자궁경부암과 유방암 선별검사 수검 및 반복수검 관련요인 조사 (Related Factors to Screening or Repeat Screening for Cervical and Breast Cancer among Women)

  • The importance of screening or repeat screening for cervical and breast cancer is well known to decrease the chance of death from cancer. Few data is avaliable regarding factors associated with participation in cervical and breast cancer screening and repeat screening examinations. This study was to investigate the associations of demographic factors, health status and cancer risk recognition, attitude to cancer screening, health behaviors, and inhibiting or facilitating factors to cervical and breast cancer screening with participation in the screening tests. Data was collected with self-administrated questionnaires from 342 women, aged 40 to 69 years. The cancer screenings were classified into have had or never groups and repeat or not repeated groups. In the case of cervical and breast cancer screening, the have had group was 90 (26.3%), and 82 (24.0%) and the repeat group was 17 (5.0%), and 13 (3.8%) respectively. According to logistic regression analysis, age (<60 vs ${\geq}60$ : OR=3.25, 95% CI=1.27-8.26), breast cancer screening (Do vs Don't : OR=14.49, 95% CI=7.46-27.78) and other person's cancer (Yes vs No : OR=4.27, 95% CI=1.01-18.05) were statistically significantly associated with participation in screening for cervical cancer. Regular exercise (Do vs Don't : OR=2.76, 95% CI=1.30-5.88) and cervical cancer screening (Do vs Don't : OR=13.70, 95% CI=7.09-26.32) were statistically significantly related to participation in screening for breast cancer.

Cost-effectiveness Analysis of Cervical Cancer Screening Strategies Based on the Papanicolaou Smear Test in Korea

  • Background: Despite the increasing number of screening examinations performed for cervical cancer utilizing the Papanicolaou smear test (Pap test), few studies have examined whether this strategy is cost-effective in Korea. Objective: This study was conducted to evaluate the cost-effectiveness of cervical cancer screening strategies incorporating the Pap test based on age at the start and end of screening as well as screening interval. Materials and Methods: We designed four alternative screening strategies based on patient age when screening was started (20 or 30 years) and discontinued (lifetime, 79 years). Each strategy was assessed at screening intervals of 1, 2, 3, or 5 years. A Markov model was developed to determine the cost-effectiveness of the 16 possible cervical cancer screening strategies, and this was evaluated from a societal perspective. The main outcome measures were average lifetime cost, incremental quality-adjusted life-years (QALYs), and incremental cost-effectiveness ratio (ICER). Results: Compared with various strategies comprising younger starting age, discontinuation age, and longer screening intervals, strategies employing annual screening for cervical cancer starting at a target age of 30 years and above were the most cost-effective, with an ICER of 21,012.98 dollars per QALY gained (with a Korean threshold of 30,000,000 KRW or US$27,272). Conclusions: We found that annual screening for cervical cancer beginning at a target age of 30 years and above is most cost-effective screening strategy. Considering the potential economic advantages, more intense screening policies for cervical cancer might be favorable among countries with high rates of cervical cancer and relatively low screening costs.

  • Many products, such as gasoline, polymer plastics, alloys, and ceramics are manufactured by mixing two or more ingredients or components. When we are to develop new mixture products, we must deal with a long list of potentially important component variables. This paper introduces some mixture screening designs, analysis tools for screening important variables, and a guideline for applying these analysis tools. The results of this paper will be helpful to engineers who work in the research and development sector of mixture product industries.

  • Errors may occur due to analysis methods and water quality during the application of the on-line particle counter In water treatment process. Errors caused by analysis methods include particle destruction by shear force due to inflow speed and tube friction, as well as interruption by screening, bubbles and contaminants. Since errors happen frequently because of these factors, it is necessary to examine and evaluate such errors during the application of a particle counter. Errors can be large due to screening and bubbles. Measurement values are effective for water analysis after filtration process. However, because of screening, only measurement values for particles above $7{\mu}m$ are valid for water with a turbidity between 3-10NTU. As particle numbers around $10{\mu}m$ increase a lot after ozone treatment, sufficient pretreatment process is necessary. Physical conditions should keep stable for inflow to decrease errors caused by shear force.

위암 선별검사 현황 (Current Status of Gastric Cancer Screening)

  • Gastric cancer is the fifth most common malignancy in the world and still remains the third and fifth leading cause of cancer death in male and female, respectively. In terms of the cancer survival, the survival of gastric cancer is low in the West with showed 5-year survival rates of 10-30%, whereas the survival rates in Asia is more than 50%. The reason of the higher survival rate from gastric cancer in Asia partially related with the availability of gastric cancer screening programs, however, the population-based screening for gastric cancer has been conducted just in Korea and Japan. Therefore, more effective method for detecting the gastric cancer is needed for countries without a population-based gastric cancer screening. Endoscopy is the most effective method for diagnosis of gastric cancer however, it requires a large infrastructure including a large number of endoscopic equipment and well-trained endoscopists. To overcome these problems, several noninvasive methods (such as serologic markers, biomarkers, cancer autoantibodies, and exhaled breath analysis) for diagnosis of gastric cancer screening are suggested. This review addresses the conventional methods and the emerging methods for gastric cancer screening.

  • This paper introduces a history and achievements of the research activities on screening of laryngeal diseases using acoustic analysis. First domestic and international research trends are introduced. Next brief introduction of the research results by the authors are mentioned. First, classification method of the laryngeal diseases using neural network is summarized. Then similar research using ARS (Automatic Response System) is mentioned. Finally, current research activities on screening of laryngeal diseases on internet is introduced.

효모 탐색을 위한 Pyrolysis Mass Spectrometry의 활용 (Application of Pyrolysis Mass Spectrometry on Yeast Screening)

  • To develop the effective microbial screening method, pyrolysis mass spectrometry (PyMS) fingerprinting was evaluated as a tool that discriminate various yeast strains. The target yeast strains were isolated from industrial wastewater. Seventeen environmental isolated yeast strains were examined by pyrolysis mass spectrometry and sequencing analysis of the large subunit rRNA gene D1/D2 region. The PyMS results were compared with those of sequencing analysis. Taxonomic correlations were observed between the PyMS data and the sequencing results. It was concluded that PyMS provides a rapid, reliable and cost-reducing method for discrimination of the yeast strains.

  • This study was aimed at preparing basic data required for establishment of a cancer screening program by examining the status of cancer screenings performed by women in a city and the factors that had an influence on cervical and breast cancers screening behavior of women. In order to find out the status of cervical and breast cancers screening and the factors having an influence on cervical and breast cancers screening behavior, 1,113 women were selected as study area and subjects by a random cluster sampling method, and the subjects were answered by questionnaires. The major results were as follows: 1. In the cases of the breast and the cervical cancers, 16.7% and 55.7% of the subjects turned out to have had one or more screenings respectively in their life-time. Also the rate of screening group of cervical and breast cancers was 16.7%, the rate of screening group of only cervical cancer was 38.9%, and the rate of non-screening group was 44.4%. 2. As to the screening pattern of cervical and breast cancer, there were significantly for age, income, occupation, married status, spouse, the factors associated with health promotion (doctor visiting, exercising), married age, number of children, and breast feeding(p〈0.05). 3. The factors associated with screening behaviors for cervical and breast cancers were age, income, occupation, married status, doctor visiting, exercising, married age, number of children, and breast feeding(p〈0.05). Based on the above-mentioned results obtained by this study, it was anticipated that this study may be playa vital role as basic data for the development and execution of cervical and breast cancers screening program for women in a community. And the analysis, done on the basis of the status of the cancer screening, of the influence factors on cervical and breast cancers screening behavior showed that for the development of a cancer screening program, the factors like age, social-economic class, married status, health promotion behavior, and reproductive characteristics should be considered.

한국여성의 Pap 도말검사 지속적 참여행위에 관한 설명모델 (Factors related to Continuous Participation in the Pap Smear Screening among Korean Women: using a Structural Model)

  • The purpose of this study was to develop an explanatory model to predict factors related to continuous participation in the Pap smear screening among Korean women. A hypothetical model was constructed on the basis of Health Belief Model and extensive review of literature on the Pap smear screening. Exogeneous variables included in this model were knowledge, perceived sensitivity, perceived severity, negative and positive emotional responses and professional support from physicians and nurses. Endogeneous variables were threat to cervical cancer, perceived benefit of the Pap smear screening, perceived barrier, and the final outcome variable was continuous participation in the Pap smear screening. The hypothetical model was tested with an empirical study. The data was collected from 623 married women whose age range was 24 - 83 using a self-reported survey questionnaire which was developed by the researcher. It's Cronbach's alpha score ranged from .6478 to .9118. Data was collected at different locations in Seoul; a university hospital, a local health center, and apartment complexes. Data analysis was done using SPSS 7.5 WIN Program for descriptive statistics and LISREL 8.12a WIN Program for covariance structural analysis. In conclusion, threat, perceived benefit, perceived barrier, positive emotion and professional support had a significant direct effect on continuous participation in the Pap smear screening among Korean women. The results of this study also showed that perceived barrier had the most significant direct effect on continuous participation in the Pap smear screening while negative emotional response had the most significant direct effect on perceived barrier. It can be suggested that decreasing perceived barrier by reducing negative emotional responses may be the most effective strategies for increasing continuous participation in the Pap smear screening among married Korean women.
