관동맥질환 치료의 최신 지견 (Updates in the Management of Coronary Artery Disease)

  • Coronary artery disease (CAD) has been increasing during the last decade and is the one of major causes of death. The management of patients with coronary artery disease has evolved considerably. There are two main strategies in the management of CAD, complementary, not competitive, each other; the pharmacologic therapy to prevent and treat CAD and the percutaneous coronary Intervention (PCI) to restore coronary flow. Antiplatelet drugs and cholesterol lowering drugs have central roles in pharmacotherapy. Drug eluting stent (DES) bring about revolutional changes in PCI. In the management of patients with 57 segment elevation acute myocardial infarction (AMI), there has been a debate on the better strategy for the restoration of coronary flow. Thrombolytic therapy is widely available and easy to administer, whereas primary PCI is less available and more complex, but mote complete. Recently published evidences in the pharmacologic therapy including antiplatelet and stalin, and PCI including DES and reperfusion therapy in patients with ST segment elevation AMI were reviewed.

급성심근경색증 환자에서 응급으로 시행한 Tc-99m Sestamibi 심근관류 SPECT와 심전도의 비교 (Comparison of Emergency Tc-99m Sestamibi Myocardial Perfusion SPECT and ECG in the Diagnosis of Acute Myocardial Infarction)

  • 저자들은 금성심근경색증으로 진단된 36명에서 응급실 도착후에 시행한 Tc-99m seataMIBl 심근 SPECT를 심전도검사와 비교하여 다음의 결과를 얻었다. 1) 좌전하행관동맥과 우관동맥의 폐쇄로 인한 심근 경색증환자 24명 모두가 안정기 Tc-99m sestamibi 심근 SPECT에서 관류결손을 나타냈고, 심전도상에서는 Q파 혹은 ST절의 상승이 관찰되었다. 2) 좌회선관동맥의 폐쇄로 인한 심근경색증환자 7명 모두 안정기 Tc-99m sestamibi 심근 SPECT에서 측벽에 관류결손이 나타났으나, 이상 Q파는 1명에서만 관찰되었고, 6명에서 ST절의 변화가 보였으며, 1명에서는 정상심전도소견을 보였다. 3) 심혈관촬영술을 시행하지 않은 3예중 2예에서는 심전도상 경색의 위치와 심근 SPECT에서의 관류결손부위가 일치하였으나, 나머지 1예에서는 심전도상 LBBB가 보였고 심근SPECT에서는 하벽에 관류결손이 관찰되 었다. 4) 극성지도를 이용하여 구한 손상된 심근의 면적을 전체심근에 대한 백분율로 표시하면 좌전하행관동맥 : $31.17{\pm}18.21%$, 우관동맥 : $31.17{\pm}13.24%$, 좌회선관동맥 : $24.71{\pm}5.94%$ 이었다. 5) 정성적인 분석에서 심근경색의 진단적 예민도는 100%였고, 관동맥 조영술을 시행하여 경색 관련 관동맥을 진단할 수 있었던 31명의 모든 예에서 심근관류 SPECT로 국소화가 가능하였다. 이상의 결과에서 응급으로 시행한 심근관류 SPECT가 심근경색의 진단에 유용하며, 특히 심전도에서 심근경색증의 판단이 어려운 측벽경색이나 좌각차단이 있는 경우에 유용한 방법으로 생각된다.

Three-Dimensional Myocardial Strain for the Prediction of Clinical Events in Patients With ST-Segment Elevation Myocardial Infarction

  • BACKGROUND: Two-dimensional (2D) strain provides more predictive power than ejection fraction (EF) in patients with ST-elevation myocardial infarction (STEMI). 3D strain and EF are also expected to have better clinical usefulness and overcome several inherent limitations of 2D strain. We aimed to clarify the prognostic significance of 3D strain analysis in patients with STEMI. METHODS: Patients who underwent successful revascularization for STEMI were retrospectively recruited. In addition to conventional parameters, 3D EF, global longitudinal strain (GLS), global area strain (GAS), as well as 2D GLS were obtained. We constructed a composite outcome consisting of all-cause death or re-hospitalization for acute heart failure or ventricular arrhythmia. RESULTS: Of 632 STEMI patients, 545 patients (86.2%) had a reliable 3D strain analysis. During median follow-up of 49.5 months, 55 (10.1%) patients experienced the adverse outcome. Left ventricle EF, 2D GLS, 3D EF, 3D GLS, and 3D GAS were significantly associated with poor outcomes. (all, p < 0.001) The maximum likelihood-ratio test was performed to evaluate the additional prognostic value of 2D GLS or 3D GLS over the prognostic model consisting of clinical characteristics and EF, and the likelihood ratio was 15.9 for 2D GLS (p < 0.001) and 1.49 for 3D GLS (p = 0.22). CONCLUSIONS: The predictive power of 3D strain was slightly lower than the 2D strain. Although we can obtain 3D strains, volume, and EF simultaneously in same cycle, the clinical implications of 3D strains in STEMI need to be investigated further.

의사결정트리를 이용한 ST분절상승 급성심근경색증 환자를 위한 장기 사망 예측 모형 (A Long-term Mortality Prediction Model for Patient with ST-segment Elevation Myocardial Infarction using Decision Tree)

  • 이 논문에서는 한국인 급성심근경색증 환자에 대한 KAMIR 데이터를 기반으로 ST분절상승 심근경색이 처음 발병한 환자의 사망에 영향을 미치는 위험요소들을 찾고, 이를 기반으로 ST분절상승 급성심근경색환자의 1년 이내 사망을 예측하는 모델을 제시한다. 총 22개의 속성 중에서 속성 선택 알고리즘을 적용한 결과 나이, 심장박출계수, 크레아티닌, 고감도 C-반응성 단백질 등 4개의 속성이 선택되었고, 이 속성들을 이용하여 더욱 정확한 예측 모델을 구축할 수 있었다. 제시된 모델을 통해서 고위험군 환자의 위험성을 평가하고 예후를 추정할 수 있을 것으로 기대한다.

Non-Contrast Cine Cardiac Magnetic Resonance Derived-Radiomics for the Prediction of Left Ventricular Adverse Remodeling in Patients With ST-Segment Elevation Myocardial Infarction

  • Objective: To investigate the predictive value of radiomics features based on cardiac magnetic resonance (CMR) cine images for left ventricular adverse remodeling (LVAR) after acute ST-segment elevation myocardial infarction (STEMI). Materials and Methods: We conducted a retrospective, single-center, cohort study involving 244 patients (random-split into 170 and 74 for training and testing, respectively) having an acute STEMI (88.5% males, 57.0 ± 10.3 years of age) who underwent CMR examination at one week and six months after percutaneous coronary intervention. LVAR was defined as a 20% increase in left ventricular end-diastolic volume 6 months after acute STEMI. Radiomics features were extracted from the oneweek CMR cine images using the least absolute shrinkage and selection operator regression (LASSO) analysis. The predictive performance of the selected features was evaluated using receiver operating characteristic curve analysis and the area under the curve (AUC). Results: Nine radiomics features with non-zero coefficients were included in the LASSO regression of the radiomics score (RAD score). Infarct size (odds ratio [OR]: 1.04 (1.00-1.07); P = 0.031) and RAD score (OR: 3.43 (2.34-5.28); P < 0.001) were independent predictors of LVAR. The RAD score predicted LVAR, with an AUC (95% confidence interval [CI]) of 0.82 (0.75-0.89) in the training set and 0.75 (0.62-0.89) in the testing set. Combining the RAD score with infarct size yielded favorable performance in predicting LVAR, with an AUC of 0.84 (0.72-0.95). Moreover, the addition of the RAD score to the left ventricular ejection fraction (LVEF) significantly increased the AUC from 0.68 (0.52-0.84) to 0.82 (0.70-0.93) (P = 0.018), which was also comparable to the prediction provided by the combined microvascular obstruction, infarct size, and LVEF with an AUC of 0.79 (0.65-0.94) (P = 0.727). Conclusion: Radiomics analysis using non-contrast cine CMR can predict LVAR after STEMI independently and incrementally to LVEF and may provide an alternative to traditional CMR parameters.

Acute left main coronary artery thrombosis as an initial presentation of systemic lupus erythematosus

  • Left main coronary artery (LMCA) thrombosis is rare and the cause should be determined. A previously healthy young man presented with severe chest pain and dyspnea. The electrocardiogram showed typical ST-segment elevation myocardial infarction with clinical instability. Emergency coronary angiography revealed complete LMCA occlusion by thrombosis. After reperfusion, the patient was admitted to the cardiac care unit. He was diagnosed with hemolytic anemia and tested positive for antinuclear antibodies. Systemic lupus erythematosus (SLE) and LMCA disease due to systemic thrombosis were diagnosed. Steroids were started and the patient was discharged without complications. We report this rare case of LMCA thrombosis as an initial presentation of SLE.

Prognostic significance of non-chest pain symptoms in patients with non-ST-segment elevation myocardial infarction

  • Background/Aims: Chest pain is an essential symptom in the diagnosis of acute coronary syndrome (ACS). One-third of patients with ACS present atypically, which can influence their receiving timely lifesaving therapy. Methods: A total of 617 NSTEMI patients from the Korea Acute MI Registry (KAMIR) and the Korea Working Group on MI (KorMI) databases were analyzed. The study population was divided into two groups by symptoms at presentation (typical symptoms group, 128; atypical symptoms groups, 128). Results: In this study population, 23% of patients presented without chest pain. After propensity score matching, the contact-to-device time ($2,618{\pm}381minutes$ vs. $1,739{\pm}241minutes$, p = 0.050), the symptoms-to-balloon time ($3,426{\pm}389minutes$ vs. $2,366{\pm}255minutes$, p = 0.024), and the door-to-balloon time ($2,339{\pm}380minutes$ vs. $1,544{\pm}244minutes$, p = 0.002) were significantly higher in the patients with atypical symptoms than in those with typical symptoms, respectively. Atypical symptoms were an independent predictor for 1-year mortality (hazard ratio, 2.820; 95% confidence interval, 1.058 to 7.515; p = 0.038). The Kaplan-Meier estimates showed higher risk for 12-month mortality in patients with atypical symptoms (p = 0.048) and no significant difference for 12-month major adverse cardiac events (p = 0.487). Conclusions: Acute myocardial infarction patients with atypical symptoms were not rare in clinical practice and showed a high risk of delayed reperfusion therapy. After imbalance between the groups was minimized by use of propensity score matching, patients who presented atypically had a high mortality rate.

급성심근경색증 환자의 일차적 관동맥 스텐트 삽입술 시 삽입된 Sirolimus-eluting stent 와 Paclitaxel-eluting stent의 임상적 안정성 및 유용성 평가 (The evaluation for Clinical usefulness and Safety of Sirolimus-eluting stent and Paclitaxel-Eluting Stents In Patients With Acute Myocardial Infarction)

  • AMI로 SES 혹은 PES 시술을 시행받은 모든 환자에서 4년 이상의 임상추적 기간이 지난 환자를 대상으로 데이터를 분석하여 두 스텐트의 안전성과 유용성을 비교해 보고자 하였다. 2004년 1월 1일부터 2006년 8월 31일까지 본원에서 ST분절 상승 혹은 ST 분절 비상승 급성심근경색증 (STEMI or NSTEMI)로 진단되어 입원 기간 중 관동맥중재술을 시행받은 환자 중 SES 혹은 PES 삽입술이 시행된 환자를 대상으로 후향적 분석을 시행하였다. 그리고 사망, 심장사. 심근경색증, 표적 혈관재관류술, 스텐트 혈전증 발생에 대해 분석하였다. 연구 기간 동안 총 668명의 급성심근경색증 환자가 중 522명만 연구 대상에 포함 사망 ($18.3{\pm}3.0%$ vs. $14.6{\pm}2.2%$, p=0.26), 심장사($11.2{\pm}2.6%$ vs. $6.8{\pm}1.52%$, p=0.39), 심근경색증 ($6.4{\pm}1.8%$ vs. $3.3{\pm}1.1%$, p=0.31), and 스텐트 혈전증 ($5.4{\pm}1.7%$ vs. $3.2{\pm}1.1%$, p=0.53) 표적 혈관재관류술(TVR) ($10.0{\pm}3.0%$ vs. $4.0{\pm}1.2%$, p=0.008) and 심혈관계 임상사건(MACE) ($29.4{\pm}3.5%$ vs. $19.4{\pm}2.5%$, p=0.003) 급성심근경색증의 초기 치료에 약물방출스텐트인 SES와 PES의 4년 장기 임상 성적을 조사한 본 연구를 통해 전체환자를 대상으로 분석하였을 때 두 스텐트의 장기 사망률, 심장사. 심근경색증, 표적 혈관재관류술, 스텐트 혈전증의 발생은 차이가 없었으나 TVR 및 MACE의 발생은 PES 삽입 환자가 SES삽입 환자보다 유의하게 높았다.

개심술 시 냉혈성 심정지액 사용에 따른 허혈 전후 심근 미세구조의 변화 (Ultrastructrual Change of Myocardium in Open Cardiac Surgery with Cold Blood Cardioplegia)

  • 본 연구에서는 사용된 혈성심정지용액에 의한 심근보호의 효과를 평가하고 심근보호를 평가하는 방법에 있어서 심근 미세구조관찰의 유용성과 다른 검사방법들과의 상관관계를 알아 보고자 하였다. 대상 및 방법 : 판막수술 및 관상동맥우회로술을 시행한 18명의 환자의 심전도, CK의 MB 동위효소, SGOT, LDH1과 LDH2의 비 등의 변화를 측정하고 반정량적인 방법을 통해 심근 미세구조의 변화를 관찰하였다. 인공심폐기 가동 직전과 인공심폐기 가동 직후 우심방 부속지에 조직검사를 시행하였고, 심근관련 혈청효소치들과 심전도는 술 후 3일 동안 검사하여 최고치를 구하였으며 심전도에서는 새로운 Q파나 ST분절의 상승을 관찰하였다. 결과: 대상환자는 남자 8명, 여자 10명이었으며 평균연령은 55.6$\pm$13세였다. 판막수술을 시행한 환자는 8명 관상동맥우회로술을 시행한 환자는 10명이었고, 평균 체외순환시간은 119$\pm$29분이었고 평균 대동맥차단시간은 75.4$\pm$24분이었다 체외순환 직전의 1.28$\pm$0.53이었던 평균 사립체 변화점수가 체외순환이후에는 2.35$\pm$0.79로 의미있게 증가했으나 술 후 심근경색을 시사하는 심근관련 혈청효소치의 증가나 심전도상의 새로운 Q파의 발견이나 ST분절의 상승은 없었다. 그리고 체외순환인 직전 및 직후 심근 사립체 변화점수, 또한 이들의 차는 체외순환시간 및 대동맥차단시간과 상관관계가 없었으며, 술 후 CK-MB, SGOT, LDH1/LDH2 등의 최고치와도 유의한 상관관계를 보이지 않았으나 체외순환시간과 LDH1/LDH2수치는 유의한 상관관계를 보였다 결론: 혈성심정지용액을 사용한 이번 연구에서 심근관련혈청수치나 심전도상에서 만족스러운 결과를 보였지만 심근의 미세구조에는 많은 변화가 있었으며 보다 많은 연구들이 이루어져야 할 것이다.

흉통을 주소로 응급실에 내원한 환자에서 심전도 촬영 소요 시간에 대한 응급벨 제도의 효과 (Effect of emergency bell on door to ECG time in walk-in patients presented to emergency department with chest pain)

  • Objectives: The aim of this study was to explore whether emergency bell could shorten door to electrocardiograms (ECG) time in chest pain patients presenting to emergency department (ED) by self-transport. Methods: This was a planned 6-month before-and-after interventional study design. We set up the emergency bell in walk-in patients' waiting room. Prior to the change, patients were triaged before an ECG was obtained. In new process, as soon as patient with chest pain push the emergency bell, emergency physicians examined patient and prioritized performing ECG. We analyzed door to electrocardiograms (DTE) times for patients with chest pain and ST segment elevation myocardial infarction (STEMI) patients between two periods. Results: During the enrollment period, a total of 63 patients called emergency bell. The median DTE time was 6 min (interquartile range: 3.0 - 9.0) and 82.5% received an ECG within 10 minutes, and only three patients were STEMI. DTE time in patient with chest pain was not different between two periods (p=0.980). Before intervention period, 15 walk-in patients admitted in ED for STEMI and 53.8% of STEMI patients received an ECG within 10 minutes. After intervention period, total 19 walk-in patients admitted in ED for STEMI. Of these, 89.5% met the time requirement. Conclusion: Because a small portion of patients with chest pain activated the emergency bell, new strategy for promotion of emergency bell must be needed.
