Effect of Pain and Shoulder Joint Function on Extracorporeal Shock Wave Therapy of Patients with Frozen Shoulder (체외충격파 치료가 동결견 환자의 통증과 견관절 기능에 미치는 영향)

  • Purpose: The purpose of this study was to assess the effectiveness of Extracorporea Shock - Wave Therapy (ESWT) on the pain and improvement of shoulder joint function which is one of the available medical treatment in patients with frozen shoulder. Object and Methods: 26 patients with frozen shoulder, with 26 patients assigned to two groups; a control treatment group (n=12) and a ESWT application group (n=14). In the ESWT application group, the patients received general physical therapy, and then they received ESWT. In the control treatment group, the patients received only general physical therapy. These medical treatments was conducted total 6 time for 3 weeks. To investigate the effectiveness of the treatment after the each group's treatment. Pain was measured by Visual Analogue Scale (VAS) which is divided into 10 ranks and shoulder joint function was measured by CSA(Constant Shoulder Assessment scale). Results: 1. There was a statistical significance of measured by shoulder pain degree in both group but, the result of a ESWT application group shows more significance than that of a control treatment group. 2. There was a statistical significance of measured by CSA in both group but, the result of a ESWT application group shows more significance than that of a control treatment group. Conclusion: We think that treatment with General physical therapy and ESWT together for the patients with frozen shoulder is more effective and more safe method to shoulder joint function improvement and decrease of pain.

Biceps Load Test: A Test of SLAP lesion in the Recurrent Anterior Dislocation of the Shoulder (이두건 부하 검사(Biceps Load Test): 견관절 재발성 전방 탈구시 SLAP 병변 진단의 새로운 검사방법)

  • The following will describe a method of evaluating the SLAP lesion in the recurrent anterior dislocation of the shoulder. We have named it the biceps load test. The biceps load test is performed with the patient in the supine position and the arm to be examined is abducted 90/sup°/, and the forearm is in the supinated position. First, the anterior apprehension test is performed. When the patient become apprehensive, the patient is allowed active flexion of the elbow, while the examiner resists elbow flexion. If the apprehension is relieved or diminished, the test is negative. If aggravated or unchanged, the test is positive. A prospective study was performed, in which 75 patients who were diagnosed as having recurrent unilateral anterior instability of the shoulder underwent the biceps load test and arthroscopic examination. The biceps load test showed negative results in 64 of these patients, of which the superior labral-biceps complex was intact'in 63 cases and only I shoulder revealed a type n SLAP lesion. E]even patients with a positive test were confirmed to have type n SLAP lesions. A positive biceps load test represents an unstable SLAP lesion in a patient with recurrent anterior dislocation of the shoulder. The biceps load test is a reliable test for evaluating the SLAP lesion in the recurrent anterior dislocation of the shoulder(sensitivity: ,9] .7%, specificity: 100%, positive predictive value: 1.00 and negative predictive value: 0.98). Biceps contraction increases the torsional rigidity ?of the glenohumeral joint and long head of biceps tendan act as internal rotator of the shoulder in the abducted and externally rotated position. These stabilize the shoulder in abduction and external rotation position in the biceps load test.

  • Shoulder joint is the most movable joint in human body with, at least, three degrees of freedom, since there are at least three bones and five joints involved in shoulder movement. Due to the complexity of the shoulder joint and the lack of appropriate anatomical data, modeling of the shoulder joint has been known to be extremely difficult. In many biomechanical models being used, shoulder joint is considered as a fixed point and it is also assumed that the shoulder joint does not noticeably move during the shoulder movement. However, such an assumption is not valid in real applications and causes inaccuracy, especially, in the area of workspace evaluation. The reachable area generated by a human becomes somewhat different from that of current models for those models fail to appropriately reflect the movement of shoulder joint's center of rotation. In this study, the location of the shoulder joint's center of rotation was obtained in relation to the location of humerus, on which a new model for reach envelope generation was developed for workspace evaluation. From the experiments conducted for three subjects, the initial location of the center of rotation was determined for each subject and subsequent changes in the instantaneous center of rotation were drawn as a function of flexion and abduction of the shoulder. Based on the regression analysis, the study suggested a new method for the generation of reach envelope. Comparisons were also made among real reach envelopes obtained from the experiment, the ones from the model, and the ones from the new method suggested in the study. As a result, the prediction errors incurred from the new method were significantly reduced when compared to the ones from the current approach.

The Effect of Self Thoracic Mobilization Exercise on Shoulder Pain and Function (자가 등 가동운동이 어깨 통증 및 기능에 미치는 효과)

  • This study evaluated the effect of self thoracic mobilization exercise (STME) on shoulder pain, disability and shoulder function in patients with shoulder dysfunction. Thirty-two patients with shoulder dysfunction were randomly assigned to the STME group and the control group. The visual analog scale, shoulder dysfunction, and shoulder range of motion were evaluated before and after intervention. Sixteen STME intervention patients used a foam roller and quadripedal position flexion and extension. Sixteen controls received placebo effects using electrotherapy pads. The two groups performed 20 minutes for three times per week for four weeks. The STME group showed significant improvement in pain and dysfunction compared to that of the control group (p <0.05), and their range of motion of the shoulder was significantly improved in flextion, abduction, and external rotation compared to that of the placebo group (p <0.05). Therefore, STME is effective in resolving shoulder pain and dysfunction, and will be able to educate patients with shoulder pain in an easier approach and mediate social treatment costs.

Arthroscopic Bankart Repair in Traumatic Anterior Shoulder Instability with Bio-knotless Anchor (Preliminary and Technical Report) (견관절 외상성 전방 불안정성에 대한 Bio-knotless 봉합 나사못을 이용한 관절경적 Bankart 병변 봉합술 (예비 보고))

  • Purpose: This study reports the clinical results of the arthroscopic Bankart repair in traumatic anterior instability of the shoulder with bio-knotless anchor. Materials and Methods: 21 cases of 21 patients (20 male and 1 female) were included in this study. The average age was 24.8 years old and the period from the first injury to operation was average 37.2 months. All cases had Bankart lesion and 12 cases had Hill-Sachs' lesion. The SLAP lesion was associated in 6 cases. Preoperative Rowe score was average 29.1. Arthroscopic Bankart repair with bio-knotless anchor were performed in all cases; 3 anchors at 3, 4, 5 O'clock position of the glenoid were used in 11 cases and 2 anchors at 4, 5 O'clock position were used in 10 cases. All the associated SLAP lesions were repaired arthroscopically with bio-knotless anchor. Thermal capsular shrinkage at the anterior and inferior shoulder capsule after the Bankart repair was performed in 3 cases. The average follow up period was 20.2 months. Results: The Rowe score improved to 92.8, excellent in 17 cases and good in 4 cases, at last follow up period and 20 cases had full range of motion of the shoulder. 1 case had mild limited range of motion of the shoulder (150 degrees in flexion, 60 degrees in external rotation and T12 level in internal rotation) without any problem in normal activity. The arthroscopic revision surgery of the shoulder was performed in 1 case because of multiple traumatic injuries of the shoulder with pain postoperatively. Conclusion: Arthroscopic Bankart repair with bio-knotless anchor in traumatic anterior shoulder instability is one of the good methods because of the good clinical results.

The Incidence of Venous Thromboembolism Following Shoulder Surgery: A Pilot Study

  • Background: To assess the incidence of venous thromboembolism (VTE) following shoulder surgery and to evaluate the role of postoperative duplex ultrasonography. Methods: The study comprised a total of 224 patients who underwent shoulder surgery, including 180 shoulder arthroscopic surgeries, 28 shoulder arthroplasties, and 16 plate fixations for proximal humerus fracture between January 2014 and December 2014. The mean age of patients was 59.0 years, and there were 81 men and 143 women. Clinical data, including body mass index, blood tests, metabolic work-up for liver and renal function, previous, and present medical history, were evaluated. Duplex ultrasonography in the operative arm was performed on 2 to 4 days after surgery. Results: The overall incidence of VTE following shoulder surgery was 0.45% (1/224). One patient with open reduction and plate fixation for proximal humerus fracture had asymptomatic deep vein thrombosis that showed complete remission after anticoagulant medication during a 2-month period. Four patients had asymptomatic superficial cephalic vein thrombosis and complete remission without any treatment. There was no case of pulmonary embolism. Conclusions: The incidence of VTE following shoulder surgery was extremely low in Asians. Duplex ultrasonography may be not considered a routine follow-up of shoulder surgery and can be selectively performed in high-risk or symptomatic patients for VTE.

The Visual Evaluation According to the Changes in the Shoulder Strap and length of Bikini Swimsuits - Focused on the Undergraduate Students in Busan - (비키니 수영복의 어깨 끈과 길이 변화에 따른 시각적 평가 - 부산지역 대학생을 중심으로 -)

  • The purpose of this study is to evaluate the differences of the visual image on variations in the shoulder strap and the length of the bikini swimsuit. Nine samples were examined: 3 variations of the shoulder strap and 3 variations of the pants length. Data have been obtained from 90 fashion design majors and analyzed using Factor Analysis, Anova, Scheffe's Test and the MCA method. The results of this study are as follows: 1) The visual image, according to changes in the shoulder strap and length of the bikini swimsuit, was composed of boldness, matureness and attraction factors. Boldness was the most important factor in the bikini swimsuit. 2) The visual images according to changes in the shoulder strap of the bikini swimsuits appeared the most (1) plain and simple image, (2) decent and neat image, (3) a wanted-not-to-dress and a natural image in the two shoulder straps, (1) unique and complicated image, (2) a lively and sexy image in the one shoulder strap and an unnatural but a wanted-to-dress image in the strapless. 3) The visual images according to changes in length of the bikini swimsuits appeared the most (1) unique and complicated image, (2) a lively and sexy image in high cut, but plain and simple image in low cut. 4) The number of shoulder straps and length do interact with each other in boldness factor: One shoulder strap and high cut of the bikini swimsuit has the most unique and complicated image. However two shoulder straps and regular cut of that has the most plain and simple image. 5) The result of matureness and attraction factors using the MCA, length affects more than the number of shoulder straps in the visual images of the bikini swimsuit.

The Convergence Effects on the Grip Strength in Change of Shoulder Angle on Horizontal Plane (수평면에서 어깨각도변화에 따른 여대생의 악력에 대한 융복합적 연구)

  • The Purpose of this study is analysis characteristic of grasping power with each different shoulder horizontal angle. Twenty female university students with no shoulder dysfuction participated subject in three different positions of shoulder horizontal flexion with standing posture, shoulder 0° flexion, elbow 90° flexion, forearm 90° pronation, different positions is followed : shoulder 0° horizontal adduction shoulder 40° horizontal adduction, shoulder 40° horizontal abduction. The One-way repeated ANOVA test was used to determine the different in grip strength on three shoulder horizontal positions. On the average, in the hand grip strength, the horizontal neutral position is higher than horizontal adduction position with significant value. In particular, shoulder horizontal aduction was measured lowest grip strength between three positions.

Effects of static stretching and hold-relax on recovering range of motion and reducing pain of patients with frozen shoulder (오십견 환자의 관절 가동 범위 회복과 통증 완화에 미치는 정적 신장 기법과 유지-이완 기법의 효과)

  • Background : Patients with frozen shoulder have pain and limited range of motion (ROM) at the affected shoulder. Shoulder pain of these patients usually decreases with recovery of range of motion. The aim of this study is to identify effects of static stretching and hold-relax on recovering ROM and reducing pain of patients with frozen shoulder. Methods : In this study, 15 patients with frozen shoulder who were only treated with physical therapy were recruited. Patients who received any surgical procedure or hydrotherapy were excluded. The subjects were divided randomly into 3 groups; control(n=5), static stretching(n=5) and hold-relax(n=5) groups. Patients were treated with stretching and the total number of treatments was 12 times. The ROM was measured at baseline, after fourth, eighth and final treatments. Shoulder pain and disability index(SPADI) scores were assessed before and after doing all treatments. Repeated measures ANOVA was used to analyze ROM change within each group. Changes of ROM between groups was analyzed by one way ANOVA followed by post hot analysis(Tukey's HSD). SPADI score within each group analyzed using Wilcoxon signed ranks test. A p-value<0.05 was considered statistically significant. Study Design : Repeated measures design. Result : There were significant differences of ROM in both treatment groups except internal rotation of static stretching group. ROM was significantly increased in treatment groups compared with control group, but the difference of ROM change between two treatment groups did not differ significantly. SPADI scores for treatment groups significantly decreased compared with pre-SPADI score. Conclusion : The result suggests that hold-relax can be more effective than static stretching to recover range of motion and alleviate shoulder pain on patients with frozen shoulder.

Shoulder blade body and the acromion and coracoid process Y-view is formed characters. $10^{\circ}$ angle between the shoulder blades and the acromion is the obstruction. $20^{\circ}{\sim}25^{\circ}$ to the Y-view of the humerus head looks superimposed, the position of the shoulder joint and seemed appropriate. $35^{\circ}$ of the humerus head superimposed on the Y-view, but above the humerus head and shoulder joints were distorted. When $45^{\circ}$ elevation observed on the side of the best has come Y-view also acromion and coracoid process is finished.
