심한 상하악 치열궁 부조화 환자의 수술적 해결: 증례보고 (Surgical and Orthognathic Treatment of Skeletal Class III Featuring Severe Transversal and Sagittal Discrepancy: A Case Report)

  • Multiple segment osteotomy orthognathic surgery serves to combine the total or segmental maxillary and mandibular correction of the dentofacial deformities with concurrent procedures to provide immediate repositioning to the dento-osseous elements. In addition, splitting the palate may often be necessary to correct a functionally poor relationship of the maxilla to the mandible or the facial skeleton by realigning the maxillary arch. In this case, the discrepancy in a bimaxillary horizontal relationship and the space between the 2nd premolar and 2nd molar was retained after lengthy preoperative orthodontic treatment. However, we could correct these dento-osseous discrepancies immediately by performing midpalatal expansion, anterior segmental osteotomy and symphyseal osteotomy with bimaxillary osteotomies. If the blood supply to each segment segments was maintained and primary closure of the operation site was feasible, multiple segment osteotomy was considered as a very effective technique for treating dentofacial deformities in vertical, transverse, and sagittal dimensions with differential repositioning of all segments.

  • First of all a good surgical access is considered among various approach methods to the cystic lesion. A poor surgical access can lead to a failure of the whole treatment. A sagittal split ramus osteotomy (SSRO) on the large cyst occurring in the mandibular ramus lets us not only reduce operation time, but can additionally contribute to a good visual field. In addition, a merit exists that it lets this operating method provide soft tissue adhesion for proximal and distal segment and decrease post operative necrosis. We experienced three cases of a large cyst on the mandibular angle and ramus. By employing a sagittal splitting of the mandible, it provided good surgical access and operation results without recurrence during a follow-up period. The surgical technique described may be helpful in treating similar large cysts.

하악전돌증 환자에서 내측 익돌근 근력측정 및 분석 (MEASUREMENT AND ANALYSIS OF THE RESISTANT MUSCLE FORCE OF MEDIAL PTERYGOID MUSCLE IN THE MANDIBULAR PROGNATHIC PATIENTS)

  • The purpose of this study was to evaluate the resistant force of medial pterygoid muscles against the mandibular advancement and distraction to anterior, and inquire into the relationship between medial pterygoid muscles and cephalometric variables. Sixty six patients with class III malocclusion underwent bilateral sagittal splitting of ramus with intraoralvertico-sagittal ramus osteotomy for mandibular set-back. The spring scale was used to measure the resistance of medial pterygoid muscles after splitting of ramus. Skeletaldental cephalometric analysis was made and statistic package was used for correlation between resistance and cephalometric variables. The resistant force of the right medial pterygoid muscle was greater than the left one in Koreans with class III malocclusion, and the force had a linear regression relationship with facial depth. The results suggested that facial depth has significant correlation with the resistance of medial pterygoid muscle, which can be acquired from patient's cephalometric analysis.

구내 경사 분열 골절단술(Intraoral oblique splitting osteotomy)에 의한 하악 전돌증의 치험례 (A CASE REPORT OF CORRECTION OF MANDIBULAR PROGNATHISM BY INTRAORAL OBLIQUE SPLITTING OSTEOTOMY OF MANDIBULAR RAMI)

  • 저자들은 2명의 하악 전돌증 환자에게 수술전 치열교정후 구내 하악지 경사 분열 골절단술을 시행하고 술후 치열교정을 계속 시행중이며 이 술식으로 다음과 같은 결과를 얻었다. 1. 전돌된 하악골의 후방이동으로 환자의 안모 추형과 심리적 열등감을 해소했으며 교합관계는 Angle 1급으로 회복하였으며 전반적인 부정교합을 치열교정과 더불어 해소했다. 2. 술후 감염과 골편 파절, 안면 지각이상, 측두하악 장애는 나타나지 않았으며 48시간 지난후 안면부 종창은 서서히 해소되었다. 3. 수술후 약간 고정은 8주간 시행했으며 악간 고정 제거 1주후 정상적인 개구가 가능했다. 4. 경조직의 후방이동에 따른 연조직의 하순과 하악 이부에서의 이동량은 본 증례에서는 거의 1:1로 나타났다. 5. 악간 고정 기관과 술후 치열고정 기간중의 회귀현상은 약간 관찰되었으나 이에 대하여는 더 오랜 기간의 연구 관찰이 필요하고 관찰중이다.

  • Odontogenic keratocyst (OKC) is a epithelial developmental cyst which were first described by Phillipsen in 1956. The frequency of OKC has been reported to vary from 3% to 11% of odontogenic cysts. The most characteristic clinical aspect of OKC is the high frequency of recurrence. The mechanism of recurrence is thought to be related to residues of cyst epithelium and an intrinsic growth potential following excision. And since the lining of the OKC is thin and friable, removal of the cyst in one piece may sometimes be difficult. Complete removal of the cyst lining without leaving behind remnants attached to the soft tissue or bone is necessary to avoid recurrence. Therapeutic approaches vary in different studies from marsupialization and enucleation, which may be combined with adjuvant therapy such as cryotherapy or Carnoy's solution, to marginal or radical resection. The recurrent rate varies from approximately 20% to 62%. And OKC in the angle-ramus region of the mandible had a higher tendency to recur, because of the difficulty in accessing and removing OKC from the ramus. By employing a sagittal splitting of the mandible a good surgical access was provided and cyst could be removed completely. We present an illustrative case of a small, lobulated OKC that involved ramus on mandible, and a review of the contemporary literature.

컴퓨터단층촬영과 파노라마상을 이용한 한국인 하치조관의 하악에서의 협, 설측 위치 관계에 대한 연구 (STUDY ON THE RELATIONSHIP OF THE INFERIOR ALVEOLAR NERVE POSITION BETWEEN BUCCAL AND LINGUAL SIDE USING CT AND ORTHPANTOMOGRAM)

  • When bilateral sagittal split ramus osteotomy or mandibular angle reduction are carried out, we have to consider the position of inferior alveolar nerve. For bone splitting or resection using a saw or an osteotome, the bucco-lingual position of the inferior alveolar nerve plays an important role in the preventing perioperative complications such as paresthesia or anesthesia. Because it is rare to find literatures concerning the mean anatomic position of the inferior alveolar nerve in Koreans, we investigated 30 patients who underwent to take CT and orthopantomogram for implant surgery, and evaluated the bucco-lingual position and vertical relationship of the inferior alveolar nerve at the mandible. The results showed that the distance between inferior alveolar nerve and buccal plate was the farthest at mandibular second molar ($7.1{\sim}7.4mm$) and the nearest at mandibular angle area ($4.4{\sim}4.8mm$). But it was no statistical relationship between the bucco-lingual postion of inferior alveolar nerve on the CT and its vertical position on the OPT. In conclusion, the results suggest that a careful surgical procedure is needed at the mandibular angle area to avoid a nerve damage and there are sufficient bone materials at the mandibular second molar are for bilateral sagittal split ramus osteotomy or mandibular angle reduction or plate fixation. And OPT is not usefull for the evaluation of a relative bucco-lingual position of inferior alveolar nerve in relation to its vertical postion on the OPT.

악안면 기형환자에서 내측 익돌근 근력측정 및 의미분석 (Analysis of the resistant muscle force against distraction in the maxilla-facial deformities)

  • Recents reports have demonstrated that force and direction is important during mandibular distraction osteogenesis. The purpose of this study was to evaluate the resistant force of internal pterygoid muscles and inquire into relationship between internal pte'Ygoid muscles and cephalometric parameters. Eighty four patients with class III malocclusion underwent bilateral sgittal splitting of ramus with intraoral vertico-sagittal ramal osteotomy. A spring scale were used for measuring for resistence of internal pterygoid mescles after splitting of ramus. Skeletal-dental cephalometric analysis was made following statistic package was used for correlation between resistence and cephalometric parameters. The resistant force of right internal pterygoid muscle was greater than left muscle in Korean with class III malocclusion and the force had a linear regression relationship with facial depth (distance between nasion and gonion). The results suggested that facial depth has significant correlation of the resistance of internal pterygoid muscle (p<0.05).

Orthognathic surgery with removal of lipoma in the asymmetric mandibular prognathism of a patient with a mandibular bone defect due to intramuscular lipoma on the medial aspect of the ramus: a case report

  • Lipomas, the most common soft-tissue mesenchymal neoplasms in adults, are characterized by the proliferation of mature white adipocytes without cytologic atypia. Lipomas are rarely observed in the head and neck region. We present a case of resection and orthognathic surgical removal of an intramuscular lipoma of the mandible with involvement of the mandibular ramus and condylar head and neck. An 18-year-old female patient was referred to our hospital for orthognathic surgery for the management of facial asymmetry and mandibular prognathism. The patient did not present with facial swelling, pain, or temporomandibular dysfunction; however, on radiographic examination, including cone-beam computed tomography and magnetic resonance imaging, an infiltrative fatty lesion was observed in the masticator space inside the right mandible, and the adjacent mandible exhibited bone thinning and deformity. Resection of the lipoma was performed along with orthognathic surgery, including a Le Fort I osteotomy for the maxilla and bilateral sagittal split ramus osteotomy (BSSRO). In this case, because the ramus was split using BSSRO, accessing the lipoma intraorally was easy. Consequently, aesthetic scarring was avoided, and no complications, such as unfavorable splitting or pathologic fracture, occurred. Although recurrence has not been observed about 1 year, long-term follow-up should be performed.

악교정 수술 후 감각소실에 관한연구 (NEUROSENSORY DEFICIT AFTER ORTHOGNATHIC SURGERY)

  • Aims: This study was designed to determine the incidence of altered sensation in patients undergoing orthognathic surgery. Method: Seventy two patients who underwent orthognathic surgery between January, 1999 and December, 1999 constituted the study group. Seven patients were excluded because of lack of follow up. Sixty five patients were followed using objective and subjective neurologic testing during the period immediately following operation, 1 month, 2 months, 6 months, and 1 year postoperatively. Age ranged from 17 to 38 years, with a mean of 24.5 years. Male patients were 21, female 44. Twenty eight bilateral sagittal splitting ramus osteotomy(BSSRO) of mandible were performed, 35 BSSRO with genioplasty, 2 genioplasties. Information on the degree of intraoperative nerve encounter was obtained from the surgical reports in 47 patients and was divided into the following three categories: (1) the nerve was not encountered in 23 patients; (2) the nerve was exposed in 11 patients; (3) the nerve was exposed and repositioned from the proximal segment in 13 patients. Results: Four patients reported altered nerve sensation of lower lip and/or chin(6.2%) at final follow up. Two patients underwent BSSRO and the other two patients BSSRO with genioplasty. Three of the patients underwent nerve exposure during the operation. Conclusion: We suggest that the nerve exposure during the operation might be partly responsible for nerve dysfunction after orthognathic surgery.
