A Study on Prevalence and Risk Factors for Varicose Veins in Nurses at a University Hospital

  • Background: Lower-limb varicose veins (VVs) are common and known to have a higher prevalence among people who work in occupations requiring prolonged standing. In the Republic of Korea, however, VV-related occupational factors have seldom been examined. This study was conducted to assess the prevalence of VVs among nurses, an occupational group considered to be at high risk of VVs, and determine the occupational risk factors of prolonged standing. Methods: Between March and August 2014, a questionnaire survey coupled with Doppler ultrasonography was conducted on the nurses working at a university hospital. Results: A total of 414 nurses participated in the survey and diagnostic testing. From the survey analysis and test results, the prevalence of VVs in nurses was estimated to be 16.18%. Significant factors for venous reflux were age [odds ratio (OR) = 1.06, 95% confidence interval (CI) = 1.03-1.10], pregnancy (OR = 2.15, 95% CI = 1.17-3.94), and delivery (OR = 2.02, 95% CI = 1.08-3.78). The statistical significance of these factors was verified after risk adjustment for sociodemographic factors (OR = 3.40, 95% CI = 1.27-9.09). Conclusion: Factors significantly associated with venous reflux were increasing age and prolonged working hours (${\geq}4hours$) in a standing position (OR = 2.80, 95% CI = 1.08-7.25), even after risk adjustment for sociodemographic factors. This study is significant in that an objective diagnosis of VVs preceded the analysis of the risk factors for VV incidence, thus verifying objectively that VVs are associated with occupations requiring prolonged hours of working in a standing position.

서울지역 국민학교(國民學校) 양호교사의 학교간호업무(學校看護業務) 수행정도(遂行程度)에 관(關)한 연구(硏究) (A Study on status of school health and analysis of factors affecting school nursing activities in the secondary school in Seoul)

  • This study was conducted in order to grasp the condition of about the school nurse's service and to offer the basic materials of improving the school health service. The objects were comprised of 98 volunteered school nurses who take service in the elementary school. The material of this study was the questionaire suited to the purpose of this research which has been made through studying references and this questionaire has been corrected and revised three times. All the questionaire written by school-nurses. The results are as follows; 1. General features of the objects of study Average age was 35.9 and average career was 9.2 years. Except working as school-nurse, the past career was that average clinical-field was 3.4 years and health service was 4 months. Their educational level was as high as 72.4% of the objects were graduated from above 3 years college and 89.8% were married. 76.5% have religions and 94.8% were working at with national and public schools. 99.0% were doing only nursing service. 2. The conditions of the school health resources. The ratio of school-nurse to students was one to 2630. School-nurse to classes, one to 49.3, and school-nurse to teachers, one to 54. For total amount of a year budget of school health, from three hundred thousands to fifty nine hundred thousands won was most common. Expenses for purchasing medicine were used most. 58.2% of school-nurses hasn't known a year budget. There was an organization for school health in 74.2% of schools. 42.9% of nursing rooms were in the center of school and 88.8% were on first floor. Nursing room were used alone without being used by another purpose and the room size of 71.6% was below 10 pyong. 3. The conditions of school health service Average users of nursing room were 413 a month. The most of them had digestive trouble. Sending letters to home was 15.9 times a year. The most contents of letters was about health education. Object spent much time managing nursing room. 4. The degree of school health service When 2 points was given to "perform" and 1 point was to "not perform" the total average was 1.75, health education 1.89, environmental management 1.86, plan of project and evaluation 1.83, management of nursing room 1.82, health management 1.78, run of school health organization 1.32. 5. Correlation between the school health services and variables (1) The part of project plan and evaluation of school health service has relationship to existence or none-existence of school health organization (P<0.01), past health service career (P<0.05), number of classes (P<0.01), number of students (P<0.01), sending letters to home about health education (P<0.01) and number of users (P<0.05). (2) The part of nursing room management has relationship to ages (P<0.05) past clinical career (P<0.05), number of classes (P<0.05), number of students (P<0.05) and sending letters to home about health education (P<0.01). (3) The part of health education has relationship to existence or none-existence of school health organization (P<0.05), past clinical career (P<0.05), the ratio of health management to school nurse's all work (P<0.05) and the ratio of health education to school nurse's all work (P<0.01). (4) The part of environmental management to ages (P<0.01), career as a school-nurse (P<0.01), salary step(P<0.01), sending letters to home about health education (P<0.01), sending all letters to home (P<0.001), the ratio of health management to school nurse's all work (P<0.05), the ratio of health education to school nurse's all work (P<0.05) and area of school-nurse's room to be used. (5) The part of school health organization management to number of classes (P<0.05). (6) The part of health management to number of classes (P<0.05), sending letters to home about health education (P<0.001), sending all of letters to home (P<0.01) and the ratio of health management to school nurse's all work (P<0.05). (7) The part of school health service to ages (P<0.05), past clinical career (P<0.05), past health career (P<0.01), number of classes (P<0.05), number of student (P<0.05), sending letters to home about health education (P<0.05), sending all letters to home (P<0.05), the ratio of health management to school nurse's all work (P<0.05), the ratio of health education to school nurse's all work (P<0.01) and area of school - nurse's room to be used (P<0.05). ## Suggestion for further studies are as follows. 1. School-nurse should exert herself to advance a quality to take care of school population's health. 2. It is necessary that systematic support required to keep school population's health. 3. Home, school and community should make efforts cooperatively and the proper roles of students, teachers, health team members and parents must be achieved.

시계열 자료를 이용한 병원 간호 인력의 변화 추이 및 병원 간호사 확보를 위한 정책의 효과 평가 (Trend analysis of the number of nurses and evaluation of nursing staffs expansion policy in Korean hospitals)

  • Purpose : The purpose of this study was to analyze the trend of the number of nursing staffs and skill mix and to assess the effectiveness of hospital nurse expansion policies in Korea. Methods : The trend of the number of nursing staffs and skill mix were analyzed using time series data, which composed of yearly series data from 1975 to 2009. The impact of hospital nurse expansion policies was estimated by autoregressive integrated moving average(ARIMA) intervention model. Results : The number of general hospital and hospital nurses per 100 beds was decreased in late 1980s and late 1990s due to rapid growth of beds. As a result of the number of nurse aids per 100 beds decreased, skill mix became high in general hospital but nurse ratio among hospital nursing staffs was about 50%. Expansion of new nurse and revised differentiated inpatient fee were only effective in expansion of hospital nursing staffs. But they had no effect in general hospitals. Conclusion : In Korea, a few policies related to expansion of hospital nurses have an effect on increasing the number of hospital nurse. Nevertheless, level of hospital nursing staffs is inferior to that of general hospital.

A comparative study of the psychological impacts of tasks related and unrelated to COVID-19 on nurses: a cross-sectional study

  • Background: This study assessed the psychological impact of the outbreak of coronavirus disease 2019 (COVID-19) on university hospital nurses. It provides an assessment of mental health issues, including depression, anxiety, distress, and burnout of nurses dealing directly and indirectly with COVID-19. Methods: In a web-based, cross-sectional study, 111 nurses from Daegu Catholic University Hospital in Korea were enrolled from August 4 to August 9, 2020. Patient Health Questionnaire-9, Generalized Anxiety Disorder-7, Impact of Event Scale-Revised, and Maslach Burnout Inventory were used to assess the psychological symptoms of depression, anxiety, psychological distress, and burnout among the study participants. Results: Of 111 nurses, 35 (31.5%), nine (8.1%), 26 (23.4%), and 49 (44.1%) experienced depression, anxiety, distress, and burnout, respectively. Nurses who performed COVID-19-related tasks were more likely to have moderate depression (related vs. unrelated, 52.0% vs. 25.6%; p=0.037). There were no differences in anxiety, distress, and burnout between nurses with and without COVID-19-related tasks. More than 50% of the participants showed receptive and positive attitudes toward caring for COVID-19 patients. Conclusion: Nurses who performed COVID-19-related tasks had a higher risk of depression. There were no significant differences in anxiety, distress, and burnout between the two groups. Since nurses who perform COVID-19-related tasks are more prone to psychological distress, continued psychiatric interventions are required for infectious disease outbreaks with a high mortality rate for healthcare workers who are emotionally vulnerable.

The Retention Factors among Nurses in Rural and Remote Areas: Lessons from the Community Health Practitioners in South Korea

  • Purpose: This study analyzed the retention factors of Korean community health practitioners who sustained over 20 years based on a multi-dimensional framework. This study suggests global implications for nurses working in rural or remote areas, even during a worldwide pandemic. Methods: The participants were 16 Korean community health practitioners who worked in rural or remote locations for over 20 years. This study identified nurses' key retention factors contributing to long service in rural and remote areas. This is a qualitative study based on the narrative method and analysis was conducted using grounded theory. A semi-structured questionnaire was conducted based on the following: the life flow of the participants' first experience, episodes during the work experience, and reflections on the past 20 years. Results: First, personal 'financial needs' and 'callings' were motivation-related causal conditions. The adaptation of environment-work-community was the contextual condition leading to intervening conditions, building coping strategies by encountering a lifetime crisis. The consequences of 'transition' and 'maturation' naturally occurred with chronological changes. The unique factors were related to the 'external changes' in the Korean primary health system, which improved the participants' social status and welfare. Conclusion: Considering multi-dimensional retention factors was critical, including chronological (i.e., historical changes) and external factors (i.e., healthcare systems), to be supportive synchronously for rural nurses. Without this, the individuals working in the rural areas could be victimized by insecurity and self-commitment. Furthermore, considering the global pandemic, the retention of nurses is crucial to prevent the severity of isolation in rural and remote areas.

  • One of the first constitutional challenges to congressional legislation in the area of school health came in 1967. The action of Ministry of Education for the school health program is within school health act constituted 20 Articles and its regulation of 14 Articles. This study was reviewed the school health act and its regulation for the development of school health program. The data were collected by 14 Boards of Education from November 1986 to January, 1987. The results of this study were identified the articles needss to change such as the articles with regard to health education activity, physical assessment activity, criteria of school health environment, control of communicable disease, First-Aid and clinic management by School nurses.

보건소장의 변혁적 리더십과 보건간호사의 사회적 자본이 임파워먼트에 미치는 영향 (Influence of transformational leadership of managers perceived by public health nurses and their social capital on empowerment)

  • 본 연구는 보건간호사의 임파워먼트 정도를 파악하고 보건간호사가 인식한 관리자의 변혁적 리더십과 보건간호사의 사회적 자본이 임파워먼트에 미치는 영향 요인을 검증하였다. 서술적 조사 연구로써 대구광역시 8개 보건소와 경상북도 23개 보건소 및 1개 보건의료원에 근무하는 정규 간호직 공무원인 보건간호사 전체 360명을 대상으로 2016년 2월 29일부터 4월 8일까지 설문지를 이용하여 자료 수집하였고, 수집된 자료는 IBM SPSS 21.0 프로그램을 이용하여 빈도, 백분율, 평균과 표준편차, 독립 t-test, ANOVA, Scheffe test, Pearson correlation coefficient 및 위계적 다중선형회귀분석으로 분석하였다. 연구 결과, 보건간호사의 임파워먼트는 보건간호사가 인식한 보건소장의 변혁적 리더십과 보건간호사의 사회적 자본이 증가할수록 보건간호사의 임파워먼트가 증가하는 것으로 나타났다. 이에 보건소장이 변혁적 리더십을 발휘한다면 보건소 업무의 중추적인 역할을 감당하는 보건간호사의 임파워먼트는 높아지고 업무수행에 발휘될 것이고. 또한 보건간호사의 사회적 자본을 증가시켜 준다면 보건 간호사의 임파워먼트는 증가시킬 수 있음을 확인하였다.

부산.경남 지역 간호학생과 간호사의 영적 요구도 비교 (A comparative study on the spiritual needs between nursing students and nurses in Busan and Gyungnam province)

  • 본 연구는 2011년 9월부터 10월까지 부산 경남지역의 병원에 근무하고 있는 간호사 총 153명 간호대학생 총 147 명을 대상으로 설문 조사 수집하였으며, 300 를 배부하여 자료가 불충분한 100 부를 제외하고 최종 200부를 분석하였다. 조사자의 직접방문을 통한 개인별 면담으로 조사하였다. 본 연구에서는 건강한 사람보다는 불건강한 즉 질병에 노출된 가능성이 많은 사람들을 간호대상으로 하는 간호학생과 간호사의 영적 요구도를 파악하고 비교하기위한 목적으로 수행하였다. 따라서 향후 연구에서는 본 연구의 결과를 토대로 간호대학생과 간호사의 영적 요구 도를 파악하고 그것을 바탕으로 간호대상자의 영적간호에 더욱 효율적이고 체계화된 교육프로그램 개발이 필요한 것으로 사료된다.

일개 대학병원 간호사들의 근무형태에 따른 건강행태 (Health Behaviors of Nurses at a University Hospital according to Type of Work)

  • 이 연구는 대학병원 간호사들을 대상으로 하여 이들의 근무형태에 따른 건강행태를 파악하고 간호사들의 건강유지 및 증진을 위한 체계적이고 효과적인 건강교육의 방향 설정을 위한 기초 자료를 얻고자 실시하였다. 연구대상은 대구지역 일개 대학병원에 근무하는 간호사 340명이었고, 자료의 수집은 2006년 3월 6일부터 3월 30일까지 대상자의 일반적 특성, 업무환경특성, 건강행태, 자신의 건강에 대한 인식, 건강증진행위 실천 점수 등에 대하여 자기기입식 설문조사를 통하여 수행되었다. 교대근무 간호사(이하 "교대")와 비교대근무 간호사(이하 "비교대")의 건강행태의 항목에서 규칙적인 식사여부(p<0.01), 음주여부(p<0.05), 건강관리(p<0.05), 규칙적인 운동(p<0.01), 근무시간 외 시간 활용(p<0.05), 충분한 휴식시간(p<0.05), 피로인식여부(p<0.01)에서 통계학적으로 유의한 차이를 보였다. 대상자의 근무형태에 따른 건강증진행위 실천 점수는 교대가 115.4점, 비교대가 121.9점으로 비교대가 건강행위를 더 잘 실천하는 것으로 나타났고, 하부영역별로 실천 점수가 유의한 차이가 있는 것은 자아실현과 영양영역(p<0.01)이었으며, 자아실현영역에서 가장 점수가 높았고, 운동영역에서 점수가 가장 낮았다. 이상의 결과로 볼 때 비교대보다 교대가 교대근무로 인해 상대적으로 시간적인 여유가 부족하고 불규칙적인 생활로 인해 시간활용을 잘 하지 못하는 것으로 생각되며, 교대의 건강행위 실천율을 높일 수 있도록 여건을 개선하고 건강행위에 대한 관심을 유도하는 방법을 모색해야 할 것이다. 또한 간호사들의 생활습관의 개선과 자신에 대한 긍정적인 평가가 이루어지도록 하고, 이에 대한 교육과 홍보가 필요하다고 생각된다.
