Association of Violent Experience and Resilience with Burnout in Emergency Medical Technicians (119구급대원의 폭력경험과 탄력성이 소진에 미치는 영향)

  • Purpose: This descriptive correlational study investigated the effects of violent experience and resilience on burnout in emergency medical technicians (EMTs). Methods: The participants were 160 EMTs working in fire stations (safety center and local center) located in U, B, and D city. The collected data were analyzed by descriptive statistics, independent t-test, ANOVA, $Scheff{\acute{e}}$ test post-hoc analysis, Pearson correlation coefficients, and multiple regression analysis using IBM SPSS Statistics 20.0. Results: The number of EMTs getting into ambulances was significantly associated with violent experience, resilience, and burnout. Position of EMTs was significantly associated with both resilience and burnout. In the multiple regression analysis, the subscales of durability and optimism in resilience were significantly associated with burnout after controlling for confounding variables. Conclusion: This study indicated that improving durability and optimism in resilience was important to cope with experience of violence and to prevent burnout in EMTs.

A Phenomenological Study on Illness Experience of Patients with Pressure Ulcer (욕창 환자들의 질병 체험에 관한 현상학적 연구)

  • Purpose: The purpose of this study was to describe the illness experience of patients with pressure ulcer. Methods: A phenomenological methodology was used for the study. The data were collected by individual in-depth interview with seven participants with pressure ulcer during 2013~2014. All interviews were audio-taped and verbatim transcripts were made for the analysis. The data were analyzed using Colaizzi's phenomenological method. Results: All participants had underlying disease, such as spinal paralysis and diabetes. Average period of having pressure ulcer was 18 months, ranged from 3 to 36 months. A total of seven theme clusters were derived from the analysis; unexpected wound, inherent vulnerability to infection, reversal of the treatment policy, unpleasant and strange feeling of wound, sweeping fear and helplessness, socioeconomic burden, and healing through specific actions and reflection. The participants faced various contradictory and paradoxical situations in managing their pressure ulcers as well as underlying diseases in their everyday life. However, they slowly overcome these situations by strictly practicing concrete action-oriented strategies that they have learned through suffering and appreciating miraculous wound healing. Conclusion: The results of this study can help developing a patient-specific intervention program with sufficient emotional support by providing insights of the paradoxical illness experience of patients with pressure ulcer.

Characteristics, Motivation of Choice and Childbirth Experience of Women Who Selected Delivery at Midwifery Clinic (조산원 분만을 선택한 여성의 특성, 선택동기 및 출산경험 조사)

  • Purpose: This study is descriptive research to identify characteristics and childbirth experience of women who selected delivery with midwives. Methods: The research methods included structured questionnaires, open questions and charts and data were collected from March 2009 to May 2010 at one midwifery clinic. The 108 data of primipara and multipara were analyzed using descriptive statistics and grouping in same meaning. Results: The general characteristics of participants were age of 31~35, housewife, college or university graduate, religion 'yes', and economic state 'middle'. The obstetric characteristics of participants were abortion rate of 23.1%, none complication to pregnancy and delivery of 97.2%, and none postpartum complication. This study was analyzed using positive experiences of 3 categories, that is 'comfortable and natural childbirth', 'satisfaction and trust to personal care of midwives', and 'experience of baby-and-family-centered childbirth' and negative experiences of 3 categories, that is 'improvement of healthcare environment', 'insufficient facilities and nursing care', and'burden of cost'. Conclusion: This study is significant, since it investigates in the absence of domestic research on the characteristics of women who delivered at midwifery clinic. Thus, this study provided basic data on the characteristics of women who delivered at midwifery clinic.

Hospital Nurses' Uses of Evidence, and Barriers to and Enablers of Evidenced-based Practice (병원 간호사들의 근거활용 경험 및 장애요소와 촉진요소에 대한 탐색)

  • Purpose: The purpose of this study was to explore nurses' experience of evidence-based nursing practice in general hospitals. Methods: Data were collected from 13 nurses through in-depth interviews about their experiences with evidence-based practice. The research questions were "What kind of evidence are you using in your practice?" and "What are the barriers to and enablers of evidence-based practice that you have experienced?" Qualitative data from field and transcribed notes were analyzed using qualitative content analysis methodology. Results: Major themes of using evidence were identified as 'research as primary valid evidence', 'information from local context and internet as realistic evidence', and 'clinical experience as pragmatic evidence'. Patient experience was not used as evidence in solving nursing problems. Barriers to and enablers of evidence-based practice were linked. They included both external, organizational factors and individual factors. Main issues were 'lack of evidence and poor work environment', and major facilitating factors were 'improving knowledge and skills related to evidence-based practice' and 'communicating and sharing evidence'. Conclusion: The study findings provide useful information for understanding nurses' experience of using external and internal evidence along with their meaning. A multidimensional approach is needed to overcome barriers to and implement evidence-based practice.

  • The purpose of this study was to explore and describe the experience of Dan-Jeon breathing training and of Qi as a essential substance in forming human body. The sample consists of 7 participants who are Dan-Jeon Breathing training in a Training center, Pusan, Korea. They were asked open-ended questions in order for them to talk about their experiences. With permission of the subjects, the interviews were recorded and transcribed. The summarized results of this research are following. 1. The purpose of Dan-Jeon Breathing The interview data was organized by themes into 4 categories : hope for health recovery, a concern about Dan-Jeon Breathing, seeking meaning of life, change of lifestyle 2. The experience of Qi during Dan-Jeon Breathing training The interview data was organized by themes into 3 categories : an autonomic movement of body, spiritual experience, conviction of existence of Qi. 3. The change after Dan-Jeon Breathing training. The interview data was organized by themes into 7 categories : physical health promotion, emotional relaxation, promoting brain function, positive attitude about life, love to others, investigation for self, improvement on Qi feeling..

Clinical Work Experience of Korean Immigrant Nurses in U.S. Hospitals (한인간호사의 미국병원 내 임상실무경험)

  • Purpose: The purpose of this qualitative study was to explore the meaning of the experience of Korean immigrant nurses working in US hospitals. Methods: Purposive sampling yielded 15 Korean immigrant nurses who had more than one year of clinical experience in US hospitals. Data were collected from March to August 2012 through in-depth interviews and thematic analysis was conducted using van Manen's hermeneutic phenomenological approach. Results: The findings were classified into eight themes: 'struggling from staff at workplace being territorial to outsiders', 'feeling oppressed due to language barrier', 'accepting rational and horizontal relationships at work', 'staying alert in the environment where lawsuits are rife', 'feeling a sense of stability from the social system that values human dignity', 'maintaining self-confidence from prominent nursing practices and senior Korean nurses' professional reputation', 'performing essential comprehensive nursing care', 'promoting self-development to be equipped with professionalism.' Conclusion: The findings indicate that the Korean immigrant nurses were able to excel in their workplace when their clinical experience at US hospitals was combined with the lived space in US politics and environment, lived time of patience, lived body to be alert, and lived others with multi cultural characteristics.

A Grounded Theory Approach on Nurses' Experience with Workplace Bullying (간호사의 직장 내 괴롭힘 경험에 관한 근거이론 연구)

  • Purpose: The purpose of this qualitative study was to explore the workplace bullying experience of Korean nurses. Methods: Participants were twenty current or former hospital nurses who had experienced workplace bullying. Data were collected through focus group and individual in-depth interviews from February to May, 2015. Theoretical sampling method was applied to the point of theoretical saturation. Transcribed interview contents were analyzed using Corbin and Strauss's grounded theory method. Results: A total of 110 concepts, 48 sub-categories, and 17 categories were identified through the open coding process. As a result of axial coding based on the paradigm model, the central phenomenon of nurses' workplace bullying experience was revealed as 'teaching that has become bullying', and the core category was extracted as 'surviving in love-hate teaching' consisting of a four-step process: confronting reality, trial and error, relationship formation, and settlement. The relationship formation was considered to be the key phase to proceed to the positive settlement phase, and the participants utilized various strategies such as having an open mind, developing human relationships, understanding each other in this phase. Conclusion: The in-depth understanding of the workplace bullying experience has highlighted the importance of effective communication for cultivating desirable human relationships between nurses.

Nurses' Experience of Career Ladder Programs in a General Hospital (간호사들의 경력개발 프로그램 참여 경험)

  • Purpose: The purpose of this study was to identify and describe nurses' experience of a career ladder program (CLP) in a general hospital in Korea. Methods: The data were collected from 15 nurses who participated in the CLP. Each nurse completed individual in-depth interviews, for which the main interview question was "How do you describe your experience of CLP?" All interview data were transcribed and were analyzed using the grounded theory method. Results: The core category of the experience of CLP was identified as "challenging to rebirth as an expert". The participants used five action/interaction strategies such as "taking some time for introspection", "appreciating the essence of nursing", "making use of feedback positively", "throwing away doubts about their own capabilities", "exerting themselves with confidence". The consequence of the process of adjusting to CLP was "positioning a differentiated expert". Conclusion: The results of this study show that participants recognized CLP as a powerful resource which leads them to professional growth and development. Thus, CLP could be utilized as a clinical career-promoting program and ultimately as a program for providing high-quality nursing services.

  • "Secret Works of Universal Benefit" was compiled by Lee Gyeonghwa(李景華, 1721-?) in Choseon Dynasty. Experience by Simple Formulae using Korean herbs in Secret Works of Universal Benefit was recorded Hangul name[鄕名], main medicinal effect and how to use etc. of 50 Korean herbs[鄕藥] to give aid to country people who didn't have medical knowledge. Secret Works of Universal Benefit have passed on traditional Korean medicine as selecting 50 Korean herbs and recording Hangul name. Though most of Experience by Simple Formulae using Korean herbs[鄕藥單方治驗] was quoted "Compendium of Matria Medica", it reflected developing of Korean traditional herb science[本草學] as selecting herbs, arranging frequent symptoms, changing main effect of herbs, and adding clinical experiences.

Homepage on the Purchase Attributes of Food Service Products (미니홈피를 통한 구전 커뮤니케이션이 외식상품 구매 속성에 미치는 영향)

  • This study aims at examining the effect of word-of-mouth communication through mini homepage on purchase attributes and mapping out a marketing strategy based on this. For this, it made an analysis of the communication level with 302 mini homepage users. The result showed that the communication level of information exchange has a positive effect on the purchase experience of food service products significantly, showing that the higher its level is, the more frequent its experience is. And it showed that the communication level of relationship retention has a negative effect on the purchase experience of food service products significantly, showing that the higher its level is, the less frequent its experience is. These results demonstrated the utility that can increase word-of-mouth communications through information exchange and relationship retention, given that they are used with the object based on the kinship of information exchange and relationship retention as word-of-mouth communications, and that they lay stress on the connection with other people in the mini homepage.
