A Study on Earth Pressure Calculating Method about Shield TBM Tunnel Segments in the Rock (암반층에서 쉴드 TBM 터널 세그먼트의 작용하중 산정방법에 관한 연구)

  • This study analyzed the differences in the analysis techniques through a comparative analysis of the various segment's modeling techniques of Shield TBM method and proposed reasonable modeling techniques. Also, this study suggested reasonable estimating methods of load to be applicable in the field through the load analysis and three-dimensional finite element analysis by estimating model of rock mass relaxation load. Estimating method of relaxation area by rock mass rating makes no odds of output in subgrade with high rock mass rating, but so the difference of output is large, that is judged to set conservative design off. In estimating result of rock mass relaxation area by three-dimensional analysis relaxation area of subgrade with low-grade soil was predicted to be positioned at medium-range of many methods, in case of designing segment in subgrade with low-grade soil it needs to actively review estimation of relaxation area through three-dimensional analysis reflecting mechanical tunnel excavation.

Job Stress Factor and Job Satisfaction in Clinical Nurse thorough of the NIOSH Job Stress Model (NOISH 직무스트레스 모형을 적용한 임상간호사의 직무스트레스 원인과 직무만족도 분석)

  • Purpose: This study was performed to identify the factors of the job stress and to analysis affecting job satisfaction in clinical nurses, using the Job Stress Model by the National Institute for Occupational Safety and Health (NIOSH). Method: Data collection was done from July 21 to July 30, 2003. The study was involved in 203 nurses who work at the hospital. It was examined the self-recording questionnaire about general characteristics, work-related factors, non-work factors, instrument revised by NIOSH Job Stress questionnaire translated by the Korea Occupational Safety & Health Research Institute(1999). Instrument about shift work was the tool developed by Kim and Gu(1984). Result: Job satisfaction of subjects at an average level with $1.96{\pm}0.37$ score on the basis of 3 points. According to general characteristics, job satisfaction was high in those with older age and married group. According to work related general factors, job satisfaction was high with working career, an officer to higher grade. According to work characteristics factors, job satisfaction was low with much role conflict at work and much workload and much interpersonal conflict. As for the relationship between job satisfaction and non-work factor, job satisfaction was did not show statistically significant differences. As a result of Stepwise multiple regression analysis, job satisfaction was influenced by shift work, conflicts of between group and grade of office. Conclusion: Therefore, it suggests that the nurses need stress management related shift work for improvement of job satisfaction.

Apatinib as a Third-Line Treatment for HER2-Positive Metastatic Gastric Cancer: A Multi-Center Single-Arm Cohort Study

  • Purpose: Treatment options are limited after the failure of first-and second-line treatments in patients with HER2+ metastatic gastric cancer (mGC). The present study aimed to explore the efficacy, safety, and prognostic factors of apatinib efficacy as a third-line therapy for patients with human epithelial growth factor receptor 2-positive (HER2+) mGC. Materials and Methods: A total of 59 HER2+ mGC patients who received apatinib as third-line therapy were retrospectively enrolled in this two-center, single-arm, cohort study; the clinical response, survival data, and adverse events were retrieved. Results: The median progression-free survival (PFS) was 5.2 months (95% confidence interval [CI], 3.9-6.5), and the median overall survival (OS) was 8.2 months (95% CI, 6.6-9.8) Furthermore, forward stepwise multivariate Cox regression analysis showed that a higher Eastern Cooperative Oncology Group performance status score and multiple metastases were independently correlated with decreased PFS and OS (both P<0.05). The main adverse events were leukopenia (45.8%), hypertension (44.1%), thrombocytopenia (39.0%), hand-foot syndrome (37.3%), and elevated transaminase (33.9%). Grade 3 adverse events mainly included hypertension (5.1%) and neutropenia (5.1%); grade 4 adverse events did not occur. Conclusions: Apatinib is efficient and well tolerated in patients with HER2+ mGC as a third-line treatment, suggesting that it may be a candidate of choice for these patients.

Cervical spine reconstruction after total vertebrectomy using customized three-dimensional-printed implants in dogs

  • Background: Sufficient surgical resection is necessary for effective tumor control, but is usually limited for vertebral tumors, especially in the cervical spine in small animal neurosurgery. Objective: To evaluate the primary stability and safety of customized three-dimensional (3D)-printed implants for cervical spine reconstruction after total vertebrectomy. Methods: Customized guides and implants were designed based on computed tomography (CT) imaging of five beagle cadavers and were 3D-printed. They were used to reconstruct C5 after total vertebrectomy. Postoperative CT images were obtained to evaluate the safety and accuracy of screw positioning. After harvesting 10 vertebral specimens (C3-C7) from intact (group A) and implanted spines (group B), implant stability was analyzed using a 4-point bending test comparing with groups A and C (reconstituted with plate and pins/polymethylmethacrylate after testing in Group A). Results: All customized implants were applied without gross neurovascular damage. In addition, 90% of the screws were in a safe area, with 7.5% in grade 1 (< 1.3 mm) and 2.5% in grade 2 (> 1.3 mm). The mean entry point and angular deviations were 0.81 ± 0.43 mm and 6.50 ± 5.11°, respectively. Groups B and C significantly decreased the range of motion (ROM) in C3-C7 compared with intact spines (p = 0.033, and 0.018). Both groups reduced overall ROM and neutral zone in C4-C6, but only group B showed significance (p = 0.005, and 0.027). Conclusion: Customized 3D-printed implants could safely and accurately replace a cervical vertebra in dog cadavers while providing primary stability.

  • The purpose of this study is to examine the relationship between children's walking safety or risk of traffic accident and the physical environments around 20 public elementary schools in Changwon-si, Gyeongsangnam-do. Field surveys were conducted to assess the street-level objective and subjective walking environments. The GIS method was used for measuring the neighborhood-level land use patterns. Children's walking safety and risk of traffic accident data were collected from the 6,381 grade 5 to 6 students attending 18 elementary schools through the questionnaire survey. Correlation analysis showed that walking safety and risk of traffic accident of the elementary students were associated with the number of temporary or permanent obstacles on the sidewalk, traffic safety signs, driveway and street intersections, street lights, and percentage of detached housing area and road area on neighbourhood-level. This research will promote to help with constructing a safe routes to school and walking-friendly healthy community.

Safety and Effectiveness of Food Allergen Immunotherapy (Oral): A Systematic Literature Review and Meta-analysis

  • Purpose: Food Allergen Immunotherapy (Oral) is a form of immunotherapy administered to patients who are allergic to foods such as egg, milk, and peanut. The food allergen is orally administered to the patient in an escalating dose for desensitization or tolerance development. The safety and effectiveness of the therapy were assessed using a systematic literature review and meta-analysis. Methods: For a literature search, 8 national databases and a number of international databases including Ovid-MEDLINE, Ovid-EMBASE, and Cochrane Library were used; and 13 articles (all from international databases) were selected. The target of Food Allergen Immunotherapy (Oral) included patients with food allergy, and the intervention was food allergen immunotherapy without limiting the food type. The safety and effectiveness of Food Allergen Immunotherapy (Oral) were assessed by reviewing all the articles reporting on the therapy. The control group received standard therapies including aversion therapy, no treatment, anti-histamine treatment, and placebo. Safety was assessed through the incidence of complication and emergency medication. Effectiveness was assessed based on therapy success rate, symptomatic improvement, and quality of life. Results: Although Food Allergen Immunotherapy (Oral) was shown to have successful desensitization in patients with food allergy, the safety of the technique has not yet reached an acceptable level; the possible reason is due to the high rate of complication and frequency of emergency medication. Also, each study employed varying protocols while relying on a small number of participants and a short monitoring period. Conclusion: The results of assessment suggest that the level of evidence from current literature review is low and further research is necessitated for the verification of the safety and effectiveness of the therapy (Grade of Recommendation: A; Level of Technology: II-b).

  • This papers describes on the experimental consideration for the intrinsically-safe explosion-proof capability of rechargeable battery's body about main item rechargeable battery and cellular phone battery which is selling in domestic that IEC(International Electrotechnical Commission) recommend the measurement of ignition limit by short circuit of rechargeable battery and temperature increase test to use a explosion grade Group IIC type of explosion-proof type apparatus test an object of hydrogen gas. Because of that there are many different results for existence or nonexistence for ignition by different company and different types. It is concluded that the maximum of self temperature increasing by spark circuit of rechargeable battery is $180^{\circ}C$ in case of Nickel-Hydrogen and $110^{\circ}C$ in case of Nickel-Cadmium. The reaction of cellular battery for external temperature have following processes. It is confirmed that the temperature of reaction is rise slantly as the ambient temperature rising, then exterior shape of one is swell up and change when the temperature of ambient reach to about $130\~140^{\circ}C$, and when reach to about $160^{\circ}C$ the battery is blown up. Therefore, it is considered that have to be in considering selection of rechargeable battery using in itself due to different ignition limits of various rechargeable battery when the portable electric containing rechargeable battery are designed, produced and used, the characteristics and the proper safety factors of devices.

Analysis of the Current Wearing Status and Point of Improvement of Warning Clothing of Elementary School Students (초등학생의 도로교통 안전의복의 착용 현황 및 개선점 분석)

  • This study analyzes the current wearing status and awareness of warning clothing for elementary school students as well as to identify points of improvement for traffic accident prevention and clothing safety education. A survey was conducted on 279 elementary $5^{th}$ and $6^{th}$ graders in the Incheon area and Ansan in 2017 June & July. The results showed that more than 2/3 of participants had no experience wearing warning clothing. Second, 63% of participants answered 'traffic accident prevention' as the main purpose of warning clothing, showing that about 2/3 are aware of the importance. Third, yellow was the most preferred color of fluorescent material for warning clothing with a significant difference of preference by grade. The favorite color combination of participants was yellow & orange followed by yellow & light green, but showed a significant difference between boys and girls. Fourth, the most preferred form of warning clothing was fluorescent short sleeves and shorts without reflective tape, suggesting that students paid more to the purpose for reflective tape. Fifth, while 68% of participants were negative towards wearing safety clothes that are currently available, 66% were willing to try safety clothes if the color and the design improved. The results suggest an improved education with more emphasis on the purpose of clothing safety. Color and design adjustment of the current warning clothing is also recommended for a more active participation by elementary school students.

A Study on the Electrical and Heat Generation Characteristics of an Induction Motor under Restrained Operation (유도전동기의 구속운전에 따른 전기 및 발열 특성 연구)

  • In this study, we determined the failure rate and fire status of electric motors widely used in domestic and industrial devices and analyzed the associated fire risks by identifying the electrical and temperature characteristics of electric motors under the normal and restrained operation modes in industrial sites and laboratories. A 2.2kW motor used for driving a conveyor during the vulcanization process in a rubber product manufacturing plant was employed as the study object and was exposed to a high- temperature environment as this motor is widely used in industrial sites. The current amplitude was 4.45-4.50 A during normal operation and 38.2-41.5 A during restrained operation due to the pinching of products and semi-finished products (i.e., 8.5 times higher than that during normal operation). The leakage current amplitude was 0.33 mA during both operation modes. The temperature of the workplace in summer was 42.38℃, indicating a poor environment for the installed motor. In the laboratory, the current and temperature of the coil inside a 3.7kW motor were measured under the restrained operation mode as performing measurements of the coil inside the motor in industrial sites is challenging. The current amplitude during normal operation was 3.5 A, whereas that during restrained operation for 30 s was 51.7-58.6 A, which is 14.8-16.7 times higher than that of normal operation. Moreover, the temperature of the motor coil increased from 22.9℃ to 101℃. Based on the experimental data, we derived the temperature increase formula according to the restrained operation time by performing a regression analysis and verified the time at which the temperature exceeded the stipulated limit for the insulation grade. The findings presented in this paper can be utilized to establish fire-prevention measures and perform safety management of motors of the same type or with a similar capacity.

The Effect to a Sense of Safety by Training the Transfer Method of Paralytic on the Guardians of Paralytic: A Preliminary Study (마비 환자 보호자의 환자 이동 방법 교육이 안위감에 미치는 효과에 대한 예비 조사)

  • This research is to consider the training effect of effective paralytic transfer method and reduce the risk of paralytic transfer. It was questioned and interviewed the guardians of 50 patients agreed this research among the guardians of patient who hospitalized in rehabilitation ward. The contents of questions was safety grade with 1~10 point which the guardians of paralytic was feeling in case transfer the patient of paralytic. The sense of safety before and after the traning of transfer method was improved about 25% more from 5.81 point to 7.26 point on the safety. The 98% of subjector supported the needs of training for transfer method of paralytic. And also 74% of subjector replied that the safety was improved after taking the training. On the base of this research, it is needed to review this kind of traning to get reliability and propriety throuth repeatitive research.
