the normal blood pressure group. In conculusion, the middle-aged women have various physical symptoms and affected by age. The obesity and BP have an important effect on the health of the middle aged women. Therefore, this study is considered significant as data for qualitative lives of the middle-aged women playing an important part of family health care by catching of their health problem complaints for prevention of disease and promotion of health.

한국에서의 단풍당뇨증, 호모시스틴뇨증, 갈락토스혈증, 선천성 부신과형성증에 대한 신생아 선별검사의 경제성 분석 (A Cost-Benefit Analysis of Neonatal Screening Tests for Maple Syrup Urine Disease, Homocystinuria, Galactosemia, and Congenital Adrenal Hyperplasia)

  • 목 적: 신생아 선별검사는 선천성 대사이상 질환을 조기에 진단하여 심각한 발달 지연이나 급성 질환, 심지어는 사망을 예방할 수 있어 점차 확산되고 있다. 본 연구에서는 우리나라에서 신생아 선별검사에 들어가는 비용과 시행하지 않을 때의 비용을 서로 비교하여 경제성 여부를 알아보고자 하였다. 대상 및 방법: 2005년 1월부터 2007년 12월까지 국내에서 단풍당뇨증, 호모시스틴뇨증, 갈락토스혈증 및 선천성 부신과형성증에 대한 신생아 선별검사를 받은 1,259,220명의 신생아를 대상으로 신생아 선별검사를 시행한 경우와 시행하지 않은 경우에 들어가는 비용을 비교하여 단풍당뇨증, 호모시스틴뇨증, 갈락토스혈증 및 선천성 부신과형성증에 대한 신생아 선별검사의 경제성 여부를 알아보고자 하였다. 결 과: 각 질환별로 신생아 선별 검사를 시행할 때와 시행하지 않을 때의 비용의 비를 분석해보면 단풍당뇨증이 1:0.5, 호모시스틴뇨증이 1:0.6로 낮은 유병률로 인해 검사를 시행하는 것이 손해였으며, 갈락토스혈증이 1:4.1, 선천성 부신과형성증이 1:2.9로 이득을 보여 전체적으로는 신생아 선별검사를 시행했을 때 총 2.0배의 이득이 있었다. 결 론: 단풍당뇨증, 호모시스틴뇨증, 갈락토스혈증, 선천성 부신과형성증에 대한 신생아 선별검사는 경제적 효용성 뿐만 아니라 개인의 삶의 질 향상을 위해서도 시행해야 하며 앞으로 윌슨병 등의 다른 유전성 대사질환에 대해서도 신생아 선별검사 시행을 고려해야 할 것이다.

Neurobehavioral Tests for Occupational Screening의 타당도 평가 : 파킨슨병 환자들의 검사 성적 (Validation of Neurobehavioral Tests for Occupational Screening(NTOS) : results of patients of Parkinson's disease)

  • 이 연구는 최근 개발된 컴퓨터를 이용한 정신운동 기능 검사법인 Neurobehavioral Tests fur Occupational Screening(NTOS)의 타당도 평가를 위해 시도되었다. 일개 종합병원 신경과에 등록된 파킨슨병 환자 12명과 신경독성물질에 폭로된 적이 없는 근로자 21명을 대상으로 NTOS의 단순반응시간 검사, 2가지 선택반응시간 검사, 양 손의 수지타진검사를 실시하였다. 평균 단순반응시간은 근로자군은 245 msec, 환자군은 378 msec였으며, 2가지 선택반응시간은 근로자군은 444 msec, 환자군은 671 msec 로 나타나 T 검정에서 유의한 차이를 보였다. 주로 쓰는 손의 수지타진검사는 68.6회를 나타내었고, 파킨슨병 환자군은 51.6회, 주로 쓰지 않는 손은 각각 60.9회, 43.1회로 유의한 차이를 보였다. 환자군과 근로자군의 연령과 학력차이를 통계적으로 보정하고 두군에서 차이가 나는지를 알아보기 위해 공분산분석을 실시한 결과, 단순반응시간, 선택반응시간, 수지타진이 유의한 결과를 보였다. 이상으로 볼 때, NTOS는 정신운동기능의 장애를 발견할 수 있는 도구라고 생각된다. 그러나 선택반응 시간 검사의 실수 횟수 같이 상대적으로 민감하지 못한 지표에 대한 고려가 필요하며, 혼란변수를 연구 설계에서 조정할 수 있는 큰 규모의 연구와 정신운동기능의 잠재적 장애를 보이는 집단에 대한 연구들이 추후에 있어야 할 것이다.

  • In order to compare the diagnostic value of Modified California Mastitis Test (MCMT), Modified Whiteside Test (MWT) and Resazurin Reduction Test (RRT) using the direct microscopic leucocyte count (DMLC) as standard, a total of 739 quarter milk samples were examined. The results obtained were as follows: 1. Of the 739 samples, 24.4% had positive DMLC value (over 500,000 leucocytes per mI.), 32.6% positive MCMT reaction, 34.9% positive RRT reaction and 39.9% positive MWT reaction. 2. The identical ratings of the three mastitis screening tests with DMLC values were 60.7% (MWT), 61. 8% (MCMT) and 72.1% (RRT). 3. The mean reaction values of the predicted mastitis screening tests were $1.09{\pm}0.01$ (MWT), $1.12{\pm}0.06$ (MCMT) and $1.25{\pm}0.40$ (RRT). The efficiency ratings of them were 34.8% (MWT), 49.3% (RRT) and 55.0% (MCMT) respectively.
