Graft Considerations for Successful Anterior Cruciate Ligament Reconstruction (성공적인 전방십자인대 재건술을 위한 적절한 이식건의 선택)

  • Several factors need to be considered for a successful anterior cruciate ligament (ACL) reconstruction, such as preoperative planning, operation technique, and postoperative rehabilitation. Graft choice, fixation, preparation method, maturation, incorporation to host bone, and graft tension should also be considered to achieve a good outcome after an ACL reconstruction. Factors to consider when selecting a graft are the graft strength, graft fixation, fixation site healing, and donor site morbidity, as well as the effects of initial strength, size, surface area, and origin of the graft on its potential for weakening during healing. There are two types of graft for an ACL reconstruction, autograft or allograft. Several autografts have been introduced, including the bone-patellar tendon-bone, hamstring tendon, and quadriceps tendon-bone. On the other hand, each has its advantages and disadvantages. The recent increased use of allografts for an ACL reconstruction is the lack of donor site morbidity, decreased surgical time, diminished postoperative pain, and good availability of source. Despite this, there are no reports suggesting that an allograft may have a better long-term outcome than an autograft. Allografts have inherent disadvantages, including a longer and less complete course of incorporation, remodeling, biomechanically inferiority to autograft, the potential risk of an immunogenic reaction and disease transmission. Higher long-term failure rates and poorer graft maturation scores were reported for allografts compared to autografts. An autograft in an ACL reconstruction should remain the gold standard, although the allograft is a reasonable alternative. If adequate length and diameter of autograft can be obtained for an ACL reconstruction, an autograft with adequate graft fixation and postoperative rehabilitation should be chosen instead of an allograft to achieve better results.

10-year trajectories of cognitive functions among older adults: Focus on gender difference and spousal loss (70대 고령자의 10년간의 인지기능수준 변화의 유형화: 성별 및 배우자 상실경험을 중심으로)

  • The purpose of this research is to investigates 10-year trajectories of cognitive functions among older adults in their 70s to understand changes in cognitive functions as a continuum until very late life. This study also examines differences in trajectories of cognitive functions by gender and by changes in marital status, especially widowhood. Among participants of the Korean Longitudinal Study of Ageing(KLoSA), the sample of this study includes 800 older adults in their 70s during the first study wave (2006) and those who reported their cognitive functions for six consecutive study waves (2006, 2008, 2010, 2012, 2014, and 2016). The analyses were conducted in two steps. First, we conducted Latent Class Growth Analyses(LCGA) to investigated heterogeneous trajectories of cognitive functions in 10 years. Then, we performed multinomial logistic regression. Three heterogeneous trajectories of cognitive functions were identified. One group of 48.7% of older adults showed high cognitive function at baseline and maintained it over 10 years. Second group of 14.7% of older adults reported low cognitive function scores at baseline and showed continuous decline over time. Third group of 36.6% were showed mid-level cognitive functions and maintained their functions over time. We also found significant gender differences but not significant differences in marital status when we consider both in our model; however, the we found significant differences in changes in marital status when we did not consider gender in the model. The results suggest that the importance of considering dynamics of gender and changes in marital status to understand changes in cognitive functions in later life.

Effect of incorporation of soy protein isolate and inulin on quality characteristics and shelf-life of low-fat duck meat sausages

  • Objective: Low fat duck meat sausages were prepared by replacing the fat in the formulations with soy protein isolate (SPI) and inulin to find the best formulation having superior shelf-life without affecting its quality attributes. Methods: Four sausage mix formulations were prepared viz.control (0% SPI and inulin), T1 (2.5% inulin), T2 (2.5% SPI), and T3 (2.5% SPI+2.5% inulin) replacing duck fat as per the recipe. Five batches of duck meat sausages of each formulation were prepared, and the final products were evaluated for physico-chemical, organoleptic, and microbiological qualities. Results: The % moisture and crude protein content of the sausages revealed an increasing trend (p<0.01) from control to the treated formulations, while the % total ash contents were found to be non-significant (p>0.05). On the contrary, the per cent ether extract decreased significantly (p<0.01) from the control to the treated groups. In terms of calorie value, control samples exhibited the highest values with a significant (p<0.01) regression from control to treated formulation, respectively. The colour profile study (L, a*, b*) of the formulations were found to be non-significant. Texture profile study in terms of springiness, cohesiveness, chewiness, and resilience revealed no significant difference in all the treatment groups except the hardness scores, which revealed a significantly (p<0.01) increasing trend from control to the treated formulations. The total viable count showed a significant decrease in the treated groups. However, there was a significant increase in the bacterial load during the storage till day 15th. The total viable psychrophilic bacterial count showed a significant (p<0.01) increase in bacterial load from day 5th to 15th day of storage. Colititre counts were negative for all the formulations until the 15th day of storage. Conclusion: The present study results may conclude that duck meat sausages could be prepared satisfactorily by replacing duck fat with SPI and inulin at the rate of 2.5% of each with superior quality attributes.

Changes in Microbial Properties and Sensory Characteristics during the Storage of Kimchi in Containers with Native Plant Extracts (자생식물 추출물을 첨가하여 개발된 저장용기의 김치 저장 중 미생물과 관능적 특성의 변화)

  • This study was conducted to investigate the quality characteristics of Kimchi during fermentation and storage according to containers with native plant extract. The containers used in the experiment were antimicrobial polypropylene containers(KAPP) developed with the addition of native plant extracts, and it was tested by comparing the microbial changes and sensory characteristics of the existing commercial containers, such as polypropylene containers(KPP), stainless steel containers(KST), and porcelain containers(KPC). Change in total microbial cell were similar for each container. Coliform maintained the lowest level from 15 days after storage to 50 days. Leuconostoc spp. and Lactobacillus spp. showed a rapid increase in all four storage containers until the 15th day of storage and then decreased. The KAPP container maintained its highest level. The sensory evaluation was carried out on Kimchi optimal condition(storage 40 days). The sensory scores of KAPP were generally higher than those of other experimental samples in characteristics of appearance, odor, taste and overall preference. As a result, KAPP container has an excellent antibacterial effect as compared with the three commercially available storage containers, is effective for fermentation of lactic acid.

Validity and reliability of Korean version of quality of life questionnaire related with music perception and engagement of the elderly (난청노인의 한국어판 음악지각과 참여와 관련된 삶의 질 설문지의 타당도와 신뢰도)

  • The purpose of this study is to develop the Korean version of Music-Related Quality of Life (K-MRQoL) for the elderly. The K-MRQoL consisted of Musical Ability, Attitude, Activity Frequency (PART 1) and Musical Ability, Attitude, Activity Important (PART 2). Each subcategory consists of Music Perception with 11 items and Music Engagement with 7 items. The validity and reliability of K-MRQoL were measured with Pearson's and Cronbach's alpha correlation coefficients and Independent t-test in total 30 elderly with normal hearing and 30 elderly with hearing loss from local welfare Centers and nursing homes. The correlation coefficients between total scores and PART 1 and PART 2 ranged from .701 to .948 and from .598 to .926, respectively. The internal consistency between total and Part 1 and Part 2 ranged from .846 to .931 and from .838 to .918, respectively. The test-retest correlations were .979, .970, and 979 for total, PART 1, and PART 2, respectively. The correlation between K-MRQoL and Quality of Communication Life Scale was .449. There were significant differences in total, PART 1, and PART 2 between the elderly with normal hearing and hearing loss. This indicates that the K-MRQoL can be used as a useful clinical tool to evaluate Music-related Quality of Life in the elderly with normal hearing or hearing loss.

Tea-Culture Therapy Program Development for Personality Education of Juvenile Reformatory Students (소년원생의 인성교육을 위한 차문화치료 프로그램 개발)

  • The purpose of this study is to develop a personality education tea culture therapy program to effectively improve juvenile delinquency prevention and social adaptation. In order to verify the effectiveness of the tea culture therapy program through social intervention, we analyzed whether there were significant differences in the scores of social characteristics such as self-efficacy, self-control, and interpersonal relationships of juvenile detention students before and after participating in the program. Ten juvenile detention students between the ages of 14 and 17 who were accommodated to a juvenile detention center in B city were selected as the experimental group for the study. The tea culture therapy program was conducted 10 times as a social intervention personality education for juvenile delinquents, and as a result, self-efficacy was found to have a pre-mean of 2.37 (SD 0.33) and a post-mean of 2.49 (SD 0.31), showing a significant difference (Z=-5.874. P=.000), self-control showed a significant difference with the pre-mean 2.06 (SD 0.20) and the post-mean of 2.16 (SD 0.19) (Z=-4.743, P=.001). The interpersonal relationship was found to have a significant difference, with a pre-mean of 1.90 (SD 0.32) and a post-mean of 2.15 (SD 0.21) (Z=-5.892, P=.000). The above results show that this program has a significant effect on improving social characteristics such as self-efficacy, self-control, and interpersonal relationships among juvenile detention students. Therefore, the tea culture therapy program developed in this study for personality education for juveniles can be used as a personality education program for various types of adolescents as well as in the field of correctional welfare in the future.

Detecting of Proximal Caries in Primary Molars using Pen-type QLF Device (펜-타입 QLF 장비의 임상적 유구치 인접면 우식 탐지 성능)

  • The purpose of this in vivo study was to assess the clinical screening performance of a quantitative light-induced fluorescence (QLF) device in detecting proximal caries in primary molars. Fluorescence loss, red autofluorescence and a simplified QLF score for proximal caries (QS-proximal) were evaluated for their validity in detecting proximal caries in primary molars compared to bitewing radiography. Three hundred and forty-four primary molar surfaces were included in the study. Carious lesions were scored according to lesion severity assessed by visual-tactile and radiographic examinations. The QLF images were analyzed for two quantitative parameters, fluorescence loss and red autofluorescence, as well as for QS-proximal. For both quantitative parameters and QS-proximal, the sensitivity, specificity and area under receiver operating curve (AUROC) were calculated as a function of the radiographic scoring index at enamel and dentin caries levels. Both quantitative parameters showed fair AUROC values for detecting dentine level caries (△F = 0.794, △R = 0.750). QS-proximal showed higher AUROC values (0.757 - 0.769) than that of visual-tactile scores (0.653) in detecting dentine level caries. The QLF device showed fair screening performance in detecting proximal caries in primary molars compared to bitewing radiography.

Comparative Analysis of ABM/P-15, Bone Morphogenic Protein and Demineralized Bone Matrix after Instrumented Lumbar Interbody Fusion

  • Objective : ABM/P-15 (anorganic bone matrix/15-amino acid peptide fragment) is a commercially available synthetically manufactured P-15 collagen peptide fragment, that is adsorbed on ABM. This study was done to investigate the efficacy of ABM/P-15 in achieving fusion in the lumbar spine and comparing it with that of recombinant bone morphogenic protein-2 (rhBMP-2) and demineralized bone matrix (DBM). Methods : A retrospective observational study of prospectively collected data of 140 patients who underwent lumbar spinal fusion surgeries in a single specialty spine hospital between 2016 and 2020, with a minimum 6-month follow-up was conducted. Based on the material used for the augmentation of the bone graft at the fusion site, the patients were divided into three categories namely ABM/P-15, rhBMP-2, and DBM group. Results : ABM/P-15, rhBMP-2, and DBM were used in 46, 44, and 50 patients, respectively. Patient characteristics like age, gender, bone mineral density, smoking history, and presence of diabetes mellitus were comparable amongst the three groups. Average follow-up was 16.0±5.2, 17.9±9.8, and 26.2±14.9 months, respectively in ABM/P-15, rhBMP-2, and DBM groups. The fusion was achieved in 97.9%, 93.2%, and 98% patients while the average time-to-union was 4.05±2.01, 10±4.28, and 9.44±3.49 months (p<0.001), respectively for ABM/P-15, rhBMP-2, and DBM groups. The average pre-operative Visual analogue scale score was 6.93±2.42, 7.14±1.97, 7.01±2.14 (p=0.900) for ABM/P-15, rhBMP-2 and DBM groups, respectively, which reduced to 1.02±0.80, 1.21±0.96, and 0.54±0.70 (p=0.112), respectively at the last follow up. Pre-operative Oswestry disability index scores were 52.7±18.02, 55.4±16.8, and 53.56±19.6 (p=0.751) in ABM/P-15, rhBMP-2, and DBM groups, which post-operatively reduced to 33.77±15.52, 39.42±16.47, and 38.3±15.89 (p=0.412) and further to 15.74±8.3, 17.41±10.45, and 16.76±9.81 (p=0.603), respectively at the last follow-up. Conclusion : ABM/P-15 appears to achieve union significantly earlier than rhBMP-2 and DBM in lumbar spinal fusion cases while maintaining a comparable clinical and complication profile.

Anesthetic efficacy of supplemental buccal infiltration versus intraligamentary injection in mandibular first and second molars with irreversible pulpitis: a prospective randomized clinical trial

  • Background: To compare the anesthetic efficacy of supplemental buccal infiltration (BI) (1.7 ml) versus intraligamentary (IL) injection containing 0.4 ml of 4% articaine with 1:100.000 epinephrine after an inferior alveolar nerve block (IANB) with 1.7 ml 2% lidocaine in the first and second mandibular molars diagnosed with irreversible pulpitis (IP). Methods: One hundred subjects diagnosed with IP of either the mandibular first (n = 50) or second molars (n = 50) and failed profound anesthesia following an IANB were selected. They randomly received either the IL or BI techniques of anesthesia. Pain scores on a 170 mm Heft-Parker visual analog scale were recorded initially, before, and during supplemental injections. Furthermore, pulse rate was measured before and after each supplemental injection. During the access cavity preparation and initial filing, no or mild pain was assumed to indicate anesthetic success. The chi-square test, Mann-Whitney U test, and independent samples t-test were used for the analyses. Results: The overall success rates were 80% in the IL group and 74% in the BI group, with no significant difference (P = 0.63). In the first molars, there was no significant difference between the two techniques (P = 0.088). In the second molars, IL injection resulted in a significantly higher success rate (P = 0.017) than BI. IL injection was statistically more successful (P = 0.034) in the second molars (92%) than in the first molars (68%). However, BI was significantly more successful (P = 0.047) in the first molars (88%) than in the second molars (64%). The mean pulse rate increase was significantly higher in the IL group than in the BI group (P < 0.001). Conclusions: Both the IL and BI techniques were advantageous when used as supplemental injections. However, more favorable outcomes were observed when the second molars received IL injection and the first molars received BI.

Factory Workers' Perception for Applying Smart Factory in Developing Country - Focusing on the survey results of the Indonesian garment manufacturing factory - (개발도상국 공장 근무자의 스마트팩토리 적용에 대한 인식 - 인도네시아 의류생산 공장 설문조사 결과를 중심으로 -)

  • Recently, major manufacturers are focusing their efforts on securing global competitiveness through smart factory, but developing countries have many difficulties in applying smart factory due to financial and technical conditions. This study is a preliminary study on the development of an ICT-based power monitoring system applicable to developing countries. The questionnaire surveyed and analyzed workers' perceptions of smart factory in a garment manufacturing factory in developing countries, Indonesia. Before and after the installation of the power monitoring system, the survey was conducted for 126 local managers and workers, and the correlation was analyzed using SPSS. As a result of analysis, factory workers in developing countries such as Indonesia are also positively aware of the necessity of introducing smart factory technology, and it is expected that the introduction of these technologies will affect job satisfaction and improve the factory environment. In addition, the result of the survey conducted after the installation of the power monitoring system increased the job satisfaction score by 5.5% compared to before the installation, and the scores on the perception of the necessity of the power monitoring system and the positive effect of the application of the system on the factory environment were increased 13% and 5.9%, respectively. It was also confirmed that managers rather than workers and female rather than male showed positive perception for the introduction of smart factory technology. The result of this study is expected to be an important reference in the direction of development of appropriate smart factory technology applicable to developing countries and the introduction of smart factory by manufacturers operating factories in developing countries.
