A Study of Methodology to Examine Organizational Root Causes through the Retrospect Error Analysis of Railroad Accident Cases

  • Objective: This study proposes a systematic process to present the analysis methods and solutions of organizational root causes to human errors on the railroad. Background: In fact, organizational root cause such as organizational culture is an important factor in the safety concerns on human errors in the nuclear power plant, railroad and aircraft. Method: The proposed process is as follows: 1) define analysis boundary 2) select human error taxonomy 3) perform accident analysis 4) draw root causes with FGI 5) review root causes analysis with survey 6) chart analysis of root causes, and 7) propose alternatives and solutions. Results: As a result, root causes of the organizations like railroad and nuclear power plant came from the educational problems, violations, payoff system, safety culture and so forth. Conclusion: The proposed process does predict potential railroad accident through retrospect error analysis by building new human error taxonomies and problem solution. Application: This study would contribute to examination of the relationship between human error-based accidents and organizational root causes.

간호사와 치위생사의 환자안전문화 인식수준 비교연구 (Comparative studies in Perception of Patient safety culture of Nurses and Dental hygienist)

  • 목적 : 본 연구는 간호사와 치위생사의 환자안전문화에 대한 인식수준을 파악하기 위해 시행되었다. 방법 : 2010년 9월부터 12월까지 부산, 경남지역의 병원에 종사하는 간호사와 치위생사 399명을 대상으로 환자안전문화 인식 도구를 사용하여 설문조사가 시행되었으며, 수집된 자료는 SPSS 프로그램을 사용하여 평균과 표준편차, t-test와 ANOVA, Scheffe test, spearman 상관관계분석을 시행하였다. 결과 : 환자안전문화에 대한 인식을 조사한 결과, 간호사는 3.48점, 치위생사는 3.51점으로 나타났다. 세부 영역에서 차이를 보인 항목은, '부서내 안전문화에 대한 인식' 영역 중 '직원배치'(t=2.841, p<.01), '직속상관 관리 태도'(t=-2.471, p<.05), '사고에 대한 피드백과 의사소통'(t=-3.356, p<.01)으로 나타났다. 간호사와 치위생사에게서 환자안전문화에 영향을 미치는 요인으로는 연령과 경력 주당 근무시간이었다. 결론 : 간호사와 치위생사의 환자안전문화 인식수준은 중간정도 수준으로, 환자안전문화 인식수준에 영향을 미치는 연령, 경력, 주당 근무시간 등이 고려된 접근과 적절한 근무시간 정착이 필요할 것으로 사료된다.

The Factors Affecting Unsafe Behaviors of Iranian Workers: A Qualitative Study Based on Grounded Theory

  • Background: Some researchers state that they are not yet able to provide a deep understanding of the underlying causes of unsafe behaviors (UBs). Therefore, the present study was conducted to investigate the attitudes and experiences of Iranian workers of UBs. Methods: This present study was conducted in 35 industries using a semistructured interview based on grounded theory. Forty participants were interviewed, including 13 industrial safety and health experts and 27 workers and supervisors. The analysis of the present study consisted of a three-step coding process including open, axial, and selective coding. Results: The results showed that the factors affecting UBs could be classified into three categories: organizational, individual, and socioeconomic factors. Organizational factors were divided into 6 parts: procedure and environmental conditions, communications, monitoring, organizational safety culture, resource allocation, and human resources. Socioeconomic factors had three subcategories: community safety culture, type of organizational ownership, and economic problems. Finally, the individual factors were classified into two categories of personality traits and individual competence. Conclusion: The results showed that organizational factors were the most categorized, and it is estimated that this factor has a more important role in the UBs. Of course, to better understand the close relationship between these factors and find the weight and importance of each factor, it needs to measure it with multicriteria decision systems.

  • Human error is often in part in the cause of accidents and the result of various factors in an organization. Accidents should be investigated to elucidate all causes. Therefore, to reduce accidents, it is necessary to identify which factors affect human error within the organization. In this study, five groups of influencing factors on human error were selected using previousresearch, and operational definitions were made based on them. In addition, a questionnaire for measuring latent variables by operational definition was developed as an observation variable, and responses were received from employees of chemical companies in Ulsan. Based on SEM (structural equation modeling) analysis, 1) confirmatory factor analysis of variables in the human error model, 2) reliability and validity of latent variables, 3) correlations among latent variables, 4) influencing coefficients among influence factors, and 5) the verification results of the paths that these influencing factors have on human error are introduced in this study.

식품 안전성에 대한 소비자 인식 및 정보요구도에 관한 연구 - 잔류농약, 식중독, 식품첨가물을 중심으로 - (Consumers' Recognition and Information Need About Food Safety - Focused on Pesticide Residues, Foodborne Illness, and Food Additives -)

  • This study was conducted to investigate the consumers' recognition and information need for food safety, especially focused on pesticide residues, foodborne illness, and food additives. The data were collected from 350 adults living in Taegu and Pusan by the self-administered questionnaire. Frequencies and chi-square tests were conducted by SPSS. The results of the survey were as follows: (1) the consumers' concerns about food safety were high, (2) many respondents worried about eating vegetables, fruits and grains in turn because of pesticide residues, and did not trust the results from food-safety tests, (3) the major factor for foodborne illness was regarded as unfreshed or contaminated food stuffs, (4) many consumers concerned highly about preservatives among food additives, and tried to consume food containing less food additives, and (5) many respondents wanted to get the information about harmfulness of pesticide residues in foods, method to choose fresh food, and safety of food additives.

객실승무원의 항공기 안전의도와 안전행동의 영향요인 분석: TPB와 SCT 통합모델 (A Study on the Relationship of Factors Related to Cabin Crews' Safety Behavior: Integration of TPB & SCT)

  • Purpose: The purpose of this study is to investigate systematic and comprehensive explanation about safety behavior by applying integrated model of Theory of Planned Behavior (TPB) and Social Cognitive Theory (SCT). By showing the interrelationship of effects of factors affecting safety behavior in a single model, comprehensive understanding regarding safety behavior will be possible. Research design, data, and methodology: This study try to integrate the two theory, TPB and SCT, to introduce background factors, such as safety culture, affecting cognitive factors of subjective norm, safety attitude, perceived behavioral control, safety consciousness, which are again affecting behavior intention of the TPB. We composed 73 questionnaire to analyse the relationship among factors affecting safety behavior. The questionnaire were distributed to stewards and stewardesses of KAL, ASIANA, and LCCs. 422 were collected and used for analysis. Factor analysis, regression analysis, and control effect analysis were conducted using SPSS windows version 2.0. Results: The safety culture, safety atmosphere, and safety education/training were turned out to affect subjective norm, safety attitude, perceived behavioral control, safety consciousness. And the cognitive factors also showed to affect safety intention significantly. The results that safety intention affects safety behavior was found. Also there are control effect of cost consciousness and punishment while safety intention affects safety behavior. Conclusions: Safety is the virtue that should be given first priority to in our daily life. To secure safety, everyone in an organization should have high level of safety intention and conduct safety behavior. The study results provide systematic and comprehensive understandings on the relationship among factors affecting cabin crews' safety behavior. The results will help design safety related regulations, education/training, and support employees to engage in safety behavior related activities.

진주시 직장인의 외식 선택 요인 (Factors Influencing Salaried Employees' Choice of a Restaurant in JinJu)

  • A total of 321 salaried employees were surveyed by a questionnaire to assess consumers perceptions on factors influencing their food choices when they go to a restaurant. The questionnaire consisted of 5 pages and each page contained a same 26 popular food list from various restaurants in JinJu in a column and 5 attributes (e.g. taste, price, fullness, healthiness) in a row. So the respondents were asked to rate a total 25 attributes on the points which the columns and the rows met, using a 5-point scale. The data were analyzed using the SAS program for factor analysis. Five factors which influence consumer's restaurant choice were emerged. They were named as follow: Factor 1:preference and purpose, Factor 2:taste, Factor 3:snack, Factor 4:convenience, Factor 5:concern about obesity. Most of respondents were satisfied with the taste and the healthiness of foods of restaurants in Jin-Ju. The younger and the female groups had more positive attitudes on eating out andi willing to use it frequently. Fullnes, healthiness, and price were important criteria in choice and preference of a restaurant. However, they didn't much consider the food safety. Taking account of the character of Korean food about fullness, healthiness, and resonable price, consumers might prefer Korean food to fast food or foreign food.

요양병원 간호사의 낙상예방행위에 미치는 영향요인 (Factors Affecting Fall-Prevention Behavior of Long-Term Care Nurses)

  • Purpose: The purpose of this study was to identify factors influencing fall-prevention behaviors of nurses working in long-term care hospitals. Methods: Participants included 147 nurses working in 10 long-term care hospitals in B city. Data were collected from September 20-October 12, 2016. SPSS/WIN 21.0 was used for analysis with t-test, ANOVA, Scheffé test, Pearson correlation coefficients, and multiple regression. Results: It was found that attitude toward fall (r=.29, p<.001) and patient safety culture (r=.25, p=.002) had a significant positive correlation with fall-prevention behaviors of nurses working in long-term care hospitals. The factors influencing fall-prevention behaviors in participants were clinical career and patient safety culture (β=.21, p=.012), contributing to 19% of the total variance in fall- prevention behaviors. Conclusion: The findings showed that systematic delivery of differentiated fall prevention education is preferred to nurse's clinical career as a private factor to improve fall-prevention behaviors of nurses in long term care hospital. Particularly, it is imperative to conduct periodical and practical fall-prevention education for nurses to prevent career discontinuity. An independent report system and open communication system as well as a scheme that can disseminate patient safety culture in individual departments to implement patient direct nursing are required to encourage patient safety culture in organizations.

Scale Development and Validation to Measure Occupational Health Literacy Among Thai Informal Workers

  • Background: The high incidence of work-related diseases and injuries among day-laborers and workers with no legal contracts (informal workers) has received the attention of the Thai authorities. Workers' low occupational health literacy (OHL) has been reasoned as one contributing factor. Absence of a valid tool has prevented assessment of informal workers' OHL. The aim of this study was to create a valid and reliable Occupational Health Literacy Scale within the context of Thai working culture (TOHLS-IF). Methods: This study used the mixed method approach to develop TOHLS-IF. Questions were generated using in-depth interviews and an extensive review of the literature. Experts' assessment confirmed the content validity of TOHLS-IF. The scales of its psychometric properties were assessed in a sample of 400 informal workers using cluster random sampling. Results: The final version of the TOHLS-IF comprises 38 items within 4 dimensions: Ability to Gain Access, Understanding, Evaluation, and Use of occupational health and safety information. Factor analysis identified items explaining 50.22% of the total variance. The final confirmatory analysis confirmed the model estimates were satisfactory for the construct. TOHLS-IF demonstrated a high internal consistency and satisfactory reliability (Cronbach's alpha = .98). Conclusion: The TOHLS-IF is a valid and reliable instrument to assess informal workers' OHL. The structural dimensions of this instrument are based on the concept of health literacy and Thai culture. Thai health professionals are encouraged to benefit from this instrument to assess their workers' OHL and apply findings as guidelines for effective occupational health and safety interventions.

A Feasibility Study on the Application of Safety Assessment to the Fishing Vessels

  • The current paper deals with the safety of fishing vessels, which needs more speculations than other ship types. The characteristics of accidents of fishing vessels are rather different compared with other commercial vessels. In the context, the methodology of the formal safety assessment, one of the important issues in the maritime world to enhance the safety of marine vehicles, are reviewed and consulted along with other methodologies to see its applicability for fishing vessels. It is anticipated that further effective measures for better safety of fishing vessels can be identified including due considerations of the aspect of human elements, i.e. cultural environment.
