연세지역(延世地域)에 대(對)한 보건기초조사(保健基礎調査) (A Basie Health Survey of the Yonsei Community Health Service Area, Seoul)

  • Introduction In order to improve medical education through the introduction of a concept of comprehensive health care of a community, an area surrounding the University Campus was chosen for the Community Health Service Project. It has been on operation for last 4 years with its major emphasis on family planning services, and maternal and child health care. The major objectives of this survey at the area are to obtain: 1) The demographic data, 2) The health need and trend of medical care, 3) The attitude and practice in maternity care to be used for further improvement of the planning and the services of the project. Population and Survey Method Out of three Dongs of the Community Health Service Area, only two Dongs namely Changchun and Yonhee were selected for the survey. Total number of households and population in the area studied was 3,683 and 21,857 respectively. An interview was performed with questionnaire schedule which was recorded by interviewers. This includes the degree of utilization of health services provided by the Community Health Service Program such as family planning, prenatal care during their last pregnancy, delivery history and complications of the delivery as well as the incidence of illnesses in general. Prior to the interview, all interviewers were trained for interviewing technique for two days. The survey was carried out during the period from October December 1967. Results 1) Demographic Data : 41.3% of the population studied were children under age 15 and only 3.5% were over 60 years of age. Crude birth rate and crude death rate of this area studied during the period of November 1966-October 1967 were 20.5 and 7.7 respectively. Infant mortality rate during the same period was 35.9. 50.4% of the 2,832 households fell into the category of middle class, 39.8% to the lower class and 9.5% to the upper class in economic condition. 19.8% of 2,832 householders had no formal education, 22.7% primary school, and 57.5% middle or higher school education. 2) Health Status and Utilization of the Community Health Service: Those who suffered from many illnesses during the month of October, 1967 were 690(4.6% of 14,891 persons). Classification of these patients into the type of disease shown respiratory diseases 27.4%, gastrointestinal diseases 18.1%, tuberculosis 10.9%, skin and genitourethral diseases 4.5% and gynecologic patients 4.5%. Only 55.9% of the patients received medical care at hospital or doctor's clinic. But among TB and gynecologic patients, 70.7% and 72.4% were treated at medical facilities. 10.6% of 2,832 householders interviewed has ever utilized the Community Health Service Program provided by the Yonsei Medical School, Classifying these clients into the type of service, 35.9% utilized the wellbaby clinic, 31.0% the family planning clinic, 14.7% the home delivery care, and the rest utilized other services such as the premarital guidance cinlic and the sanitary inspection service. 3) Maternity Care: 23.6% of 2,151 deliveries were done at medical facilities such as hospital, private clinic, while 76.4% were done at home. Acceptance rate of prenatal care was 32.6% as whole, but 49.6 of 774 women who had the prenatal care service had their deliveries at medical facility. 45.1% of total deliveries were attended by medical and or paramedical personnel. 75.8% of the deliveries of those received prenatal care were attended by medical and or paramedical personnel while only 27.8% of the deliveries of those who did not have prenatal care attended by medical and or paramedical personnel. 49.8% of deliveries of the upper class, 29.8% of the middle class and 9.9% of the lower class were attended by medical and or paramedical personnel. 6.2, 3.3% and 24.8% of mothers reported about their xeperience of edema, coma and fever during the period of trimester of pregnancy and puerperium. 4) Family Planning: The rate of practice of family planning was 27.9%. 31.7% of them were by IUD, 2.9% by oral pill, 15.2% by sterilization and the rest by traditional methods. Those women who had 3 to 4 children had highest(30.2%). Practice rate among the various methods of family planning, oral pill was the most popular method to whom had 2 or less children. In relation between the practicing rate of family planning and living standard, the upper, middle and lower class practiced 37.5, 29.4 and 19.9% respectively.

반복성 복통증 환아의 역학적 특징에 관한 조사 (A Study on Epidemiologic Characteristics of Recurrent Abdominal Pain in Elementary School Children)

  • 목 적: 학동기 소아들의 반복성 복통의 발생빈도, 임상증상, 의료기관 이용양상, 선택기준 및 관련된 요인을 조사하여 반복성 복통 환아의 특징, 증상의 발현 및 유지에 영향을 줄 수 있는 요인을 알아보고, 반복성 복통 환아의 의료기관 이용양상과 문제점을 파악하여 향후 치료를 위한 기초 자료를 제공하고자 실시하였다. 방 법: 1998년 6월 광주시내 초등학교 학부모 1417명을 대상으로 반복성 복통의 발생빈도, 임상증상, 의료기관 이용양상을 알아보기 위하여 설문조사를 실시하였다. 결 과: 1) 총응답자는 1417명, 남자 715명, 여자 702명이었고, 남녀비 1.02:1 평균나이는 10.3세였다. 2) 반복성 복통증의 발생 빈도는 18.9%이었다. 남자는 18.4%, 여자는 19.2%였다. 복통의 지속시간은 10분 이내가 68.5%였다. 3) 정확한 진단을 받지 못한 경우가 62.5%였고, 66.3%가 치료를 받았고, 67.5%가 치료시 증상 호전을 보였다. 민간요법을 실시한 경우도 66%를 차지하였다. 4) 치료 의료기관은 소아과 35.2%로 많았으나, 약국 및 내과의 이용률도 30%정도를 차지하였다. 연령이 증가함에 따라 소아과 이용률이 감소하고 내과의 이용율이증가하였다. 의료기관의 선택기준은 거리의 근접성이 36.3%로 가장 많았고, 의료인의 친절도, 치료의 질 순이었다. 5) 동반증상은 55.5%에서 있었고, 두통이 30.9%로 가장 많았고, 흉통, 현기증, 구토 순이었고, 동반 증상이 없는 경우도 44.5%였다. 6) 반복성 복통의 발생시기는 하루 중 식후가 35.3%로 가장 많았고, 식사전, 수업중 순이었다. 주내 변동은 월요일이 21.4%로 가장 많았으며, 점차 감소하여 토요일이 가장 낮았다. 7) 복통의 발생부위는 배꼽주위가 38%로 가장 많았고, 심와부 등 중앙부에 위치하였으며, 통증의 성격은 작열통이 36.9%로 가장 많았다. 결 론: 반복성 복통 환아는 대부분 정확한 진단을 받지 못하고, 전문적인 치료없이 대증적인 치료 및 민간요법이 많이 시행되고 있었다. 연령이 증가함에 따라 소아과 이외의 타과의 이용률이 증가하였다. 반복성 복통에 대한 올바른 이해와 함께 정확한 진단 및 전문적인 의료기관의 선택이 필요하고 이에 대한 홍보 및 대응책이 필요하다.

일본 '고증파(考證派)' 의학에 관한 연구 (A Study on The 'Kao Zheng Pai'(考證派) of The Traditional Medicine of Japan)

  • 1. The 'Kao Zheng Pai(考證派) comes from the 'Zhe Zhong Pai' and is a school that is influenced by the confucianism of the Qing dynasty. In Japan Inoue Kinga(井上金娥), Yoshida Koton(吉田篁墩) became central members, and the rise of the methodology of historical research(考證學) influenced the members of the 'Zhe Zhong Pai', and the trend of historical research changed from confucianism to medicine, making a school of medicine based on the study of texts and proving that the classics were right. 2. Based on the function of 'Nei Qu Li '(內驅力) the 'Kao Zheng Pai', in the spirit of 'use confucianism as the base', researched letters, meanings and historical origins. Because they were influenced by the methodology of historical research(考證學) of the Qing era, they valued the evidential research of classic texts, and there was even one branch that did only historical research, the 'Rue Xue Kao Zheng Pai'(儒學考證派). Also, the 'Yi Xue Kao Zheng Pai'(醫學考證派) appeared by the influence of Yoshida Kouton and Kariya Ekisai(狩谷掖齋). 3. In the 'Kao Zheng Pai(考證派)'s theories and views the 'Yi Xue Kao Zheng Pai' did not look at medical scriptures like the "Huang Di Nei Jing"("黃帝內經") and did not do research on 'medical' related areas like acupuncture, the meridian and medicinal herbs. Since they were doctors that used medicine, they naturally were based on 'formulas'(方劑) and since their thoughts were based on the historical ideologies, they valued the "Shang Han Ja Bing Lun" which was revered as the 'ancestor of all formulas'(衆方之祖). 4. The lives of the important doctors of the 'Kao Zheng Pai' Meguro Dotaku(目黑道琢) Yamada Seichin(山田正珍), Yamada Kyoko(山田業廣), Mori Ritsi(森立之) Kitamura Naohara(喜多村直寬) are as follows. 1) Meguro Dotaku(目黑道琢 1739${\sim}$1798) was born of lowly descent but, using his intelligence and knowledge, became a professor as a Shi Jing Yi(市井醫) and as a professor for 34 years at Ji Shou Guan mastered the "Huang Di Nei Jing" after giving over 300 lectures. Since his pupil, Isawara Ken taught the Lan Men Wu Zhe(蘭門五哲) and Shibue Chusai, Mori Ritsi(森立之), Okanishi Gentei(岡西玄亭), Kiyokawa Gendoh(淸川玄道) and Yamada Kyoko(山田業廣), Meguro Dotaku is considered the founder of the 'Yi Xue Kao Zheng Pai'. 2) The family of Yamada Seichin(山田正珍 1749${\sim}$1787) had been medical officials in the Makufu(幕府) and the many books that his ancestors had left were the base of his art. Seichin learned from Shan Ben Bei Shan(山本北山), a 'Zhe Zhong Pai' scholar, and put his efforts into learning, teaching and researching the "Shang Han Lun"("傷寒論"). Living in a time between 'Gu Fang Pai'(古方派) member Nakanishi Goretada(中西惟忠) and 'Kao Zheng Pai' member Taki Motohiro(多紀元簡), he wrote 11 books, 2 of which express his thoughts and research clearly, the "Shang Han Lun Ji Cheng"("傷寒論集成") and "Shang Han Kao"("傷寒考"). His comparison of the 'six meridians'(3 yin, 3 yang) between the "Shang Han Lun" and the "Su Wen Re Lun"("素問 熱論) and his acknowledgement of the need and rationality of the concept of Yin-Yang and Deficient-Replete distinguishes him from the other 'Gu Fang Pai'. Also, his dissertation of the need for the concept doesn't use the theories of latter schools but uses the theory of the "Shang Han Lun" itself. He even researched the historical parts, such as terms like 'Shen Nong Chang Bai Cao'(神農嘗百草) and 'Cheng Qi Tang'(承氣湯) 3) The ancestor of Yamada Kyoko(山田業廣) was a court physician, and learned confucianism from Kao Zheng Pai 's Ashikawa Genan(朝川善庵) and medicine from Isawa Ranken and Taki Motokata(多紀元堅), and the secret to smallpox from Ikeda Keisui(池田京水). He later became a lecturer at the Edo Yi Xue Guan(醫學館) and was invited as the director to the Ji Zhong(濟衆) hospital. He also became the first owner of the Wen Zhi She(溫知社), whose main purpose was the revival of kampo, and launched the monthly magazine Wen Zi Yi Tan(溫知醫談). He also diagnosed and prescribed for the prince Ming Gong(明宮). His works include the "Jing Fang Bian"("經方辨"), "Shang Han Lun Si Ci"("傷寒論釋司"), "Huang Zhao Zhu Jia Zhi Yan Ji Yao"("皇朝諸家治驗集要") and "Shang Han Ja Bing Lun Lei Juan"("傷寒雜病論類纂"). of these, the "Jing Fang Bian"("經方辨") states that the Shi Gao(石膏) used in the "Shang Han Lun" had three meanings-Fa Biao(發表), Qing Re(淸熱), Zi Yin(滋陰)-which were from 'symptoms', and first deducted the effects and then told of the reason. Another book, the "Jiu Zhe Tang Du Shu Ji"("九折堂讀書記") researched and translated the difficult parts of the "Shang Han Lun", "Jin Qui Yao Lue", "Qian Jin Fang"("千金方"), and "Wai Tai Mi Yao"("外臺秘要"). He usually analyzed the 'symptoms' of diseases but the composition, measurement, processing and application of medicine were all in the spectrum of 'analystic research' and 'researching analysis'. 4) The ancestors of Mori Rits(森立之 1807${\sim}$ 1885) were warriors but he became a doctor by the will of his mother, and he learned from Shibue Chosai(澁江抽齋) and Isawaran Ken and later became a pupil of Shou Gu Yi Zhai, a historical research scholar. He then became a lecturer of medical herbs at the Yi Xue Guan, and later participated in the proofreading of "Yi Xin Fang"("醫心方") and with Chosai compiled the "Jing Ji Fang Gu Zhi"("神農本草經"). He visited the Chinese scholar Yang Shou Jing(楊守敬) in 1881 and exchanged books and ideas. Of his works, there are the collections(輯複本) of "Shen Nong Ben Cao Jing"(神農本草經) and "You Xiang Yi Hwa"("遊相醫話") and the records, notes, poems, and diaries such as "Zhi Yuan Man Lu"("枳園漫錄") and "Zhi Yuan Sui Bi"("枳園隨筆") that were not published. His thoughts were that in restoring the "Shen Nong Ben Cao Jing", "the herb to the doctor is like the "Shuo Wen Jie Zi"("說文解字") to the scholar", and he tried to restore the ancient herbal text using knowledge of medicine and investigation(考據). Also with Chosai he compiled the "Jing Ji Fang Gu Zhi"("經籍訪古志") using knowledge of ancient text. Ritzi left works on pure investigation, paid much attention to social problems, and through 12 years of poverty treated all people and animals in all branches of medicine, so he is called a 'half confucianist half doctor'(半儒半醫). 5) Kitamurana Ohira(喜多村直寬 1804${\sim}$1876) learned scriptures and ancient texts from confucian scholar Asaka Gonsai, and learned medicine from his father Huai Yaun(槐園). He became a teacher in the Yi Xue Guan in his middle ages, and to repay his country, he printed 266 volumes of "Yi Fang Lei Ju("醫方類聚") and 1000 volumes of "Tai Ping Yu Lan"("太平禦覽") and devoted it to his country to be spread. His works are about 40 volumes including "Jin Qui Yao Lue Shu Yi" and "Lao Yi Zhi Yan" but most of them are researches on the "Shang Han Za Bing Lun". In his "Shang Han Lun Shu Yi"("傷寒論疏義") he shows the concept of the six meridians through the Yin-Yang, Superficial or internal, cold or hot, deficient or replete state of diseases, but did not match the names with the six meridians of the meridian theory, and this has something in common with the research based on the confucianism of Song(宋儒). In clinical treatment he was positive toward old and new methods and also the experience of civilians, but was negative toward western medicine. 6) The ancestor of the Taki family Tanbano Yasuyori(丹波康賴 912-955) became a Yi Bo Shi(醫博士) by his medical skills and compiled the "Yi Xin Fang"("醫心方"). His first son Tanbano Shigeaki(丹波重明) inherited the Shi Yao Yuan(施藥院) and the third son Tanbano Masatada(丹波雅忠) inherited the Dian You Tou(典藥頭). Masatada's descendents succeeded him for 25 generations until the family name was changed to Jin Bao(金保) and five generations later it was changed again to Duo Ji(多紀). The research scholar Taki Motohiro was in the third generation after the last name was changed to Taki, and his family kept an important part in the line of medical officers in Japan. Taki Motohiro(多紀元簡 1755-1810) was a teacher in the Yi Xue Guan where his father was residing, and became the physician for the general Jia Qi(家齊). He had a short temper and was not good at getting on in the world, and went against the will of the king and was banished from Ao Yi Shi(奧醫師). His most famous works, the "Shang Han Lun Ji Yi" and "Jin Qui Yao Lue Ji Yi" are the work of 20 years of collecting the theories of many schools and discussing, and is one of the most famous books on the "Shang Han Lun" in Japan. "Yi Sheng" is a collection of essays on research. Also there are the "Su Wen Shi"("素問識"), "Ling Shu Shi"("靈樞識"), and the "Guan lu Fang Yao Bu"("觀聚方要補"). Taki Motohiro(多紀元簡)'s position was succeeded by his third son Yuan Yin(元胤 1789-1827), and his works include works of research such as "Nan Jing Shu Jeng"("難經疏證"), "Ti Ya"("體雅"), "Yao Ya"("藥雅"), "Ji Ya"("疾雅"), "Ming Yi Gong An"("名醫公案"), and "Yi Ji Kao"("醫籍考"). The "Yi Ji Kao" is 80 volumes in length and lists about 3000 books on medicine in China before the Qing Dao Guang(道光), and under each title are the origin, number of volumes, state of existence, and, if possible, the preface, Ba Yu(跋語) and biography of the author. The younger sibling of Yuan Yin(元胤 1789-1827), Yuan Jian(元堅 1795-1857) expounded ancient writings at the Yi Xue Guan only after he reached middle age, was chosen for the Ao Yi Shi(奧醫師) and later became a Fa Yan(法眼), Fa Yin(法印) and Yu Chi(樂匙). He left about 15 texts, including "Su Wen Shao Shi"("素間紹識"), "Yi Xin Fang"("醫心方"), published in school, "Za Bing Guang Yao"("雜病廣要"), "Shang Han Guang Yao"(傷寒廣要), and "Zhen Fu Yao Jue"("該腹要訣"). On the Taki family's founding and working of the Yi Xue Guan Yasuka Doumei(失數道明) said they were "the people who took the initiative in Edo era kampo medicine" and evaluated their deeds in the fields of 'research of ancient text', 'the founding of Ji Shou Guan and medical education', 'publication business', 'writing of medical text'. 5. The doctors of the 'Kao Zheng Pai ' based their operations on the Edo Yi Xue Guan, and made groups with people with similar ideas to them, making a relationship 'net'. For example the three families of Duo Ji(多紀), Tang Chuan(湯川) and Xi Duo Cun(喜多村) married and adopted with and from each other and made prefaces and epitaphs for each other. Thus, the Taki family, the state science of the Makufu, the tendency of thinking, one's own interests and glory, one's own knowledge, the need of the society all played a role in the development of kampo medicine in the 18th and 19th century.

He then became a lecturer of medical herbs at the Yi Xue Guan, and later participated in the proofreading of "Yi Xin Fang"("醫心方") and with Chosai compiled the "Jing Ji Fang Gu Zhi"("經籍訪古志"). He visited the Chinese scholar Yang Shou Jing(楊守敬) in 1881 and exchanged books and ideas. Of his works, there are the collections(輯複本) of "Shen Nong Ben Cao Jing"("神農本草經") and "You Xiang Yi Hwa"("遊相醫話") and the records, notes, poems, and diaries such as "Zhi Yuan Man Lu"("枳園漫錄") and "Zhi Yuan Sui Bi"(枳園隨筆) that were not published. His thoughts were that in restoring the "Shen Nong Ben Cao Jing", "the herb to the doctor is like the "Shuo Wen Jie Zi"(說文解字) to the scholar", and he tried to restore the ancient herbal text using knowledge of medicine and investigation(考據), Also with Chosai he compiled the "Jing Ji Fang Gu Zhi"("經籍訪古志") using knowledge of ancient text. Ritzi left works on pure investigation, paid much attention to social problems, and through 12 years of poverty treated all people and animals in all branches of medicine, so he is called a 'half confucianist half doctor'(半儒半醫). 5) Kitamurana Ohira(喜多村直寬, $1804{\sim}1876$) learned scriptures and ancient texts from confucian scholar Asaka Gonsai(安積艮齋), and learned medicine from his father Huai Yaun(槐園), He became a teacher in the Yi Xue Guan in his middle ages, and to repay his country, he printed 266 volumes of "Yi Fang Lei Ju"("醫方類聚") and 1000 volumes of "Tai Ping Yu Lan"("太平禦覽") and devoted it to his country to be spread. His works are about 40 volumes including "Jin Qui Yao Lue Shu Yi"("金匱要略疏義") and "Lao Yi Zhi Yan"(老醫巵言) but most of them are researches on the "Shang Han Za Bing Lun". In his "Shang Han Lun Shu Yi"("傷寒論疏義") he shows the concept of the six meridians through the Yin-Yang, Superficial or internal, cold or hot, deficient or replete state of diseases, but did not match the names with the six meridians of the meridian theory, and this has something in common with the research based on the confucianism of Song(宋儒). In clinical treatment he was positive toward old and new methods and also the experience of civilians, but was negative toward western medicine. 6) The ancestor of the Taki family Tanbano Yasuyori(丹波康賴 $912{\sim}955$) became a Yi Bo Shi(醫博士) by his medical skills and compiled the "Yi Xin Fang"("醫心方"). His first son Tanbano Shigeaki(丹波重明) inherited the Shi Yao Yuan(施藥院) and the third son Tanbano Masatada(丹波雅忠) inherited the Dian You Tou(典藥頭). Masatada's descendents succeeded him for 25 generations until the family name was changed to Jin Bao(金保) and five generations later it was changed again to Duo Ji(多紀). The research scholar Taki Motohiro was in the third generation after the last name was changed to Taki, and his family kept an important part in the line of medical officers in Japan. Taki Motohiro(多紀元簡 $1755{\sim}1810$) was a teacher in the Yi Xue Guan where his father was residing, and became the physician for the general Jia Qi(家齊). He had a short temper and was not good at getting on in the world, and went against the will of the king and was banished from Ao Yi Shi(奧醫師). His most famous works, the "Shang Han Lun Ji Yi"("傷寒論輯義") and "Jin Qui Yao Lue Ji Yi"("金匱要略輯義") are the work of 20 years of collecting the theories of many schools and discussing, and is one of the most famous books on the "Shang Han Lun" in Japan. "Yi Sheng"("醫勝") is a collection of essays on research. Also there are the "Su Wen Shi"(素問識), "Ling Shu Shi"("靈樞識"), and the "Guan Ju Fang Yao Bu"("觀聚方要補"). Taki Motohiro(多紀元簡)'s position was succeeded by his third son Yuan Yin(元胤 $1789{\sim}1827$), and his works include works of research such as "Nan Jing Shu Jeng"(難經疏證), "Ti Ya"("體雅"), "Yao Ya"("藥雅"), "Ji Ya"(疾雅), "Ming Yi Gong An"(名醫公案), and "Yi Ji Kao"(醫籍考). The "Yi Ji Kao" is 80 volumes in length and lists about 3000 books on medicine in China before the Qing Dao Guang(道光), and under each title are the origin, number of volumes, state of existence, and, if possible, the preface, Ba Yu(跋語) and biography of the author. The younger sibling of Yuan Yin(元胤 $1789{\sim}1827$), Yuan Jian(元堅 $1795{\sim}1857$) expounded ancient writings at the Yi Xue Guan only after he reached middle age, was chosen for the Ao Yi Shi(奧醫師) and later became a Fa Yan(法眼), Fa Yin(法印) and Yu Chi(禦匙). He left about 15 texts, including "Su Wen Shao Shi"("素問紹識"), "Yi Xin Fang"("醫心方"), published in school, "Za Bing Guang Yao"("雜病廣要"), "Shang Han Guang Yao"("傷寒廣要"), and "Zhen Fu Yao Jue"("診腹要訣"). On the Taki family's founding and working of the Yi Xue Guan Yasuka Doumei(矢數道明) said they were "the people who took the initiative in Edo era kampo medicine" and evaluated their deeds in the fields of 'research of ancient text', the founding of Ji Shou Guan(躋壽館) and medical education', 'publication business', 'writing of medical text'. 5. The doctors of the 'Kao Zheng Pai' based their operations on the Edo Yi Xue Guan, and made groups with people with similar ideas to them, making a relationship 'net'. For example the three families of Duo Ji(多紀), Tang Chuan(湯川) and Xi Duo Cun(喜多村) married and adopted with and from each other and made prefaces and epitaphs for each other. Thus, the Taki family, the state science of the Makufu, the tendency of thinking, one's own interests and glory, one's own knowledge, the need of the society all played a role in the development of kampo medicine in the 18th and 19th century.

종합병원의 뇌.척수.근골격계 입원환자의 가정간호요구 (The Demand for Home Nursing Care of Hospital Inpatients in Brain-Spine and Musculoskeletal Diseases)

  • 본 연구는 입원환자들의 가정간호사제도에 대한 찬반여부 및 가정간호 서비스 내용과 그 요구도를 파악하기 위하여 대구시내 3개 종합병원에서 1993년 9월 6일부터 10월 9일까지 뇌질환, 척추질환 및 근골격계질환으로 입원한 환자중 퇴원예정에 있는 환자 242명을 대상으로 설문지에 응답하도록 하여 자료를 수집하였다. 가정간호제도를 찬성한 환자는 66.1%(161명)이었으며 반대는 33.5%(81명)이었다. 환자의 일반적 특성별로는 30세에서 49세 사이가 75.5%, 중소도시에 거주하는 75.9%로 높은 찬성율을 보였다. 특히 의료보호 환자군에서는 40.0% 낮은 찬성율을 보였다. 척추손상 환자의 75.6%가 찬성한 반면 복합질 환자는 54.5%의 찬성율을 보였다. 그리고 거동 가능한 환자의 69.4%, 임종을 기다리는 환자의 80.0% 및 조기퇴원을 희망하는 환자의 73.9%가 가정 간호사제도를 찬성하였다. 가정간호사제도를 찬성하는 환자(161명)의 찬성 이유는 주치의와 계속적인 연결을 갖고 싶어서가 37.3%로 가장 높았고, 그 다음 병원에 오기가 블편해서였고 기능적 상태에 따라 찬성 이유간에는 유의한 차이가 있었다(p<0.05). 가정간호사제도를 반대하는 환자(81명)의 이유를 보면 병원보다 질이 낮을 것같다가 가장 많았고, 방문이 번거롭다 및 가정에서 가족이 돌볼 수 있어서의 순이였다. 조기퇴원을 원하는 환자는 74.8%였으며 조기퇴원 이유는 병원생활이 지루하기 때문에가 57.5%로 가장 많았고, 조기퇴원을 원하지 않는 환자는 질병상태에 대한 불안감 때문에가 54.0%로 가장 높았다. 가정간호사제도를 찬성한 환자들에게 가정간호서비스 내용을 23개 항목으로 나눈 요구 빈도에서 앞으로 수혜받기를 원하는 가정간호활동 내용은 회복촉진, 합병증예방, 상담 및 건강관리지도로 76.4%로 가장 많았으며, 그 다음의 투약관리 및 지도 (62.1%), 활력증상의 정기적인 측정(555.9%)의 순이었고 임종간호(3.7%) 및 호흡유지를 위한 간호(9.9%) 등은 요구빈도가 가장 낮았다. 가정간호서비스 항목에 있어서 진단내용, 환자의 기능적 상태, 합병증 유무 등에 따라서는 큰 차이 없이 회복촉진, 합병증예방상담 및 건강관리지도 항목에서 가장 높은 요구도를 나타냈고 나머지 간호요구는 큰 차이가 없었다.

물리치료사의 업무 스트레스 현황과 대응수준 -부산지역을 중심으로- (An Analysis of Work Stress of Physical Therapist and Reaction)

  • This study aims to enhance work efficiency and satisfaction by offering data that make a physical therapist cope with stress coming from the job efficiently by identifying and analyzing the job satisfaction and work stress perceived by a physical therapist, and the level of reaction of a physical therapist. The study distributed survey questionnaires to 300 physical therapists working in medical institutions in Busan via mail and by visit from March 3, 2007 by selecting them simply and randomly from the physical therapist list, and collected the questionnaires by March 20, 2007, 103 respondents were working at 17 general hospitals including a university hospital, 65 respondents at 12 medical centers, and 79 respondents at 39 doctor's offices. The study collected 251 copies, which showed the collection rate of 83.7%, and analyzed 247 copies (82.3%) excluding 4 copies of insincere answers. As a research tool for measuring job satisfaction the study used a tool employed for research into the job satisfaction of physical therapists who work at medical institutions in Gwangju, Jeonnam, and Jeonbuk by Kim Hee-Gwon(1992) and research regarding job satisfaction by Jeong Jeong-Hee(2004) as well as research regarding the factors of job satisfaction by Flippo(1980) & Seberhagen(1970) after adjusting the research tools to the purpose of the study. Also for questions about work stress, the study employed nurses' job stress measurement tool developed by Kim Mae-Ja and em Mi-Ok(1984) by modifying the tool to the purpose of the study, and for a measurement tool for reaction to stress, the study used a tool employed for research into reaction to stress of nurses at general hospitals by Choi Eun-Deok(2005) without modification. For data analysis, the study used the SPSS12.0 as a statistical method, and then used t-test or ANOVA for verifying actual numbers, percentile, average :score, standard deviation, rank, and difference. Also, the study conducted which is a post-test method for variables that show a significant difference at the level of p<.05 level after the analysis. The findings include the following. 1) The respondents' job satisfaction score was 3.21 points on the average (out of 5 full points). The peer relationship ranked the highest, posting 4.02 points on the average, and the job satisfaction with rewards was proven the lowest, posting 2.51 points. For the job satisfaction level by characteristics, there were significant differences (p<.05) in gender, hospital type, weekly working hours, monthly working days, number of patients per day, department in charge of therapy, and number of peers, and there was no significant difference in characteristics other than that. 2) The respondents' work stress score was 2.72 points (out of 5 full points) on the average. The respondents were shown to be under the highest stress when they suffered from excessive workload, posting 3.49 points on the average, and they were shown to be under the least stress when they had a conflict with peers at another department, recording 1.90 points on the average. for the job stress level by the characteristics of job, there was a significant difference in the reflection of job assessment(p<.05). 3) 1n respondents' reaction to stress, most of them were shown to make efforts in coping with stress, posting 2.80 points (out of 5 full points). For their experience of being wider stress, they answered that 'they felt depressed (2.85 points)" for their experience of coping with stress, they answered that 'they were indifferent to it or thought about something else' (2.62 points). Also, for their efforts in coping with stress, they answered that 'they were motivated to remove their strain by taking leave, playing, or using their preferences' (3.52 points), which ranked higher. For the level of reaction to stress by characteristics, there were significant differences by age, gender, marital status, total service years as a physical therapist, monthly working days, and department in charge of therapy(p<.05). It is necessary to offer correct information by conducting an in-depth analysis of the stressful situations of physical therapists who exert efforts in rehabilitating patients at hospitals by factor, and seeking management plans based on the research results. Also, it is necessary to develop a program for coping with stress efficiently for removing stress and to conduct research into the understanding and cooperation of administrators and persons in charge of physical therapists for reducing physical therapists' stress at hospitals.

초등학생들의 의료기관 이용양상 및 선택 기준에 관한 연구 (A Study on the Status of Utilization and Criteria for Selection of Medical Care Facilities of Elementary School Children)

  • 목 적 : 학동기 소아들의 상병상태, 의료기관 이용 양상, 선택기준 및 관련된 요인 조사하여 현재 소아 환자의 진료의 실태와 문제점을 파악하여 향후 대책을 위한 자료를 제공하고자 실시하였다. 방 법 : 학동기 아동의 의료기관 이용율과 그 관련 요인을 조사 분석하기 위하여 1998년 6월 광주시내 초등학교 학부모 2,036명을 대상으로 설문 조사를 하여 자료를 분석하였다. 결 과 : 1) 총응답자는 2,036명, 남자 1,035명, 여자 1,001명이었고, 남녀비 1.03 : 1 평균나이는 10.6세였다. 2) 질환의 유병률은 32.3%였고, 질환을 계통별로 보면 호흡 및 알레르기 질환 64.7%, 소화기 질환 12.8%, 외상, 치과질환, 기타 순이었다. 치료율은 89.9%였으며, 치료하지 않은 이유로 경미한 증상, 경제적 부담, 바쁜 생활 순이었다. 3) 질환에 이환 된 경우 주로 이용하는 의료기관은 소아과 46.7%, 이비인후과 19.8%, 약국 13.2%, 내과 12.2%, 가정의학과 순이었다. 4) 의료기관의 선택이유는 거리의 근접성, 치료효과, 교통의 편리성, 의사의 유명도, 의료인의 친절도 순이었다. 각 의료기관별 선택이유는 소아과의 경우 거리의 근접성, 이비인후과의 경우 치료 효과, 내과의 경우 의사의 평판이 많은 빈도를 차지하였다. 5) 호흡기 증상의 의료기관 이용도를 보면, 기침의 경우 소아과, 약국, 이비인후과, 내과 순이었고, 콧물의 경우 소아과, 약국, 이비인후과, 내과 순이었다. 열이 있는 경우 소아과, 약국, 이비인후과, 내과 순이었다. 6) 소화기 증상의 의료기관 선호도를 보면, 설사의 경우 소아과, 약국, 내과 순이었고, 변비의 경우 소아과, 약국, 내과 순이었다. 복통의 경우 소아과, 내과, 약국 순이었다. 예방접종은 보건소, 소아과, 내과 순이었고 피부 증상이 있는 경우에 소아과, 피부과, 약국, 내과 순이었다. 7) 연령에 따른 의료기관 선호도는 호흡기 질환의 경우 소아과 이용율의 경우 7세 50%가 점차 감소하여 13세 때는 33.8%로 감소한 반면, 이비인후과의 경우 7세 26.7%, 13세 때 22%로 별 변화 없었으나 내과의 경우 7세 6.7%, 13세 때 10.1%로 약간 증가하였다. 소화기 질환의 경우 소아과 이용율은 7세 때 45.4%였으나 13세 때는 20.2%로 크게 감소한 반면, 내과 이용율은 7세 때 25%, 13세 때 34.8%로 증가하였다. 특히 11세경부터 내과 이용율이 소아과의 이용율을 앞서고 있었다. 8) 의료기관선택에 가장 많은 영향을 미치는 사람은 어머니, 아버지, 의사의 추천, 친구 이웃 순이었다. 9) 소아과 진료의 적정연령에 대하여서는 12세까지 47.8%, 10세까지 22.4%, 15세까지 18.5%, 8세까지 10.1% 순이었다. 결 론 : 소아과 진료영역의 많은 부분이 타과에서 쉽게 진료되고 있어 이에 대한 홍보와 대응책이 필요할 것이다. 소아는 성인과 달리 독특한 성장과 발달과정이 있으므로 전문적인 진료 및 적절한 치료기관의 선택이 필요할 것이다.

47계대 Oka주 약독화 생백신 접종 후 2년간 임상 및 면역성의 추적 연구 (Tow-Year Follow-up Study for Clinical Feature and Immunity of The Children, Vaccinated by 47 Passaged Oka Strain Live Attenuated Varicella Vaccine)

  • 목 적 : 국내 건강한 소아에서 47계대 배양 Oka주 수두 약독화 생백신을 접종한 후 단기간 내의 면역원성과 안전성에 관한 연구를 시행한 다음 이들 접종아의 장기간에 걸친 접종효과와 면역성 유지에 관한 연구계획에 따라 일차적으로 접종 2년내에 수두 발생 여부와 발생시 임상적 특성을 확인하고 접종 2년 후 수두에 관한 면역상태의 평가와 잠복감염의 활성 여부를 파악하고자 하였다. 대상 및 방법 : 1997년 4월에서 8월 사이 가톨릭의대 소아과에서 시행하였던 47계대 Oka주 수두 약독화 생백신의 단기간 면역원성과 안전성의 연구 대상아 99명을 본 연구의 지속적인 동일 대상으로 하였다. 연구 대상아를 2년간 지속적으로 추적하여 수두의 발생여부를 확인하였다. 접종아중 수두에 이환 되었을 경우 발병기간, 발열을 포함한 피부병변의 정도, 합병증의 발생 여부, 치료투약의 여부 등을 파악하여 임상적 평가를 실시하고, 동시에 발생의 역학 상태를 파악하였다. 그리고 이 연구 기간 동안에 연구 대상아에서 잠복감염에 따른 대상포진의 발생을 동시에 관찰하였다. 또한 접종 2년후에 EIA와 FAMA의 면역학적 검사를 통하여 수두 대상포진 바이러스 IgG 항체와 수두 대상포진 바이러스의 세포막 항원에 대한 항체를 측정하여 접종 2년 후의 면역상태를 평가하였다. 그리고 이 결과를 통하여 2년 전에 확인하였던 접종 1개월 후 면역상태와 비교하였다. 결 과 : 연구대상자 99명을 2년간 추적한 결과 6명에서 수두가 발생되었다. 이들은 3일에서 5일 정도의 짧은 임상 경과를 보였고 2례에서 발열이 있었고 전 예에서 발진의 정도는 수포의 수나 확산 정도가 매우 미약하였으며 합병증을 보인 경우는 없었다. 그리고 치료를 위한 투약을 받은 경우는 없었고 전 예에서 가족내 또는 집단 발생 후 발생되었다. 2년 후 항체검사가 가능하였던 46명에서 EIA에 의한 수두 대상포진 바이러스 IgG 항체를 측정한 결과 1례를 제외한 전 예에서 양성을 보였으며 높은 항체가를 유지되었음을 알 수 있었고, FAMA에 의한 수두 대상포진 바이러스 세포막 항원에 대한 항체가는 전 예에서 양성을 보였다. 이들 추적 항체검사가 가능하였던 46명에서 접종 1개월 후 항체가와 2년 후 항체가를 비교한 결과 수두 대상포진 바이러스 IgG 항체와 수두 대상포진 바이러스 세포막 항원에 대한 항체는 모두 접종 2년 후에 높게 유지됨을 알 수 있었다. 그러나 전 예에서 연구기간 동안 대상포진의 발생은 관찰되지 않았다. 결 론 : 47계대 배양 Oka주 수두 약독화 생백신 접종 후 2년간 추적한 결과 접종아에서 집단 발생과 더불어 소수에서 수두가 발생되었으나 그 정도는 매우 미약하여 임상적으로 큰 문제가 없고 접종 2년 후에도 수두 대상포진 바이러스에 대한 면역상태는 잘 유지되고 있음을 확인하였다.

보건소(保健所) 행정(行政)의 기선을 위(爲)한 연구(硏究) (A Study on the Administrative Enhancement for Health Center Activities)

  • This survey was conducted to evaluate not only the present status of health center directors-their personal histories, their will to private practice in the future, their responses to governmental policies, -but also the distribution of doctorless myons, budget and subsidy, and director's opinions to the enhancement of health center activities. This survey questioned 116 health center directors and 16 health personnel from August to October of 1970 and obtained the following results; 1) The average ages of directors of kun, city, and total health centers were $43.2{\pm}7.8,\;42.1{\pm}7.7,\;and\;42.9{\pm}10.3$ respectively. 2) The average family sizes of directors of kun, city, and total health centers were $5.6{\pm}2.7,\;5.6{\pm}2.1,\;and\;5.6{\pm}2.6$ respectively. 3) Directors holding M. D. degrees were 79.3%, those holding qualified M. D. degrees ('approved director') were 20.7%. 4) M. P. H., M. S., and Ph. D. holders were 6.0%, 6.1%, and 4.3% respectively. 5) The average duration of present directorship in kun and city were 30.2 months and 20.4 months respectively. 6) The majority of directors had been employed in related fields before assuming current position : directorship at other health center 26.7%, army 22.4%, health subcenter 21.6%, private practice 19.0%. 7) Average length of directorship is 41.8 months. Average length of public health career, including health subcenter and present position, is 56.5 months. 8) Both rural and urban experience in health centers for regular directors is 16.3% and for approved directors, 12,5%. A total of 15.5% of all survey directors had experience in both rural and urban health center. 9) A total of 70.7% of health center directorships were staffed by local doctors. 10) Nearly 40% wanted to quit the directorships within 3 years and 60.3% had already experienced private practice. 11) Of the regular directors 17.4% felt strongly about devoting their lives to public health fields, but only 4.1% of the approved approved directors felt so. 12) There wire 432 doctorless myons among 996 respondent myons and 4.5 doctorless myons per kun. 13) The percentage of doctorless myon by Province are as follows, Cholla buk-do 57.2%, Cholla nam-de 55.0%, Kyungsang nam-do 52.0%, Kyungsang buk-do 49.7%, Chungchong but-do 42.4%, Kyonggi-do 32.9%. Cheju-do 30.8%, Kangwon-do 25.8%. 14) Two thirds of health critters have experienced the abscence of the director for a certain period since 1966 and the average span of the abscence was 18.2 months. 15) The percentage of doctorless myons increased proportionally with the span of the director's abscence. 16) The average budgets of health centers, kun, city and ku, were $W15.03\;million{\pm}W4.5\;million,\;W22.03\;million{\pm}W17.80\;million,\;W13.10\;million{\pm}W7.9\;million$ respectively. 17) Chunju city had the highest health budget per capita(W344) while Pusan Seo ku had the lowest(W19). 18) Director's medical subsidies are W30,000-50,000 in kun, and roughly W20,000 in city. 19) The older of priority in health center activities is T.B. control(31.1%), Family Planning and M. H.(28.0%), prevention of acute communicable disease and endemic disease (18.2%) and clinical care of patients(14.3%). 20) Nearly 32% opposed in principle the governmental policy of prohibiting medical doctors from going abroad. 21) Suggestions for immediate enhancing the position of director of health centers and subcenters: (1) Raise the base subsidy (48.2%), (2) Provide more opportunities for promotion (20.7%), (3) Exemption from army services(12.1%), (4) Full scholarship to medical students for this purpose only (7.8%). 22) A newly established medical school was opposed by 56.9% of the directors, however 33.6% of them approved. 23) Pertaining to the division of labor in Medicine and Pharmacy, the largest portion (31.9%) urged the immediate partial division of antibiotics and some addictive drugs to be given only by prescription. 24) More than half wanted a W70,000 level for the director's medical subsidies, white 36.2% stated W50,000. 25) Urgently needed skills in the kun are clinical pathologist (38.6%) and doctor (health center director) (25.5%); while in the city nurse (37.1%), doctors(clinical)(31.4%) and health educators(14.4%) are needed. 26) Essential treatment for the better health center administration; raising the base subsidy (22.7%), obtaining the power of personal management (19.3%) and the establishment of a Board of Health (14.3%). etc.

가족 건강관리 행위에 관한 조사연구 -서울시내 일부 기혼부인들을 대상으로- (A Survey Research on Family Health Care : Focusing on Married Women in Seoul)

  • Recently diseases related to personal health habit and lifestyle have become common in modern industrial society. These kinds of diseases can be prevented simply by changing one's lifestyle to be more healthy. As a result of realization our interest in general health has become stronger. The most basic environment for human-being in society is the home. Humans secure their livelihood, physically. mentally, and socially at home. Therefore health care at home is very important. In modern society the responsibility for this task is traditionally given to housewives. The purpose of this study was to measure the degree of the health knowledge, health concern, health behavior and family health care of the married women and to analyze its related factors. The subjects for this study. 1,100 married women who studied at social education institutes and who had children attending an elementary school or a kindergarten, were surveyed with questionnaires. The preliminary survey was carried out from Aug. 7, to Aug. 19, 1995. With complement of questions, the main survey was carried out from Sep. 11, to Sep. 30, 1995. The data was analysed by using the SAS program. The results were as follows. 1. General Characteristics (1) In the individual characteristics of the respondents, the married women aged 30-39 were 54.8%, the average age was 39.8 years old. 33.8% of respondents had 6-10 years of marriage period, and the average marriage period was 14.9 years. Most of them(96.5%) lived with their husband. Those who graduated from college and graduate school were 53.4%. And 68.3% of respondents had no job. (2) In the family characteristics, 69.3% of the married women had 3 or 4 family members and the average family size was 4.1 person. 60.0% of the respondents had 2 children. Most of the respondents(90.9%) had no married children. 84.8% of the respondents lived with their parents. Those who reported that the total family income was more than 2,500,000 won a month were 32.3%. When making the decisions, 68.5% of the married women discussed the family matter with their husband. (3) In the individual characteristics of the respondents, 51.5% answered they were in good health. 61.7% of the married women answered they obtained the health knowledge through mass media. 24.3% of the women answered they had patients in their family in these days. 67.5% of the respondents answered they could generally control their health by themselves. 2. The Health Knowledge, Concern and Behavior. (1) For the health knowledge, the average score was 11.8. The lowest percent of correct answer(27.8%) was in the item about the skin tests for tuberculosis. And the highest percent(97.%) was in the item about taking a rest. (2) For the health concern, the married women had the highest concern about washing hands. But they were indifferent to smoking. (3) For the health behavior, the highest score was in "changing socks and underwear everyday", and the lowest one was in "taking a regular dental examination". 3. The Family Health Care (1) For the family health care, the item of "using a drug with the order of doctor or pharmacist" had the highest grade(4.78), and "consulting with the family physician about the health problem" had the lowest grade(2.03). (2) Older women and the women with a longer period of marriage had the highest level of the family health care(p<0.001). The married women who had 3 children had the highest level of the family health care(p<0.001). Those who had 5 or 6 family member and higher income had the highest level had the high level of the family health care(p<0.01). Women in good health and those who had the health knowledge from health experts had a high level of the family health care. (3) For the correlation of the family health care and other variables, the health behavior showed the highest correlation with family heath care practice(r=0.74) and the second was health concern(r=0.43). The variables which could explain the family health care were health behavior, the health concern and married women's health status(r²=55.87). The most closely associated with family health care was health behavior(r²=54.93)
