Lung function tests in preschool children (5세 미만의 폐기능 검사)

  • Measurement of lung function is an integral component of respiratory physiology and of clinical assessment of lung diseases in school age children and adults. Pulmonary function test of infants and children under the age of 2 years have now been standardised and are being used both in research and as an adjunct to clinical management. By contrast, until recegntly, children of preschool age, i.e. between 2-6 years represented a major challenge for pulmonary function test assessment, this particular period commonly being referred to as the 'dark ages' of Pediatric Pulmonology. Measurement of lung function in preschool-aged children is now feasible. However, much work remains to be done in standardizing how these tests are performed, and in understanding the most appropriate role for the various tests in the study of growth and development of the respiratory system and in the clinical management of children in this age group. As the field develops and the knowledge of respiratory physiology in this age group expands, investigation of different and more appropriate algorithm use in preschool children, together with development of more appropriate reference data, may result in improved disease discrimination.

Children's Dramatic Play Behaviors in Same-Age and Mixed-Age Preschool Classrooms (유치원 단일연령 교실과 혼합연령 교실에서의 아동의 극놀이 행동)

  • The purpose of this study was to examine children's dramatic play and dramatic play themes in the same-age and mixed-age kindergarten classrooms. The subjects were 45 children in three classrooms of 4-year-olds, 69 children in three classrooms of 5-year-olds, and 60 children in three mixed-age classrooms of 4-and 5-year-olds. Observations were conducted by videotape recordings. Observation periods were of five-minutes duration. There were ten observations of children's indoor free-play periods. Four-and five-year-olds in mixed-age classrooms were more likely to engage in group-dramatic play than 4-and 5-year-olds in same-age classrooms. Four-year-olds in mixed-age classrooms were more likely to engage in domestic and family, war/violence, school, animal, and vehicle play themes than 4-year-olds in same-age classrooms. However, 5-year-olds in mixed-age classrooms were more likely to engage in family and school play themes than 5-year-olds in same-age classrooms. 5-year-olds in same-age classrooms were more likely to engage in vehicle and animal play themes than 5-year-olds in mixed-age classrooms. 5-year-olds in same-age classrooms did not differ from 5-year-olds in mixed-age classrooms on the war/violence themes used during dramatic play. In terms of sex differences, boys were more likely to engage in violence, adventure fantage themes than girls. Conversely girls were more likely to engage in family/marriage static play themes than boys.

Predicting and Understanding School-Age Children위s Health Behavior (학령기 아동의 건강 행위 실천의 예측 요인)

  • The purpose of the study was to investigate the level of health behavior of school-age children and to identify the predicting variables of the school-age children's health behavior. The subject were 467 children in grades four to six, enrolled in two elementary schools located in two cities. The mean age of the subject was 10.03(SD=1.33). The data were analyzed using t-test, ANOVA, correlation analysis, and stepwise multiple regression. The result are as follows : 1. The mean of the score of health behavior of the school-age children was 154.6, showing that they are practicing health behavior relatively well. 2. There were significant differences in the mean scores of health behavior according to grade (F=6.53 p=.001), sex(t=-3.70, p=.000), educational level of the parents(F=4.92, p=.002 ; F=4.47, p=.004), occupation of the parents(F=3.31, p=.003 ; F=4.76, p=.000), and socioeconomic status(F=11.87, p=.000). 3. There were significant correlations between health behavior and health motivation(r=.53, p=.000), self-concept(r=.32, p=.000), perceived health status(r=.16, p=.000), and health locus of control (r=.15, p=.001). 4. Health motivation, self-concept, grade, socioeconomic status, and health locus of control were identified as predictor variables of health behavior of the school-age children from the stepwise multiple regression analysis. The total percent of variance accounted for by these five variables was 35.0%. From the result, it is suggested that in the development of a school health education program, the effect of health motivation and self-concept to promote student's health behavior in school-age children should be considered.

A Qualitative Study on Experiences of Parenthood Among Mothers of Early School-Age Children (학령 초기 자녀를 둔 어머니들의 부모됨 경험에 대한 질적 연구)

  • Objective: The purpose of this study was to gain an understanding of parenthood of mothers of early school-age children, with emphasis on cognitive, behavioral, and emotional aspects of parenthood. Methods: Nineteen mothers whose children were 1st and 2nd grade elementary school children participated in this study. Data were collected through individual in-depth interviews. During the interview, mothers were asked to speak freely about their experiences as parents of early-school age children, including parenting beliefs, parenting behaviors, and emotional experiences related to parenting. These interviews were transcribed verbatim and analyzed in a thematic way using MAXQDA. Results: Using qualitative methods, five major themes were emerged: (1) Recognizing the role of parents as a supporter of growth; (2) recognizing parenthood of the present generation that imposes expanded roles to mothers; (3) co-parenting of early school-age children and marital relationship; (4) co-existence of different parenting styles based on specific mother-child interaction situations; (5) experiencing a wide spectrum of emotions including anxiety, guilt, and depression, as well as happiness. Conclusion: This study revealed the unique cognitive, behavioral, and emotional aspects of parenthood experienced by mothers with early school-age children. Results of this study can fill a gap in our understanding of parenthood of mothers when their children entered into the new developmental stage of middle childhood. The results could also be used as a basis for developing parent education and family relationship programs.

The Effect of Children's Age on Married Women's Career Reinterruption (자녀 연령이 기혼여성의 경력 재단절에 미치는 영향)

  • Purpose - The main purpose of this paper is to examine the effect of children's age on maternal labor supply in Korea using survival analysis. Specifically, we focus on the career re-interruption of women having children under age 12, which has rarely been studied in the existing literature. Research design, data, and methodology - We use micro data from the Korea Labor and Income Panel Study (KLIPS) surveyed from 1998 to 2016. Instead of using a pre-school child dummy or the number of young children as an explanatory variable, 9 children's age dummies are included to capture the effect of nurturing 0 to 9 years old children. This study estimates the hazard of a woman's exiting the labor market after her first experience of the career interruption, rather than the hazard of the first career interruption itself. A Cox proportional hazard model is applied to numerically capture the impact of children's age on behavioral changes in maternal labor supply. The sample used in this analysis is women between 15 and 54 years old. Most of all, we restrict the sample to women who had at least a child between 0 and 12 years old at the time of quitting their jobs. Results - The Cox proportional hazard model estimates show a strong negative effect of a 0-year-old child on maternal labor supply. Mothers with newborns have a high hazard ratio of labor force exit after the re-entry. The hazard of women with infants is three times higher than those with children aged 10 to 18. Additionally, the results show that not only newborns, but also children in the age of school-entry have a negative impact on their mother's labor supply. Conclusions - The findings reveal that children's ages need to be properly expanded and included when analyzing the effect of children and their ages on married women's labor supply, especially on women's career re-interruption. A large negative effect of 7-year-old children on maternal labor supply found here indicates that supporting mothers with school age children as well as pre-school children is necessary to prevent mothers from leaving the labor market.

Influence of age at complementary food introduction on the development of asthma and atopic dermatitis in Korean children aged 1-3 years

  • Background: Complementary food in infancy is necessary for human growth, neurodevelopment, and health. However, the role of allergen consumption in early infancy and its effects on the development of food allergy or tolerance remain unclear. Purpose: To investigate the influence of age at the time of complementary food introduction on the development of asthma and atopic dermatitis in Korean children aged 1-3 years. Methods: We combined data from the Korea National Health and Nutrition Examination Survey collected from 2010 to 2014 and analyzed 1619 children aged 1-3 years who were included in the survey. Multivariate regression analysis was used to identify associations among type of feeding, age at the time of complementary food introduction, and doctor-diagnosed atopic dermatitis and asthma. Results: Age at the time of complementary food introduction was not significantly associated with doctor-diagnosed atopic dermatitis and asthma in children aged 1-3 years. In the univariate analysis, children with asthma showed higher water and sodium intake levels than nonasthmatic children. However, this relationship was not significant in the multivariate regression analysis. Conclusion: The present study revealed no statistically significant relationship between age at the time of complementary food introduction and the risk of atopic dermatitis and asthma in young Korean children. A national prospective study is needed to clarify the influence of age at the time of complementary food introduction on the development of allergic diseases.

Perceived competence of Children and Perception of responsibilities of Parents Based on Age, Sex, Sociometric Status (연령, 성별, 사회측정적지위에 따른 아동의 자기능력지각과 부모의 책임지각)

  • The purpose of this study is to see difference based on age, sex, sociometric ststus for perceived competence of children and difference about perception of responsibilities degree of parents of these children. the content of study as follows. 1. How is the difference about perceived competence of children based on age, sex, sociometric status. 2. How is the difference about perception of responsibilities of children based on age, sex, sociometric status. 3. How is the relationship between father and mother. The subject of this study is children and their parents of 126 persons of the third grade, sixth grade of S elementary school, the second grade of S middle school, the second grade of S girls' middle school which are located in Pusan city. This study used a peer nomination among sociometric assesment method as sociolity assessment means, self-perception profile for children of Harter(1985) as perceived competence assessment means of children and used self-perception profile for children which amends fits for parents. the collected data are managed by SPSS/PC+ program. We analyze three-way ANOVA to verify study question No, one, two, analyze t-verification and Pearson's product moment correlations to verify study question No. three. The result of this study as follows. First, in investigating difference about perceived compretence of children based on age, sex, sociometric status, we can see that children perceive high, self-ability for low ages. Also, boys than girls, popular children than rejected children, neglected children perceive high self-ability. Second, in investigating difference about perception of responsibilities of children based on age, sex, sociometric status, we can see that mother perceive responsibilities in learning sphere, exercise sphere, the whole self-value sphere and father perceive responsibility only in social sphere. Third, in investigating relation between perception of responsibilities of father and mother, this shows that difference between perception of responsibilities of father and mother is not showed ane have (+) interrelation of r=.46(P<.001) between perception of responsibilities of father and perception of responsibilities of mother.

Ten-year trends and prevalence of asthma, allergic rhinitis, and atopic dermatitis among the Korean population, 2008-2017

  • Background: Major questions remain regarding the agestratified trends of allergic diseases and asthma in Korea. Purpose: To identify the estimated recent prevalence and 10-year trends in asthma, allergic rhinitis, and atopic dermatitis among the Korean population from 2008 to 2017. Methods: This nationwide cross-sectional survey (Korean National Health and Nutrition Examination Survey) over 10 years (2008-2017) examined representative samples of the Korean population (n=85,006) including 2,131 infants, 4,352 preschool children, 12,919 school-age children, 44,200 adults, and 21,404 elderly adults. Results: In the 2016 to 2017 population, the estimated prevalence of asthma was 0.9% in infants, 2.3% in preschool children, 4.1% in school-age children, 2.3% in adults, and 4.1% in the elderly. The estimated prevalence of allergic rhinitis was 9.0%, 20.2%, 27.6%, 17.1%, and 6.9%, respectively. The estimated prevalence of atopic dermatitis was 5.9%, 11.3%, 14.6%, 3.9%, and 1.6%, respectively. Ten-year trends revealed a significant decrease in asthma prevalence in infants, preschool children, and the elderly. and in atopic dermatitis prevalence in infants and preschool children (P<0.05 for all trends). Furthermore, 10-year trends demonstrated a significant increase in allergic rhinitis prevalence in school-age children, adults, and the elderly, and in atopic dermatitis prevalence in school-age children and the elderly (P<0.05 for all trends). Conclusion: These results improve our understanding of the age-stratified epidemiology of allergic diseases in Korea and suggest the need for the development of tailored and precise strategies to prevent allergic diseases in different age groups.

Predicting and Understanding School-Age Children's Visual Health Behavior (학령기 아동의 시력 건강 행위 이행의 예측 요인)

  • Purpose: The purpose of the study was to investigate the status of visual health behavior of school-age children and to identify the predicting variables of the school-age children's visual health behavior. Method: The subject was 764 children in grades 4 to 6, enrolled in 2 elementary schools located in urban area. The data were analyzed using t-test, ANOVA, correlation analysis, and stepwise multiple regression. Result: The findings were as follows: 1. The mean of the score of the visual health behavior of school-age children was 41.59(SD=7.22) and there was a significant difference according to grade. 2. There were significant correlations between visual health behavior and self-efficacy for visual health behavior (r=.51, p=.000), motivation for visual health(r=.45, p=.000), perceived benefits(r= .34, p=.000), self-concept(r=.32, p=.000), knowledge of visual health(r=.30, p=.000), health perception for vision(r=.16, p=.000), health locus of control(r=.11, p=.002), and perceived barriers(r=.-.12, p=.000). 3. Self-efficacy for visual health behaviors, knowledge of visual health, motivation for visual health, self-concept, health perception for vision, and perceived benefits were identified as predictor variables of the visual health behavior of the school-age children from the stepwise multiple regression analysis. The total percent of variance accounted by these 6 variables were 36.9%. Conclusion: From the result, it is suggested that the development of comprehensive visual health education program including the component of self-efficacy, health motivation and self-concept to promote the visual health of school-age children.

Kidney size estimation in Korean children with Technesium-99m dimercaptosuccinic acid scintigraphy

  • Purpose: Renal size is an important indicator to determine adequate organ growth in children. The aim of this study was to estimate renal size with Technesium-99m dimercaptosuccinic acid (DMSA) scan and propose a simple formula for predicting renal length in normal Korean children. Methods: This study included 346 children (148 boys and 198 girls; age range, 1 month to 17 years) in whom renal length was measured using the DMSA scan. Patients with anatomical renal abnormalities or acute pyelonephritis were excluded. Children were divided into two groups: 214 children (61.8%) were less than a year old (group 1) and 132 (38.2%) were ${\geq}1$ year (group 2). Results: Renal length was larger on the left side than the right side, and there was no significant gender-related difference in renal length. We propose the following formula for renal length based on the analysis of the 346 children in our study: the formula was as follows: $4.682{\times}age(month)^{0.137}$, $R^2=0.780$. In group 1, the formula was renal length $(cm)=0.127{\times}age(month)+5.144$, $R^2=0.354$, and in group 2, the formula was $0.334{\times}age(year)+6.477$, $R^2=0.829$. Conclusion: It is difficult to establish simple formulae in infants ($R^2=0.354$). Therefore, further studies including relevant variables are needed for this age group. We proposed formulae to estimate renal length in Korean children over 1 year of age by using the DMSA scan.
