견관절 운동면과 상완골 회전에 따른 견관절 가동범위의 차이 (Shoulder Range of Motion According to Sagittal, Coronal and Scapular Plane and Humeral Rotation)

  • For effective rehabilitation of the shoulder, physical therapists must have correct knowledge of shoulder movements. The purpose of this study was to determine the relationship between shoulder movements and the rotation of the humerus in the sagittal, coronal and scapular planes. Fifty normal subjects(25 male, 25 female) were tested using a Dualar-plus digital goniometer and an air-splint. The subjects performed active shoulder elevation in each plane with the humerus rotated in both medial and lateral directions. The range of motion(ROM) of the glenohumeral joint was measured three times. The paired t-test was used to determine the difference in ROM between medial and lateral rotation of the humerus. Results showed that, in the sagittal and the coronal planes, there was a significant difference(p < 0.01) in ROM of the shoulder between medial rotation and lateral rotation which was greater. But in the scapular plane, there was no difference between medial and lateral rotation. Physical therapists should consider these results when the goal of treatment is to increase ROM of the shoulder.

경추 전방 유합 환자의 장기 수술 결과에 영향을 주는 방사선학적 요인 분석 (Analysis of Radiologic Factors Affecting Longterm Surgical Results of Anterior Cervical Fusion Patients)

  • Object : Anterior cervical discectomy and fusion is accepted as appropriate surgical intervention for disease processes of the ventral cervical spine. In some cases, however, improvement of symptom following operation develop myelopathy or rhizopathy at longterm follow-up. We studied to clarify the correlation between clinical results and plain radiologic findings at long term follow-up. Methods : A total of 86 patients who underwent anterior cervical discectomy and fusion were grouped into deteriorated and good recovery group. These two groups were compared in lateral functional roentgenograms on the cervical kyphosis and disc height at fused level, sagittal plane rotation and dynamic sagittal canal diameter at adjacent level. Results : The presence of cervical kyphosis and disc height at fused level do not correlate with long term followup results. Sagittal plane rotation of more than 20 degrees were identified in 36% of the cases in deteriorated group, whereas the same findings were identified in 15% of the cases in good group. Sagittal diameter of less than 12mm were identified in 48% of the cases in deteriorated group, whereas the same finding were identified in 8% of the cases in good group. Preoperatively, sagittal plane rotation of more than 20 degrees at adjacent level were identified in 28% of the cases in deteriorated group, whereas the same findings were identified in 13% of the cases in good group. Preoperatively sagittal diameter of less than 12mm at adjacent level were identified in 40% of the cases in deteriorated group, whereas the same finding were identified in 7% of the cases in good group. Conclusion : Large sagittal plane rotation and small dynamic sagittal diameter at adjacent level were factors that might be associated with later clinical deterioration after cervical anterior fusion.

Restoration of Sagittal Balance in Spinal Deformity Surgery

  • The prevalence of patients with adult spinal deformity (ASD) has been reported as high as 68%. ASD often leads to significant pain and disability. Recent emphasis has been placed on sagittal plane balance and restoring normal sagittal alignment with regards to the three dimensional deformity of ASD. Optimal sagittal alignment has been known to increase spinal biomechanical efficiency, reduce energy expenditure by maintaining a stable posture with improved load absorption, influence better bony union, and help to decelerate adjacent segment deterioration. Increasingly positive sagittal imbalance has been shown to correlate with poor functional outcome and poor self-image along with poor psychological function. Compensatory mechanisms attempt to maintain sagittal balance through pelvic rotation, alterations in lumbar lordosis as well as knee and ankle flexion at the cost of increased energy expenditure. Restoring normal spinopelvic alignment is paramount to the treatment of complex spinal deformity with sagittal imbalance. Posterior osteotomies including posterior column osteotomies, pedicle subtraction osteotomies, and posterior vertebral column resection, as well anterior column support are well known to improve sagittal alignment. Understanding of whole spinal alignment and dynamics of spinopelvic alignment is essential to restore sagittal balance while minimizing the risk of developing sagittal decompensation after surgical intervention.

Condylar positioning changes following unilateral sagittal split ramus osteotomy in patients with mandibular prognathism

  • Background: This study was performed to evaluate three-dimensional positional change of the condyle using three-dimensional computed tomography (3D-CT) following unilateral sagittal split ramus osteotomy (USSRO) in patients with mandibular prognathism. Methods: This study examined two patients exhibiting skeletal class III malocclusion with facial asymmetry who underwent USSRO for a mandibular setback. 3D-CT was performed before surgery, immediately after surgery, and 6 months postoperatively. After creating 3D-CT images by using the In-vivo $5^{TM}$ program, the axial plane, coronal plane, and sagittal plane were configured. Three-dimensional positional changes from each plane to the condyle, axial condylar head axis angle (AHA), axial condylar head position (AHP), frontal condylar head axis angle (FHA), frontal condylar head position (FHP), sagittal condylar head axis angle (SHA), and sagittal condylar head position (SHP) of the two patients were measured before surgery, immediately after surgery, and 6 months postoperatively. Results: In the first patient, medial rotation of the operated condyle in AHA and anterior rotation in SHA were observed. There were no significant changes after surgery in AHP, FHP, and SHP after surgery. In the second patient, medial rotation of the operated condyle in AHA and lateral rotation of the operated condyle in FHA were observed. There were no significant changes in AHP, FHP, and SHP postoperatively. This indicates that in USSRO, postoperative movement of the condylar head is insignificant; however, medial rotation of the condylar head is possible. Although three-dimensional changes were observed, these were not clinically significant. Conclusions: The results of this study suggest that although three-dimensional changes in condylar head position are observed in patients post SSRO, there are no significant changes that would clinically affect the patient.

Evaluation of mandibular condyle position in Class III patients after bimaxillary orthognathic surgery: A cone-beam computed tomography study

  • Objective: This retrospective study evaluated the mandibular condyle position before and after bimaxillary orthognathic surgery performed with the mandibular condyle positioned manually in patients with mandibular prognathism using cone-beam computed tomography. Methods: Overall, 88 mandibular condyles from 44 adult patients (20 female and 24 male) diagnosed with mandibular prognathism due to skeletal Class III malocclusion who underwent bilateral sagittal split ramus osteotomy (BSSRO) and Le Fort I performed using the manual condyle positioning method were included. Cone-beam computed tomography images obtained 1-2 weeks before (T0) and approximately 6 months after (T1) surgery were analyzed in three planes using 3D Slicer software. Statistical significance was set at P < 0.05 level. Results: Significant inward rotation of the left mandibular condyle and significant outward rotation of the right mandibular condyle were observed in the axial and coronal planes (P < 0.05). The positions of the right and left condyles in the sagittal plane and the distance between the most medial points of the condyles in the coronal plane did not differ significantly (P > 0.05). Conclusions: While the change in the sagittal plane can be maintained as before surgery with manual positioning during the BSSRO procedure, significant inward and outward rotation was observed in the axial and coronal planes, respectively, even in the absence of concomitant temporomandibular joint disorder before or after the operation. Further long-term studies are needed to correlate these findings with possible clinical consequences.

Cone-beam computed tomographic evaluation of the condylar remodeling occurring after mandibular set-back by bilateral sagittal split ramus osteotomy and rigid fixation

  • Objective: To evaluate condylar head remodeling after mandibular set-back sagittal split ramus osteotomy (SSRO) with rigid fixation in skeletal class III deformities. The correlation between condylar head remodeling and condylar axis changes was determined using cone-beam computed tomography (CBCT) superimposition. Methods: The CBCT data of 22 subjects (9 men and 13 women) who had undergone mandibular set-back SSRO with rigid fixation were analyzed. Changes in the condylar head measurements and the distribution of the signs of condylar head remodeling were evaluated by CBCT superimposition. Results: The subjects showed inward rotation of the axial condylar angle; reduced condylar heights on the sagittal and coronal planes; and resorptive remodeling in the anterior and superior areas on the sagittal plane, superior and lateral areas on the coronal plane, and anterior-middle and anterior-lateral areas on the axial plane (p < 0.05). Conclusions: The CBCT superimposition method showed condylar head remodeling after mandibular set-back SSRO with rigid fixation. In skeletal class III patients, SSRO with rigid fixation resulted in rotation, diminution, and remodeling of the condylar head. However, these changes did not produce clinical signs or symptoms of temporomandibular disorders.

스탠스 자세와 운동면의 차이에 따른 위팔어깨관절의 돌림운동이 어깨돌림근군과 몸통근군의 근전도 반응에 미치는 영향 (Effects of shoulder rotation according to stance posture and plane of motion on EMG response of shoulder rotator cuff and Trunk muscles.)

  • 본 연구는 스탠스 자세와 운동면의 차이가 어깨돌림동작 시 활동근의 %MVIC를 비교분석하여 어깨강화운동을 위한 기초자료를 제공하는데 목적이 있다. 남성 8명을 무선배정하여 두발지지 자세, 한발지지 자세, 런지자세와 이마면, 수평면, 시상면에서 회전동작을 10회씩 수행하였다. 수행 중 가시위근, 가시아래근, 작은원근, 앞세모근, 배곧은근, 척수세움근, 큰가슴근, 넓은등근의 근활성도를 측정하였다. SPSS 22.0 통계프로그램을 사용하여 반복측정 일원분산분석(repeted one-way ANOVA)을 실시하였다. 첫째, 이마면에서 바깥돌림 동작 시 척추세움근은 두발지지 자세와 한발지지 자세보다 런지자세에서 높게 나타났고 안쪽돌림 동작 시 가시위근은 두발지지 자세보다 한발지지 자세, 한발지지 자세보다 런지자세에서 높게 나타났다. 둘째, 수평면에서 바깥돌림 동작 시 앞세모근은 두발지지 자세보다 한발지지 자세과 런지자세에서 높게 나타났고 안쪽돌림 동작 시 가시아래근은 두발지지 자세와 한발지지 자세보다 런지자세에서 높게 나타났으며, 큰가슴근에서는 한발지지 자세보다 두발지지 자세, 한발지지 자세보다 런지자세가 높게 나타났다. 셋째, 시상면 바깥돌림 동작 시 배곧은근은 두발지지 자세보다 한발지지 자세와 런지자세에서 유의하게 높게 나타났다. 안쪽돌림 동작 시 가시위근은 두발지지 자세보다 한발지지 자세와 런지자세에서 높게 나타났고 가시아래근은 두발지지 자세와 한발지지 자세보다 런지자세에서 높게 나타났다. 배곧은근에서 두발지지 자세와 런지자세보다 한발지지 자세에서 높게 나타났고 척추세움근은 한발지지 자세보다 두발지지 자세와 런지자세에서 높게 나타났다. 결론적으로 스탠스와 어깨의 운동면의 차이가 어깨위팔관절의 돌림운동 시 활동근들의 근활성도에 미치는 영향은 스탠스자세와 운동면에 따라서 다른 양상이 나타났으며, 이를 어깨 강화 운동프로그램에 적용한다면 보다 긍정적 효과를 기대할 수 있는 운동프로그램이 될 것으로 생각 된다.

A Study on Asymmetric Lifting Capacity Due to Spine Deformity

  • Scoliosis can be biomechanically described as a three dimensional deformity of the spine, with deviations from the physiologic curves in the sagittal and frontal planes, usually combined with intervertebral rotation. Various factors are suspected such as genetic defects, uneven growth of the vertebrae, hormonal effects, abnormal muscular activity, postural problems, or a mix of some of these elements, but its initial cause is known in only 15-20% cases. The screening test for diagnosing scoliosis is called the Adams Forward Bend Test. During the experiment, the subjects were asked to bend over, with arms dangling, until a curve could be observed. The Scoliometer was placed on the back of the subjects and used to measure the difference between the left and right apex of the curve in the thoracic, thoracolumbar and lumbar area. Then, the subjects were asked to perform Maximum Voluntary Contractions (MVCs) using the digital back muscle dynamometer in three different postures: (1) 0o (sagittally symmetric); (2) 30o from the mid-sagittal plane (clockwise); and (3) 30o from the mid-sagittal plane (counterclockwise). In addition to the experimental data, subject-dependent variables including Body Mass Index (BMI), percentage of body fat and muscle mass of left/right arms and legs were employed to reveal the cause of difference among three MVC conditions. All those variables were tested using statistical methods.

CT기반 3차원 모델을 이용한 고관절 운동범위 예측 (Hip Range of Motion Estimation using CT-derived 3D Models)

  • 본 연구에서는 CT영상기반 3차원 고관절모델을 이용한 컴퓨터시뮬레이션을 통해서 고관절의 운동범위(Range of Motion)를 측정하는 방법을 제시하였다. 본 연구에서는 그 측정방법에 대한 기술적인 사항을 제시하고, 그 기술이 재현성 있게 실현할 수 있도록 대퇴골두 중심점의 결절, 대퇴골 외전(Abduction)/내전(adduction)회전축, 굽힙(flexion)/신전(extension) 회전축을 정의하고 측정하는 명확한 방법을 제시하였다. 외전각은 해부학적인 시상면(Sagittal plane)상의 Anterior-Posterior축에 대해 아래쪽(Inferior)면으로부터 Lateral 쪽으로의 회전각으로 정의된다. 최대외전각은 대퇴골두가 엉덩이뼈(Pelvis)의 절구(Acetabulum)의 테두리와 겹치지 않고 Anterior-Posterior축을 중심으로 회전할 수 있는 최대 외전각으로 결정된다. 굴곡각은 해부학적인 관상면(Coronal plane)상의 Medial-Lateral축에 대해 아래쪽(Inferior)면으로부터 회전각으로 정의된다. 최대굴곡각은 대퇴골이 Medial-Lateral축을 중심으로 엉덩이뼈(Pelvis)의 절구(Acetabulum)의 테두리와 겹치지 않고 회전할 수 있는 최대 굴곡각으로 결정된다. 정상고관절에 비해 인공고관절술을 받은 해당 환자의 경우, 외전에서는 60도 정도, 굽힘에서는 4도 정도 운동범위가 줄어들 수 있다는 예측이 나왔다. 본 연구에서 행한 시뮬레이션을 해보고 외전의 경우 운동범위의 감소가 예측되므로, 대퇴골두를 조금 큰 것을 고르거나 대퇴골목부의 길이 (femoral neck offset)를 길게 시술해야 할 필요가 있음을 의미한다.

A Study on Changes in Lower Limb Joint Angles during Stair Walking with High Heel

  • Purpose: The purpose of this study is to compare kinematics on lower limbs between stair walking with high heel and barefoot in healthy adult women. Methods: 18 healthy adult women were recruited in this study. The subjects performed stair ascent and descent with high heels and barefoot. The experiment was conducted in random order and repeated three times for each stair walking with high heels and barefoot. The movements of lower limb joints were measured and analyzed using a three-dimensional analysis system. Results: The ankle, knee, and hip flexion angles on the sagittal plane exhibited statistically significant differences between stair ascent and descent with high heels and barefoot. The pelvic forward tilt angles showed statistically significant differences only during stair ascent. The ankle inversion, hip abduction and pelvic lateral tilt angles on the frontal plane showed statistically significant differences between stair walking with high heels and barefoot. On the transverse plane, the hip rotation angles showed statistically significant differences between the high-heeled and barefoot gait during stair ascent and descent. However, the pelvic rotation angles showed no statistically significant differences. Conclusion: Therefore, wearing high-heeled shoes during stair walking in daily life is considered to influence lower limb kinematics due to the high heel, and thus poses the risks of pain, and low stability and joint damage caused by changes in the movement of lower limb joints.
