분만직전 한우의 행동 (Prepartum Behaviors of Bos taurus coreanae)

  • 본 연구는 분만직전 한우의 전반적인 행동 패턴을 관찰하고자 수행되었다. 본 연구에서, 행동의 정확한 관찰을 위해 분만직전의 한우를 격리된 우사에 두고 time lapse VCR을 이용하여 48시간동안 행동을 촬영하여 scan point sampling 방법을 통하여 행동을 분석하였다. 그 결과 유지행동, 사회행동, 섭식행동을 주로 관찰할 수 있었다. 분만직전 한우의 48시간 동안의 행동별 분포도를 살펴보면, LD(가슴을 대고 누워있는 자세)가 38.2%로 가장 높은 빈도를 보였고, 다음으로 ST(서있는 자세)가 24.7%, EA(놓여있는 여물 또는 사료를 먹는 동작)가 10.7%, WA(걸어다니는 동작)가 7.2%, LR(가슴을 대고 누워서 반추 행동을 하고 있는 동작)이 5.6%, SR(서서 반추 행동을 하고 있는 동작)이 3.3%, TW(서서 꼬리를 흔드는 동작)이 3.1%, 그리고 SG(서서 자신을 핥는 동작)가 1.8%의 빈도를 보였으며, 그 외의 PG(새끼나 상대방을 핥아주는 동작), PC(머리를 좌 우로 흔들며 파리 등을 쫓아내는 동작)와 같은 행동들은 1% 미만이었다.

  • 상업적 복합균주와 효모를 sourdough starter로 사용하여V5가지의 시료를 다음과 같이 준비하였다. 제빵용 효모만을V사용한 control군, 상업용 복합균주를 사용한 LAB 1군, 상업용 복합균주와 효모로 각각 발효한 후 합친 LAB 2군, 상업용 복합균주와 효모를 함께 사용하여 발효한 LAB 3군, 상업용 복합균주와 효모를 사용하여 각각 발효하여 합친 후 다시 10시간 발효한 LAB 4군을 사용하여 제조한 발효식빵의 제빵특성은 다음과 같다. CrumbScan에 의한 영상분석의 결과로 LAB 4의 부피가 2,140 cc로 가장 큰 것으로 발효율이 가장 높았던 LAB 4의 부피가 가장 크게 나타났다. 기공의 조밀도는 부피가 가장 큰 LAB 4가 806로 가장 낮게 나타났고, 기공의 찌그러짐 정도는 기공의 조밀도가 가장 높았던 LAB 1이 1.56으로 가장 높은 것으로 나타났다. 껍질의 두께는 대조구가 가장 얇았고 LAB 4가 가장 두껍게 나타나 유의적인 차이를 보였다. Sourdough bread의 비용적의 측정결과는 발효율이 가장 높았던 LAB 4의 비용적은 5.14 mL/g으로 가장 높게 나타났다. Firmness 측정결과는 93.2 g인 대조구에 비해 LAB 2, LAB 3과 LAB 4에서의 firmness 측정치는 각각 113.67, 111.97, 113.50 g로 높게 나타났다. 관능검사 결과에서 식빵의 내부색은 전반적으로 밝게 평가되었으며, LAB 4의 기공이 가장 균일한 것으로 나타났고 촉감도 가장 부드러운 것으로 평가되었고, 전체적인 품질평가에서도 3.7을 나타내어 가장 선호하는 것으로 나타났다. 결과적으로 대부분의 평가항목에서 LAB 4로 만든 sourdough bread를 선호하는 것으로 나타났다.

Surgical Outcomes and Complications Following All Posterior Approach for Spinal Deformity Associated with Neurofibromatosis Type-1

  • Objectives : The purpose of this study was to evaluate surgical outcomes and complications of spinal deformity associated with neurofibromatosis type-1 (NF-1). Methods : From 2012 to 2018, patients suffering from spinal deformity associated with NF-1 who underwent surgical correction were identified. Demographic data and radiographic measures were retrospectively reviewed. Pre- and postoperative whole spine radiograph images were used to determine both coronal and sagittal Cobb angles. All of patients underwent 3-dimentional computed tomographic scan and magnetic resonance imaging scan to confirm dystrophic features. For evaluation of clinical outcomes, we surveyed the pre- and postoperative scoliosis research society-22r (SRS-22r) score. Results : Seven patients with spinal deformity associated with NF-1 were enrolled in this study. The mean age of patients was 29.5±1.2 years old. The mean follow-up period was 2.8±1.4 years. The apex of the deformity was located in cervicothoracic (n=1), thoracic (n=4), and lumbar region (n=2). Most patients have poor bone quality and decreased bone mineral density with average T-score of -3.5±1.0. All patients underwent surgical correction via posterior approach. The pre- and postoperative mean coronal and sagittal Cobb angle was 61.6±22.6° and 34.6±38.1°, 56.8±18.5° and 40.2±9.1°, respectively. Mean correction rate of coronal and sagittal angle was 44.7% and 23.1%. Ultimate follow-up SRS-22r score (average score, 3.9±0.4) improved comparing to preoperative score (average score, 3.3±0.9). Only one patient received revision surgery due to rod fracture. No serious complication occurred, such as neurological deficit, and viscerovascular injury. Conclusion : The surgical correction of patients having spinal deformity associated with NF-1 is challenging, however the radiographic and clinical outcomes are satisfactory. The all posterior approach can be a safe and effective surgical option for patients having dystrophic curves associated with NF-1.

방사성의약품 검사 시 진단(CT)영상에 미치는 영향 (The Effects of Diagnostic Radiology Image on Radiopharmaceutical Testing)

  • This research attempts to qualitatively evaluate the intensity change by radiopharmaceuticals and obtain computed tomography using phantom injected with various nuclide. Cylindrical phantom is used for comparing and analysing the effect on diagnosis image during radiopharmaceuticals inspection. Inside of the phantom, water is injected and computed tomography image is scanned. During nuclear medicine invitro, frequently used radiopharmaceuticals, $^{99m}TcO_4$ 20 mCi and $^{18}F$ 14 mCi, is diluted in the water phantom and scanned in the same method. Traverse image obtained by CT scan is divided into six traverse image in the same slice of each scanned image. CT-number(HU) value of 10 measuring point is measured in 2 cm interval based on the center of the phantom. Measured HU value, based on the water phantom, is compared with the image after injecting $^{99m}TcO_4$ and $^{18}F$. Average scale of water is 2.8~1.6 HU, $^{99m}TcO_4$ is 3.0~1.6 HU and $^{18}F$ is 1.2~0 HU. Average of water is $2.3{\pm}0.17$ HU, $^{99m}TcO_4$ is $2.2{\pm}0.85$ HU and F-18 is $0.7{\pm}0.95$ HU. Based on water, reduced value of about 0.1 HU and about 0.5 HU is acquired from $^{99m}TcO_4$ and F-18. Radionuclide used in nuclear medicine inspection utilizes 100~200 KeV energy and obtains image through scintillation camera and PET-CT utilizes 511 KeV positron annihilation energy to obtain image. What we learned from this research is that gamma rays from these energies used in CT scan for diagnosis purpose or radioactive therapy plan can change the intensity of the image. The nuclear medicine inspection for reducing the effect of emitted gamma ray diagnosis image should be obtained after a period of time considering half-life which would be reduced distortion or changed in image.

한국인 조기난소부전 여성의 배란양상 (Pattern of Ovulation in Korean Women with Premature Ovarian Failure)

  • 목 적: 조기난소부전 환자의 10$\sim$20%에서 난소기능이 회복될 수 있다고 하며 간헐적으로 배란이 되고 심지어는 임신이 가능할 수 있다는 보고가 있다. 그러나 한국의 조기난소부전 여성의 임상적 특징과 배란가능성은 논의된 적이 없다. 이 연구의 목적은 한국인 조기난소부전 환자에서 자연 난포의 발생 빈도와 배란유도의 가능성을 평가하는 것이다. 연구방법: 특발성 조기난소부전으로 진단된 15명 여성의 전체 진료기록을 후향적으로 연구하였다. 환자별로 6번 이상의 초음파 검사를 시행하였고 36개월 이상의 기간 동안 추적관찰을 하였다. 또한 4명의 환자에서 6주기의 배란유도시도를 하였다. 조사항목은 초음파 검사에서 난포의 발견율, 자연 난포 성장률과 배란유도를 시행한 경우에 그 반응을 평가하여 결과를 분석하였다. 결 과: 15명 중 6명은 초음파상 난포가 발견되었나 (40%) 초음파 검사 당 난포 발견 빈도는 7%였다 (115회 중 8회). 6명 중 한 명은 초음파 추적관찰 상 배란이 확인되었고 나머지 5명은 크기에 변화가 없었다. 배란유도를 시도한 6명 중 성공적인 난포 성장이 관찰된 예는 없었다. 결 론: 한국인 조기난소부전 환자에서 난포의 발견율은 이전 서양 연구에 비하여 낮다고 생각되지만 관찰 방법에 차이가 있어 직접비교는 불가능하다. 배란유도의 성공율은 0%로서 이전 연구에서 발표된 수준보다 낮았다.

Elliptical Centric과 TRICKS 기법의 임상 적용에 관한 유용성 연구 (Elliptical Centric Techniques and Tricks About the Usefulness of the Clinical Application)

  • To prospectively determine the diagnostic performance a combination of standard bolus-chase magnetic resonance (MR) angiography and MR angiography with time-resolved imaging of contrast kinetics (TRICKS) for depicting severity of the head and neck vascular diseases. Over a period of two months, A total of 100 patients(average ages : $50{\pm}8$, male : 60, female : 40) with head and neck vascular diseases were performed on the GE excite 3.0 T units with 8-channel head coil and 4-channel NV coil. Imaging parameters for a typical study were as follow: SBC(TR/ TE/ FA/ SliceThicken./ Slab/ Freq./ FOV/ BW/Scan Time) = 5.4/ min/ 30/ 2/ zip2/ 70/ $224{\times}448$/ 30/ 62.50/ 28, TRICKS(TR/ TE/ FA/ Slice Thicken/Slab/ Freq./ FOV/ BW/ Temp Res./ Scan Time = 3.6/ min/ 25/ 4/ 30/ $160{\pm}384$, zip512/ 30/ 100/ 1 to 1.5/ 23). The analysis of all MR images, which have respect-ively classified two techniques into some diseases. The results of the former were divided into two groups(SBC, TRICKS)with 4 grading of two reader, respectively. Wilcoxon signed rank test was used to determine if a significant difference between imaging techniques existed(p < 0.05). In 33 of 100 patients, arterio-venous malformation was 11% at TRICKS, subclavian vein stenosis : 8%, fistular sinus : 4%, jugular vein stenosis:6%, Middle Cerebral Artery bypass surgery : 4%, p < 0.05). The rest of 67 patients were considered as the results of SBC(14% in the basilar artery stenosis, carotid stenosis : 16%, vertebral stenosis : 17%, central neuro-cytoma : 5%, meningioma : 5%, Not appliable : 10%, p < 0.05). Sensitivity and specificity of TRICKS MR angiography in SVS, FS, JVS, MCABS were improved compared with those at standard MR angiography. In SBS MR angiography which were improved in BAS, CS, VS, CN, Meningioma. In conclusion, TRICKS MR angiography of the SVS, FS, JVS, MCABS is superior to standard MR angiography regarding the number of diagnostic grading. The SBS MR angiography were improved in BAS, CS, VS, CN, Meningioma. and assessment of the degree of luminal narrowing on both TRICKS and SBS.

Visibility of Internal Target Volume of Dynamic Tumors in Free-breathing Cone-beam Computed Tomography for Image Guided Radiation Therapy

  • Respiratory-induced dynamic tumors render free-breathing cone-beam computed tomography (FBCBCT) images with motion artifacts complicating the task of quantifying the internal target volume (ITV). The purpose of this paper is to study the visibility of the revealed ITV when the imaging dose parameters, such as the kVp and mAs, are varied. The $Trilogy^{TM}$ linear accelerator with an On-Board Imaging ($OBI^{TM}$) system was used to acquire low-imaging-dose-mode (LIDM: 110 kVp, 20 mA, 20 ms/frame) and high-imaging-dose-mode (HIDM: 125 kVp, 80 mA, 25 ms/frame) FBCBCT images of a 3-cm diameter sphere (density=0.855 $g/cm^3$) moving in accordance to various sinusoidal breathing patterns, each with an unique inhalation-to-exhalation (I/E) ratio, amplitude, and period. In terms of image ITV contrast, there was a small overall average change of the ITV contrast when going from HIDM to LIDM of $6.5{\pm}5.1%$ for all breathing patterns. As for the ITV visible volume measurements, there was an insignificant difference between the ITV of both the LIDM- and HIDM-FBCBCT images with an average difference of $0.5{\pm}0.5%$, for all cases, despite the large difference in the imaging dose (approximately five-fold difference of ~0.8 and 4 cGy/scan). That indicates that the ITV visibility is not very sensitive to changes in imaging dose. However, both of the FBCBCT consistently underestimated the true ITV dimensions by up to 34.8% irrespective of the imaging dose mode due to significant motion artifacts, and thus, this imaging technique is not adequate to accurately visualize the ITV for image guidance. Due to the insignificant impact of imaging dose on ITV visibility, a plausible, alternative strategy would be to acquire more X-ray projections at the LIDM setting to allow 4DCBCT imaging to better define the ITV, and at the same time, maintain a reasonable imaging dose, i.e., comparable to a single HIDM-FBCBCT scan.

스캔통계량 분석을 통한 상수도 누수 및 수질 민원 발생 클러스터 탐색 (Cluster exploration of water pipe leak and complaints surveillance using a spatio-temporal statistical analysis)

  • In light of recent social concerns related to issues such as water supply pipe deterioration leading to problems like leaks and degraded water quality, the significance of maintenance efforts to enhance water source quality and ensure a stable water supply has grown substantially. In this study, scan statistic was applied to analyze water quality complaints and water leakage accidents from 2015 to 2021 to present a reasonable method to identify areas requiring improvement in water management. SaTScan, a spatio-temporal statistical analysis program, and ArcGIS were used for spatial information analysis, and clusters with high relative risk (RR) were determined using the maximum log-likelihood ratio, relative risk, and Monte Carlo hypothesis test for I city, the target area. Specifically, in the case of water quality complaints, the analysis results were compared by distinguishing cases occurring before and after the onset of "red water." The period between 2015 and 2019 revealed that preceding the occurrence of red water, the leak cluster at location L2 posed a significantly higher risk (RR: 2.45) than other regions. As for water quality complaints, cluster C2 exhibited a notably elevated RR (RR: 2.21) and appeared concentrated in areas D and S, respectively. On the other hand, post-red water incidents of water quality complaints were predominantly concentrated in area S. The analysis found that the locations of complaint clusters were similar to those of red water incidents. Of these, cluster C7 exhibited a substantial RR of 4.58, signifying more than a twofold increase compared to pre-incident levels. A kernel density map analysis was performed using GIS to identify priority areas for waterworks management based on the central location of clusters and complaint cluster RR data.

치매환자(患者) 32례(例)에 대(對)한 임상적(臨床的) 고찰(考察) (The Clinical Analysis on 32 Cases of Dementia)

  • A clinical analysis was carried out for 32 patients who were treated in Dept. of Dong-shin Oriental Medicine Hospital from 10st January to 16st May in 1998 and ruled out as dememtia. The results were summarized as follows. 1. In the distribution of sex, female was more than male. And the age of seventies(17 cases, 53.125%) was highest in the frequency of age. 2. In the distribution of having history of drinking and smoking were each 4 cases(12.5%) and 7 cases(21.875%). 3. In the distribution of seasons, the ratio of spring and winter were each 16 cases(50.0%), 15 cases(46.875%), that of summer and fall were comparatively low. 4. In the period from onset to admission, the period within a week had 20 cases(62.5%) as top, and the most period of treatment was 2 and 3 weeks(28.125%). 5. The number of cases that had preceding disease was 35 cases(71.875%), a major portion of preceding diseases were hypertension, diabetes mellitus, stroke. 6. The most common symptom was disorientation(78.125%), and the next common symptom was motor desability(71.875%), and memory disturbance, verbal disturbance was each 59.375 and 47.1%. 7. In the distribution of the CT scan films, 15 cases(46.875%) showed cerebral infarction and that of another 5cases (15.625%) showed cerebral hemorrhage, that of another 2 cases(6.25%) showed brain atrophy. 8. In the lipid density of blood, T-Cholesterol, TG, HDL-Cholesterol, LDL- Cholesterol was within normal limit. 9. In the MMSE-K, the most score was from 16 point to 20 point, and second only was from 11 point to 15 point, and the next was from 6 point to 10 point. 10. In the Ischemic score, 20 cases(62.5%) was diagnosed as brain vascular type dementia, and mixed type dementia was 9 cases(28.125%), Alzheimer type dementia was 3 cases(9.375%). 11. The most used recipes were a sort of recipes to activate of flow the vital energy and the blood, to remove the phlegm(41 cases 33.9%), especially DODAMHOALHEL TANG (12 cases, 9.9%), ANSINCHUNGNOITANG (8 case, 6.6%) was the most used recipe. 12. In the judgement of efficacy, 17 cases(53.125%) showed excellent efficacy, 10 cases(31.25%) showed good efficacy, 5 cases(15.625%) showed no change and that according to MMSE-K, 4 case(18.18%) showed excellent efficacy, 15 cases(68.18%) showed good efficacy, 15 cases(68.18%) showed no change.

고열을 동반한3개월 미만 요로 감염 영아에서 척추천자와 배뇨성 방광요도조영술의 필요성 (A Necessity for Lumbar Puncture and VCUG in Febrile UTI Infants less than 3 Months of Age)

  • 목 적 : 3개월 미만 영아에서 요로감염(UTI)과 동반된 세균성 뇌수막염 및 방광 요관 역류(VUR)의 유병률을 확인하여 척추 천자 및 배뇨성 방광 요도 조영술(VCUG) 시행의 적용 대상 범위를 줄이고자 하였다. 방 법 : 2001년 1월에서 2008년 6월까지 고열을 주소로 입원한 3개월 미만의 영아를 대상으로 연구하였다. 이 중 UTI, 패혈증, 뇌수막염 및 UTI와 동반된 세균성 뇌수막염의 유병률을 알아보았다. UTI 영아에서 신장초음파, Technetium-99m dimercaptosuccinic acid (DMSA) 신스캔, VCUG검사 결과를 검토하여 신장초음파와 DMSA 신스캔 모두 정상인 경우를 1군, 그렇지 않는 경우를 2군으로 나누어 각각의 VUR의 유병률을 비교하였다. 또한, 1군 중 VUR이 있는 환아들의 임상 경과를 관찰하였다. 결 과: 총 1,962명의 영아 중 UTI는 620명(31.6%), 패혈증은 63명(3.2%), 세균성 뇌수막염은 8명(0.4%) 이었다. UTI환아 중 CSF천자 검사를 시행한 환자는 413명(66.6 %)이었고, 세균성 뇌수막염이 동반된 경우는 없었다. 신장초음파, DMSA신스캔, VCUG를 모두 시행한 환자는 348명(56.1%)이었다. Group 1은 110명(31.6%) 이었고, 이 중 VUR이 있는 경우는 4명(3.6%)이었으며 Group 2는 238명(68.4%)이었고, 이 중 VUR이 있는 경우는 51명(21.4%)이었다. 신장초음파 혹은 DMSA 의 이상 소견과 고도의 VUR사이에는 유의한 상관관계가 있었다. Group 1 중 VUR이 있었던 4명은 추적관찰기간 동안 대체로 양성경과를 밟았다. 결 론 : 고열을 동반한 3개월 미만의 영아들 중 입원 첫날 UTI로 진단된 경우에는 CSF천자 검사를 선택적으로 시행하는 것이 바람직하다고 사료된다. 또한, 침습적인 검사로 널리 알려진 VCUG의 시행에 대한 새로운 임상지침이 필요할 것으로 사료된다.
