This study investigated the status of infectious disease inpatients at long-term care hospitals in Korea. A descriptive study was conducted on patients with 14 infectious diseases at 798 long-term care hospitals during 2016-2017. The number of infected patients, total admission days, and total medical expenses were higher in 2017 than in 2016. The most common infectious diseases were enterocolitis due to Clostridium difficile, influenza, and scabies. The number of hospitals with patients who had enterocolitis due to C. difficile and resistance to carbapenem was higher in 2017 than in 2016. Hospitals with 150-299 beds had higher numbers of infectious disease patients than those with under 150 or over 300 beds. Therefore, intensive efforts are needed to control the most common diseases at long-term care hospitals, such as enterocolitis due to C. difficile, influenza, and scabies. It is recommended to apply relevant guidelines related to infection control management as well as implement educational programs. It will also be necessary to develop applicable infection monitoring standards and support the facilities and health workforce of long-term care hospitals under 300 beds through an effective infection surveillance system.
From 1996 to 1999, potato-growing areas in Korea were surveyed for identification and distribution of potato scab pathogens. Potato scab was widely distributed in the mass cultivation areas, especially in Jriu island, southern areas of Chonnam and Gyounggi provinces, and the alpine area of Gangwon province. Jeju island was the most affected area by this disease. A total of 55 Streptomyces strains were isolated from potato scab lesions, among which 40 strains were pathogenic on progeny tubers. Among the pathogenic strain, 21 strains were identified as previously described S. scabies, 7 Strains as S. turgidiscabies, and 5 Strains as S. acidiscabies, while 7 strains were observed as having distinct phenotypic properties. These strains were classified into six distinct clusters based on phenotypic characteristics and selected representative strains for each cluster. S. scabies (S33) had grey spores in a spiral chain. Mean-while, S. turgidiscabies (S27) had grey spores, S. acidiscabies (S71) had white spores, S. luridiscabiei (S63) had yellow-white spores, S. puniciscabiei (S77) had purple-red spores, and S. niveiscabiei (S78) had thin and compact white spores, all in a rectiflexuous chain. Pathogenicity was determined by the production of thaxtomin A and homologs of necl and ORFtnp genes. In TLC, representative strains S27, S71, S63, S77, and S78 produced a yellow band that co-migrated with the authentic thaxtomin A. However, thaxtomin A was not detected in chloroform extracts from oatmeal broth culture and Slice tuber tissue of S. luridiscabiei (S63) and S. puniciscabiei (S77) by HPLC analysis. In addition, no homologs of necl and ORFtnp genes in S. acidiscabies (S71), S. luridiscabiei (S63), S. puniciscabiei (S77), and S. niveiscabiei (S78) were detected by PCR and Southern hybridization analysis.

Potato scab, an important disease that affects developing tubers, causes a major problem in potato cultivation. The major potato cultivation areas in Korea are located in two Northern provinces, Gangwon and Gyeonggi, and two Southern provinces, Jeju island, and South Jeolla. In these areas, potato scab is widely distributed and has caused severe problem in potato cultivation. Therefore, potato-growing areas were surveyed for identification and distribution of potato scab pathogens from 1996 to 1999. Pathogenic Streptomyces strains were isolated from potato scab lesions and six representative Streptomyces species were characterized based on their phenotypic and molecular characteristics including, pathogenicity, physiological and morphological properties, analyses of 16SrRNA genes and 16S-23S ITS region, DNA relatedness, production of thaxtomin A, and the presence of nec1 and ORFtnp gene homologs. Three species were identified as previously described Streptomyces scabies, S. turgidiscabies, and S. acidiscabies, while other three species having distinct phenotypics properties were identified as novel S. luridiscabiei, S. puniciscabiei, and S. niveiscabiei.
Evodia daniellii $H_{EMSLEY}$ (Rutaceae) is cultivated in Korea and used as a folkmedicine for gastric inflammation, extermination of noxious insects, headache and antiinflammatory. The seed oil of this plant also has been used in various diseases, for example, dermatitis, scabies and so forth. Fructus Evodiae seed oil sterols were obtained from the nonsaponifiable matters of ethereal extract of the seed. The composition of sterols have been determined by gas liquid chromatography and thin layer chromatography on the preparative plates. It was noted that ${\beta}-sitosterol$ was the major sterols in the Evodia daniellii $H_{EMSLEY}$ in Korea. The results showed that contents of Evodia daniellii $H_{EMSLEY}$ sterols were campesterol 14.01%, stigmasterol 4.36%, ${\beta}-sitosterol$ 71.26%, ${\delta}^7-sterol$ 9.21% and unknown sterol 1.16% by method of triangulation and campesterol 13.02%, stigmasterol 3.87%, ${\beta}-sitosterol$ 74.34%, ${\delta}^7-sterol$ 8.67% and unknown sterol 1.85% by method of planimetry.
The bark of Ulmus parvifolia Jacq. (Ulmaceae) has been used for the treatment of gonorhea, edema, scabies and eczema marginatum. Previous investigations conducted with the heartwood and leaves have demonstrated it to contain sesquiterpenes as well as fat acids from the heartwood and flavonol glycosides from leaves. However, no phytochemical work has been done on the bark parts of this plant. Investigation of the phytochemical constituents in the barks of U. parvifolia has resulted in the isolation of sterols, sterol glucoside and a catechin glycoside, $(+)-catechin\;7-O-{\alpha}-{_L}-rhamnopyranoside$, all of which were isolated for the first time from this plant. Sterols were consisted of the three components, ${\beta}-sitosterol$, stigmasterol and campesterol in a ratio of 92.1:4.1:3.8, and sterol glucoside was identified as ${\beta}-sitosterol\;3-O-{\beta}-{_D}-glucoside$. The structure of the catechin $7-O-{\alpha}-{_L}-rhamnoside$ was established primarily by analysis of $^1H-and$ COSY-45 NMR, HMQC and HMBC and EI mass spectra of the heptaacetate. Especially, HMBC spectrum provides effective way for the determination of the point of attachment of the rhamnosyl group to catechin moiety.
It was conducted to find out the effect of chemical treatment on seed tubers and soil to control potato common scab in Jeju Island in 2001. Although the treatment of soil disinfection with Dazomet Gr. (30kg/10a) was more effective comparing to the treatment of seed disinfection and the untreated, the control value was merely 40.5%. However, the combined treatment of soil and seed disinfection was considerably more effective to reduce the percentage of infection and infection area, and increase the production of marketable tubers and also increase the control value. Therefore, the combined treatments are demanded to reduce the scab incidence in Jeju field conditions.
Objectives : Sophorae Radix (SR), the dried root of Sophorae Flavescens Aiton, has been used to treat atherosclerosis, arrhythma and skin diseases including scabies and eczema. The present study was examined to evaluate antimicrobial activities of SR extracts against oral microorganism. Methods : Antimicrobial properties of SR extracts were determined by agar diffusion method and minimum inhibitory concentration (MIC) against Streptococcus mutans, Streptococcus sobrinus and Actinomyces viscosus. Analysis of kurarinone from SR extracts was conducted using UPLC (Ultra Performance Liquid Chromatography). Results : The ethanolic extracts of SR showed stronger antimicrobial effect than methanolic extracts, while the aqueous extracts of SR had no activity. In addition, the higher content of kurarinone was found in ethanolic extracts than methanolic extracts. The purified kurarinone from ethanolic extracts showed potent antimicrobial activity with the MIC value of $3.9{\sim}7.8{\mu}g/m{\ell}$. Conclusion : An ethanolic extract of SR showed antimicrobial properties against several oral microorganisms, and kuranrinone contributed to antimicrobial action of SR. Thus, ethanolic extracts of SR or purified kurarinone should be beneficial for the preparation of the useful agent for treating oral disease including anticaries.
The secY gene of Streptomyces lividans TK24 was cloned by the PCR method with synthetic oligonucleotide primers designed on the basis of the conserved regions of Ll5-secY-adk operon from E. coli, B. subtilis, and M luteus. The deduced amino acid sequences of the SecY are highly homologous to those of other known SecY. It has 46%, 43%, 57%, 44%, 42%,56%, 90% similarity to Escherichia coli, Bacillus subtilis, Micrococcus luteus, Bacillus licheniformis Staphylococcus carnosus, Brevibacterium flavum, Streptomyces scabies, respectively and almost the same with Streptomyces coelicolor, The gene organization of Ll5- SecY-Adk is also similar to those of other bacteria. SecY and Adk are very likely translationally coupled that is overlapping stop codon of SecY and start codon of Adk with one base pair, which is common structure among high GC content strains of gram positive bacteria.
We analyzed parasitic diseases diagnosed by tissue biopsy specimens at KyungHee Medical Center (KMC) from 1984 to 2005. The total number of parasite infection cases was 150 (0.07%) out of the total 211,859 biopsy specimens submitted for histopathological examinations. They consisted of 62 cysticercosis, 23 sparganosis, 16 paragonimiasis, 15 amebiasis, 11 anisakiasis, 11 clonorchiasis, 3 ascariasis, 2 scabies, 2 enterobiasis, 2 trichuriasis, 1 leishmaniasis, 1 taeniasis, and 1 thelaziasis. Out of 62 cysticercosis cases, 55 were detected in subcutaneous tissues or the central nerve system. Eighteen out of 23 sparganosis cases were involved in muscular and subcutaneous tissues. In most anisakiasis cases, the involved organ was the stomach. The lung and the pleura were the most common site of paragonimiasis. The incidence of parasitic diseases during the first 5 years (1984-1988) was the highest of all observed periods. After 1989, similar incidences were shown throughout the period. Whereas cysticercosis was diagnosed in 34 cases during 1984-1988, no case has been diagnosed since 2000. In the case of sparganosis, the chronological incidence was almost uniform throughout the period 1984-2005. Paragonimiasis showed a similar tendency to cysticercosis. In gender and age distribution of parasitic diseases, men showed higher incidence rates than females, and the age groups of the 40s or older indicated higher infection frequencies than other age groups. Therefore, these results are a significant report to appear the tendency of human parasitic disease diagnosed by tissue biopsy in association with parasitosis at KMC in Seoul.
Purpose: This study was to investigate the recognition of skin infection (RSI) and management practice (MP) on caregivers in geriatric hospital. Methods: The subjects were 209 caregivers who work at geriatric hospital in G city and J do. Data were analyzed with ${\chi}^2$-test, t-test. and ANOVA using SPSS 18.0 program. Results: Prevalence rate of caregivers' skin diseases was 76.6%. Diagnosis of Skin disease was contact dermatitis 42.5%, scabies 26.9%, and skin xerosis 25.0%. The Mean RSI score was 3.81 and MP was 4.12. In addition, MP was significantly different by number of bed hospitals (F=4.63, p=.011) and number of caring patients (F=2.67, p=.049). Conclusion: This study will be a useful on continuing education for caregivers and basis for the guidance of medical infection control standards development.
