  • Objective : The purpose of this study was conducted to find scapular movement instability related with shoulder complex and provide strengthening the scapular muscles with proprioceptive neuromuscular facilitation(PNF). Methods : This is a literature study with books and articles, seminar note and book for PNF international course. Results : The scapular serve as the platform for humeral motions. The scapulothoracic articulation is stabilized and controlled, in part, by the scapular muscles. Therefore, if scapular muscle function is altered, then dysfunctional scapulothoracic kinematics may result. Strengthening exercise for scapular muscles are a common part of rehabilitation programs designed for patients with scapular instability. Conclusion : Treatment of the scapular instability is provided. It is that treatment with combination of isotonic, stabilize reversal on different patient's positions.

  • In general, reverse shoulder arthroplasty revision is performed using the same implant for both the humerus and glenoid components. However, the authors of the present case used different implants from what was used previously for treating instability with scapular notching and glenoid aseptic loosening and report the case.

Thoracic Hyperkyphosis affects Scapular Orientation and Trunk Motion During Unconstrained Arm Elevation

  • Background: Shoulder function is achieved by the coordinated movements of the scapula, humerus, and thoracic spine, and shoulder disorders can be associated with altered scapular kinematics. The trunk plays an important role as the kinematic chain during arm elevation. Objects: The purpose of this study was to determine the effects of thoracic hyperkyphosis on scapular orientation and trunk motion. Methods: Thirty-one subjects (15 in the ideal thorax group and 16 in the thoracic hyperkyphosis group) performed right-arm abduction and adduction movements in an unconstrained plane. The scapular orientation and trunk motion were recorded using a motion analysis system. Results: Those subjects with thoracic hyperkyphosis displayed greater scapular posterior tilting at a $120^{\circ}$ shoulder elevation, greater scapular internal rotation throughout the arm raising phase, and greater trunk axial rotation at the upper ranges of the shoulder elevation, compared to those subjects with an ideal thorax (p<.05). Conclusion: Thoracic hyperkyphosis can cause scapular instability, greater trunk rotation and greater scapular posterior tilting, and may contribute to preventing the achievement of a full range of humeral abductions in an unconstrained plane.

  • Objective : The purpose of this study was conducted to find treatment of forward head posture and shoulder instability with proprioceptive neuromuscular facilitation concept. Methods : This is literature study with books, seminar note and international course. Results : Forward head posture and shoulder instability has related each other. Forward head posture will make muscle instability, weakness and stiffness on neck and shoulder girdle. It will make pain also. Important muscle are suboccipital muscle, omohyoid muscle, sternoclaidomastoid muscle, scaleni, pectoralis minor, levator scapular and digastric. Conclusion : Treatment of the forward head posture and shoulder instability is provided. It is that treatment of stiff muscle with eccentric muscle work, muscle elongation, muscle strengthening at the structure level and at the functional level for daily activities.

Bankart 병변과 SLAP 병변에서 경견관절와 술식시 Guide Pin 출구의 분석 (Analysis of Exit Site of Guide Pin Using Tansglenoid Suture Technique in Bankart and SLAP Lesion)

  • Purpose: To Analyze the exit site of pin inserted at the anterior glenoid rim in the reconstruction of the Bankart lesion and SLAP lesion using transglenoid suture technique. Materials and Methods: In the twenty adult right cadeveric scapula, insertions of pin were performed using guide at the position of 1, 2, 3 O'clock of glenoid rim. We measured the exit site of dorsal surface of the scapula by medial distance from sagittal plane of lateral border of scapular spine and the vertical distance from posterior border of the scapular spine. Results: When the pin was inserted caudally within 10 degree, at the position of 1, 2, 3 O'clock, the medial distance from lateral border of the scapula is averaged 29.4, 19.2, 34.0 mm respectively and the vertical distance from posterior border of the scapular spine is averaged 15.0, 18.6, 17.2 mm respectively. When the pin was inserted caudally within 20-30 degree, the medial distance is averaged 14.6, 14.2, 15.8 mm respectively and the vertical distance is averaged 31.6, 31.9, 32.1 mm respectively. Conclusion: When the pin was inserted caudally within ten degrees using the guide, the pin exit appeared at the more medial side of the base of scapular spine and the more inferior of scapular spine. This can make the firm suture tied over scapular spine during repair SLAP and the Bankart lesion, and also prevent the injury of suprascapular nerve.

Immediate Effects of Flexi-bar Exercise With Knee Push-up Plus on Shoulder Joint Position Sense and Muscle Activity in Subjects With Scapular Winging

  • Background: Individuals with scapular winging may have proprioceptive dysfunction which is important for motor control and causes shoulder instability. Reduced serratus anterior (SA) and lower trapezius (LT) muscle activity accompanied by over-active upper trapezius (UT), and pectoralis major (PM) may be contributing factors. Flexi-bar (FB) exercise may be used to increase joint position sense (JPS) and alter the target muscle activities. Objects: This study aimed to investigate the immediate effects of flexi-bar exercise prior to knee push-up plus (FPK) versus knee push-up plus (KPP) on JPS and muscle activity of SA, LT, UT, and PM in subjects with scapular winging. Methods: Eighteen subjects with scapular winging were recruited. JPS was investigated at baseline, after KPP and after FPK. Passive and active JPS errors were calculated by isokinetic equipment. Surface electromyography was used to record muscle activities during KPP and FPK. One-way repeated-measures analysis of variance and post hoc analyses were used to analyze the JPS error measured at baseline, after KPP and after FPK. Paired t-tests were used to compare muscle activities between KPP and FPK. Results: Passive JPS error was significantly decreased after KPP (p = 0.005) and after FPK (p = 0.003) compared to the baseline. Active JPS error was also significantly decreased after KPP (p = 0.016) and after FPK (p = 0.012) compared to the baseline. There was no significant difference in the passive and active JPS errors between KPP and FPK. SA activity during FPK was significantly increased (p = 0.024), and LT activity during FPK was significantly increased (p = 0.006). There were no significant differences in the UT and PM activity. Conclusion: FB might be recommended to immediately improve passive and active JPS and to selectively increase SA and LT muscle activities during KPP in individuals with scapular winging.

상지 패턴을 결합한 어깨 복합체 안정화 운동이 초등학교 야구선수들의 제구력과 구속에 미치는 영향 (The Effects of a Shoulder Complex Stabilization Exercise Combined with Upper Extremity Patterns on Ball Control and Ball Speed in Elementary School Baseball Players)

  • Purpose: The purpose of this study was to determine how a shoulder complex stabilization exercise affects ball control and ball speed in elementary school baseball players with an instable scapula. Methods: The subjects of the study were 16 baseball players attending S elementary school in Kwangju Metropolitan City. A shoulder complex stabilization exercise was conducted three times per week for one hour for four weeks. Then, the participants were divided into a scapular instability group or stability group based on their lateral scapular slide test scores. The measurement tool was measured using the target, the ball speed tester (PR1000-BC). All inspections were measured before and after the mediation period to examine the change in the subjects according to the mediation period. Results: As a result of measuring ball control, there was a statistically significant difference in the variation over time (p < 0.05), and there was no statistically significant difference in the interaction between time and group (p > 0.05). As a result of the test of effectiveness between subjects, there was no statistically significant difference found among the groups (p > 0.05). In terms of ball speed, there was no statistically significant difference found in the variation over time (p > 0.05) as well as in the interaction between time and group (p > 0.05). In terms of the effectiveness between subjects, there was no statistically significant difference found among the groups (p > 0.05). Conclusion: It was found that the shoulder complex stabilization exercise could improve ball control for elementary school baseball players. It is believed that this will help prevent and solve possible sport damages experienced during training or competitions, thereby helping athletes use training methods to improve their exercise capacity and continue their careers.

견갑골 골절 환자의 기능회복에 대한 추나 요법을 포함한 한방재활치료: 증례 보고 (The Clinical Effects of Korean Medicine Rehabilitation Including Chuna Manual Therapy on Scapular Fracture Patient's Functional Recovery: A Case Report)

  • To evaluate the effects of Korean medicine and Chuna treatment on patient with pain and limited range of motion (ROM) after scapular fracture. Patient was treated with Chuna manual therapy, acupuncture, herbal medicine, pharmacoacpuncture, cupping therapy and measured by ROM, visual analog scale (VAS), shoulder pain and disability index (SPADI), American shoulder elbow surgery scale (ASES), Korean shoulder score for instability (KSS). After the 3 weeks treatment, the patient's ROM, VAS, SPADI, ASES, KSS scores were improved. Rehabilitation program including Chuna manual therapy can be effectively used to improve scapular fracture patient's pain and symptom during rehabilitation period. Therefore it is recommended to use Chuna manual therapy for the improvement of movement and pain from fracture.

Shoulder Arthrokinematics of Collegiate Ice Hockey Athletes Based on the 3D-2D Model Registration Technique

  • Objective: There is a lack of studies using the 3D-2D image registration techniques on the mechanism of a shoulder injury for ice hockey players. This study aimed to analyze in vivo 3D glenohumeral joint arthrokinematics in collegiate ice hockey athletes and compare shoulder scaption with or without a hockey stick using the 3D-2D image registration technique. Method: We recruited 12 male elite ice hockey players (age, 19.88 ± 0.65 years). For arthrokinematic analysis of the common shoulder abduction movements of the injury pathogenesis of ice hockey players, participants abducted their dominant arm along the scapular plane and then grabbed a stick using the same motion under C-arm fluoroscopy with 16 frames per second. Computed tomography (CT) scans of the shoulder complex were obtained with a 0.6-mm slice pitch. Data from the humerus translation distances, scapula upward rotation, anterior-posterior tilt, internal to external rotation angles, and scapulohumeral rhythm (SHR) ratio on glenohumeral (GH) joint kinematics were outputted using a MATLAB customized code. Results: The humeral translation in the stick hand compared to the bare hand moved more anterior and more superior until the abduction angle reached 40°. When the GH joint in the stick hand was at the maximal abduction of the scapula, the scapula was externally rotated 2~5° relative to 0°. The SHR ratio relative to the abduction along the scapular plane at 40° indicated a statistically significant difference between the two groups (p < 0.05). Conclusion: With arm loading with the stick, the humeral and scapular kinematics showed a significant correlation in the initial section of the SHR. Although these correlations might be difficult in clinical settings, ice hockey athletes can lead to the movement difference of the scapulohumeral joints with inherent instability.
