A Case of Epithelial-Myoepithelial Carcinoma in the Nasopharynx (비인두에 발생한 상피-근상피암종 1예)

  • Epithelial-myoepithelial carcinoma is rare, low grade malignant tumor of the salivary glands that exhibits a dual composition of epithelial and myoepithelial cells. Most of these tumors arise in the parotid gland, and only few occur in the submandibular gland or minor salivary glands. We describe here a rare case of epithelialmyoepithelial carcinoma arising from a minor salivary gland in the nasopharynx, one of the most unusual locations. The clinical and biological behavior of this tumor is not yet known.

Swelling-activated $Cl^-$ Channels in Human Salivary Gland Acinar Cells

  • The role of $Cl^-$ channels in regulatory volume decrease (RVD) in human salivary gland acinar cells was examined using a whole-cell patch clamp technique. Human tissues were obtained from healthy volunteers or from patients with oromaxillofacial tumors. During the measurements, $K^+$-free solutions were employed to eliminate contamination of whole-cell conductance by $K^+$ currents. When the cells were exposed to a 70% hypotonic solution, outward-rectifying currents, which were not observed in the resting state, were found to have significantly increased both in human labial and parotid gland acinar cells. The amplitudes of the currents were reduced in a low $Cl^-$ bath solution. Furthermore, the addition of $100{\mu}M$ 5-Nitro-2- (3-phenyl propylamino) benzoic acid (NPPB) or $100{\mu}M$ 4,4'-diisothio cyanatostilbene-2,2'-disulphonic acid (DIDS), known to partially block $Cl^-$ channels, significantly inhibited these currents. Its outward-rectifying current profile, shift in reversal potential in a low $Cl^-$ bath solution and pharmacological properties suggest that this is a $Ca^{2+}$-independent, volume activated $Cl^-$ current. We conclude therefore that volume activated $Cl^-$ channels play a putative role in RVD in human salivary gland acinar cells.

  • Acinic cell carcinoma is a rare salivary gland carcinoma, usually being found in the parotid gland and is uncommon in the other major and minor salivary glands. The tumor cells consist of either serous or mucous acinar cells with few ductal or myoepithelial cell elements. The tumor is a low-grade malignancy with slow growth potential. Surgical therapy depends on tumor size and the extent of infiltration into neighboring tissues. Superficial parotidectomy or total parotidectomy is the initial method of therapy in case of acinic cell carcinoma on parotid gland. When regional neck lymph nodes are involved, the operation is combined with a neck dissection, or with radiation therapy. In the short follow up period, acinic cell carcinoma has good prognosis with 5 year survival rate after surgery is over 80%. In the long-term follow-up, however, there is a tendency to increase in recurrence or metastasis. We experienced a case of acinic cell carcinoma of parotid gland in a 57-year-old female, so we report it with literatures review.

  • Although fine needle aspiration cytology (FNAC) has become one of the primary tools for diagnosing salivary gland lesions, some of these methods continue to confuse pathologists. The most common problems occur so-called basaloid neoplasms. Basal cell adenomas are frequently misdiagnosed as pleomorphic adenomas, and in worse cases, as adenoid cystic carcinomas. The cytologic diagnostic accuracy of basaloid neoplasms could be increased by a better understanding of the histology and the nature of the tumor cells. These are displayed well in aspiration smears. A consideration of differential points on the basis of the epithelial-stromal relationship is offered in this paper.

  • Salivary duct carcinoma is rare high grade adenocarcinoma, arising from larger excretory duct in major salivary gland, resembling the ductal carcinoma of the breast histologically. We report a case of salivary duct carcinoma of left parotid gland in which fine needle aspiration cytology was performed. Cytologic examination revealed 1) moderate cellularity, 2) flat sheets or three dimensional, tightly cohesive clusters on the necrotic background, 3) sometimes cribriform or papillary configuration, 4) polygonal or cuboidal cells with moderate anisonucleosis with abundant, granular and eosinophilic cytoplasm, 5) round to oval nuclei with irregular chromatin clumps, and 6) 1 or 2 inconspicuous nucleoli. Histologically, the tumor is mainly composed of cribriform intraductal component with central comedonecrosis, and small foci of infiltrating component including well differentiated ductal structures or irregular sheets in a desmoplastic stroma.

Efficacy of corticosteroid ductal irrigation in acute salivary gland inflammation induced in a rat model

  • Purpose: This study aimed to compare the therapeutic effects of corticosteroid irrigations and normal saline irrigations in the early inflammatory state of the salivary gland. Materials and Methods: Adult male Wistar rats were divided into experimental (n=6) and control (n=3) groups. Inflammation was induced in the experimental subjects on both sides of the submandibular gland with ligation. After 14 days, both sides of the glands were de-ligated and retroductal irrigation using saline (n=3) and a corticosteroid (n=3) was performed on the left sides only. The controls (n=3) were used to normalize the gland state for the effects of diet and aging. Magnetic resonance imaging was performed to confirm inflammation and post-irrigation gland recovery by measuring relative signal intensity (SI). The glands were excised for histological examination. Results: All experimental animals showed inflamed glands with increased SI and subsequent recovery of the gland with decreased SI to varying degrees. The SI of the controls showed no significant changes during the overall period. The mean SI change of the irrigated gland was higher than that of the non-irrigated side, without a significant difference. The corticosteroid-irrigated glands showed a greater change in SI than that of the saline-irrigated glands. Histology revealed that inflammation was not observed in most of the irrigated glands, while mild to moderate quantities inflammatory cells were found in non-irrigated glands. Conclusion: Corticosteroid irrigation mitigated the early stages of salivary gland inflammation more effectively than normal saline.

Culture of Rat Salivary Acinar Cells (백서 타액선 선포 세포의 배양)

  • We investigated the culture condition and effects of various growth factors on the culture of salivary gland acinar cells. Male, Sprague-Dawley rats (about 6 weeks old) were sacrificed and their submandibular, sublingual, and parotid glands were used as specimens. High oxygen level more than 90% and coating of Matrigel on culture dish were important factors to help increase the survival time of acinar cells, Proper concentration of enzymes such as collagenase and hyaluronidase during isolation steps was also important. Addition of various growth factors such as dexamethasone, insulin, transferrin, selenous acid, reduced glutathione, epidermal growth factor, isoproterenol, and putrescine in culture medium helped to increase lifetime of cultured salivary acinar cells.

  • Acinic cell carcinoma is a rare salivary gland tumor of low-grade malignancy. It comprises only about 2.5% of all salivary gland tumors. We recently experienced a case of fine needle aspiration cytology of acinic cell carcinoma of the parotid gland. The characteristic cytopathologic features were 1) cellular aspirate consisting of monomorphic cells in large sheets or singly, 2) formation of acini and/or microcysts, 3) abundant granular cytoplasm with sharp cytoplasmic borders, 4) bland nuclei with micronucleoli, and 5) clear background.

Spindle cell myoepithelioma of the parotid gland

  • Myoepithelioma was recognized as a histological distinct entity by the World Health Organization (WHO) in 1991. Myoepithelial cells are believed to be of ectodermal origin. In salivary glands, the myoepithelial cells that surround the intercalated ducts are spindled, which is in contrast to the large stellate ones that envelop the acini. Myoepithelioma is a benign salivary gland tumor that consists entirely of myoepithelial cells. A 53-year-old man presented with a 1-year history of a painless mass originating from the right parotid gland. The mass grew rapidly reaching a size of approximately 6 cm. The patient had no facial paralysis. The authors performed right parotidectomy. Immunohistochemistry study of this tumor showed that it was positive for vimentin, positive for S-100, focally positive for pancytokeratin, and focally positive for p63 and that it had a Ki-67 labeling index (below 10%). Additionally, the tumor was negative for epithelial membrane antigen, negative for actin, negative for desmin, negative for CD34 and negative for anaplastic lymphoma kinase. The authors present a case of benign spindle cell myoepithelioma of the parotid gland in a 53-year-old man diagnosed after immunohistochemistry study, describing its importance, along with a brief review of the literature.

  • The fine structure of the salivary glands of the pine moth, Dendrolimus spectabilis Butler, at the last larval period is observed using light and electron microscopes. The moths have single paired tubular salivary glands which openings are connected to the oral cavity through the upper jaw. By the external morphology and its functions, the glands are subdivided into three regions which are anterior reabsorptive region, middle storage region and posterior secretory region. Along the inner canal of the salivary gland two columns of the large glandular cells are connected each other and oriented to ring-like forms. By this cellular orientation, the glands have long and large tubular structure. From anterior to posterior region large nuclei of the glands are ramified like twigs of the tree, and in the cytoplasm of the cell numerous mitochondria and vacuoles are seen. Moreover, basal plasma membranes of the gland cells are heavily infolded. The anterior region of the glands keeps several characteristics related to the reabsorption of the material from the inner cavity to the glandular cells whereas, main salivary material is synthesized and secreted through the long and convoluted posterior region. The apical plasma membranes of the cells are the most heavily invaginated at the posterior regoin, but trachea and tracheoles are distributed only at the middle and posterior regions. In the cytoplasm of the middle region Golgi complexes appeared at the vicinity of the vesicles, and at the posterior region of the salivary glands multivesicular bodies are also observed.
