Vascularisation of Urethral Repairs with the Gracilis Muscle Flap

  • Background The ability to achieve a long-term, stricture-free urethral repair is one of the ongoing challenges of reconstructive urologic surgery. A successful initial repair is critical, as repeat procedures are difficult, owing to distortion, scarring, and short urethral stumps. Methods We describe a technique in which the gracilis muscle flap is laid on or wrapped around the urethral repair site to provide a well-vascularised soft tissue reinforcement for urethral repair. This technique promotes vascular induction, whereby a new blood supply is introduced to the repair site to improve the outcome of urethral repair or anastomotic urethroplasty. The surface contact between the muscle flap and the repair site is enhanced by the use of fibrin glue to improve adherence and promote inosculation and healing. We employed this technique in 4 patients with different urethral defects. Results After a follow-up period of 32 to 108 months, all of the urethral repairs were successful without complications. Conclusions Our results suggest that the use of a gracilis muscle flap to vascularise urethral repairs can improve the outcome of challenging urethral repairs.

  • Various surgical techniques, such as endoscopic surgery and robotic surgery, are developed to optimize the esthetic outcome even in operations for malignancy. A modified face-lift or retroauricular approach are used to minimize postoperative scarring. Recently, robot-assisted surgery is being done in various fields and considered as favorable treatment method by many surgeons. However its high cost is a nonnegligible fraction for many patients. On the other hand, endoscopic surgery, which is cheaper than robotic surgery, is minimally invasive with contentable neck dissection. Although it is a difficult technique for a beginner surgeon due to its limited operation view, we suppose it as an alternative method for robotic surgery. Herein, we report two cases of endoscopic neck dissection via retroauricular incision with a discussion regarding the pros and cons of endoscopic neck dissection.

EFFICACY AND BIOLOGICAL ACTIVITIES OF A NEW ANTI-AGING AGENT OBTAINED FROM ARECA CATECHU

  • Inhibitory effects of the new material obtained from Areca catechu seed (CC-516) according to a special process, and its applicability to the skin as a cosmetic raw material in terms of its efficacy were presented. Areca catechu extract out of 150 medicinal plants, exhibited high inhibitory effect on the porcine pancreatic elastase ($IC_{50}$ : $40.8{\mu}$g/ml). It also had an inhibitory effect on the human leukocyte elastase ($IC_{50}$ : 48.1$\mu$g/ml), hyaluronidase ($IC_{50}$ : $416{\mu}$g/ml), antioxidative activity ($IC_{50}$ : $45.4\mu$g/ml) and free radical scavenging activity ($SC_{50}$ : $10.2{\mu}$g/ml). The cream contained 3% of CC-516 improved skin hydration above 16.5%. Especially, the skin elasticity increases more than 35% and skin wrinkles decreased more than 23%. The CC-516 was designed to be utilized in cosmetology. The cream containing 3% of this product has not only protecting effect on the skin mechanical properties provided by the collagen and the elastin in the derm but also restructuring effect of scarring or aging tissue.

흉터복원술과 자동 미세침(Auto Microneedle Therapy System, AMTS)을 이용한 여드름 흉터의 치험례 (Case Study of Treating Acne Scars Using Reconstruction of Skin Scars and Auto Microneedle Therapy System)

  • Objective : Acne is very common disease, it can leave permanent scars. However, there are few reports about it in oriental medicine academia. This study performed to evaluate the effect of reconstruction of skin scars and auto microneedle therapy system on acne scars. Methods : Six patients were treated by using reconstruction of skin scars and auto microneedle therapy system. We compared the degree of treatment of acne scars on pre and post case photos and Qualitative Global Acne Scarring Grading System(QGASC). Satisfaction degree were asked from the patients. Results & Conclusion : After comparing case photos and QGASC, observing patients satisfaction, it can be considered that reconstruction of skin scars and auto microneedle therapy system to be valid acne scars treatment.

의료용 화상환자 압박복의 제조 국가별 비교 (Comparison of Medical Compression Garments by Manufacturing Country)

  • A special medical compression garment has been developed to be worn after surgery or treatment using an elastic fiber in which the amount of compression can be adjusted to a specified targeted area to reduce the formation of hypertrophic scarring. In order to develop this medical compression garments, specialized technical skill in fiber, compression class and body measurements are needed. This study provides product survey data for the development of medical compression garments. An investigation analyzed medical compression garments of 16 brands from 6 countries sold in the Korean market & online, such as Make Me Heal, Jobst, Bio Concepts, Design Veronique$^{(R)}$ Nouvelle and Leonisa$^{(R)}$ (USA), Respire(Germany), Malcom$^{(R)}$, Holistic garments and Jobskin(UK), Technomed, kanav Healthcare and Sindhoori surgicals(India), Soo medical and C&C medical(Korea), Secondskin(Australia). The main fibers of compression garments were nylon and spandex: in addition, the fiber content was different by country and brand. The number of compression classes of products was USA(4), UK(4), and India(4). For body measurements, USA and many brands used (bust, under bust, waist, hip, thigh, knee, calf, ankle, upper arm, elbow, wrist, armhole bicep) circumference and length.

안면부 비후성 반흔에 스테로이드 주사: 증례보고 (Steroid Injection on Facial Hypertrophic Scar: Report of 3 Cases)

  • Traumatic lacerations are common in the orofacial region as a result of accidents. Due to the frequent movement of the skin around the mandible, scars in that area are more likely to widen or become hypertrophic. Treatment of facial laceration was performed on three patients and followed by regular check-up. It was discovered that the scars have become hypertrophic, so steroid injections were used. The results were satisfactory with the decrease in sizes and hardnes of the scar. Facial scarring is a particularly distressing phenomenon and has always been a challenge to treat as the scars are more likely to widen or become hyphertrophied due to the frequent movement of the muscle in the facial area. We confirmed that the positive effect of steroid on hypertrophic scars. So, we suggest that proper treatment and periodic follow-up, adjuvant treatments especially steroid injection is necessary in patients with lacerations.

Scarred scalp reconstruction with a rectangular expander

  • Scalp reconstruction is challenging because the scalp is inelastic, stiff, and has hair follicles. Tissue expansion offers aesthetically pleasing outcomes with minimal donor-site morbidity. However, in a scarred scalp, the extent of possible dissection for the expander insertion may be limited and surgeons must make use of the limited scalp tissue. We successfully reconstructed scarred scalps using rectangular expanders. This report presents two cases: a 4×3 cm chronic defect with widespread scarring and osteomyelitis and an 11×7.5 cm scar tissue following a skin graft. Tissue expanders were inserted in the subgaleal plane and were inflated by 195 mL and 400 mL over periods of 2 and 3 months, respectively. Subgaleal elevation of a fasciocutaneous flap was achieved with the expanded tissue. The defects were well covered, with good color, texture, and hair-bearing tissue. There were no complications involving the tissue expanders. Rectangular expanders yield more available tissue than round or crescent-shaped expanders. Moreover, since the base of the flap is well defined, the expander can be easily inserted in a limited space. Therefore, rectangular expanders are recommended for the reconstruction of scarred scalps.

요추 추간판 탈출증에 대한 경피적 내시경 레이저 수핵 제거술 후 임상 결과 (Clinical Outcomes of Percutaneous Endoscopic Laser Lumbar Discectomy)

  • Background: Over the years, disc surgery has progressively evolved in the direction of decreasing trauma and its invasiveness. Conventional open surgery has many complications, such as scarring, instability, bleeding and a relative high mortality rate. Minimally invasive spinal surgery is now an alternative to a traditional discectomy. Herein, we present an operative technique, and the early results, for a percutaneous endoscopic lumbar discectomy in herniated lumbar disc disease. Methods: 43 patients, including 27 men and 16 women, with ages ranging from 18 to 66 years, were enrolled in this study. All the patients showed a protruded or extruded soft disc herniation at the lumbar level on magnetic resonance imaging and computed tomography. A percutaneous endoscopic lumbar discectomy was applied to the patients, and clinical responses evaluated using MacNab's criteria. Results: 40 patients were regarded as showing successful responses (93.1%), and there were no severe complications, such as a hematoma, nerve injury, postoperative dysesthesia or death. One patient underwent fusion surgery for remnant back pain six month later. Conclusions: We conclude that, in properly selected patients, a percutaneous endoscopic lumbar discectomy is a safe, noninvasive and effective treatment modality for herniated lumbar intervertebral disc disease.

간질성 폐질환의 연구 동향 (Research Trends of Interstitial Lung Disease)

  • Interstitial lung disease (ILD) is characterized by progressive scarring of the lung leading to restriction and diminished oxygen transfer. Clinically, the presenting symptoms of ILD are nonspecific (cough and progressive dyspnea on exertion) and are often attributed to other diseases, thus delaying diagnosis and timely therapy. In this study, I analyzed the 10 chinese papers of interstitial lung diseases(ILD). The etiology are body resistance weakness(本虛) and pathogenic factor prevailing(標實). The body resistance weakness(本虛) including deficiency of the lung(肺虛), deficiency of the kidney(腎虛), deficiency of the spleen(脾虛), deficiency of Qi and Yin(氣陰兩虛), pathogenic factor prevailing(標實) including stagnation of phlegm(痰濁), blood stasis(瘀血), noxious heat(熱毒). As an treatment aim at supplementing lung and kidney(益肺腎), resolving phlegm and blood stasis(化痰瘀).

새싹테라피 3단계를 이용한 위축성 여드름 흉터 치료 20례 (Clinical Reports on Atrophic Acne Scar Treatment with Sae-ssack Therapy)

  • Objectives : The purpose of this study was to evaluate the therapeutic efficacy of Sae-ssack(sprout) therapy on atrophic acne scars. Methods : Twenty patients had atrophic acne scars on the face were treated with Sae-ssack therapy. Sae-ssack therapy consists of a total of three treatments, a first-stage Sae-ssack acupuncture, a second-stage texture treatment, and a third-stage anti-pigmentation therapy. The evaluation of the treatment effect was made by the Korean medical doctor who did not participate in the procedure, and the PGA(Physician's Global Assessment), QGASC(Qualitative Global Acne Scarring Grade System) and patient satisfaction were used as the evaluation index. Results and Conclusions : According to the above indicates, the Sae-ssack therapy was evaluated to be effective in the repair of atrophic acne scars. There were no visible or consistent side effects and patient satisfaction was also very high.
