Effect of Hyulbuchukeo-tang and its Component Groups on Alloxan Induced Hepatoxicity in Rats (혈부축어탕(血府逐瘀湯)과 그 구성약물군(構成藥物群)이 Alloxan 유도 당뇨(糖尿) 백서(白鼠)의 간독성에 미치는 영향)

  • This study was performed to observe the effect of hyulbuchukeo-tang and its component groups on recovery of hepatoxicity in alloxan treated rats. The experimental group was divided into 3 groups: hyulbuchukeo-tang(HCT) and its components groups, sayeok-san(H-sa) and dohong-samul-tang (H-do). The results were obtained as follows: 1. The level of Glucose was considerably reduced by HCT, H-sa, H-do with high significancy. 2. In the change of GOT activity, HCT and H-do were decreased with high significancy, H-do was decreased with some significancy. 3. In the change of GPT activity, HCT was decreased with high significancy, H-sa and H-do were decreased with some significancy. 4. In the change of ${\gamma}-GPT$ activity, HCT was decreased with high significancy, H-do was decreased with some significancy. 5. In the change of ALP activity. only HCT was decreased with some significancy. 6. In the change of LDH activity, HCT was decreased with high significancy, H-sa and H-do were decreased with some significancy. 7. In the change of bilirubin contents. only HCT was decreased with some significancy. As result, hyulbuchukeo-tang(HCT) has significant effects on recovery of hepatoxicity, and its components groups. sayeok-san(H-sa) and dohong-samul-tang(H-do) have some effects. It suggests that sayeok-san(H-sa) and dohong-samul-tang(H-do) have more effects on recovery of hepatoxicity in case of the combination of the two (that is hyulbuchukeo-tang) than in case of the separation.

Effects of Sayeok-tang on Papain-Induced Osteoarthritis in Mice (Papain으로 유도된 골관절염 생쥐 모델에서 사역탕(四逆湯)의 항골관절염 효능에 관한 연구)

  • This study intends to clarify how Sayeok-tang(here in after reffered to SYT) affect C57BL/10 mice whose osteoarthritis was induced by papain. Osteoarthritis was induced by injecting papain in the knee joint of 3 groups(n=6) of mice. Normal group was non-treatment group and was not injected papain, whereas control mice were orally administered with $200{\mu}{\ell}$ of physiological saline. Positive comparison group was medicated with 100 mg/kg of Joins$^{(R)}$ mixed with $200{\mu}{\ell}$ of physiological saline. Experimental group was medicated with 400 mg/kg of SYT mixed with $200{\mu}{\ell}$ of physiological saline. Both Positive and experimental comparison groups were orally medicated once per day for 4 weeks. After the experiment, the functions of liver and kidney, inflammation cytokine values within serum, degree of revelation for inflammation cytokine genes, immune cells within blood, metabolism of arachidonic acid and amount of cartilage were measured and histopathological changes in the knee joint structures were observed. As results, SYT had no significant effect on the liver and kidney functions. Interleukin-$1{\beta}$(IL-$1{\beta}$), interleukin-6(IL-6), monocyte chemo attractant protein-1(MCP-1) and tumor necrosis factor-${\alpha}$(TNF-${\alpha}$) were significantly decreased. Inflammation cytokines in joints were all significantly decreased. Prostaglandin $E_2(PGE_2)$, thromboxane $B_2(TXB_2)$ were significantly decreased. Destruction of cartilage on micro computed tomography(CT)-arthrography was meaningfully decreased. In terms of histopathology, infiltration of inflammation, proliferation of synovial membrane, subsidence of cartilage and bone due to penetration of excessive formation of synovial cell and destruction of cartilage were small. Based on all results mentioned above, Sayeok-tang(SYT) is believed to be meaningful for suppressing the progress of osteoarthritis and its treatments because of its anti-inflammatory effects and alleviation of pain with histopathological effective efficacy.

Recent Research Trends of Sayeok-tang Used in Ischemic Heart Disease (허혈성 심질환에 활용된 사역탕(四逆湯)의 최신 연구 동향)

  • Purpose : Sayeok-tang (Sini decoction, SND) is a cold-dispelling formula used for cold deficiency syndrome and is composed of Aconiti Lateralis Radix Preperata, Zingiberis Rhizoma, and Glycyrrhizae Radix et Rhizoma. It is used for diseases such as myocardial infarction, heart failure, acute and chronic gastroenteritis, and gastric effusion. This study proposes the possibility of expansion of basic research and clinical applications for ischemic heart disease (IHD) through systematic analysis of domestic and foreign studies on SND. Methods : We collected studies within the last 10 years on the use of SND in IHD and excluded those lacking relevance. Selected studies were classified by research method and the main themes of the studies were analyzed for each classification. Result: Out of 15 studies, there were 5 animal studies, 8 metabolite analyses in animals, 1 in vitro study, and 1 systematic review. Our review suggests that SND may be used as an adjuvant to nitroglycerin and percutaneous transluminal coronary angioplasty, and may improve symptoms and quality of life of patients with IHD. Myocardial protective effects through antioxidant, anti-inflammatory, anti-apoptotic, and anti-hypertensive actions were confirmed through these studies. Effects on carbohydrate, protein, and lipid metabolism were also reported. Conclusions : This study suggests that SND has potential as a treatment for IHD.

A Case Report of Psoriasis Treated by Hoeyeok-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 회역탕(回逆湯)을 투여하여 호전된 전신 건선 증례 보고)

  • Objective : There was an improvement in a patient with psoriasis following the intake of Hoeyeok-tang for 317 days. By analyzing these cases, we reviewed the use of Hoeyeok-tang in skin diseases and attempted to interpret the related Shanghanlun articles. Methods : The diagnostic process was analyzed according to the disease pattern identification diagnostic system based on the Shanghanlun provisions. The progression of psoriasis, which was addressed, was assessed using the PASI score. Results : The patient was diagnosed with Soeum-byung 324, and the PASI score decreased from 13.6 to 0.4 after the intake of Hoeyeok-tang for 317 days. The patient's compliance and expectation from treatment were good, and no specific adverse effect was identified. Conclusions : This study suggests the clinical use of Hoeyeok-tang in psoriasis based on Shanghanlun.

Nummular Eczema Treated by Hoeyeok-tang Based on the Shanghanlun Disease Pattern Identification Diagnostic System (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 회역탕(回逆湯) 투여 후 호전된 화폐상 습진 증례 1례)

  • Objective: This study aimed to report the improvement of a patient with nummular eczema treated with Hoeyeok-tang based on Shanghanlun provisions. Methods: According to the disease pattern identification diagnostic system (DPIDS) based on the Shanghanlun provisions, the patient was diagnosed with lesser yin disease according to the 324th provision. The patient was treated with Hoeyeok-tang for 90 days. The severity of pruritus decreased, and changes in symptoms were estimated by a visual analog scale, Scoring Atopic Dermatitis Index and the Validated Investigator Global Assessment for Atopic Dermatitis. Results: The patient's symptoms were improved. The severity of pruritus decreased and they recovered from abrasions. Conclusions: This study suggests that the 324th provision of Shanghanlun refer directly to nummular eczema.

Recovery of an Elderly Kwashiorkor Patient by Korean Medicine: A Case Report

  • Objectives: The present study reports case of an elderly kwashiorkor patient who was treated with Korean Medicine. Methods: A 68-year-old female patient with weight loss, general weakness, chills, anorexia, and dizziness after over 100 episodes of diarrhea visited the hospital. Blood tests showed hypoalbuminemia and anemia, and ultrasonography revealed fatty liver disease. The patient was diagnosed with kwashiorkor, and her symptom differentiation was Yang deficiency followed by Both Qi-Blood deficiency. Sayeok-Tang, Soshiho-Tang, Insamyangyeong-Tang, and Gongjin-Dan, herbal drugs, were given to the patient during 40 days of hospitalization. Results: After 40 days of hospitalization, her symptoms were reduced, and the blood test results improved. Conclusion: This case presents the therapeutic potential of Korean medicine in the treatment of kwashiorkor.

Study of structural analysis on formulas from 『Onbyungjobyun』 using network analysis (네트워크 분석법을 이용한 『온병조변』 처방의 구조적 분석 연구)

  • Objective : This study aims to diversify interpretation of formulas from "Onbyungjobyun" by analyzing various formulas from "Onbyungjobyun" as basic formulas. Method : This study found herbal communities of combination by analyzing herbal combinations based on network analysis of formulas from "Onbyungjobyun", and has analyzed each community of combination as basic formulas. Result : The results of network analysis showed a total of 3 herbal communities of combination; first was medicinal herbs from Eungyo-san(銀翹散), Jeungaek-tang(增液湯), Bokmaek-tang(復脈湯), Gyeji-tang(桂枝湯), Sogeonjung-tang (小建中湯) series; second was medicinal herbs from Angungwoohwang-hwan(安宮牛黃丸); third was medicinal herbs from Baekho-tang(白虎湯), Jaseol-dan(紫雪丹), Sayeok-tang(四逆湯) series. Conclusion : The formulas from "Onbyungjobyun" are consisted of herbal communities of combination; that treat warm-heat pathogen and supplies yin essence or yang qi; treat reverse transmission to the pericardium(逆傳心包); and treat heat in the qi phase in Onbyeong and cold-dampness in the middle energizer.

A case study of a patient with atopic dermatitis who improved with Shanghanlun Hoeyeok-tang (『상한론(傷寒論)』 회역탕(回逆湯)으로 호전된 아토피피부염 환자 1례에 대한 증례 연구)

  • Objective : The objective of this study is to report a case of atopic dermatitis (AD) treated with Hoeyeok-tang. Methods : We retrospectively reviewed the course of herbal medicine treatment in a 20-year-old male patient with a topic dermatitis. Changes in main symptoms were confirmed based on the gross findings and assessed using SCORing Atopic Dermatitis(SCORAD) Index. Accompanying symptoms and side effects that can be referred to for diagnosis were analyzed By referring to the clinical records. Results : According to the Shanghanlun provision, the patient's AD was diagnosed with Terminal-yin Gwakran 389th provision. The patient was treated with Hoeyeok-tang for 90 days. The severity of pruritus decreased, and SCORAD Index was decreased from 44.6 to 8.4. VAS for eye fatigue and nausea was decreased from 10 to 5, also. No particular side effects occurred to the patient while taking the Hoeyeok-tang. Conclusions : The result suggests the clinical applicability of 389th provision of Shanghanlun.

A Research of Definition and Treatment of Dizziness in the Books on Cold Damage (상한문헌에 나타난 현훈의 정의와 치료법에 대한 연구)

  • Objectives to study definition and treatment of dizziness in the books of Cold damage which are classified as symptoms of all time. Methods 1. Quote provisions related to dizziness in "Sanghanlon(傷寒論)" 2. Among the books of Cold damage which are classified as symptoms, select 19 literatures on dizziness with table of contents and collect data and categorize in two perspectives on theories and disease pattern. 3. Compare and draw a chart all data collected in above methods. Results & Conclusions 1. In the books on Cold damage, dizziness is expressed in 2 ways such as head dizziness(頭眩) and fainting(鬱冒). fainting is much more several symptoms than head dizziness and it is the difference that it has mental confusion. 2. The cause of head dizziness after promoting sweating, vomiting and purgation is that source qi(元氣) of upper energizer(上焦) is deficiency, and cause of head dizziness before using method of treatment is wind(風), heat(熱), phlegm(痰), blood deficiency(血虛) and etc. 3. Main cause of fainting is that deficiency is getting severe so that cold invades, and symptoms are blood deficiency, after giving birth and excess pattern of fire and heat. 4. Remedies for head dizziness are using Yeonggyechulgam-tang(苓桂朮甘湯), jinmu-tang(眞武湯), sosiho-tang(小柴胡湯), sagunja-tang(四君子湯), samul-tang(四物湯) and etc. 5. Insamsambaek-tang (人蔘三白湯) and Sayeok-tang(四逆湯) are used for deficiency pattern of fainting, and Dojeok-san(導赤散), Daeseunggi-tang(大承氣湯), and Hwangryeonhaedok-tang(黃連解毒湯) are used for excess pattern(實證).

Remissions of Crohn's Disease at Onset and Relapse Using Herbal Medicine: A Case Report (발병과 재발에 한약으로 관해가 유도된 크론병 증례 보고)

  • This case report details two remissions at onset and relapse after 4 years each with Korean herbal medicine treatment in a 33-year-old male patient diagnosed as Crohn's disease (CD) by clinical and colonoscopic examination. He presented bloody diarrhea and severe abdominal pain, and his symptoms were persistent even with usual Western medications, so he was offered surgery and biologics. He was then given Sayeok-Tang for 8 months and remission was induced. Four years later he had a relapse and this time he was given Banha-Sasim-Tang, and Baekduong-Ga-Gamcho-Agyo-Tang, based on pattern changes with intense symptom monitoring. Clinical and endoscopic remission was confirmed after 4 months of herbal medicine treatment and no adverse events were reported. This case report shows that individualized Korean herbal medicine treatment has the potential for induction of remission of CD and further research is warranted.
