Reoccurred Multiple Basal Cell Carcinomas: A Case Report (전신에 새로이 발생한 다발성 기저세포암 1예)

  • Basal cell carcinoma (BCC) is the most common skin cancer. Ultraviolet radiation exposure and genetic predisposition are known to be the most important etiological factors. Multiple BCC is often associated with genetic familial conditions such as BCC syndrome, basal cell nevus syndrome. We present a case of 54-year-old female who had multiple BCC that had reoccurred. She was completely cured after receiving radio-chemotherapy for leukemia 16 years ago. She had multiple lesions (scalp, left thigh, right popliteal fossa, and right buttock), and had underwent wide excisions of all lesions. All biopsies revealed BCC. Six years later, she had also multiple lesions; left forehead, frontal vertex scalp, parietal vertex scalp, right occipital scalp, and lower abdomen. We performed wide excision. Histopathological examination revealed BCC. She had no signs of any BCC associated syndrome. We report a rare case of nonsyndromic multiple BCC that reoccurred at the new site.

Output Voltage Control in a Serise Multilevel H-bridge Inverter with SHE-PWM Method (직렬 멀티레벨 H-bridge inverter에서 SHE-PWM방식을 사용한 출력 전 압의 제어)

  • This paper proposes a method of voltage control for three-phase multilevel H-bridge inverters with selective harmonic elimination (SHE) PWM The full-bridge configuration of H-bridge inverter cells enables voltage control with a fixed PWM pattern by means of phase shifting between the legs, which greatly simplifies the control while maintaining the harmonic elimination characteristics. The series combination of the cells in multilevel configuration can be exploited to further improve the hormonic elimination characteristics with proper phase shifting between the ceil volitage. A complexor-based control method is introduced to control the magnitude and phase angle of cell voltages that form three-phase multilevel output voltages. Simulation results show that the proposed method along with SHE PWM would provide satisfactory performance in spite of its simplicity.

A study on the constructive principle and Techniques of Vionnet's works (Vionnet 작품의 구성원리와 기법에 대한 연구)

  • The purpose of this study was to identify the constructive principle and techniques of Vionnet's works. According to analysis of Vionnets works, it was found that Vionnet conceived of the human body as a continuous shaping of a cylinder. Vionnet's drapery denied the planner presumption of woman and gave real three-dimension to form and to fashion. The techniques of Vionnet's works were as fallow : (1) She introduced the diogonal "bias cut" and changed dress design by her sensuous use of fabrics. (2) She eliminated interfacing to keep the silhouette and the fabric soft. (3) She manipulated the fabric to remove the excess for fit and decoration at the same time. (4) She treated the fabric with a synthesis of geometry and grace. (5) The sale luxury allowed was to note the choice of seams, facings, hems, and stitches. (6) Vionnet's dresses were not lined.

Recurrent Herpes Zoster on the Supraorbital Counter Area -A case report- (반대편 눈확위신경 영역에 재발된 대상포진 1예)

  • An 85-year old female patient visited our pain clinic because of pin pricking pain and allodynia on the left forehead area for 2 days. Vesicular eruptions were seen along the left supraorbital nerve distribution. She experienced similar pain and eruptions on the contralateral forehead, the supraorbital counter area, 8 years previous. She had been taking antihypertensive medications for 15 years. She also had suffered from diabetes mellitus. She received a total hysterectomy and anterior posterior colporrhapy due to procidentia uteri and severe cystocele and rectocele. She had been treated intermittently for back pain due to advanced osteoarthritis and spondylosis. She was treated with famciclovir and triamcinolone acetonide with daily stellate ganglion block and supraorbital nerve block. Nortriptyline (a tricyclic antidepressant) and midazolam was prescribed to relieve pain and difficulty in sleeping. After 3 days, all treatment was ended because it was impossible to assess the severity of pain due to the senile psychosis of the patient. She eventually expired after 2 months. We report this case because it is rare for herpes zoster to recur, and particularly on the contralateral counter area.

A Case Report of a Guillain-Barre Syndrome Patient Suspected of Having Acute Transverse Myelitis Treated with Korean Medicine (급성 횡단성 척수염으로 의심된 길랑바레증후군 환자에 대한 한방 치료 증례보고 1례)

  • A 67-year-old female patient diagnosed with acute transverse myelitis visited our hospital in February 2023. Consultation papers indicated that she was diagnosed with acute transverse myelitis and that she complained of quadriparesis and numbness in both hands and feet. However, her spine magnetic resonance imaging (MRI) did not clearly indicate that she suffered from acute transverse myelitis. She was then sent to a high-level hospital and diagnosed with Guillain-Barre Syndrome through electromyography, cerebrospinal fluid test, and spine MRI. On March 2023, she visited our hospital again, still complaining about weakness in both hands and inability to walk. She was treated with Korean medicine, including acupuncture, electroacupuncture, pharmacopuncture, and herbal medicine such as Yanggyeoksanhwatang and Sipimijihwang-tanggami. After three months, she was discharged on cane walking. The manual muscle test (MMT) grade of her upper limbs increased from 3-/3-/3-/3- (shoulder/elbow/wrist/fingers) to 4/4/4/3+, and the MMT grade of her lower limbs increased from 2/2/2/2 (pelvis/knee/ankle/feet) to 4/4/4-/4-. Her numbness in both feet disappeared, and her numbness in both hands decreased from a numeric rating scale of 7 to 2. No adverse events were reported during treatment. This case implies the therapeutic potential of Korean medicine for Guillain-Barre Syndrome, especially for those who had delayed diagnosis and missed the opportunity to get plasma exchange.

  • The purpose of this study is to investigate Modernism of Gabrielle Chanel in respect to her fashion business. Chanel always considered the work of a fashion designer 'a technique, a craft, a trade' and chastised couturiers who looked upon themselves as artists. She was the first to cater to the public in its broader sense and to produce standard which appealed to every taste, the first to democratize the art of dressmaking for purely economic reasons. The results were as follows; 1. Chanel personified ~his new spirit of independence and evolved a style of dress for the modern, liberated woman. 2. Chanel excelled at fabrics, their interpretation, and an ability to use them. She had taken a humble material, one that was used by men and that they had shunned, and turned it into a fashionable fabric. In the process she also accelerated the growth of the ready-to-wear industry for it was a fabric within the financial reach of the majority of woman who wanted to dress fashionably but were not well off. 3. Chanel thought black chic and would never go out of fashion. As American Vogue for 1 October 1926 prognosticated, her little black dress became a kind of uniform. 4. Chanel represented an exception among couturiers because she was flattered that her styles were so popular and widely copied. She believed that her style would be affirmed by high-street copies-after all, copying is the sincerest form of flattery. 5. Chanel changed the concept of costume jewellery. It was not an original idea of Chanel's. Whereas Poiret had pioneered the original idea of costume jewellery, it was Chanel's avant-garde way with it not to mention her usual deceptive simplicity and supreme artistry that made costume jewellery evolve a successful and lucrative part of the fashion industry.

A Case Report about Pregnancy and Delivery with Primary Amenorrhea by Oriental Medicine in Oocyte Donation (원발성 무월경 환자의 난자공여 시술시 한약치료를 병행한 임신 및 출산 1례)

  • Objectives: The aim of this case is to report the effects of oriental medicine on one patient with primary amenorrhea for endometrial preparation and implantation. Methods: A patient with primary amenorrhea had symptoms of sleep disorder, diarrhea, colpoxerosis. For preparing endometrium and implantation in oocyte donation after one previous failure, she was treated by twice a day herb medication for 75 days. And we observed the effects of treatments by improvement of symptoms and following up endometrial proliferation ultrasonography. After implantation, for maintaining pregnancy and live birth, she was also treated by twice a day herb medication for 45 days. Results: After treatments, Symptoms of sleep disorder, diarrhea, colpoxerosis were improved and the thickness of endometrium was prepared for implantation in oocyte donation. So she was pregnant and gave birth to a healthy baby 36 weeks later. Conclusions: This case shows that oriental medicine has its effective implementation for the implantational surroundings on patients with primary amenorrhea in oocyte donation programs.

A Case Study of Sandplay Therapy with a Middle-Aged Woman Having Difficulty in Relationships (관계에 어려움이 있는 중년기 여성의 모래놀이치료 사례연구)

  • This study explored the sandplay therapy case of a middle-aged woman having difficulty with interpersonal relationships and wanting to increase her self-identity. The goal of the therapy was to change her relationships with others by self-examination and becoming conscious of her negative animus and femininity in a free and protected space provided by sandplay therapy. Forty-five therapy sessions were held. The client exhibited her lonesomeness and fantasies by making sandtrays of a house she would like to live in, a park she would like to relax at and dreams she would like to fulfill in the initial phase of therapy (1~8, Who am I?). In the intermediate phase of therapy (9~33, Meeting myself), she displayed scenes of her negative self and breaking away from her negative self by making sandtrays of shadows, deaths, creations, a soaring scene and a disappearing alligator. In the final phase of therapy (34~45, Discovery of the real Self), she showed scenes of her meeting a positive self by making sandtrays of hope, coexistence, start, harmony and community. This study showed the effectiveness of sandplay therapy since the client' relationships with others were improved through her self-awareness by sandplay therapy in the free and protected space.

The Principles of Clothing Design and the Way of Design Approach in Lamanova (라마노바의 의상디자인 이론과 접근 방식)

  • The purpose of this study is to examine the principles of clothing design and the way of design approach in Lamanova. She caused a revolution in the art of dress and created the foundation for costume design theory. She Addressed Art in everyday life to self-taught dressmakers. The distinguishing feature of the design is simplicity and functionality. In an article "The Russian Style", Lamanova raised the question of the new Soviet costume and traditional costume. In "On Contemporary Costume" she classified the new forms of clothes into everyday and holiday attire, showed the principles of costume, and analyzed in detail the necessity of constructing a costume to suit the individual figure. In "On the Rationality of Costume", she found a basis theory for clothing design which is for whom, from what, and for what purpose. Lamanova's theory was made public in full in 1928 and was presented at the exhibition of Handmade Textiles and Embroidery in Women's Contemporary Costume. The main contents are the costume's purpose, it's material, the figure of its wearer, and its form. She argued that the new costume could be in line with the new life and her theory could be the creed of clothing designers. The principles of Lamanova's theory can still be applied today, not only in Russian traditional and contemporary costume, but in contemporary world fashion.

A Research of Madeleine Vionnet's Work II (Madeleine Vionnet의 작품에 관한 연구II -연구대상 작품의 제작되어진 패턴을 중심으로-)

  • Madeleine Vionnet, a representative designer of the 1920′s, who made prefectly organized clothes with unique formativeness was the first to express fabric in a modern method and was called "The architect of fashion". This is because she developed a three dimensional design by expressing cloth-she rejected corset, should pad, etc-into elegant curved lines that do not stick to the body. By granting meaning to the beauty of the body and its movement in her own unique ways, she emphasized the formativeness in her work and gave shape to creative artistry. Expecially with the "Bias-Cut" she could express the lines of the body more flexibly and could make geometrical styles like the diamond shape dress or the triangle dress more effectively. Using the "Tired Bias cut" and "Handkerchief point" she let the skirt hemline dangle irregularly in geometrical forms, thus showing modern formative sense which forms a three dimensional solidness along the movement of the human body. Thus far, analyzed how the contemporary trend of art was reflected in her designs by studying her work; also investigated through her artistic characteristics and pattern method. Also be tried to find out what can be learned through her artistic view and superb formativeness as a designer.
