비침습적 인공호흡기 적용 환자의 불편감 연구 (Undergoing Noninvasive Mechanical Ventilation)

  • Purpose: The purpose of this preliminary study was to describe the physical and psychosocial distress of critically ill patients undergoing noninvasive ventilation(NIV). Method: Open-ended question interviews were conducted with four patients using NIV. Considering the outcomes of the interviews, literature, and experts'opinions, a survey questionnaire was developed. Twenty patients in medical and surgical intensive care units answered the questions. Results: Through open-ended question interviews and survey, the patients using NIV reported feeling heavy, impotent feeling, pain, loss in feeling, loss in communication, and lack of sleep as physical distress and a loss in sense of time, pain, anxiety, mind of desiring to die, worry about family, and a sense of burden for medical-cure expenses as psychosocial distress. Conclusion: Critically ill patients undergoing NIV experience physical and psychosocial distress to artificial respiratory ventilation treatment. Further research should be performed with a large sample for generalization of the study result.

소음과 수면양상에 관한 연구 - 중환자실을 중심으로 - (The Relationship between Noise and Sleep Patterns in Intensive Care Units)

  • The environment in the ICU leads to negative changes in a patient's usual sleep pattern and so contributes negatively to the patient's health condition as compared to patients in general wards. Therefore, it is thought that an important nursing intervention would be to identify the relation between noise and sleep patterns which play an important role in illness recovery. The purpose of the present study was to explore the relationship between noise in the ICU and the sleep pattern of patients admitted to the ICU. A descriptive correlation design was used to examine the relationship. Thirty-four subjects were recruited from a Medical ICU (MICU), Surgical ICU (SICU) and Coronary Care Unit (CCU) at a large university hospital in Suwon. Data were collected from September 28 to October 31 in 1999. In the present study, noise was categorized into noise level and patients' perception of noise. The objective noise level was measured using the A-Weighted Sound Level Meter. The patients' preception of noise was measured using a self-reported questionnaire developed by the researcher. Sleep patterns in this study includes both quantity and quality of sleep. These were measured using open ended questionnaires and the 'Korean Sleep Scale A' developed by Oh, Song, Kim(1998). The data was analyzed using the SPSS-WIN to test the research question, Pearson product moment correlation coefficient was run. Ancillary analysis were conducted with demographic variables to determine their relation to the main study variables. For the ancillary analysis, t-test and one-way ANOVAs were performed. The results of the present study are summerized as follows : 1. The total mean of objective noise level (10pm-6am) was 56.2dB. The means for night time noise level in individual ICUs for the SICU, MICU and CCU, were 58.7dB, 58.6dB and 48.3dB, respectively. The total mean for patients' noise perception was 42.8 out of a maximum possible score of 76. For item means of noise perception, the one ranked highest was "conversations between doctors and nurses" (3.2). The one ranked lowest was "noise from the radio" (1.2). Regarding the degree of perception for each type of noise source, the one ranked highest was "equipment noise" (2.6), the second was "conversation between medical staff" (2.4), the third was "conversation between patients, caregivers and visitors" (2.3), and the one ranked lowest was "environment noise" (1.8). 2. Looking at quantity of sleep of ICU patients, the mean nocturnal sleep time was found to be 4.9 hours. The total mean of sleep quality for ICU patients was 21.0 out of a maximum possible score of 40. 3. The relationship between perception of noise and quantity of sleep was statistically significant(r= - .41, p<.05). The relationship between perception of noise and quality of sleep was also statistically significant(r= - .47, p<.01). The results of the study indicate that personal perception of noise is related to sleep patterns. Therefore, it is suggested that nursing interventions be developed to reduce the degree of personal perception of noise and, thus, decrease sleep pattern disturbances in patients in the ICU.

  • A 11-month-old infancy was referred for treatment of cleft palate. The patient's congenital deformities had included oronasal fistula on the soft palate and bifid uvula. Furrow double-opposing Z-plasty was performed under general anesthesia. However, postoperative bleeding developed at ward. Despite the pressure of operative wound, venous oozing continued and hemoglobin concentration and RBC were decreased (Hb: 5.5g/dL, RBC: $1.98{\times}10^6/mm^3$). The patient was pale and confused and transferred to Surgical Intensive Care Unit(SICU) We could not keep the peripheral venous line and there was a high risk of hypovolemic shock. Emergency transfusion was performed through the central femoral venous line and emergency operation for bleeding control was carried out. We explored the operation site and found bleeding point from the posterior pharyngeal wall. Electrocoagulation and surgicel packing were carried out and bleeding was stopped.

호기 및 흡기호흡운동이 상복부 수술을 한 노인 환자의 폐환기능과 폐 합병증에 미치는 효과 비교 (Comparison of Effects of Exhalation and Inhalation Breathing Exercises on Pulmonary Function and Complications in Elderly Patients with Upper-abdominal Surgery)

  • Purpose: The purpose of this study was to identify the effects of exhalation breathing exercises using expirometer and that of inhalation breathing exercises using incentive spirometry on pulmonary function and complications in elderly patients with upper-abdominal surgery. Methods: The research design was a nonequivalent control group non-synchronized design. Participants were 63 patients who underwent upper-abdominal surgery under general anesthesia (32 in experiment group, 31 in control group). They were recruited at P university hospital from August 1 to November 30, 2015. Effects were evaluated by measuring pulmonary functions (Forced Vital Capacity [FVC], Forced Expiratory Volume in 1 second [FEV1]) and pulmonary complications. Data were analyzed using SPSS/WIN 18.0 program. Results: There was no difference in FVC between the experimental group and the control group, but FEV1 in the experimental group increased significantly compared to the control group by time change (p=.001). Also, there were no pulmonary complications in the experimental group but there were 5 cases (16.1%)(p=.018) in the control group. Conclusion: Findings indicate that exhalation breathing exercises by elderly patients following upper-abdominal surgery is an effective nursing intervention in enhancing pulmonary function and preventing pulmonary complications.

  • Purpose: The purpose of this study was to identify the effect of supportive nursing intervention on the anxiety and nursing satisfaction of mothers with children undergoing open-heart surgery. Methods: The research design involved a non-equivalent control group pretest-posttest non synchronized design. The subjects consisted of 43 mothers of children who were expected to undergo open heart surgery, and were divided into two groups; an experimental group of 22 and a control group of 21. The experiment was administered to the experimental group a total of four times, following supportive nursing intervention protocols. The research tools used were Spielberger's State Anxiety Inventory for anxiety and Park Jung-Eun's nursing satisfaction. The data were analyzed using the $x^2$-test, and t-test with SPSS/WIN 12.0 program. Results: Hypothes 1, 'The experimental group who are given supportive nursing intervention will have a lower anxiety score than the control group' was supported(t=5.658, p=.000). Hypothesis 2, 'The experimental group who are given supportive nursing intervention will have a higher nursing satisfaction score than the control gropu wasl supported(t=-9.549, p=.000). Conclusion: The supportive nursing intervention protocol was effective in reducing anxiety and increasing the nursing satisfaction of mothers with children undergoing open-heart surgery.

중환자실 환자의 경험 (The Experiences of Patients in Intensive Care Units(ICU))

  • Purpose: The purpose of this study was to describe the essence of the experiences of patients in an ICU, and to understand them from the patients' point of view. Methods: Participants in this study were six patients in P hospital. Data collection consisted of in-depth interviews and an observation method done from January to April in 2005. The method was analysis using the phenomenological method proposed by Colaizzi(1978). Results: The themes were classified into eight theme clusters. The eight theme clusters were finally grouped into four categories, 'shock', 'pain', 'gratefulness' and 'pleasure of revival'. Conclusion: The ICU patients had negative experiences in physical mental critical situations, but also positive experiences in consolation and nurses and families' encouragement. Therefore, ICU nurses must support patients and their families to minimize the negative experiences and maximize the positive experiences.

병원간호사의 환자안전문화에 대한 인식과 안전간호활동에 관한 연구 (A Study on Hospital Nurses' Perception of Patient Safety Culture and Safety Care Activity)

  • Purpose: The purpose of this study was to investigate perception of patient safety culture and safety care activity among hospital nurses and to identify factors associated with the safety care activity. Methods: A total of 399 nurses working at secondary or tertiary hospitals in B city were participated in. Data were collected using 'Questionnaire on Patient Safety' and 'Safety Care Activity Questionnaire'. Results: The mean score of patient safety culture was 3.41 out of possible 5. The mean score of safety care activity was 4.40 out of possible 5. There was a positive relation between the perception of patient safety culture and the safety care activity. Through stepwise regression analysis with 22.4% of accountability, it was found that the perception of the safety care activity was associated with communication process, a sub-domain of safety culture, marital status, experience of incidence reporting, and level of patient safety. Conclusion: The findings show the importance of communication among nurses to improve the safety care activity. To provide high quality of care for patients, it is necessary to educate nurses on fire safety, patient education, and medication safety and improve their communication skills.

뇌손상 환자에게 적용한 흉부물리요법의 비교연구 (The Effect of the Chest Physiotherapy in Brain Injury Patients)

  • The purpose of this study was to compare chest percussion with chest vibration in brain injury patients. 30 patients in SICU of one general hospital in T city were randomely divided by 3 groups and the arterial oxygen tension(PaO2) and the retained secretion were measured. The chest vibration was done among the first group, the chest vibration by hand after the chest percussion was done among the second group and the mechanical chest vibration after the chest percussion was done among the third group. The data was analyzed using SPSS 7.0(5% significance) and the results are below. 1) It was adopted the hypothesis that the amount of retained secretion of endotracheal suction after stopping the actions among the first, second and the third groups is different from each other.(F=41.62, p=0.00) 2) It was rejected the hypothesis that the arterial oxygen tension(PaO2)s are different from each other among the first, second and third group.(F=1.22, p=0.31) The amount of the retained secretion after chest percussion, chest vibration by hand or mechanical chest vibration was significantly different from each other. Therefore, chest physiotherapy could be regarded as the effective nursing intervention for the unconscieus patients who have the inappropriate airway cleaness and it was more effective to be together than to be alone.

중환자실 경장영양 환자의 영양지원, 위 잔여량 및 영양상태 (Nutritional Support, Gastric Residual Volume and Nutritional Status during Enteral Nutrition in Intensive Care Unit Patients)

  • Purpose: The purpose of this study was to investigate the nutritional support, gastric residual volume, and nutritional status of the intensive care unit (ICU) patients on enteral feeding. Methods: A descriptive longitudinal design was used to collect 5 day data on enteral nutrition of 52 ICU patients in an university hospital. Nutritional support was calculated with actual caloric intake compared to individual caloric requirement. Residual volumes were measured prior to routine feedings, and the serum albumin levels and the total lymphocyte counts were checked to evaluate nutritional status. The data were analyzed using one group repeated measures ANOVA, paired t-test, and Spearman's bivariate correlation analysis. Results: The subjects received their first enteral feeding on the $5.75^{th}$ day of ICU admission. The mean nutritional support rate was 49.1% of the requirement, however prescription rate and support rate were increased as time goes by. Gastric residual volumes were less than 10 cc in 95% cases. A significant negative correlation was found between nutritional support and nutritional status. Conclusion: The nutritional support for ICU patient was low compared to the requirement, and their nutritional status was worse than at the time of ICU admission. Further studies are necessary to develop nursing interventions for improving nutritional support for ICU patients.

중환자실 가족면회 시간 연장의 효과 (The Effects of Extended Family Visiting Hours in the Intensive Care Unit)

  • Purpose: This study aimed to examine the effects of extended family visiting hours in the intensive care unit (ICU). Methods: The subjects were 168 ICU patients and their family members. Two 30-minute visits a day were allowed to the control group according to current policy, while four 30-minute visits a day were allowed to the experimental group. Patients' state anxiety was measured at the first day of ICU admission, and on the third day of ICU admission patients' anxiety and family satisfaction were measured. For the infection rate, comparison was made between the experimental and control data-collecting periods. Results: The patients' state anxiety significantly decreased in the experimental group. Family satisfaction of experimental group was significantly higher than that of control group. There was no significant difference in the infection rate. Nurses positively evaluated extension of visiting hours because it could stabilize patients, reduce the number of arrangements for additional visits, and help establish trust relationship with families. Conclusion: Extended family visiting hours in the ICU reduced patients' anxiety and improved family satisfaction but had no effect on the infection rate. Extended family visiting hours in the ICU is expected to improve the quality of critical care.
