Effects of an Agro-Healing Activity Program on the Physiological Condition of Adults with Chronic Metabolic Diseases

  • This study was conducted to find out the effects of agro-healing activities and strolling on the physiological conditions of patients with chronic metabolic diseases. A total of 11 subjects participated in agro-healing activities, and their average age was 49.6±11.3. Changes in their salivary cortisol were compared and analyzed before and after participating in the agro-healing activity program, and it was found that the stress hormone decreased after participating in the program. Differences in brain wave between before and after participating in agro-healing activities and strolling were measured, and the ratio of alpha to high beta (RAHB) of subjects increased in most of the measured points after participating in agro-healing activities. In addition, the increase in the value in the temporal lobe (T3) that controls mortor skills and occipital lobe (O2) that controls visual functions indicates that the activities were effective in improving relaxation and stability. The relative low beta (RLB) power spectrum of subjects also statistically significantly decreased in the right occipital lobe (O2) that controls visual functions, the relative mid beta (RMB) and relative high beta (RHB) power spectrum decreased in the left temporal lobe (T3) and the right occipital lobe (O2). These results indicate that the activities were effective in improving relaxation under no stress and tension. The systolic and diastolic blood pressure of subjects after participating in agro-healing activities decreased from the prehypertension level to the normal level. These results indicate that horticultural activities and visual elements in healing farms, such as agro-healing activities and strolling, have positive impacts on patients with chronic metabolic diseases accompanied with diabetes or high blood pressure by increasing the brain activity and psycho-physiological conditions of participants.

Interrelationship between the Oral Disease and the Systemic Disease to Inpatient(I) (전신질환으로 입원한 환자의 구강질환과 전신질환의 상관관계(I))

  • Purpose : To investigate the actual conditions of diagnosis and treatment of oral disease of inpatient with systemic disease. Methods : A total of 110 subjects, inpatient due to systemic disease for diagnosis and treatment of oral disease was requested to answer the medical history and dental treatment record. Results : In the main systemic disease, Endocrine, nutritional and metabolic diseases is composed of Gingivitis and periodontal diseases 44.9%, Diseases of salivary glands 22.4%, Within Normal Limit, Dental caries 12.2%, Diseases of pulp and periapical tissues 4.1%, Embedded and impacted teeth, Other diseases of hard tissues of teeth 2%. In the main oral disease, Gingivitis and periodontal diseases is composed of Non-insulin-dependent diabetes mellitus 39.2%, Cerebral infarction 29.4%, Nerve root and plexus disorders 5.6%, Intracerebral hemorrhage 3.9%, Malignant neoplasm of stomach, Thyrotoxicosis, Schizophrenia, Alcoholic liver disease, Nephrotic syndrome 2%. Conclusion : These findings indicate that inpatient due to the systemic disease is significantly correlated to the oral disease. The patients of oral disease interrelationship between inpatient and outpatient of systemic disease should be validated by future research.

An Investigation into Oral Medicine Inpatients by Systemic Disease (전신질환으로 입원한 환자의 구강내과질환 발현실태)

  • Purpose : To investigate the actual conditions of diagnosis and treatment of oral medicine inpatient with systemic disease. Methods : A total of 54 oral medicine subjects, inpatient due to systemic disease for diagnosis and treatment of oral disease was requested to answer the medical history and dental treatment record. Results : The ratio of gender is composed of male 44% and female 56%, the distribution of age is the order of the 50-59 group 37%, the 60-69 group 26%, the 40-49 group 22%. Systemic disease is composed of Endocrine, nutritional and metabolic diseases 36%, Diseases of the circulatory system 36%, Diseases of the nervous system 10%. Chief complain of oral disease is composed of routine check for oral health 26%, craniomandibular disorders 18%, soft tissue problem 18%. Oral disease is composed of Diseases of salivary glands 32%, Gingivitis and periodontal diseases 23%, Dentofacial anomalies 16% Conclusion : These findings indicate that oral medicine inpatient due to the systemic disease is significantly correlated to the oral disease. The patients of oral disease interrelationship between inpatient and outpatient of systemic disease should be validated by future research.

  • Purpose: Cigarette smoking is a major risk factor in periodontal diseases. The pathogenesis of periodontal diseases may be affected by alterations of the inflammatory response by smoke. Nitric oxide (NO) is a gaseous, colorless, highly reactive, short-lived free radical with a pivotal role in the regulation of various physiological and pathological mechanisms in the body. It is important in host defense and homeostasis, on the one hand, whereas, on the other hand, it modulates the inflammatory response in periodontitis, leading to harmful effects. The aim of this study was to assess the levels of NO in both the serum and saliva of smokers and nonsmokers having chronic periodontitis and to compare them with periodontally healthy controls. Methods: Sixty subjects participated in the study and were divided into three groups: group I, healthy nonsmoking subjects; group II, nonsmoking patients with chronic periodontitis; group III, smoking patients with chronic periodontitis. Each group consisted of twenty subjects. The biochemical estimation of NO in the collected serum and in the saliva was performed using the Griess colorimetric reaction. Results: The results showed that the mean value of the salivary and serum NO was greater in group II than in group I, and also greater in group III than in group II. Conclusions: NO appears to play an important and rather complex role in the immuno-inflammatory process and in the remodeling and maintenance of osseous structures. It is therefore logical that modulation of this mediator has potential for the treatment of a number of inflammatory conditions including periodontal disease.

  • Acute epiglottitis is a life threatening inflammatory. disease of the upper airway mainly in children, however, the recent reports about acute epiglottitis in adults are increasing. The common symptoms are sore throat, dysphagia, dyspnea and salivary drooling. As the laryngeal edema progresses, the patient sits up, leans forward, with the chin thrust forward, having obvious difficulty breathing. Early recognition and proper airway maintenance until the inflammatory edema subsides are essential steps to avoid a possible life threatening upper airway obstruction. We experienced two cases of acute epiglottitis with sitting up position, chin thrust forward, having dyspnea.

A Report of Two Cases of Mucoepidermoid Carcinoma (폐에 생긴 점액표피양 암종 2예)

  • Mucoepidermoid carcinoma of the lung can be divided into low grade and high grade varieties, like their major salivary glands counterparts. This carcinoma comprises only. about 0.5% of all pulmonary neoplasma. Their clinical and biologic behaviors are closely related with histologic grade of carcinoma. We have experienced two cases of mucoepidermoid carcinoma. Case 1 occurred in a 16 years old male and showed the histologic features of low grade type. This carcinoma arised from the right upper lobe bronchus which was removed by the right upper lobectomy. Case 2 occurred in a 41 year old male was the high grade carcinoma located at the distal left main bronchus which was removed by the left pnemonectomy.

Primary Adenoid Cystic Carcinoma of Salivary Gland with Metastatic to the Lung (악하선 종괴를 동반한 고립성 폐결절)

  • A 22-year old female visited CNUH due to palpable neck mass. Cytologic examination of a fine needle aspiration was performed and the result was Pap class II. Routine chest x-ray shows solitary pulmonary nodule. For rule-out malignancy, FNA at neck mass was repeated and pathologic finding was dysplasia. She was admitted to MI department for evaluation of solitary pulmonary nodule and percutaneous needle aspiration was done. Pathologic diagnosis was adenoid cystic carcinoma. Thereafter, the lesions were treated by excisional biopsy of submandibular gland mass with left supraomohyoid neck dissection and wedge resection of right lower lobe at ENT department and thoracic and cardiovascular surgery department, respectively. Final diagnosis was adenoid cystic carcinoma arising in submandibular gland with solitary lung metastasis. According to TMN staging system, surgical staging is stage IV of T2N0M1. Clinical follow-up to postoperative 13 months in this case showed that she is alive and well without evidence of recurrence.

  • Inflammatory pseudotumor is a infrequent, benign tumor that affects at all ages and involves the lung, gastrointestinal tract, and salivary gland. They are the most common, isolated, primary lesion of the lung in children, and usually present as circumscribed, peripheral, parenchymal tumors or endobronchial tumors, which may be usually static or increase slowly in size without causing symptoms. Surgical excision is the treatment of choice because of the location and benign nature of this lesion. The prognosis after resection is excellent. Other therapeutic modalities include radiation and steroid therapy. We experienced two cases of inflammatory pseudotumor of the lung parenchyme and endobronchus. We report these cases with a review of literatures.

  • The pulmonary lymphomas were thought to originate in specialized lymphoid tissue that is associated with bronchial mucosa(bronchus-associated lymphoid tissue(BALT)), and they were categorized as mucosa-associated lymphoid tissue(MALT) lymphoma. MALT lymphoma consists of a monoclonal population of cell, in contrast to reactive lymphoid proliferation, which consists of polyclonal cells. Lymphoma arising from MALT(=MALToma) represents a distinct clinicopathologic features. It is usually localized 10 their original site for a long time and shows much more favorable prognosis than lymphoma at other site. Some MALT lymphoma could arise simultaneously or successively in different organ or that cells from MALT lymphoma might circulate and give rise to another lymphoma by homing in the MALT of another organ, such as breast, salivary gland, stomach etc, and can be multifocally disseminated or recurred. We report a case of low-grade B-cell lymphoma of the mucosa-associated lymphoid tissue(MALT) of the lung, which was confirmed by open lung biopsy, immunohistochemistry and PCR assay.

  • Inflammatory pseudotumor, also known as plasma cell granuloma, is a rare, benign tumor that affects at all ages and frequently involves the lung, gastrointestinal tract, and salivary gland. They are the most common, isolated, primary lesion of the lung in children less than 16 years of age, and usually present as circumscribed, peripheral, parenchymal tumors, which may be static or increase slowly in size without causing symptoms. Surgical excision is the treatment of choice bacause of the location and benign nature of this lesion. The prognosis after resection is excellent. Adjuvant therapeutic modalities include radiation and steroid therapy. Recently, we experienced a case of inflammatory pseudotumor of the lung, the diagnosis of which was made by percutaneous fine needle aspiration cytology examination. We tried steroid and the lesions of the lung was completely resolved. We report this case with a review of literatures.
