  • Fine needle aspiration cytology (FNAC) is a very useful tool in the preoperative diagnosis of lesions of the salivary gland. Surgical therapy of high-grade malignancies (salivary duct carcinoma, mucoepidermoid carcinoma, squamous cellcarcinoma, carcinoma ex pleomorphic adenoma, small cell carcinoma, and sebaceous carcinoma) is different from that of benign lesions or low-grade malignancies. Therefore, the recognition of high-grade malignancies is important in salivary gland FNAC. Although recognition of high-grade malignancies of the salivary gland by FNAC is not difficult, precise classification of these malignancies is often impossible. Additionally, because of its rarity, FNAC of spindle cells and mesenchymal lesions of the salivary glands is a tool that is not familiar to many cytopathologists. The characteristic cytomorphologic features of these lesions are reviewed here with a discussion of specific diagnostic problems.

  • Background: Conventionally, indirect radiography has been used to diagnose salivary gland diseases. However, with the development of sialendoscopy, diagnosis and treatment of salivary gland diseases have become more effective. Herein, we report a case of sialolithotomy treated with sialendoscopy and compare it with the existing methods through a literature review. Case presentation: Two patients with a foreign body sensation under the tongue and dry mouth visited the Anam Hospital, Korea University. Radiographic examination revealed salivary stones inside the right Wharton duct, and the patients underwent sialolithotomy under local or general anaesthesia. The stones were totally removed, and there were no postoperative complications such as bleeding or pain. Conclusion: The development of sialendoscopy has enabled better definitive diagnosis of salivary gland diseases compared with the conventional methods; better treatment outcomes can be obtained when sialendoscopy is used in appropriate cases.

Comparative Studies on the Ultrastructure of Salivary Ducts between the Two Species of Snails, Achatina fulica and Incilaria fruhstorferi (두 종의 달팽이류 (Achatina fulica and Incilaria fruhstorferi) 사이의 타액관의 미세구조에 관한 비교연구)

  • We observed the salivary ducts of two species of snails, Achatina fulica and Incilaria fruhstoferi with an electron microscope, and obtained the following results. The intralobular and interlobular ducts of Achatina fulica assume the forms of round or ellipsoidal doughnuts. The boundaries between the endothelial cells are not clear. It is also found that the cytoplasm of the endothelial cells consists of the membrane infolded in interdigital form, and there are well -developed microvilli at the apical portion of the cytoplasm. On the other hand, the intralobular and interlobular ducts of Incilaria fruhtoferi consist of the irregular simple columnar epithelia. The high electron dense cytoplasm is filled with the irregular round granules. The microvilli at the apical portion of the cytoplasm are not so well-developed as those in Achatina fulica. In the salivary duct of Achatina fulica, the lumen has narrow and long tubular structure. The boundaries between the endothelial cells are not clear. The cytoplasm is full of many vacuoles and electron lucent granules. At the apical portion of the cytoplasm, lots of short and thin microvilli are found. The salivary duct of Incilaria fruhstorferi is wider ($65\times250{\mu}m$ in diameter) than that of Achatina fulica, and consists of endothelial cells of the same structures. At the apical portion of those endothelial cells, a lot of junction apparatus such as desmosomes are observed. The vessels in the salivary ducts of Achatina fulica and Incilaria fruhstoferi are observed mainly in the connective tissues between the salivary glands. The endothelial cell of the vessel has the irregular structure and looks dark due to the high electron density. These cells protrude their filopodia and phagocytosize foreign bodies.

Sialolithiasis of minor salivary gland: a challenging diagnostic dilemma

  • Minor salivary gland sialolithiasis (MSGS) is a not uncommon oral mucosal disease. Its clinical appearance may mimic a mucocyst or other benign submucosal overgrowth. Stasis of saliva, which accompanies MSGS, usually results in minor salivary gland inflammation, with a chronic sialadenitis appearance. MSGS typically is a painless lesion but can become painful when the salivary gland parenchyma or excretory duct becomes infected, with or without pus. However, misdiagnosis of this condition is rather common, as the clinical appearance is asymptomatic. The most common location is the upper lip, and MSGS affects males and females, with a slight predilection for males. The sialolith causing MSGS may be obvious during surgical excision, as in the case reported. In other cases, sialolith may be absent or fragmented. Differential diagnosis includes mucocele, swelling due to local irritation like fibroma and diapneusia, chronic abscess of the oral mucosa, and neoplasms either benign (lymphangioma, pleiomorphic adenoma) or malignant. Histopathological examination is needed to establish clinical diagnosis.

Quality Assuarance on Fine Needle Aspiration Cytology of Malignant Salivary Gland Neoplasms (악성 침샘종양의 세침흡인 세포검사의 정도관리)

  • To evaluate the quality of fine needle aspiration cytology diagnosis on malignant salivary gland neoplasms, cytologic findings were correlated with histologic diagnosis of 56 surgically removed malignant salivary gland tumors. Seven cases (12.5%) were insufficient, 23 cases (41.1%) were diagnosed as malignant, 17 (30.4%) cases were accurately diagnosed by histologic subtype, and 9 cases (16%) were diagnosed as benign. Five out of 9 false negative cases were misdiagnosed as pleomorphic adenomas. Except the cases with insufficient specimen, overall sensitivity was 81.6%, and the sensitivity varied according to the histologic subtype; 91% in salivary duct carcinoma, 100% in carcinoma ex pleomorphic adenoma, 50% in mucoepidermoid carcinoma, 63% in adenoid cystic carcinoma, and 50% in acinic cell carcinoma. The diagnostic accuracy differed among cytopathologists irrespective of periods after acquisition of board of pathologists. These results confirm that salivary gland neoplasm can be easily misdiagnosed in fine needle aspiration cytology and a great caution should be given in diagnosing the benign appearing salivary aspirates to avoid under-diagnosis of malignant neoplasm with low grade cytologic atypia.

Change of the Amylase Secretion on the Rat Submandibular Gland in the Restraint Stress Condition (구속스트레스에 의한 백서 악하선의 Amylase 분비 변화)

  • In currently, stress diseases are increased that present several sign and symptoms. Under stress condition, there are dry mouth, burning mouth syndrome, oral mucosa diseases and halitosis more frequently. Changing of salivary proportion is checked in almost patients with changing of function and structure in salivary gland. This study purpose are what effect stress does on salivary gland, and a-amylase on salivary gland. This study was resulted that 1. Under restraint stress, acinar cells are vacuolization and changing of intercellular spaces are separated, and peripheral tissues of duct are changed 2. Acinar cells were shrunk after 3 hours under restraint stress, intercellular space was separated after 6hours, peripheral tissues of duct started to change after 72 hours, and acinar cells and peripheral tissues of duct were all severely changed after 168hours. 3. In immunohistochemical study, amylase reaction was showed partially and irregularly after 3 hours, was getting little milder after 6 hours. And amylase reaction was gradually increased from the time of 12 hours after experiment up to the time of 48 hours after experiment. But after 168 hours, amylase appearance was diminished. According this result, emotional stress can change of salivary gland structure, and amylase secretion, the important digestive enzyme from salivary gland is changed and it is supposed to make digestive disorder and to make halitosis efficiency. So, we need to study about secretion of amylase.

Characterization of Primary Epithelial Cells Derived from Human Salivary Gland Contributing to in vivo Formation of Acini-like Structures

  • Patients with head and neck cancer are treated with therapeutic irradiation, which can result in irreversible salivary gland dysfunction. Because there is no complete cure for such patients, stem cell therapy is an emerging alternative for functional restoration of salivary glands. In this study, we investigated in vitro characteristics of primarily isolated epithelial cells from human salivary gland (Epi-SGs) and in vivo formation of acini-like structures by Epi-SGs. Primarily isolated Epi-SGs showed typical epithelial cell-like morphology and expressed E-cadherin but not N-cadherin. Epi-SGs expressed epithelial stem cell (EpiSC) and embryonic stem cell (ESC) markers. During long-term culture, the expression of EpiSC and ESC markers was highly detected and maintained within the core population with small size and low cytoplasmic complexity. The core population expressed cytokeratin 7 and cytokeratin 14, known as duct markers indicating that Epi-SGs might be originated from the duct. When Epi-SGs were transplanted in vivo with Matrigel, acini-like structures were readily formed at 4 days after transplantation and they were maintained at 7 days after transplantation. Taken together, our data suggested that Epi-SGs might contain stem cells which were positive for EpiSC and ESC markers, and Epi-SGs might contribute to the regeneration of acini-like structures in vivo. We expect that Epi-SGs will be useful source for the functional restoration of damaged salivary gland.

ANALYSIS OF 67 MALIGNANT SALIVARY GLAND TUMORS IN KOREAN POPULATION (한국인에서 발생한 67례의 악성타액선종양에 대한 연구)

  • Malignant salivary gland tumor is rare neoplasm. In Korean population, retrospective study of malignant salivary gland tumor has not been performed. We analyzed 67 cases of malignant salivary gland tumors from 2001 to 2005 in Seoul National University Dental Hospital in Seoul, Korea. The mean age is 51.7 and the male to female ratio is 1:1.39. The most affected site is the palate. Histologically, the tumors were classified as adenoid cystic carcinoma(34.4%), mucoepidermoid carcinoma(31.3%), adenocarcinoma, NOS(11.9%), polymorphous low grade adenocarcinoma(3.0%), salivary duct carcionoma(6.0%), carcinoma ex pleomorphic adenoma(4.5%), myoepithelial carcinoma(4.5%), epithelial-myoepithelial carcinoma(1.5%), cyatadenocarcinoma(1.5%) and adenosquamous carcinoma(1.5%).

Effect of the Streptozotocin Induced Diabetes in the Rat Submandibular Glands (Streptozotocin유도 당뇨병이 백서 악하선에 미치는 영향에 관한 병리조직학적 연구)

  • The purpose of this study was to observe the microscopic change of salivary gland tissues, which is the cause of xerostomia in diabetic condition: for this target the author injected STZ 0.1ml/100gm b.w. on rat to produce diabetes, and than observed microscopic change in submandibular gland through the histopathologic method, obtaining as follows : 1. All of the experimental specimens suffered diabetes after injection of STZ, but the blood glucose level was irregular. 2. There were not interrelationship between the blood glucose level and microscopic change on salivary gland tissues. 3. The salivary gland changed after diabetes initiation in lapse of times; after 14 days,suffered severe destruction, however after 17 days, it was regenerated. 4. Salivary glands showed congested, destructive acini cells, and hyperplastic ductal cells as well as salivary gland duct-like structures. 5. Then were accumulation of fat granules within the cytoplasm of the acini cells on mucous gland in diabetic condition. 6. According to insulin injection, there were no more changes on salivary gland tissues, even in the accumulation of fat granules. 7. Histological changes of the serous gland were obvious more than the mucous gland in this experimental condition.
