A Study on the Facility and Equipment of Laboratory Medicine in General Hospital - Focused on more than 550 bed sized hospitals (종합병원 진단검사의학과 검사실의 시설 설비 현황 조사 - 550 병상 이상 종합병원을 중심으로)

  • Purpose: Though Korean healthcare services have been upgraded, infection and fire had been broken out in general hospitals. And higher concerns about quality assessment made it to clinical laboratory design guideline studies. So, this study investigates the facilities, equipment and personnel of laboratory medicine focusing on more than five hundred fifty bed hospital, and contributes to make guidelines for safety and efficiency in lab. Methods: Questionnaires to supervisor technologist and field surveys to medical laboratories in korean hospitals have been conducted for the data collection. 16 answers have been analysed statistically by MS Excel program. Results: Most of the sample tests such as hematology, clinical chemistry, immunology, transfusion, urinalysis, microbiology and molecular diagnosis are performed by more than 80% in large sized general hospital laboratory. In the test methods, automatic analyzers are used up to 80%, total laboratory automation up to 43% in clinical chemistry and immunology, and manual tests in all sorts of the test. There are placed in single lab or two and three labs above the ground, which are all in semi-open lab. There is some correlation with the number of specimens and the number of lab people depending on the number of hospital beds. Laboratory environment shows that work distance is good, but evacuation path width, visibility, separation of staff area from automatic analyzer, and equipment installations are needed to have more spaces and gears. Most of the infection controls are equipped with mechanical ventilation, air-conditioning, washbasin and wastewater separation, BSC installation and negative pressure lab room. Implications: Although the laboratory space area is calculated considering the number of hospital beds, type of tests and number of staff, hospital's expertise and the samples numbers per year should be taken into account in the planning of the hospital.

The Removal Study of Restoration Materials for the Ceramics (토기의 복원재료로 사용되는 합성수지 제거 연구)

  • This study examines the removal effects of solvents which were applied on three adhesives and three filling materials for ceramics. The chosen materials are Cemedine C, Loctite$^{(R)}$ 401$^{TM}$, Araldite Rapid Type, plaster of paris, CDKI 520A+B, Araldit$^{(R)}$ SV427-2+HV427-1. For the first examination, each different materials were soaked into different solvents, and then the soften level were compared by compressive pressure. The result of the first exam showed that Cemedine C was dissolved easily in acetone. Furthermore Araldite, CDK and SV427 were soften efficiently by Dichloromethan, Tetrahydrofuran and DMX(Dichloromethan+Methanol+Xylene). However, Loctite and plaster were not able to make sample for the first test. Therefore, the second test carried on only. The second examination, chosen effective solvents from the first test were applied with two different methods; soaking and saturated atmosphere. Almost every sample could removed by soaking method in ten minutes, and saturated atmosphere method needs 24 hours for the safety.

Local Heat Transfer Coefficients for Reflux Condensation Experiment in a Vertical Tube in the Presence of Noncondensible Gas

  • The local heat transfer coefficient is experimentally investigated for the reflux condensation in a countercurrent flow between the steam-air mixture and the condensate, A single vertical tube has a geometry which is a length of 2.4m, inner diameter of 16.56mm and outer diameter of 19.05mm and is made of stainless steel. Air is used as a noncondensible gas. The secondary side has a shape of annulus around vertical tube and the lost heat by primary condensation is transferred to the coolant water. The local temperatures are measured at 11 locations in the vertical direction and each location has 3 measurement points in the radial direction, which are installed at the tube center, at the outer wall and at the coolant side. In three different pressures, the 27 sets of data are obtained in the range of inlet steam flow rate 1.348∼3.282kg/hr, of inlet air mass fraction 11.8∼55.0%. The investigation of the flooding is preceded to find the upper limit of the reflux condensation. Onset of flooding is lower than that of Wallis' correlation. The local heat transfer coefficient increases as the increase of inlet steam flow rate and decreases as the increase of inlet air mass fraction. As an increase of the system pressure, the active condensing region is contracted and the heat transfer capability in this region is magnified. The empirical correlation is developed by 165 data of the local heat transfer. As a result, the Jacob number and film Reynolds number are dominant parameters to govern the local heat transfer coefficient. The rms error is 17.7% between the results by the experiment and by the correlation.

Immediate application of vacuum assisted closure dressing over free muscle flaps in the lower extremity does not compromise flap survival and results in decreased flap thickness

  • Background Free muscle flaps are a mainstay for reconstruction of distal third leg wounds and for large lower extremity wounds with exposed bone. However a major problem is the significant postoperative flap swelling, which may take months to resolve. We studied the efficacy and safety of immediate application of a vacuum assisted closure (VAC) dressing after a free muscle flap to the lower extremity. Methods Over a 19 months period, all consecutive free muscle flaps for lower extremity reconstruction at a Level I trauma center were evaluated prospectively for postoperative flap thickness, complications and flap survival. Immediate application of a VAC dressing was performed in 9 patients, while the flap was left exposed for monitoring in 8 patients. Results There was no statistically significant difference in flap survival between both cohorts. Mean flap thickness at postoperative day 5 for the VAC group was $6.4{\pm}6.4mm$, while flap thickness for the exposed flap group was $29.6{\pm}13.5mm$. Flap thickness was significantly decreased at postoperative day 5 for the VAC dressing group. Conclusions Immediate application of VAC dressing following free muscle flaps to the lower extremity does not compromise flap survival or outcomes and results in decreased flap thickness and a better aesthetic outcome.

Effect of Chungyeoldodam-tang on Hypertension (청열도담탕이 고혈압에 미치는 영향)

  • To access the safety and efficacy of Chungyeoldodam-tang(CDT), a traditional herbal medicine prescription, on hypertension we examined various parameters involved in the pathogenesis of hypertension. CDT seems to be safe because CDT at the concentrations lower that 250 ug/ml showed no toxic effects in cultured human fibroblast and no toxic effects on liver function. The production of reactive oxygen species (ROS) were greatly decreased in CDT treated group compared with control, and angiotensin converting enzyme activities were reduced by CDT in a dose dependent manner. There was no differences in weight of hearts between control and CDT treated group. The blood pressure and pulse rate were significantly decreased. CDT greatly reduced the levels of plasma hormones including aldosterone, dopamine, and norepinephrine, but not epinephrine, and serum electrocytes including Na$^+$ and Cl$^-$, but not K$^+$. were also decreased. The levels of uric acid, BUN and creatinine were significantly decreased compared with control. These results suggested that CDT has suppressive effects on various pathologic factors in hypertension, and CDT has potential as a safe and effective therapeutics for hypertension.

Safety Evaluation of LB10522, a New Cephalosporin Antibiotic

  • All the pharmacological studies of LB17522 described here were carried out with high doses (fifteen to sixty times of the therapeutic dose) to determine an indication of potential side effects in clinical use in terms of the acute clinical signs, cardiovascular and central nervous system. LB10522 does not produce any observable clinical signs except for the symptoms such as moist eye, skin rash, slight salivation, vomitting, and slightly reduced activity. The effects of LB10522 on the hemodynamics and cardiac function of anesthetized beagle dogs are as follows; heart rates and mean arterial blood pressure had a tendency to increase mildly, which is a normal finding in anesthetized dogs. All the animals except for one showed relatively stable respiratory rates throughout the observation period. Each animal treated with LB10522 showed slight increase in the left cardiac work and left ventricular stroke work which are mainly related to corresponding increases in cardiac output. Femoral blood flow were shown to be increased in some animals treated with LB10522. The epileptogenic activities of various cephalosporins were assessed by a direct intracerebral injection of appropriate concentration of test articles. The CD$_{50}$ values (nmol) obtained from the analysis of the dose-response data are as follows; 78.2, 175.3, 156.3, and 53.5 for cefazolin, cephaloridine, ceftazidime, and LB 10522, respectively. LB10522 seems to be equipotent with cefazolin or to be three times more potent than cephaloridine and ceftazidime in causing adverse CNS stimulation. Taken into consideration all the information obtained, LB10522 is not supposed to induce much changes in the functions examined in these studies in man at therapeutic doses.s.

  • General pharmacology of LB20304, a quinolone antibiotic, were examined in terms of general behaviour, cardiovascular, and central nervous system. LB20304 at oral dose of 2,000 mg/kg did not induce significant behavioural changes in mice. In contrast with ciprofloxacin, LB20304 at dose of 20 mg/kg, iv. did not show any observable effects on the blood pressure in rats. Displacement of [$^3$H]muscimol binding to the rat brain synaptic membranes was measured. LB20304 was shown to be about five times less potent than ciprofloxacin in specific GABA receptor binding. Drug interaction between LB20304 and 4-biphenyl acetic acid, an active metabolite of fenbufen, was assessed in mice by measuring convulsion and/or subsequent death. A single oral pretreatment with 4-BPA at 400 mg/kg increased the incidence of convulsion and death after oral administration of ciprofloxacin at the doses of 25, 50, and 100 from zero of five to three of five, two of five, and four of five, respectively, whereas LB20304 alone or combination with 4-BPA caused neither convulsions nor death at the doses of 12.5, 25, 50, and 100 mg/kg, respectively. Quinolones-induced epileptogenic activities were assessed by a direct intracerebral injection of test articles. The CD$_{50}$ values (nmole) are as follows; 169.47, 35.36, 105.29, and 88.67 for LB20304, ciprofloxacin, of loxacin, and lomefloxacin, respectively. From these data, LB 20304 at therapeutic doses seems to be much more safe than any other quinolones tested.d.

General Pharmacology of LB71350, a New HIV-1 Pretense Inhibitor

  • Safety evaluation of LB71350, a new HIV-1 protease inhibitor, was performed in mice, rats and dogs. For the general behavior of mice, LB71350 at an oral dose of 200 mg/kg did not show any significant effects on muscle tone and locomotor activity. In terms of central nervous system, at oral doses of 200 mg/kg and 1000 mg/kg, LB71350 inhibited acetic acid-induced pain response approximately 41% and 83% of control. respectively. At oral doses of 200 mg/kg and 500 mg/kg, it reduced the rectal body temperature in rats. Pentylenetetrazole-induced seizure in mice was slightly potentiated by oral administration of LB71350 at doses ranging from 200 mg/kg to 1000 mg/Ag. Single or five day treatment of LB71350 doubled the hexobarbital- induced sleeping time in mice at oral doses ranging from 50 mg/kg to 500 mg/kg. It did not cause any effects on gastric secretion and acidity in rat at oral doses of 200 mg/kg and 1000 mg/kg and also it did not change intestinal motility in mice up to 1000 mg/kg. Blood coagulation indices such as prothrombin time (PT), activated partial thromboplastin time (aPTT), and thrombin time (TT) in rats were not affected by the treatment of LB71350 up to 500 mg/kg. LB71350 caused no significant effects on the cardiac output, stroke volume, heart rate, and mean blood pressure when infused intravenously to the anesthetized rats and dogs. Taken together, LB71350 at high oral doses caused significant pharmacological effects on the central nervous system and the hexobarbital-induced sleeping time.

  • Steam generator is one of the major components of nuclear power plant and steam generator tubes are the pressure boundary between primary and secondary side, which makes them critical for nuclear safety. As the operating time of nuclear power plant increases, not only damage mechanisms but also scaled deposits on steam generator tubes are known to be problematic causing tube support flow hole blockage and heat fouling. The ability to assess the extent and location of scaled deposits on tubes became essential for management and maintenance of steam generator and eddy current bobbin data can be utilized to measure thickness of scale on tubes. In this paper, tube reference standards with various thickness of scaled deposit has been set up to provide information about the overall deposit condition of steam generator tubes, providing essential tool for steam generator management and maintenance to predict and prevent future damages. Also, methodology to automatically measure scale thickness on tubes has been developed and applied to field data to estimate overall scale amount.

A Study on the Allowances of Aircraft Landing Distance (항공기 착륙거리의 여유분 산정에 관한 연구)

  • Among the phases of flight operations pilots feel much pressure in landing segment. There is a number of factors affecting landing safety while pilots reduce aircraft speeds and make a touchdown and stop completely. If runway length is sufficient for landing, there maybe is no problem. But it is not the case all the time. So it is necessary to confirm whether landing performance is within limits or not. Required landing distance is actual landing distance demonstrated by flight test pilot plus allowances for average airline pilots. FAR(Federal Aviation Regulations) AFM certification is based upon manual landing for dry and wet runway. Other runway conditions are not the certification basis. JAR dictates even contaminated/slippery runway is included by prescribed allowances. Automatic landing is not certification basis, so actual landing distances are provided. In this paper I would like to analyze distance allowances included in each type of runway condition. In addition there is no regulation about allowances for specific runway condition, I would suggest adequate allowances for that case.
