Myoepithelioma is composed exclusively of myoepithelial cells. Myoepithelial cells are ectodermally derived contractile cells that can be routinely identified in many normal tissues having secretory function such as major and minor salivary glands, lacrimal gland and sweat gland. Tumors composed exclusively of myoepithelial cells, so-called myoepitheliomas, are rare-less than 1% of all salivary gland tumors. Grossly, these tumors are well demarcated. The external surface is smooth and may be bosselated. The cut surface is white and homogenous. Microscopically, these tumors are surrounded by a thin fibrous capsule. They are composed of benign-appearing spindle- shaped and/or polygonal cells. Mitoses are rare. Frequently these tumors contain myxomatous stroma which is susceptible to alcial blue stain. Clinically, myoepitheliomas present as slow-glowing, painless masses and can not distinguished from pleomorphic adenomas. Treatment is the same as for pleomorphic adenoma, and the surgical excision should include a margin of normal tissue. Although the majority of myoepitheliomas have behaved in a beingn manner, pleomorphism and mitotic activity have been associated with local aggressiveness.

Mucoepidermoid carcinoma was initially recognized as occurring only in the salivary gland and only later was it appreciated that it occurred in the bronchus and trachea as well. Mucoepidermoid carcinoma of bronchial gland origin is extremely rare, and little is known about their natural history. This carcinoma is derived from the minor salivary gland of the proximal tracheobronchial tree and it is divided into low-grade and high-grade by gross, histologic, and ultrastructural criteria. Also its clinical and biologic behaviors are closely related with histologic grade of carcinoma. We have experienced a rare case of bronchial mucoepidermoid carcinoma associated with adenocarinoma which obstructed the left main bronchus and was successfully removed by the pneumonectomy.
Purpose: This study investigated the effects of 1 year of training on imaging diagnosis, using static ultrasonography (US) salivary gland images of Sjögren syndrome patients. Materials and Methods: This study involved 3 inexperienced radiologists with different levels of experience, who received training 1 or 2 days a week under the supervision of experienced radiologists. The training program included collecting patient histories and performing physical and imaging examinations for various maxillofacial diseases. The 3 radiologists (observers A, B, and C) evaluated 400 static US images of salivary glands twice at a 1-year interval. To compare their performance, 2 experienced radiologists evaluated the same images. Diagnostic performance was compared between the 2 evaluations using the area under the receiver operating characteristic curve (AUC). Results: Observer A, who was participating in the training program for the second year, exhibited no significant difference in AUC between the first and second evaluations, with results consistently comparable to those of experienced radiologists. After 1 year of training, observer B showed significantly higher AUCs than before training. The diagnostic performance of observer B reached the level of experienced radiologists for parotid gland assessment, but differed for submandibular gland assessment. For observer C, who did not complete the training, there was no significant difference in the AUC between the first and second evaluations, both of which showed significant differences from those of the experienced radiologists. Conclusion: These preliminary results suggest that the training program effectively helped inexperienced radiologists reach the level of experienced radiologists for US examinations.
The ultrastructure of submandibular gland was examined in the lesser white-toothed shrew, Crocidura suaveolens. The submandibular gland of C. suaveolens was a mixed gland composed of serous and mucous acinar cells. Secretory granules from the acini were discharged through the salivary ducts into the oral cavity. Serous and mucous acinar cells had well developed rough endoplasmic reticulum and mitochondria and large amount of granules. In case of serous acinar granules, an immature granule was formless and had only dense specks, and a matured granule was a complete round type delimiting by a single membrane and had a homogeneous dense center with dense specks on the border. In case of mucous acinar granules, while an immature granule was a round type and had an only homogeneous matrix and an indistinct limiting membrane, a mature granule was an even round type having a variety of pattern with several dense bands into the homogeneous matrix and had a distinct membrane. Therefore, a mature mucous acinar granule of C. suaveolens was not only distinct from those of the other mammalian species to have a variety of pattern but also from those of C. lasiura to have an even round type. A great serous-like secretory granules and Myelin-like body were observed in the cytoplasm and lumen of granular duct cells. Myelin-like body, a characteristic structure only reported in salivary gland of three shrews, was discharged from secretory cell into lumen by the manner of exocytosis which has little differences from discharging manner of secretory granules.

Epithelial-myoepithelial carcinoma (EMC) is a low-grade malignant salivary gland neoplasm that was first described in 1972. EMC occurs in the older age group, there is a female predilection and mainly involves the parotid gland. Most authors recommend superficial parotidectomy as a treatment for low-grade malignant tumor in the superficial lobe of parotid gland. The treatment of epithelial-myoepithelial tumors typically includes surgical excision aimed at achieving a R0 resection. This paper reports a case of EMC of the parotid gland treated only by a conservational surgical excision. The lesion was exposed by the retromandibular approach and detached. After the parotid gland envelop was exposed, the mass was observed and was easy to remove due to capsulation. The preoperative diagnosis was a pleomorphic adenoma on the left parotid gland. The tumor was removed surgically with a conservative extracapsular dissection. The postoperative diagnosis was EMC, so superficial parotidectomy or radiation therapy was considered. Nevertheless, the patient was observed and no additional treatment was attempted because the patient was old and a successfully excision of the tumor had been achieved.
Purpose: A plunging ranula is relatively uncommon and represents a mucus escape reaction occurring from a disruption of the sublingual salivary gland. It is a common condition found in young adults, even though the reported age range is 2 - 61 years. We report our experience of a complete excision of a plunging ranula via the intraoral and submandibular approach. Methods: A 23-year-old man had a large protruding mass in the right submandibular area. Initially, the protruding mass appeared bilaterally but the left side disappeared spontaneously. The MRI findings revealed a homogenous fluid attenuation mass in the submandibular space, suggesting a ranula. The sublingual gland was extirpated through the intraoral approach and the ranula excised totally via the submandibular approach. Results: The patient had an uneventful postoperative course without infection, paralysis and tongue sensory changes, etc. The pathology findings were characteristic of a pseudocyst without a lining epithelium or endothelium but with a vascular fibro-conective tissue wall filled with mucinous fluid. No recurrence was observed on the submandibular area during the 8 month follow-up period. Conclusion: The combined intraoral approach and submandibular approach is an effective and highly recommended method for sublingual gland extirpation and complete excision of a plunging ranula.
The tumor in accessory parotid gland (APG) is rarely occurred and its incidence is about 7.7% of all parotid gland neoplasms, but has a higher frequency of malignancy than major salivary glands. The mucoepidermoid carcinoma is the most common malignancy in APG, while B-cell lymphoma is less than 2%. It is often appeared as superficial mass in mid-cheek area. This lesion requires differential diagnosis with epidermoid cyst, lipoma, neurogenic tumors, Stensen's duct stone, lymphadenopathy and hemangioma etc. The mucosa associated lymphoid tissue (MALT) lymphoma, which is also termed extra-nodal marginal zone B-cell lymphoma tends to be localized disease for long time and has a relatively indolent course. We recently encountered a 69-year-old man with superficial solitary mass on the right cheek area that finally diagnosed as MALT lymphoma in APG. We report the rare and unique case with brief literature review.
Adenoid cystic carcinoma(ACC) is a rare malignant tumor that occurs in exocrine glands such as major and minor salivary glands, lacrimal glands, ceruminal glands. It has a tendency for delayed distant metastasis and long clinical course. ACC of the lacrimal gland is generally found in adults and is usually managed by radical orbitectomy and supplemental external beam irradiation. The authors present two cases of recurrent lacrimal gland ACC with review of the literature.
Carcinoma ex pleomorphic adenoma is rare, aggressive, poorly understood malignancy that occurs in the salivary glands. The patient was 70-year-old male presenting ovoid elevated submandibular mass that has been present for 30 years and rapid growing for 6 months. The initial cytologic finding by fine needle aspiration biopsy showed highly malignant cells and the radiologic finding revealed a $3.6{\times}3.7{\times}4.8cm$ sized mass with cervical lymphadenopathy. After wide excision of submandibular gland and modified radical neck dissection, the histologic examination revealed that tumor was composed partly of a benign pleomorphic adenoma and partly of an adenocarcinoma component with areas of calcification and frequent angiolymphatic invasion. We present a rare case of adenocarcinoma ex pleomorphic adenoma with tumor thrombus in the submandibular gland.

Malignant oncocytoma(oxyphil cell adenoma) is a extremely rare malignant tumor of the salivary glands and almost occurs in the parotid gland. A case of malignant oncocytoma of the parotid gland occurring in a 63 year old man is reported. Establishing a diagnosis of malignant oncocytoma essentially requires the application of two sets of criteria to the tumor under examination. First, the tumor cells must be identified as oncocyte and second, malignancy must be determined. Histochemical stains are not as useful as generally believed. Electron microscopy confirms the diagnosis by demonstrating an abundance of mitochondria. The diagnosis of malignant oncocytoma depends on several additional criteria in addition to cellular and nuclear pleomorphism, including lack of encapsulation, local infiltration, perineural and intravascular growth, and regional and/or distant metastasis. Satisfactory results from surgical resection and postoperative radiotherapy in our patient serve as additional experience in the treatment of this rare malignancy.
