The Effectiveness of Upper Limb Offload Dynamic Taping Technique on Scapular Muscles Activation During Elevation in Healthy Subjects (상지 부하감소 다이나믹 테이핑 기법이 정상인의 어깨 올림 시 어깨뼈 주위근의 근활성도에 미치는 영향)

  • Background: For performing various movements well, cooperation between the muscles around the scapula and shoulder has been emphasized. Taping has been widely used clinically as a helpful adjunct to other physiotherapy methods for shoulder pathology and dysfunction treatment. Previous studies have evaluated the effect of taping techniques using dynamic tapes on shoulder function and pain. However, no study investigated the electromyographic (EMG) changes in the shoulder muscles. Objects: This study aimed to investigate the effect of the upper limb offload taping technique using a dynamic tape on EMG activities of the upper trapezius (UT), lower trapezius, serratus anterior (SA), and middle deltoid (MD) muscles during scaption plane elevation. Methods: A total of 26 healthy subjects (19.85 ± 6.40 years, male = 20) volunteered to participate in this study. The subjects were instructed to perform scaption elevation with and without dynamic taping on the shoulder. Shoulder elevation strength tests were performed at 100%, 75%, 50%, and 25%, for the maximal isometric contraction force. Results: There were statistically significant interaction effects between the taping application and shoulder scaption elevation force in EMG activities in the UT (p < 0.05) and MD (p < 0.05). EMG activities in the UT showed significant increases in 50%RVC (reference voluntary contraction, p < 0.05) and 25%RVC (p < 0.01). Furthermore, the EMG activity of the SA significantly increased in 50%RVC (p < 0.01) and 25%RVC (p < 0.01) after dynamic taping. For the MD, the EMG activity level significantly decreased in 100%RVC (p < 0.05). Conclusion: These results indicated that upper limb offload dynamic taping application affects the muscle activities of some shoulder muscles depending on different scaption elevation strength levels. Therefore, we suggest that the upper limb offload dynamic taping can be applied to the shoulders when patients need middle deltoid inhibition or upper trapezius facilitation, such as patients with shoulder impingement syndrome.

The Visual Effects by Variation of the Shoulder Length and Puff Volume in Puff Sleeve Blouse (퍼프슬리브 블라우스의 어깨길이와 퍼프량의 변화(變化)에 따른 시각적(視覺的) 효과(效果))

  • The purpose of this study is to evaluate the differences of visual effects by variations in the shoulder length and puff volume of the puff sleeve blouse. The stimuli are 21 samples: 3 variations of the shoulder length and 7 variations of the puff volume. The data has been obtained from 40 fashion design majors. The data has analyzed by Frequency, Factor Analysis, ANOVA, Scheffe's test and MCA method. The result of the study are as follows. The visual effects by the shoulder length and puff volume are composed of 4 factors : the width of the upper body, the shape of the breast part, the shape of the neck part and the shape of the upper arm. In these factors, the width of the upper body is estimated by most important factor. In view of the visual effects by variation of the shoulder length, the shorter shoulder length goes, the more positive visual image comes in the shape of the breast part. In comparison of the differences of visual effects by the 7 variations of puff volume, the arm looks thin and long when the puff volume goes enough and the breast part has a better visual assesment in case the puff volume goes poor. In the interaction effects between the shoulder length and puff volume, the shoulder length have significant differences in the shape of the breast part, the shape of the neck part and the shape of the upper arm and puff volume has significant difference in the width of the upper body.

Arthroscopic Treatment of Synovial Chondromatosis of the Shoulder Joint with Mini-open Procedure for the Lesions of Biceps Tendon Sheath

  • Synovial chondromatosis is an uncommon condition, and the involvement of the shoulder joint is rare. A 15 year old female patient presented to author's institution for right shoulder pain. We checked the plain radiographs and MRI. And they showed that a diagnosis of synovial chondromatosis in the shoulder, and they also demonstrated that the disease involved the bicipital tendon sheath as well as glenohumeral joint. We removed all loose bodies with total synovectomy by arthroscopic procedure, and a mini-open procedure for the lesions of biceps tendon sheath. Arthroscopic treatment affords excellent visualization of the shoulder joint with less morbidity. However, with current arthroscopic techniques, it is difficult to manage the synovial chondromatosis of biceps tendon in bicipital groove. The authors suggest that the complete elimination of synovial chondromatosis involving shoulder requires a mini-open procedure for the lesions of biceps tendon sheath in addition to the arthroscopic resection of the affected synovium and loose body removal in the glenohumeral joint.

Deltoid Middle Fiber of the Isometric Contraction According to Elastic Band and Dumbbell on Various Shoulder Abduction Angle (탄력밴드와 아령에 의한 저항이 다양한 어깨관절 외전 각도에서 중간 어깨 세모근의 등척성 수축에 미치는 영향)

  • PURPOSE: The purpose of this study was to compare isometric contraction of deltoid middle fiber between elastic band and light dumbbell on various shoulder abduction angle. And this study intends to examine the compare with isometric contraction on deltoid middle fiber in various abduction angle of shoulder joint. METHODS: The participants of this study were Korean healthy adult in their 20s (n=60). The 8 channel surface electromyography was used to measure activity of the deltoid middle fiber at isomeric contraction on various abduction angle: $0^{\circ}$, $30^{\circ}$, $60^{\circ}$, $90^{\circ}$, $120^{\circ}$ of shoulder joint using elastic band: red color Theraband and dumbbell: 2kg. The data used in the analysis of the present study was the root mean square. RESULTS: The deltoid middle fiber activation was not significantly different between elastic band and dumbbell at each abduction angle. The deltoid middle fiber activation significantly increased as more to increased shoulder abduction angle in elastic band and dumbbell. CONCLUSION: The elastic band and dumbbell was to provide the similarly load to the deltoid middle fiber isometric contraction on all of the shoulder abduction angle. The deltoid middle fiber activation increased as more to increased shoulder abduction angle in elastic band and dumbbell. These results may be an important reference in development of exercise on shoulder joint.

Clinical Study of the Effects of Juglandis Semen Pharmacopuncture Therapy on Chronic Shoulder Pain (호도약침이 만성견비통에 미치는 임상적 연구)

  • Objectives : The purpose of this study is to examine the effect of Juglandis Semen Herbal Acupuncture Therapy on Chronic Shoulder Pain. Methods : Clinical studies on shoulder pain were carried out on 34 patients who were treated at Department of Acupuncture & Moxibusition, Samse Oriental Hospital from June 1, 2009 to October 15, 2009. Subjects were divided into two groups: Experimental group(Group A, n=17) and Control group(Group B, n=17). Group A were treated by common acupuncture therapy, cupping therapy and Juglandis Semen Pharmacopuncture Therapy and Group B were treated by common acupuncture therapy & cupping therapy. All patients in both groups were treated three times a week over 3 weeks. For evaluating change of pain, we used Shoulder Pain and Disability Index(SPADI), Visual Analogue Scale(VAS), Degree of feeling inconvenient in everyday life and Degree of subjective symptoms of shoulder pain. Evaluations were made at baseline and after one week, two weeks and three weeks of treatment. Results : Both group showed chronic shoulder pain decrease after 3 weeks of treatment. But Group A showed better effect on decreasing SPADI, VAS, Degree of feeling inconvenient in everyday life and Degree of subjective symptoms of chronic shoulder pain than Group B. Conclusions : According to the above-mentioned results, it seems that Juglandis Semen Pharmacopuncture Therapy could be applied as the effective method for reducing shoulder pain.

The Effects of Kinesiology Taping and Pectoralis Minor Self-Stretching on Posture Change and muscle tone in Adults with Rounded Shoulder Posture (키네시오 테이핑과 작은가슴근 자가스트레칭이 둥근어깨자세를 가진 성인의 자세변화와 근긴장도에 미치는 영향)

  • PURPOSE: This study examined the effects of Kinesiology taping followed by pectoralis minor muscle self-stretching on the rounded shoulder posture, neck range of motion, and upper trapezius muscle tone in subjects with a rounded shoulder posture. METHODS: Thirty adults with a rounded shoulder posture were assigned randomly to one of two groups, either one that only performed pectoralis minor muscle self-stretching or a group that performed pectoralis minor muscle self-stretching after applying Kinesiology taping ; the subjects underwent four weeks of intervention. Three rounds of intervention were carried out over four weeks, and before and after the experiment, rounded shoulder posture, neck range of motion, and upper trapezius muscle tone creep were measured. RESULTS: Following the interventions, both the experimental and control groups showed significant improvement in the rounded shoulder posture, neck range of motion, upper trapezius muscle tone, stiffness, relaxation, and creep. Significant differences in the post training gains in the rounded shoulder posture and neck range of motion were observed between the experimental and control groups. CONCLUSION: These results showed that a combination of Kinesiology taping and pectoralis minor muscle self-s tretching led to more significant improvement than that seen when only utilizing the application of pectoralis minor muscle self-stretching to change the posture of the subjects with a rounded shoulder posture.

The Effect of Preoperative Interscalene Block Using Low-Dose Mepivacaine on the Postoperative Pain after Shoulder Arthroscopic Surgery (어깨 관절경 수술에서 저용량 Mepivacaine을 이용한 술전 사각근간 차단이 수술 후 진통에 미치는 효과)

  • Background: Shoulder arthroscopic surgery is frequently associated with severe postoperative pain, which can be difficult to manage without the use of high-dose opioids. Although an interscalene brachial plexus block (ISBPB) can be used to provide anesthesia for shoulder arthroscopic surgery, its effect using low-dose mepivacaine on postoperative pain management has not been reported. We hypothesized that ISBPB using a low-dose mepivacaine can provide effective postoperative analgesia for shoulder arthroscopic surgery without the need for high-dose opioids and act as a significant motor or sensory block. Methods: This study examined a total of 40 patients, who underwent shoulder arthroscopic surgery, and received ISBPB with 10 ml of normal saline (group NS; n = 20) or 10 ml of 1% mepivacaine with epinephrine 1:200,000 (group MC; n = 20). The block was performed preoperatively. The postoperative pain score, opioid consumption, and side effect were recorded. Results: The visual analog scale scores were significantly lower in group MC than in group NS at 120 minutes after shoulder arthroscopic surgery ($1.9{\pm}1.0$ versus $4.0{\pm}1.4$). Group MC showed significantly lower fentanyl consumption after shoulder arthroscoic surgery than group NS ($27{\pm}32.6$ versus $79{\pm}18.9{\mu}g$). The degree of motor and sensory block after surgery was minimal. Conclusions: ISBPB using low-dose mepivacaine reduced the level of postoperative pain and fentanyl consumption without significant side effects. ISBPB using low-dose mepivacaine is a useful analgesic technique for shoulder arthroscopic surgery.

A Changes of Electromyogram Activities in Deltoideus and Supraspinatus During Shoulder Abduction Using TheraBand (TheraBand를 이용한 견관절 외전시 삼각근과 극상근의 근전도 변화)

  • The purpose of this study was to compare the changes of muscle activities in deltoid with those in supraspinatus using electromyography(EMG) while subjects abducted their shoulder to different angles with different resistance. Methods : 20 volunteers who were comprised of 10 males(mean age: $21.2{\pm}2.0$) and 10 females(mean age: $20.2{\pm}1.6$) were collected. Surface electrodes were attached on deltoid and supraspinatus muscle for recording. Each reference electrode was located 3 cm to the each recording electrode. Muscle action potentials were recorded with changing the angle of shoulder abduction, $30^{\circ}$, $60^{\circ}$, $90^{\circ}$. This procedure was repeated with different resistance 0 pound, 2 pounds, 4.5pounds. The angle of shoulder abduction was determined by clinical goniometer. SPSS(Statistical Program for Social Science)/WIN 10.0 was used for statistics. Analysis included ANOVA, T-test. Results : The following results were obtained in this study. 1. There was significant differences during isometric shoulder abduction $30^{\circ}$, $60^{\circ}$, $90^{\circ}$ comparing muscle activity in deltoid and supraspinatus muscles at 0 pound, 2pound, 4.5pound resistance(p<0.05). 2. There was no useful significant in the deltoid and supraspinatus muscles activity EMG compared by physical condition. 3. Their was significant in the deltoid EMG compared by gender t=-5.41, P<0.01, but no difference in the supraspinatus EMG(P=0.333 >a=0.05). Conclusions: There are many previous studies on influence of shoulder angles and speeds of muscle activity. However most of them placed the focus on isotonic or isokinetic exercise, or shoulder function. But this study was done during isometric exercise, which is better for early assessment and treatment for injured patients. There is strong relationship between the shoulder angle with resistance in deltoid supraspinatus muscle activity. We conclude that it is important to apply isometric shoulder abduction exercise with correct angle and resistance especially in early stage.

A Study on the Princess Line by Body Types (Part I) - Focused on Body Types of A & H -

  • The main purpose is to study lines on different body types and to disguise any imperfections by using diverse princess lines. We separated festers body shapes into specific body types, A, H by applying both the direct and the indirect measurements. Following are the results of the study: 1. nose are the resulting illusion effects when the shoulder width of the princess line was fixed. In case of the armhole princess line, the illusion that the waist intervals were narrow or wide in A-line silhouette. In case the shoulder princess line, when the waist interval was narrow, it appeared to show narrow shoulder in A-line silhouette. While the wide waist interval shows the wide shoulder width in H-line silhouette. It regards body type H in the same light as H-line silhouette when the waist interval is narrow, it appeared to show narrow shoulder width in A-line silhouette. When the waist interval was wide, it appeared to give the narrow shoulder width. 2. When the princess lines waist width was fixed in order to study illusion effects of waist widths. Generally, for body type A, Whether the waist interval princess line is narrow or wide, the princess line is located comparatively wide, in A-line silhouette appeared to show relatively slender waist. For body type H, whether the waist interval is narrow or wide, the princess line us situated comparatively narrow interval, H-line silhouette appeared to show sum waist but the differences are not significant 3. The illusion of the hip were studied by fixating the width of the skirt and varying the locations of princess line and waist widths. For the location of both armhole and shoulder princess line in H and A-line silhouette, the narrow shoulder width and the waist appeared to show narrow hips. mile they are wide appeared to show wide hips. 4. With both waist and skirt width fuel, all two body types showed taller and slender postures when the princess line originated from the shoulder compare to the armhole.

Arm Armor System Performance Study: Net Effect (Perceptual Response) Analysis

  • This study compares the net effect of wearing different shoulder/arm armor systems on garment impediment perception and wearer acceptability. Two independent variables in this study were armor systems and shoulder/ arm movements. There were four armor systems of control garment and arm armor systems A, B, and C as well as five types of arm/shoulder movements, (shoulder flexion, should extension, shoulder abduction, shoulder horizontal flexion, and shoulder horizontal extension). Ten male volunteers wearing size medium battle dress uniform (BDU) with recent relevant military experience participated in this study. The volunteers performed shoulder/arm movements (while wearing each armor treatments) and completed the garment impediment perception as well as wearer acceptability scales. The body areas of neck side, shoulder top, and armscye front showed the highest frequency of reported impediments. Resistance to movement and localized pressure were the most frequently mentioned types of impediment. The armor system B had the most areas of impediment, and was rated as more restrictive than the control garment and armor system A for each movement. For wearer acceptability, no significant differences were found between the control garment and armor system A for all eight items; this indicated that subjects did not perceive a difference between wearing the control garment and armor system A. There was a trend for wearer acceptability to decrease from wearing the control garment to armor systems A to C to B.
