A Study on the Standards of Medical-Nutritional-Education by the Type of Bariatric Surgery in Morbid-obesity Patients (고도비만 환자의 수술적 치료방법에 따른 영양교육 기준 설정에 관한 연구)

  • This study examined the effects of postoperative medical nutrition therapy on patients who had undergone bariatric surgery. Eighty seven patients who underwent bariatic-surgery at Yeouido St. Mary's Hospital from January 2007 to April 2009 were evaluated. The bariatric surgery patients included 42 Laparoscopic Roux-en Y gastric bypass (LRYGB) and 45 Laparoscopic adjustable gastric banding (LAGB) patients. Weight loss was more significant after LRYGB than after LAGB after 9 months (p<0.05). The LRYGB group was more satisfied with the weight loss (LRYGB 4.4/5.0, LAGB 3.0/5.0 p<0.001). The mean albumin, hemoglobin and hematocrit levels were significantly lower in the LRYGB group than in the LAGB group at the time of discharge (p<0.05~0.001). The GOT/GPT was significantly higher in the LRYGB group at the time of the operation than the LAGB group (p<0.01). The LRYGB group showed significantly lower intakes of total energy, carbohydrates, protein and fat from 1 week after surgery than the LAGB group. Multiple regression showed that the weight change after LRYGB was significantly more associated with the intakes of total energy at 1 week after surgery (p<0.01), SWS (sweets and high-calorie beverages) at 1 and 6 months after surgery (p<0.001), and fat at 3 months after surgery (p<0.01). In addition, LAGB was significantly more associated with the intakes of protein and NLS (non-liquid sweets) at 1 week after surgery (p<0.001, p<0.01), carbohydrate at 1 months after surgery (p<0.01), total energy at 3 months after surgery (p<0.001), HCL (high-calorie liquids) at 6 months after surgery (p<0.05), and fat at 9 months after surgery (p<0.01). These results suggest that continuous-follow-up medical nutrition therapy is needed according to the types of bariatric surgery, particularly during the weight loss phase (the first 1 week to 12 months).

Nutritional Status of Recipients of Allogeneic Hemopoietic Stem Cell Transplantation by Types of Conditioning Regimen (동종 조혈모세포 이식환자의 이식 전 처치 형태에 따른 영양상태)

  • This study was conducted for 39 patients who are recipients of allogeneic hemopoietic stem cell transplantation at BMT ward of St. Mary's hospital affiliated to Catholic University of Korea from April to September 1999. The subjects were devided into two groups; those who received both TEl and chemo therapy as conditioning regimen (TEl group). and those who used chemo agents as singular conditioning regimen (chemo group). The oral intake status of the two groups were compared through physical assessment and blood chemistry exam of the subjects, and factors influencing their nutritional change and oral intake were explored in each stage of the transplantation (six stages: admission, conditional stage, date of transplantation, one week after transplantation, two weeks after transplantation, and three weeks after transplantation). The prior aim of the study was to provide baseline data to minimize delayed treatment from nutritional deficiency of the subjects. The results were as follows: 1. TBI group was significantly decreased of oral calorie intake in two weeks after transplantation compared to admission and conditioning stage while that of chemo group was significantly decreased on the date of transplantation. 2. TBI group was significantly decreased of protein intake in two weeks after transplantation compared to admission and conditioning stage. In chemo group, protein intake was significantly decreased on the date of transplantation compared to admission. It was remarkable that TBI group showed lesser protein intake than chemo group. 3. Both group were significantly decreased of BMI in one week and three weeks after transplantation compared to admission. TBI group showed significantly higher BMI than chemo group. 4. Both group were significantly decreased of Triceps Skinfold Thickness (TST)on the date of transplantation compared to admission stage. 5. TBI group was significantly decreased of mid-arm muscle circumference (MAMC) in two weeks after transplantation compared to admission, conditioning, date of transplantation. 6. TBI group was significantly decreased of albumin level in two weeks after transplantation compared admission stage. In chemo group, it was significantly decreased on the date of transplantation compared to admission, three weeks after the transplantation. 7. TBI group was significantly decreased of transferrin level in two weeks after transplantation compared admission, conditioning, date of transplantation and one week after transplantation. In chemo group, it was decreased of transferrin level in 3 weeks after transplantation. 8. Oral intake of TEl group was impacted by vomiting before transplantation and gingivitis after transplantation. In chemo group, it was impacted by vomiting before transplantation and by two factors, gingivitis and nausea, after transplantation. The results showed oral calorie intake was not different between the two groups while protein intake was significantly lower in TBI group than chemo group. Oral intake was significantly impacted by vomiting before transplantation in both groups, but affected by oral gingivitis in TBI group and gingivitis and nausea in chemo group after transplantation. This findings present that standardized strategies to manage nutrition and gingivitis more effectively are desperately needed to enhance oral intake and protein intake of patients who receive TBI as conditioning regimen.

Immobilization of Lipases on Amberlite and Their Interesterification Reaction Characteristics (Amberlite에 고정화된 Lipase 제조 및 효소적 Interesterification을 이용한 반응 특성 연구)

  • Immobilized lipases were prepared by physical adsorption using lipase AK, AY, AH, PS and R on Amberlite$^{(R)}$XAD$^{(R)}$7 HP resin. With the immobilized lipases (10%), structured lipid was synthesized by enzymatic interesterification of canola oil, palmitic ethyl ester, and stearic ethyl ester in order to study the reaction characteristics. Among the lipase, the highest protein content was obtained from lipase AH (11.41%) before immobilization, while the highest levels of bound protein was observed from immobilized lipase AK (63.91%). Immobilized lipase AK had the highest interesterification activity (38.3% of total saturated fatty acid). Lipase AK was also used for a continuous reaction in which the slow flow of reactant resulted in increased reaction rate. Reusability of immobilized AK, AH and PS increased at the second reaction (120-196.5%). However, the activity of immobilized AK, which had the highest bound protein content (63.91%) decreased after the third reaction, while the activity of immobilized AH and PS was maintained until the sixth reaction.

The Effect of Aqua-acupuncture of Total Saponin on the Damaged Liver Induced by Carbon Tetrachloride in Rats (인삼 총 사포닌 약침이 사염화탄소에 의한 랫트 간손상에 미치는 영향)

  • The effect of aqua-acupuncture of total saponin on the recovery of liver function in -intoxicated rats was exemined. Rats were divided into normal, control and three experimental groups (As-I group : aqua-acupuncture of saline to Gansu locus and Aa-III group : aqua-a-cupuncture of total saponin to blank locus at the thigh). Changes of some serum biochemical parameters (ALT, AST and ALP, total protein and albumin, and albumin/globulin (A/G) ratio) were determined. Serum ALT activities were increased by $CCI_{4}$T and decreased in all aqua-acupuncture groups compared with the control group. Aa-I group showed significant decrease of ALT activities on the 2nd (P<0.05) and 3rd (P<0.05) day, respectively. Aa-II group showed significant decrease on the 3rd day (P<0.05) day. Serum AST activities were increased by $CCI_{4}$T and decreased in all apua-acupuncture groups compared with the control group. Aa-I group showed significant decrease on the 1st (P<0.05), 2nd (P<0.001) and 3rd (P<0.001) day, respectively. Aa-II group showed significant decrease on the 3rd (P<0.001) day. Aa-III group showed significant decrese on the 1st (P<0.05) and 3rd (P<0.001) day, respectrvely. Serum ALP activities were increased by $CCI_{4}$T and decreased in all aqua-a-cupuncture groups compared with the control group. Aa-I group showed significant decrease on the 3rd (P<0.01) day. Total protein, albumin and A/G ratio were decreased by $CCI_{4}$T and increased in all aqua-acupuncture groups compared with control group. Aa-I group showed significant increase only on the 3rd day in albumin (P<0.01). It was thought that the aqua-acupuncture of total saponin to Gansu locus was effective on the recovery of liver function in $CCI_{4}$T-intoxicated rats.

Effects of Application of Nightsoil Sludge Compost on Dry Matter Yields and Nutritive Value of Orchardgrass(Dactylis glomerata L.) (분뇨 슬러지 퇴비 시용이 Orchardgrass의 건물 수량과 영양가에 미치는 영향)

  • This experiment was carried out to investigate the effects of application levels of nightsoil sludge compost(NSC) in 3 and 4 cuttings per annum on the dry matter yields and nutritive value of orchardgrass(Dactylis glomerata L.). Annual nightsoil sludge compost(NSC) were applied at levels of 0, 10, 20, 40 and 80 ton $㏊^{respectively}$ . Relative yields of each cut to annual dry matter yield were 29.9%, 46.% and 23.2% for 1st cut, 2nd cut and 3rd cut in 3 cutting system, 23.7%, 29.3%, 34.0% and 13.0% for 1st cut, 2nd cut, 3rd cut and 4th cut in 4 cutting system, respectively. Significantly higher dry matter yield of orchardgrass obtained were 8.11 ton $㏊^{-1}$ at level of 40 ton ㏊$^{-1}$ $yr^{-1}$ in 3 cutting systems, ranges of 7.50~10.71 ton $㏊^{-1}$ at level of 10~80 ton ㏊$^{-1}$ $yr^{-1}$ in 4 cutting systems than those of non application level of NSC, respectively(p〈0.05). The mean efficiency of dry matter production to application of NSC(kg DM/ton NSC) were 46 and 102 kg in 3 and 4 cutting systems, respectively. Although th contents of crude protein(CP, %) of orchardgrass were significantly higher at the application levels of 10~80 ton $㏊^{-1}$ $yr^{-1}$ -1/ by NSC than those of the application levels at the 0 ton $㏊^{-1}$ $yr^{-1}$ in 3 and 4 cutting systems(p〈0.05), the contents of neutral detergent fiber(NDF, %) and acid detergent fiber(ADF, %) were lower in NSC plots than those at the non-application plots of NSC. The contents of total digestible nutrient(TDN, %) and relative feed value(RFV) of orchardgrass were significantly increased with increase the applicaton levels of NSC(p〈0.05).

A Study of Predicting the Severity Following Glufosinate Ammonium Containing Herbicide Poisoning Experienced in Single Emergency Medical Institution (단일 응급의료기관에서 경험한 글루포시네이트 암모니움 포함 제초제 중독 후 중증도 예측에 관한 연구)

  • Purpose: Owing to the increased agricultural use of the herbicide glufosinate ammonium (GLA), the incidence of GLA poisoning has recently increased. Therefore, we investigated the possible predictive factors associated with severe complications following GLA poisoning. Methods: A retrospective analysis of medical records was conducted based on 76 patients who had visited our regional emergency medical center with GLA poisoning from 2006 to 2017. Severe complications were defined as respiratory failure requiring intubation, systolic blood pressure less than 90 mmHg, Glasgow Coma Scale (GCS) less than 8, and presence of seizure. Results: Age, ingested amount and ingested amount per weight were significantly greater in the severe group (p<0.001). PSS grade 2 or higher was more common in the severe group (p<0.001), and In addition, the APACHE II score was significantly higher in the severe group (p<0.001), as were the SOFA scores (p=0.002). Serum ammonia levels were significantly higher in the severe group (p=0.007), while MDRD-GFR was smaller in the severe group (p=0.002). The spot urine protein levels were significantly higher in the severe group (p=0.005), as was the urine protein to creatinine ratio (p=0.001). Upon multivariate analysis, the amount ingested per weight and PSS grade 2 or higher were identified as significant predictors. Conclusion: Our study showed that MDRD-GFR was significantly lower in the severe group after GLA poisoning. PSS grade 2 or higher and ingested amount per weight may be useful to evaluate the severity of complications after GLA poisoning.

Changes in the Contents and Composition of Dietary Fiber during the Growth of Soybean Sprout (콩나물 성장중에 식이섬유 함량과 조성의 변화)

  • This study was conducted to determine changes in th contents and composition of dietary fiber during the growth of soybean sprout. Soybean was soaked in water at $25^{\circ}C$ for 2hrs and cultivated at 2$0^{\circ}C$ for 7 days under dark condition. The soybean sprouts were divided into cotyledon and axis and sampled every 24hrs. The analysis methodlogies used were Van Soest's NDF, AOAC's ADF and lignin and Prosky's IDF, SDF, TDF. The weight of 100 sprouts increased gradually from 20.26g to 90.12g during the growth periods. The weight increased to 344.9% of the original weight. The germination rate was 100% after soaking at $25^{\circ}C$ for 2hrs. Root length increased gradualy from 0.6cm at 1st day to 17.2cm at 7th day. The crude ash and crude fat contents showed no significant change in the cotyledon and axis. The crude protein contents increased in the cotyledon and axis, whereas the total carbohydrate content didn't have general tendency. The insoluble dietary fiber(IDF), soluble dietary fiber(SDF) and total dietary fiber(TDF) contents of cotyledon were no significantly different from 20.01%, 1.45%, 21.46% at 1st day to 22.75%, 2.07%, 24.82% at 7th day on dry basis. In axis those contents increased from 23.19%, 1.97%, 25.16% at 1st day to 32.78%, 3.02%, 35.80% at 7th day, respectively. The neutral detergent fiber(NDF) contents of cotyledon and axis increased from 4.35% to 6.39% and from 6.44% to 26.60% respectively on dry basis. The acid detergent fiber (ADF) contents of cotyledon and axis increased from 2.84% to 4.91% and from 2.5% to 4.7%, but there were no significantly different in the hemicellulose and lignin contents on dry basis. The hemicellulose and lignin contents of axis increased with culture periods from 1.70% to 4.41% and from 0.20% to 2.11%, respectively. The cellulose contents increased from 4.54% to 20.35% on dry basis.

A Novel Therapeutic Effect of a New Variant of CTLA4-Ig with Four Antennas That Are Terminally Capped with Sialic Acid in the CTLA4 Region

  • Rheumatoid arthritis (RA) is a multifactorial immune-mediated disease, the pathogenesis of which involves different cell types. T-cell activation plays an important role in RA. Therefore, inhibiting T-cell activation is one of the current therapeutic strategies. Cytotoxic T-lymphocyte antigen 4-immunoglobulin (CTLA4-Ig), also known as abatacept, reduces cytokine secretion by inhibiting T-cell activation. To achieve a homeostatic therapeutic effect, CTLA4-Ig has to be administered repeatedly over several weeks, which limits its applicability in RA treatment. To overcome this limitation, we increased the number of sialic acid-capped antennas by genetically engineering the CTLA4 region to increase the therapeutic effect of CTLA4-Ig. N-acetylglucosaminyltransferase (GnT) and α2,6-sialyltransferase (α2,6-ST) were co-overexpressed in Chinese hamster ovary (CHO) cells to generate a highly sialylated CTLA4-Ig fusion protein, named ST6. The therapeutic and immunogenic effects of ST6 and CTLA4-Ig were compared. ST6 dose-dependently decreased paw edema in a mouse model of collagen-induced arthritis and reduced cytokine levels in a co-culture cell assay in a similar manner to CTLA4-Ig. ST6- and CTLA4-Ig-induced T cell-derived cytokines were examined in CD4 T cells isolated from peripheral blood mononuclear cells after cell killing through irradiation followed by flow- and magnetic-bead-assisted separation. Interestingly, compared to CTLA4-Ig, ST6 was substantially less immunogenic and more stable and durable. Our data suggest that ST6 can serve as a novel, less immunogenic therapeutic strategy for patients with RA.

Digestibility and fermentation rate or Alfalfa , Orchar grass with different cutting times (예취시기에 따른 Alfalfa , Orchar grass고정물의 소화율 , 발효율 및 발산속도 측정)

  • Chemical composition, dry matter and cellulose digestibilities and fermentation rates of alfalfa and orchard grass cut at different time were estimated in vitro method and the results summurized as follows: 1. Crude protein, crude fiber cwc and cellulose content of Alfalfa were decreassed as advancing cutting time. Crude protein, crude fiber and cwc content of Orchard grass were decreased as advancing cutting time up to 3 cutting, but crude protein was slightly increased at 4th cutting, crude fiber and cellulose content were higher at 2nd cutting, but decreased thereafter. 2. DM digestibility of Alfalfa was 51.80, 51.86, 52.92 and 59.52% at 1, 2, 3 and 4th cutting time, respectibly, thus slightly increased as advancing cutting time, and cellulose digestibility of Alfalfa was not much different with different cutting time. DM digestibility of Orchard grass was 62.21, 66.10, 60.95 and 66.32% at 1, 2, 3 and 4th cutting time, respectibly, and cellulose digestibility of Orchard grass was slightly increased at 3rd cutting time and then increased at 4th cutting time. 3. Fermentation rate of DM of alfalfa was the highest 1st cutting time (0.83%/hr.) and was not different at 3 and 4th cutting time. Cellulose fermentation rate of Alfalfa was the highest at 1st cutting time (1.29%/hr.), decreased at 2 and 3th and then increased at 4th cutting time. Fermentation rate of DM of Orchard grass was 1.42, 1.58, 1.60 and 1.57%/hr. and of cellulose was the highest at 2nd cutting time (1.77%/hr.)

Waterlogging induced oxidative stress and the mortality of the Antarctic plant, Deschampsia antarctica

  • We investigated the mortality and the oxidative damages of Deschampsia antarctica in response to waterlogging stress. In field, we compared the changes in the density of D. antarctica tuft at the two different sites over 3 years. The soil water content at site 2 was 6-fold higher than that of site 1, and the density of D. antarctica tuft decreased significantly by 55.4% at site 2 for 3 years, but there was no significant change at site 1. Experimental results in growth chamber showed that the $H_2O_2$ and malondialdehyde content increased under root-flooding treatment (hypoxic conditions-deficiency of $O_2$), but any significant change was not perceptible under the shoot-flooding treatment (anoxic condition-absence of $O_2$). However, total chlorophyll, soluble sugar, protein content, and phenolic compound decreased under the shoot-flooding treatment. In addition, the catalase activity increased significantly on the 1st day of flooding. These results indicate that hypoxic conditions may lead to the overproduction of reactive oxygen species, and anoxic conditions can deplete primary metabolites such as sugars and protein in the leaf tissues of D. antarctica. Under present warming trend in Antarctic Peninsula, D. antarctica tuft growing near the shoreline might more frequently experience flooding due to glacier melting and inundation of seawater, which can enhance the risk of this plant mortality.
