  • Primary cilia are solitary, non-motile, axonemal microtubule-based antenna-like organelles that project from the plasma membrane of most mammalian cells and are implicated in transducing hedgehog signals during development. It was recently proposed that aberrant SHH signaling may be implicated in the progression of idiopathic pulmonary fibrosis (IPF). However, the distribution and role of primary cilia in IPF remains unclear. Here, we clearly observed the primary cilia in alveolar epithelial cells, fibroblasts, and endothelial cells of human normal lung tissue. Then, we investigated the distribution of primary cilia in human IPF tissue samples using immunofluorescence. Tissues from six IPF cases showed an increase in the number of primary cilia in alveolar cells and fibroblasts. In addition, we observed an increase in ciliogenesis related genes such as IFT20 and IFT88 in IPF. Since major components of the SHH signaling pathway are known to be localized in primary cilia, we quantified the mRNA expression of the SHH signaling components using qRT-PCR in both IPF and control lung. mRNA levels of SHH, the coreceptor SMO, and the transcription factors GLI1 and GLI2 were upregulated in IPF compared with control. Furthermore, the nuclear localization of GLI1 was observed mainly in alveolar epithelia and fibroblasts. In addition, we showed that defective KIF3A-mediated ciliary loss in human type II alveolar epithelial cell lines leads to disruption of SHH signaling. These results indicate that a significant increase in the number of primary cilia in IPF contributes to the upregulation of SHH signals.

  • Images of microcalcification specks showed large variation in conventional radiographs of phantoms which are approved for mammography image quality standard by the American College of Radiology (ACR). This kind of variation is not appropriate for image quality standards because the number of specks are visually counted in images and that number is important in image quality evaluation. Our study using synchrotron radiation (SR) imaging revealed the overlapping of micro-sized air bubble(s) to some specks, and also the structural deformation or crackings. Eight phantoms approved by ACR from two different makers and an air-bubble phantom were examined. SR imaging was performed at a synchrotron radiation facility, SPring-8, in Japan. The image-detector was a fluorescent-screen optical-lens coupling system using a CCD camera with a spatial resolution of 6 $\square$m. Objects when imaged with longer sample-to-detector distance show edge enhancement due to a difference in refraction indices, that is refraction enhancement. Refraction-enhanced SR images revealed that some of specks carried foreign objects, which were proven to be air. In phantoms provided by one maker, attaching/overlapping airs were observed for 62 out of 150 specks (41%) , with a higher incidence for the smallest specks. A speck becomes hardly visible in a conventional radiograph when air(s) overlaps the majority part of a speck, though depending on the size of the air-inclusion and on its configuration. Those airs might have been adsorbed on a speck surface before being embedded and then introduced into the matrix together with specks. Our study using SR imaging has clearly shown the nature of defects in some mammography phantoms which seriously degrade the quality as an image standard.

  • Purpose: To evaluate the effect of shortening scarf osteotomy on pain relief and range of motion (ROM) of the first metatarsophalangeal joint in hallux rigidus patients. Materials and Methods: Twenty-three cases of 19 patients who had been treated with shortening scarf osteotomy for the hallux rigidus between January 2007 and December 2013 were reviewed. The mean follow-up period was 21.4 months, and the mean age was 59.2 years. The first metatarsal bone was shortened until the ROM of the first metatarsophalangeal joint was greater than $80^{\circ}$ or $40^{\circ}$ of dorsiflexion. The length shortened by scarf osteotomy was measured. The authors also measured and compared the joint interval difference of the standing foot using an anteroposterior radiography. Moreover, the difference of ROM of the first metatarsophalangeal joint between the preoperative and final follow-up periods was also compared. The clinical results were evaluated and compared using the American Orthopaedic Foot and Ankle Society (AOFAS) scoring system and visual analogue scale (VAS) score. Results: The mean shortening length was about 6.5 mm (range, 4~9 mm). The joint space has been increased to 1.8 mm, and the ROM of the first metatarsophalangeal joint has also been increased to $18.4^{\circ}$ after the operation. In three cases, the postoperative ROM has been decreased to less $10^{\circ}$. The AOFAS score has been improved from 41.7 (range, 32~55) to 86.2 (range, 65~95), and the VAS score was also decreased from 3.7 (range, 3~5) to 1.3 (range, 0~3). Two cases have shown no decrease in pain even after the operation. Conclusion: Shortening scarf osteotomy was found to decrease joint pain by decompressing the pressure of the first metatarsophalangeal joint. This osteotomy also helped improve the ROM of the first metatarsophalangeal joint. Shortening scarf osteotomy can be considered one of the effective methods for joint preservation.

  • Objective : Cranioplasty is required to protect underlying brain, to correct major aesthetic deformities, or both. The ideal material for this purpose is autogenous bone. When this is not available, alloplastic or artificial materials may be used. In this study authors compared the infection rate according to the cranioplasty materials(the frozen autologous bone vs. bone cement), and duration of the skull defect. Materials : Between May 1994 and December 1999, 111 patients with skull defect treated with cranioplasty(82 cases of frozen autologous bone and 29 cases of artificial bone material) were included in this study. There were 77 males and 34 females with a mean age of 41.4 years(range 1-85 years). 57 patients had head trauma and 54 had non-traumatic insults. According to the duration of skull defect, there were 28 cases under 1 month, 33 cases of 1-2 months, 15 cases of 2-3 months, 20 cases of 3-6 months and 15 cases over 6 months of duration. Results : Overall infection rate was 9.9%. In cases with frozen autologous bone and artificial bone material, the infection rate was 8.5% and 13.7%, respectively. The infection rate according to the duration of skull defect was 3.6%(among 28 cases) under 1 month of age, while those were 12%(4 among 33 cases) at 1-2 months, 20%(3 among 15 cases) at 2-3 months, 5%(1 among 20 cases) at 3-6 months and 13%(2 among 15 cases) over 6 months. Accoring to the underlying disease, the infection rate in traumatic cases was 12%(7 among 57 cases) and that in non-traumatic one was 3.7%(2 among 54 cases). Conclusion : From this study, it appears that skull defect should be repaired as soon as possible, because early cranioplasty can lower the infection rate. And surgeons could save the patients' cranial bone as possible as they can because autologous bone is not only cost effective in cosmatic purpose but lower the infection rate.

  • These studies were carried out to investigate the quality of green tea processed by different 1st-pan-firing time and to select the best conditions of green tea processing. The results were as follows. At harvesting tea plant leaf, tea plants had 4 leaves, each leaf size was $6.4cm\;{\times}\;2.4cm$, and weight of 100 buds was 40.2g. The content of moisture of green tea was decreased with the prolongation of 1st-pan-firing time, and the yield of green tea at 10 min. pan-firing time was the highest value of 20.12%. After processing green tea, 1st 10 min. pan-fired one had much more contents of total nitrogen(TN 5.41%) and total amino acid(T.A.A; 3,282mg/100g) and less content of tannin(13.22%) and caffeine(1.64%) than the other treatments. The content of chlorophyll and vitamin C went down with the extension of 1st pan-firing period. Sixteen kinds of free amino acid including theanine as a good taste component of green tea were isolated and measured by HPLC. T.A.A contents were in the range of $2,447{\sim}3,192mg/100g$ and among various pan-firing methods, 10 min. 1st pan-fired one had the highest content of T.A.A. The taste-determinants such as theanine, aspartic acid, serine, glutamic acid and arginine in green tea liquor, occupied as much as $89.4{\sim}90.0%$ of T.A.A.s. Among these, theanine was more than 50%, Lightness and yellowness of green tea were increased with delay of pan-firing time, while green color appeared to be more distinct at 10 min. 1st pan -firing time than other treatments. Besides, it gained the highest score of sensory evaluation among them. These above results suggested that 10 min. pan-fired green tea was considered to be the best level as pan-firing time of green tea processing.

  • Purpose : Intraluminal high dose rate brachytherapy is an accepted treatment for the tumors of GI tract. However, there is only some limited clinical data for intraluminal high dose rate brachytherapy for the tumors of GI tract. Materials and Methods : Between February 1991 and July 1993, 18 Patients who have the tumors of GI tract (esophageal cancer-8 cases, rectal cancer-10 cases) were treated with high dose rate Iridium-192 afterloading system (Microselectron-HDR, Nucletron CO, Netherland) at the department of therapeutic radiology, St. Mary's hospital, Catholic university medical college. Age range was 47-87 years with a mean a9e 71 years. All patients were treated with intraluminal high dose rate brachytherapy within two weeks after conventional external radiation therapy and received 3-5 Gy/fraction 3-4 times per week to a total dose 12-20 Gy (mean 17 Gy). Standard fractionation and conventional dose were delivered for external radiation therapy. Total dose of external radiation therapy ranged 41.4-59.4 Gy (mean 49.6 Gy). Median follow up was 19 months Results : The analysis was based on 18 patients, The complete response and partial response in esophageal cancer was similar (38%). Two year rates for survival and median survival were 13% and 10 months, respectively. Among 10 patients of rectal cancers, partial response was obtained in 6 patients (60%). There was no complete response in the patients with rectal cancer, but good palliative results were achieved in all patients. Conclusion : Although the number of patients was not large and the follow-up period was relatively short, these findings suggested that intraluminal high dose rate brachytherapy could be useful in the treatment of the patients with advanced tumors of GI tract.

  • Patent and utility indicate international competitiveness in the knowledge-based society of the $21^{st}$century where both the quantity and quality of the nation's scientific intelligence and innovative technology represent key criteria to evaluate its strength. Thus, discerning the trends of patents is inevitable for further development. This research is centered on apprehending the technological current of the functional clothing of Korea, through an analysis of patents and utility models. The number of patent applications in Korea was low until the mid-1990s. However, it began to grow rapidly in the 2000s and the number of patents surpassed the number of utility starting in 2006. The technological level of invention in this field has been turned into a higher level. The IPC code with the strongest application was the field related to temperature controllable clothing (A41D 13/005), followed by surgeon or patient apparel related fields (A41D 13/12), and reflective or luminous safety devices (A41D 13/01).The main technological idea was to give functionality that could protect the human body from various hazards and represents the goal of various applied techniques. About 66% of domestic patent applications belong to individuals; however, the proportion of corporate or institutional applications(including universities) remains poor. Consequently, more systematic and long-term support for research on patents is required.

  • Purpose: The aim of this study was to examine the validity and reliability on Psychometric Arthritis Impact Measurement Scale-Short Form (AIMS-SF) in patients with knee osteoarthritis(OA). Methods: The sample consisted of 62 patients who had received physical therapy at the physical therapy units of the Andong Medical Center, Sungso Hospital, Ahn Orthopedic Surgery Clinic, and St. Luke Clinic in Andong city in June 2006. Questionnaire on the AIMS-SF was recruited by 5 physical therapists. The internal structure and reliability of the scales were evaluated by means of item-internal consistency(Cronbach's alpha coefficient: ${\alpha}$), item-discriminant validity, Pearson's relation coefficient. To explore construct validity we conducted a principal component factor analysis with varimax rotation analysis. The criterion for factor extraction was an eigenvalue >1.0. Results: 62 OA patients's an average $age{\pm}standard$ deviation was $53.8{\pm}14.4$ years (range: $40{\sim}81$ yr). The internal consistency reliability of 11 items, as estimated by Cronbach's ${\alpha}$ coefficient, was high ranging $0.60{\sim}0.78$ (except for 0.40 for mobility level and -0.48 for mood). The internal consistency reliability of item-each scale was also high $0.82{\sim}0.93$ (except for 0.48 for mood). Of 11 items, the item-discriminant validity on 6 items was high (${\alpha}$ coefficient range: $0.11{\sim}0.25$), however, others (0.57 for tension level, 0.48 for arthritis pain, 0.41 for walking and bending, and 0.40 for work) were a little low. The construct validity by factor analysis was a little low. Conclusion: In conclusion, the results reported here confirm the validity and reliability of the AIMS-SF in patients with OA of the knee. The Collection of information on health status using this instrument was acceptable to patients. A further prospective multi-center study will be necessary to prove the construct validity.
