Experiences of Korean-American Women with High Risk Hereditary Breast Cancer (고위험 유전성 유방암을 지닌 한국계 미국 여성의 질병경험)

  • Purpose: This micro-ethnographic study aimed to understand coping experiences of Korean-American (K-A) women after diagnosis with breast cancer due to a hereditary gene mutation. Methods: Participatory observation and in-depth interviews were performed at one breast cancer screening center in Southern California, in 2005 with eleven first generation K-A immigrant women. All transcribed interviews and field notes were analyzed using ethnographic methodology. Results: K-A women's experience varied based on acculturation risk factors including: limited English speaking ability; disrupted family relationships, individualistic family values, or intergenerational communication barriers; lack of Korean speaking nurses; and Korean physicians' who lacked knowledge about hereditary breast cancer risk. These risk factors led to isolation, loneliness, lack of emotional and social support. In comparison to Korean homeland women in a similar medical situation, these K-A immigrants felt disconnected from the healthcare system, family support and social resources which increased their struggling and impeded coping during their survivorship journey. These women were not able to access self-support groups, nor the valuable resources of nurse navigator programs. Conclusion: Professional oncology associations for nurses and physicians have a moral obligation to support and promote knowledge of hereditary cancer risk and self-help groups for non-native speaking immigrants.

  • To enhance the efficiency of national R&D programs, it is the most important thing to establish the optimized evaluation system. This is because the distorted evaluation could be an obstacle to select the best suited research team and lead the desirable research result. In current evaluation process of national R&D program, it is the most frequently used method for eliminating an evaluator's biased view to exclude both the maximum and minimum scores. However, there are several limitations in this stereotype method. Therefore, this study analyzes some real cases to derive the problems from the stereotype method and then recommends the complementary alternatives which can enhance the reliability of the evaluation system for national R&D programs. The results acquired from this study seem to be helpful to increase the agreement of the applicants on the evaluation results and ultimately to maximize the efficiency of national R&D program. In addition, they can be applied to various field with respect to the methodology for ranking decision.

The Effect of Urban Shade Trees on the WBGT(Wet Bulb Globe Thermometer Index) (도심 녹음수의 체감온도지수(WBGT) 조절효과)

  • Focusing on WBGT(Wet Bulb Globe Thermometer Index) according to the LAI(Leaf Area Index) variation of trees, this study verifies the effects of urban shade trees on the outdoor thermal environment. As for methodology, air$.$globe temperature, air humidity and WBGT were measured under three shade trees whose LAIs were 2.1, 4.0 and 8.2 respectively at midday(12:00-13:00) of 14 sunny days from the 4th through the 29th day of September 2003. Those factors were also measured at the unshaded areas and compared with the values of shaded areas. The measured site was paved with interlocking concrete bricks. The measurements were analyzed through the ANCOV A(Analysis of Covariance) and the regression routines of SPSS11 for windows (SPSS Inc., 2001). The major findings were as follows. 1. The direct correlation between WBGT and LAI was very low. On the contrary, the WBGT showed close correlation with air$.$globe temperature and air humidity, and the LAI also showed very close correlation with globe temperature. These results tell that dominant shading effect by the tree is on the screening of direct solar radiation which lower the globe temperature and WBGT consequently. 2. While the average globe temperatures and WBGT at unshadowed area were 40.4$^{\circ}C$ and 26.2$^{\circ}C$ respectively, the former under the shade tree with LAI 2.1, 4.0 and 8.2 were 34.5$^{\circ}C$, 32.6$^{\circ}C$ and 30.2$^{\circ}C$, and the latter were 24.6$^{\circ}C$, 24$^{\circ}C$ and 23.4$^{\circ}C$ respectively. 3. The relationship between LAI(x) and WBGT(y) can be presented with the following equation: y = 24.23+1.53 $e^{-x}$+0.36x $e^{-x}$+0.46 $x^2$ $e^{-x}$ ($R^2$ =.98) =.98)

FUNDAMENTAL UNDERSTANDING OF CRACKING AND BULGING IN COKE DRUMS

  • Cracking and bulging in welded and internally lined pressure vessels that work in thermal-mechanical cycling service have been well known problems in the petrochemical, power and nuclear industries. However, published literature and industry surveys show that similar problems have been occurring during the last 50 years. A better understanding of the causes of cracking and bulging causes is needed to improve the reliability of these pressure vessels. This study attempts to add information required for increasing the knowledge and fundamental understanding required. Typical examples of this problem are the coke drums in the delayed coking units refinery process. This case was selected for experimental work, field study and results comparison. Delayed coking units are among the refinery units that have higher economical yields. To shut down these units represents a high negative economical impact in refinery operations. Also, the maintenance costs associated with repairs are commonly very high. Cracking and bulging occurrences in the coke drums, most often at the weld areas, characterize the history of the operation of delayed coking units. To design and operate more robust coke drums with fewer problems, an improved metallurgical understanding of the cracking and bulging mechanisms is required. A methodology that is based field experience revision and metallurgical analyses for the screening of the most important variables, and subsequent finite element analyses to verify hypotheses and to rank the variables according to their impact on the coke drum lives has been developed. This indicated approach provides useful information for increasing coke drum reliability. The results of this work not only order the most important variables according to their impact in the life of the vessels, but also permit estimation of the life spans of coke drums. In conclusion, the current work shows that coke drums may fail as a combination of thermal fatigue and other degradation mechanisms such as: corrosion at high and low temperatures, detrimental metallurgical transformations and plastic deformation. It was also found that FEA is a very valuable tool for understanding cracking and bulging mechanisms in these services and for ranking the design, fabrication, operation and maintenance variables that affect coke drum reliability.

Comparison of Short-Term Toxicity Tests Based on Feeding Behavior and Temperature Control by Ceriodaphnia dubia (Ceriodaphnia dubia의 먹이섭생 기작과 온도조절에 근거한 급성독성조사법의 비교)

  • Two methods, a Ceriodaphnia algal uptake suppression test (CAUST) and a new toxicity test based on temperature control (TTBTC) which are based on feeding behaviour and temperature control, respectively, were developed and compared for the adoption as the better methodology for short-term toxicity screening. As previously published by Lee et aI., (1997), the CAUST method is based on the feeding behaviour of C. dubia and requires as little as 1 hour of contact time between C. dubia neonates and toxicant. However, even though CAUST requires only 1 hour of contact time, this method still take many hours for the preparation and measurement. Before the test starts, neonate digestive tracts were cleared by feeding yeast to the daphnids, Neonates were then exposed to toxicant, followed by addition of Scenedesmus subspiatus into the bioassay vessels. Daphnids were examined under the bright-field microscope with the presence of algae (indicated by a green colored digestive tract) or the absence of algae. Uptake indicated no toxic effect, whereas, absence of uptake indicated toxic inhibition. Unlike CAUST, the newly developed method (TTBTC) is based on just temperature control for the toxicity test of C. dubia. Initially, neonates are exposed to toxicants while the temperature of water bath containing media increased to $35.5^{\circ}C$. After 1.25 hour of contact time, the number of the daphnids, either live (no toxic effect) or dead (toxic effect), is counted without the aid of any instrument. In both methods, median effective concentrations ($EC_{50}$ values) were computed based on the results over a range of dosed toxicant concentrations. It showed that TTBTC was as sensitive as the standard 48-hour acute bioassay and CAUST. TTBTC and CAUST were much more sensitive than the I-hour I.Q. test and 30-minute Microtox. This study indicates that TTBTC is an easier and more rapid toxicity test than the standard 48-hour acute bioassay and even CAUST.

Round Robin Analysis for Probabilistic Structural Integrity of Reactor Pressure Vessel under Pressurized Thermal Shock

  • Performed here is a comparative assessment study for the probabilistic fracture mechanics approach of the pressurized thermal shock of the reactor pressure vessel. A round robin consisting of one prerequisite deterministic study and five cases for probabilistic approaches is proposed, and all organizations interested are invited. The problems are solved by the participants and their results are compared to issue some recommendation of best practices and to assure an understanding of the key parameters in this type of approach, like transient description and frequency, material properties, defect type and distribution, fracture mechanics methodology etc., which will be useful in the justification through a probabilistic approach for the case of a plant over-passing the screening criteria. Six participants from 3 organizations responded to the problem and their results are compiled and analyzed in this study.

Practice and Barriers of Mammography among Malaysian Women in the General Population

  • Objective: The objective of this study was to determine the practice and barriers of mammography and associated factors among Malaysian women in the general population. Methodology: A cross-sectional study was conducted among 200 women in Shah Alam, Selangor; Malaysia. The questionnaire contained 27 questions and was comprised of two sections; socio-demographic characteristics and practices, knowledge and barriers of mammography. All the data were analyzed using the Statistical Package for the Social Sciences (SPSS) 13.0. Results: Of the 200 Malaysian women who participated in this study, the majority were under the age of 50 years (65.5%), Malay (86%), and married (94.5%). Regarding any family history of cancer in general, the majority of the participants had none (78%). However, some did report a close relative with breast cancer (16.5%). While the majority of them knew about mammography (68%), 15% had had a mammogram once in their life and only 2% had the procedure every two or three years. Univariate analysis showed that age, family history of cancer, family history of breast cancer, regular supplement intake, regular medical check-up and knowledge about mammogram were significantly associated with mammogram practice among the general population (p=0.007, p=0.043, P=0.015, p=0.01, p=0.001, p<0.001; respectively). Multivariate analysis using multiple linear regression test showed that age, regular medical check-up and knowledge about mammography testing were statistically associated with the practice of mammography among the general population in Malaysia (p=0.035, p=0.015 and p<0.001; respectively). Lack of time, lack of knowledge, not knowing where to go for the test and a fear of the test result were the most important barriers (42.5%, 32%, 21%, 20%; respectively). Conclusion: The practice of mammogram screening is low among Malaysian women.

Improvement of in vitro Sun Protection Factor Measurement (In vitro SPF 측정법 개선에 관한 연구)

  • The major advantage of the in vitro test is that it is a rapid, objective and cost-effective screening methodology. In vitro tests can provide a formulation tool to identify new fillers that are optimized by combinations of old ones and they can be used to pre-screen protective formulas prior to in vivo testing in humans. Therefore, the accuracy of in vitro SPF measurement is very important. In this study, improvement of application method of samples was tried to improve the accuracy of in vitro SPF measurement. The outer part of Transpore$\^$(R)/ tape was used to apply samples as the substrates and the standard drying time was set at 15 min. The new method, topical applications at light scan areas, results in more accurate and reliable results. This result suggests that more accurate prediction system can be established for in vivo SPF with in vivo SPF measurement.

The Analysis of Screen Composition Method in 19th Presidential Candidate TV Debates (제19대 대통령후보 TV토론회 화면구성방법 분석)

  • This study is based on the screen composition(shot size, split screen, candidate placement, lighting, stage) that fits the discussion process (round table, sitting, standing method) adopted from the 19th presidential candidates' TV joint discussion to analyze the adequacy and limitations of the methodology(design, candidate background, etc.). In addition, the progress and screen composition of the 45th US Presidential Candidates Debate in 2016 and the 25th Presidential Candidates of France TV Joint Debate in 2017 have been analyzed to compare and contrast the methods used in Korea. Through this, we apply the screening method to the 20th Presidential Candidate TV Joint Debate, which will be held in 2022, to fit the fixed discussion process that can fully identify candidates' qualities, policies, and vision.

Rapid Bioassay Technique Based on Temperature Control of Ceriodaphnia dubia (Ceriodaphnia dubia의 온도조절에 근거한 단기급성독성 조사법)

  • A method for rapid acute toxicity test based on temperature control of Ceriodaphnia dubia has been developed and evaluated. A new toxicity test based on temperature control (TTBTC) which are based on temperature control, was developed and compared for the adsorption of the better methodology for short-term toxicity screening. Initially, daphnid larval are exposed to toxicants and at the same time the temperature of the water bath containing media is increased to high temperature $(35.5^{\circ}C).$ After 1.25 hrs of contact time, the number of the daphnids, either living (no toxic effect) or dead (toxic effect), is counted by the naked eyes. Effect of exposure time on test sensitivity was not significantly different between 1 to 1.5 hr. Comparison of the rapid 1.25 hr acute toxicity test developed in this study and the standard 48 hr acute toxicity test using heavy metals, cyanide and pentachlorophenol indicated that the 1.25 hour test provides an acceptable level of sensitivity in toxicity test for C. dubia.
