태음인체질병증 임상진료지침: 표병 (Clinical Practice Guideline for Taeeumin Disease of Sasang Constitutional Medicine: Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease)

  • Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Taeeumin Disease of Sasang Constitutional Medicine(SCM): Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease. This CPG was developed by the national-wide experts committee consisting of SCM professors. Methods First, collection and organization of literature related to SCM such as Donguisusebowon, Text book of SCM, Clinical Guidebook of SCM and Fundamental research to standardize diagnosis of Sasang Constitutional Medicine was performed. Secondly, journals related to clinical trial or Human complementary medicine of SCM were searched. Finally, 7 articles were selected and included in CPG for Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease. Results & Conclusions The CPG of Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease in Taeeumin Disease include classification, definition and standard symptoms of each pattern. Esophagus Cold-based Exterior Cold (Wiwansuhan-pyohan) disease consists of two aspects : Esophagus-Cold (Wiwanhan) and Esophagus-Cold Lung-Dry (Wiwanhan-paejo) symptomatology. Esophagus-Cold (Wiwanhan) symptomatology is classified into mild and moderate pattern by severity. Mild pattern of Esophagus-Cold (Wiwanhan) symptomatology is classified into Supraspinal Exterior (Baechu-pyo) initial and Wheezing-Dyspnea (Hyocheon) pattern. Moderate pattern of Esophagus-Cold (Wiwanhan) symptomatology is classified into Cold-reversal (Hanguel) and Cold-reversal (Hanguel) advanced pattern. And Esophagus-Cold Lung-Dry (Wiwanhan-paejo) symptomatology is classified into severe and critical pattern by severity. Severe pattern of Esophagus-Cold Lung-Dry (Wiwanhan-paejo) is classified into Dry-Cold (Johan) pattern and Dry-Cold (Johan) advanced pattern. Critical pattern of Esophagus-Cold Lung-Dry (Wiwanhan-paejo) symptomatology consists of Dry-Cold (Johan) intense pattern (Eumhyeol-mogal handa pattern).

Purpose The purpose of this article was to find the change of Dong-mu's conceptions about constitutional disease through comparing with ${\ulcorner}$Sasang-Yihak-chobonguen${\lrcorner}$, ${\ulcorner}$Dongyi Soose Bowon${\lrcorner}$ written in 1894(Old edition) and ${\ulcorner}$Dongyi Soose Bowon${\lrcorner}$ published in 1901(In Edition) 2. Methods Comparing with ${\ulcorner}$Sasang-Yihak-chobonguen${\lrcorner}$, ${\ulcorner}$Dongyi Soose Bowon${\lrcorner}$ written in 1894(Old edition) and ${\ulcorner}$Dongyi Soose Bowon${\lrcorner}$ published in 1901(In Edition) we found the change of Dong-mu's conceptio ns about constitutional disease 3. Result 1) In ${\ulcorner}$Sasang-Yihak-chobonguen${\lrcorner}$, diseases were classified 'Oi-Gam(外感)‘, ’Nae-sang(內傷)‘, ’Ok-roi(牢獄)‘. ’Wi-gyoung(危傾)‘. In ${\ulcorner}$Sasang-Yihak-chobonguen${\lrcorner}$ ordinary symptoms was more impotant than disease, and good life style was emplasized for health 2) In ${\ulcorner}$Dongyi Soose Bowon${\lrcorner}$ written in 1894(Old edition), diseases were classified 'Oi-Gam(外感)‘, ’Nae-sang(內傷)‘. siseases from exterior causes was classified into 'Oi-gam-noi-chu-byoung(外傷腦?頁病)’, and diseases from interior causes was classified into 'Nae-sang-wi-wan-byoung(內觸胃脘病).‘ Herbal medication yet was't used by Disease 3) In ${\ulcorner}$Dongyi Soose Bowon${\lrcorner}$ published in 1901(In Edition), diseases was classified not by causes of disease but by mechanism and symptom of disease, so into Exterior-cold disease' and 'Interior-hot disease'. Herbal medication was used by pattern of Disease. 4) Conception of constitutional diseases was gradualy complished through edition.

  • Objectives This study was purposed to develop the clinical practice guideline for examination of Sasangin disease and diagnosis for Sasang Constitution. Methods "Donguisusebowon" and many articles were reviewed and examined for developing clinical practice guidelines. And "Guideline for Educating Public Health Korean Medical Doctors on Diagnosis of Sasang Constitution" was basically used to develop clinical guidelines. Results & Conclusions By researching and discussing the examination of Sasangin disease and diagnosis for Sasang Constitution, we make the principle of the clinical practice guideline, including methods using body shape, facial shape, vocal characteristic and personality etc.

  • Objective The purpose of this study was to examine the correlation between the Cold-damage Six-meridian disease of Qibo (岐伯六經病證) and Sasang Constitutional Symptomatology (四象體質病證), presented in Discourse on the Origin of Eastern Medicine (醫源論) of Longevity and Life Preservation in Eastern Medicine (Donguisusebowon, 東醫壽世保元). Method The process of development from Cold-damage Six-meridian disease of Qibo (岐伯六經病證) to Six Meridians Physical Symptoms and Medicines (六經形證用藥) in the chapter Cold(寒門) of the Treasure Mirror of Eastern Medicine (Donguibogam, 東醫寶鑑) was investigated. And the correlation between Six Meridians Physical Symptoms and Medicines (六經形證用藥) and Sasang Constitutional Symptomatology (四象體質病證) was considered. Results and Conclusions 1. The Cold-damage Six-meridian disease of Qibo (岐伯六經病證) in the chapter Heat Treatise (熱論篇) of Basic Questions (素問) had evolved into Six Meridians Physical Symptoms and Medicines (六經形證用藥) in the chapter Cold(寒門) of the Treasure Mirror of Eastern Medicine (Donguibogam, 東醫寶鑑) through Book for Life Saving (Huorenshu, 活人書), a work of Zhu Gong (朱肱), Six Books on Cold Damage disease (Shanghanliushu, 傷寒六書), a work of Tao Hua (陶華) and Introduction to Medicine (YixueRumen, 醫學入門), a work of Li Chan (李梴). 2. The correlation between the Cold-damage Six-meridian disease of Qibo (岐伯六經病證) and Sasang Constitutional Symptomatology (四象體質病證) can be analyzed and understood through Six Meridians Physical Symptoms and Medicines (六經形證用藥) in the chapter Cold(寒門) of the Treasure Mirror of Eastern Medicine (Donguibogam, 東醫寶鑑). 3. Greater Yang meridian disease of Qibo (岐伯) is related to Soyangin early stage of Lesser-Yang Wind-Injury symptomatology and Soyangin early stage of Chest-Heat symptomatology, Yang Brightness meridian disease and Greater Yin meridian disease to Taeeumin Liver-Heat symptomatology, Lesser Yin meridian disease to Soyangin Chest-Heat symptomatology, Lesser Yang meridian disease to Soyangin early stage of Lesser-Yang Wind-Injury symptomatology and Reverting Yin meridian disease to Soeumin Reverting Yin symptomatology of Greater Yang disease.

사상처방을 활용한 약물 유발성 속발성 파킨슨증 치험 2례 (Two Cases of Drug-induced Secondary Parkinsonism Treated with Sasang Constitutional Therapy)

  • Objectives : This study is about two patients of drug-induced secondary parkinsonism caused by antipsychotic drugs. The purpose of this study is to report the clinical effects of Sasang constitutional medicine. Methods : These two patients were treated by Sasang constitutional herbal medications based on "Donguisusebowon". We evaluated the symptoms through the score of the Unified Parkinson's Disease Rating Scale(UPDRS). Results and Conclusions : The patients' chief complaints were improved. This study shows that Sasang constitutional herbal medications are an effective treatment for drug-induced secondary parkinsonism patients, but further studies are still necessary.

일개 한방병원에 파킨슨병으로 내원한 환자의 사상의학적 특성에 관한 고찰 (The Analysis on Characteristics according to Sasang Constitution of Patients with Parkinson's Disease in a Korean Medicine Hospital)

  • Objectives The aim of this study was to make an analysis of clinical characteristics and status of Sasang constitutional medicine of patients with Parkinson's Disease who visited a Korean medicine hospital. Methods We studied 129 patients with Parkinson's Disease who visited a Korean medicine hospital from September 1, 2012 to June 30, 2018. We retrospectively collected medical history and characteristics of subjects using data of EMR (electronic medical record). Results The proportion of female was higher than male. The average age was 60s. The patients visited a hospital more than one year after the occurrence of symptoms. And they were mostly under western medicine treatment. Among the 129 patients, 56 persons(43.4%) were Taeeumin, 45 persons(34.9%) Soyangin and 28 persons Soeumin(21.7%). In the distribution of prescription according Sasang Constitutional Medicine symptomatology, there was no significant difference in the proportion of exterior cold disease(48.9%) and interior heat disease(51.1%) in Soyangin. Meanwhile the proportion of heat disease's prescription was higher than cold disease in Taeeumin(73.2%) and Soeumin(84.1%). The majority of disease were Dry-febrile symptom(73.2%) of heat disease in Taeeumin, Yang depletion-syndrome(64.3%) of heat disease in Soeumin, and Gyolhyung-symptom(33.3%) of exterior cold disease and Wasting thirst-syndrome(22.2%) of interior heat disease in Soyangin, Conclusions By Developing of guideline for treatment of Parkinson's Disease through analyzing on clinical characteristics and distribution of Sasang Constitutional Medicine symptomatology could contribute to the improvement of the cure rate and accuracy of constitution pattern.

소음인(少陰人) 소음병(少陰病)환자의 치험 1례 (A Clinical Case study of Soeum-Disease in Soeumin Patient)

  • 1. Objectives The purpose of this study is to give an account of the effects of the Korean herbal medicine therapy which is based on the Sasang constitutional medicine for the patient who has suffered from Soeum-disease(少陰病). 2. Methods The subject is a 73-year old woman who has the symptoms caused by sequella of cerebral infarction and depression mood which are headache, dizziness, ostalgia, chilling, anorexia, general weakness. 3. Results We have diagnosed her as Soeumin(少陰人), and have prescribed Kwankae bujaijung-tang(官桂附子理中湯). 4. Conclusions The consequence is the accompanied headache, dizziness, ostalgia, chilling, anorexia, and general condition has improved.

태음인 조열증으로 진단한 파킨슨병 환자의 치험 1례 (A Case Study of a Taeeumin Patient with Parkinson's Disease Diagnosed as Dry-heat Symptomatic Pattern)

  • Objectives This case study is about a Taeeumin patient with Parkinson's disease identified as Dry-heat (Joyeol) pattern. In this study, we report significant improvement of motor and non-motor symptoms of this patient after Sasang constitutional medicine treatment. Methods The patient was identified as Taeeumin Dry-heat pattern and treated with Cheongsimyeonja-tang and acupuncture. The Unified Parkinson Disease Rating Scale(UPDRS) was used to assess the overall function of the patient. And the Global Assessment Scale (GAS) was used to assess the change of bradykinesia, tremor and dry mouth after the treatment. Results The UPDRS total score decreased from 145 points to 77 points after 5 weeks treatment. And symptoms of bradykinesia, tremor and dry mouth showed significant improvement in GAS after the treatment. Furthermore, constipation, sleep disturbance, dysuresia and anorexia were reported to be improved after treatment. Conclusions This study shows that Sasang constitutional medicine can be effective treatment for motor and non-motor symptoms caused by Parkinson's disease.

  • 1. Objectives This study is written to investigate the biblographical facts on the book ${\ulcorner}$ Geumge Bibang${\lrcorner}$(Secret Prescriptions of Sasang (Golden Box). 2. Methods The Biblographical study through chronological research of several books concerning Sasang Constitutional Medicine including ${\ulcorner}$Dongyisusebowon${\lrcorner}$,${\ulcorner}$Dongyisasangjintyoeuijeon${\lrcorner}$after Dr. Lee Je-Ma's death was accomplished. 3. Results and Conclusions There are so many herb medicines using just one herb drug not several herb drugs according to many diseases in this book. Accrording to the writer of this book, Lee Min- Bong, JUSt one herb drug to each disease was a useful way to treat that disease in a poor countryside life. There are 85 adult's diseases, 8 women's diseases, 47 children's disease, 11 smallpox's diseases and 30 grotesque diseases. This book of ${\ulcorner}$Geumgebibang${\lrcorner}$ was written by Lee Minbong not by Cheondeoksanin in 1935 and published in 1936 and it was the first book covering the herb drug and diets to treat diseases according to Sasang Constitutional Types. It was first found in this book that Sasang could be explained with Yin- Yang relationship and Sasang Constitutional Types could be explained with characters like dragon, cow, donkey and horse, and with classrncatibn of faces and behaviors.

소양인 파킨슨병 환자의 한양방 병행치료 장기 경과관찰 보고 3례 (Long-term Clinical Efficacy on Three Soyang Patients with Parkinson's Disease Improved by Combined Treatment of Korean-Western Medicine)

  • Objective This case study was about a long-term clinical effects after using Korean medicine treatment and western treatment in three Soyangin patients with Parkinson's disease. In this study, we reported significant improvement of non-motor and motor symptoms of these patients after Sasang constitutional medicine treatments for more than a year. Method The patients were identified as Soyangin and treated with Yangguksanhwa-tang, Hyeongbangdojeok-san and acupuncture. The Hoehn & Yahr scale, Movement Disorder Society-sponsored revision of the Unified Parkinson's Disease Rating Scale (UPDRS) were used to assess the overall function of the patients. And the Global Assessment Scale (GAS) was used to assess the change of chief complaints after the treatments. Result and Conclusion The non-motor and motor symptoms of Parkinson's disease improved after Sasang constitutional medicine and western medicine treatments for more than a year. Furthermore these patients' chief complaints improved without increasing of amount of western medicine (levodopa). In conclusion, this study showed that Sasang constitutional medicine with western medicine can be effective treatment for long-term clinical care of Parkinson's disease.
