Purpose: The purpose of this study was to compare depression and quality of life(QOL) according to the severity of insomnia in elderly. Method: The survey participants were 463 community-dwelling adults aged 65 or older. Severity of insomnia was measured by assessing a time-length to get to sleep, the number of breaks in sleep, a time-length to get to sleep after awakening, and a usual sleep pattern. Depression and QOL was measured, respectively, a modified Zung's depression scale and SF-36. The data were collected from May to June in 2005 and analyzed using the SPSS program for descriptive statistics, ${\chi}^2$-test, t-test, and ANOVA with Scheffe test. Results: A majority of the participants(75.6%) presented mild(65.0%) or severe(10.6%) insomnia. Severity of insomnia was different depending on sex, a marriage status, and sleep patterns such as hours of sleep on average, time to go to bed, and time of awakening(p<0.05). Depression and QOL were related to the severity of insomnia(p<0.05). The elderly suffering from insomnia needs nursing interventions considering demographics and sleep patterns in oder to prevent depression and to improve QOL. Future research is in need for predicting and preventing depression and QOL in elderly.
Purpose: Terminal cancer influences on patients as well as their family members. This research was performed to evaluate the quality of lift of primary family caregivers and to investigate the influencing factors. Methods: The results of survey were collected from 81 family caregivers who were taking care of hospitalized terminal cancer patients at an oncology department of university hospital in Daejeon from March 2005 to January 2006 with questionnaires. The questionnaires were consisted of the general characteristics of the subjects, and 36-items short-form Health Survey (SE-36) Korean version to evaluate the quality of life of family caregivers, the characteristics of patients and family caregivers' caring trait. Results: Family caregivers' mean (${\pm}SD$) SF-36 score was 47.9 (${\pm}20.7$). Influential factors on family caregivers' life quality were daily raring hours, economic burden, type of treatment; only supportive care, caring duration, sex, the numbers of ADLs (activity of daily livings) items that patients needed help in order by stepwised multiple logistic regression analysis (overall $R^2=0.639$, P=0.044). Conclusion: Daily raring hours and economic burden were two influential modifiable factors on family caregivers' quality of life. Therefore, social supportive systems are required to reduce family caregivers' daily caring hour and economic burden.

Purpose: We investigated the effect of isometric resistance exercise on the vastus medialis oblique muscle with inelastic tape and EMG biofeedback training applied to the patello-femoral joints of patients with patella malalignment. Methods: The 39 elderly subjects that had patella malalignment but no neuromuscular disorders were divided into a control group, taping group, and EMG biofeedback training group. Evaluations of function improvement performed before and after the treatment, as well as 4 weeks after treatment. Results: Change in pain in the knee joint were significantly different among groups (p<0.05). Maximum voluntary isometric contractility in the quadriceps muscle was significantly in the EMG biofeedback group (p<0.001). The WOMAC (Western Ontairo & McMaster Questionnaire) index showed a significant change (p<0.05) in pain, function, and total score. Taping and EMG biofeedback training showed a lasting effect until measurement 4 weeks after treatment. SF-36 (Medical outcome short form-36), which assesses the quality of life, did not significantly change. Conclusion: In osteoarthritis patients with a loss of patello-femoral joint function, isometric resistance exercise of the vastus medialis oblique muscle with taping seems effective.

Purpose: Transsexualism is considered to be the extreme end of the spectrum of gender identity disorders characterized by, among other things, a pursuit of sex reassignment surgery (SRS). We evaluated psychologic status, health-related quality of life in female-to-male (FTM), male-to-female (MTF) transgender individuals. Methods: We used the Minnesota Multiphasic Personality Inventory, Beck Anxiety Inventory, Beck Depression Inventory, Moudsley obsessive-compulsive Inventory, SCL-90-R, Short-Form 36-Question Health Survey version 2 (SF-36v2). We enrolled 40 transsexual participants. Results: Analysis of quality of life health concepts demonstrated statistically significant (p<0.01) diminished quality of life among the transsexual participants as compared to the Korea male and female population. FTM transgender participants reported more higher hostile, phobic than MTF transgenders. Overall, in all psychologic status examination, Transgender individuals are within normal population boundary. On all category, result is improved post-operatively. Conclusion: Transgender participants reported mental status within normal boundary. SRS improved their quality of life and mental stability.

PURPOSE: This study aimed to evaluate the effects of health-promoting behaviors on the subjective well-being of a physical therapist. METHODS: Survey data were collected from 247 physical therapists in Daejeon city. The baseline for health-promoting behaviors was determined using Health Promoting Lifestyle Profile (HPLP), subjective well-being was determined using Positive Affect and Negative Affect Schedule (PANAS), quality of life was measured using the SF-36 (Short-Form 36), and pain level was determined using the Quadruple Visual Analogue Scale (QVAS). Health-promoting behaviors and related factors were analyzed using an average comparative analysis (t-test), and the factors relevant to subjective well-being and health-promoting behaviors were evaluated using correlation analysis (Pearson's correlation coefficient). Factors affecting health-promoting behaviors were evaluated using stepwise multiple regression analysis (stepwise multiple regression). RESULTS: The mean score for health-promoting behavior was 3.73, with emotional support having the highest score (3.90) and regular meals having the lowest score (3.16 points; p<0.01). Health-promoting behavior was significantly correlated with positive emotions and negative emotions are subjective well-being, negative emotions and pain were noted (p<0.01). Health-promoting behaviors showed a significant correlation with quality of life (r=0.04), positive emotions (r=0.21; p<0.01), negative emotions (r=0.16; p<0.05), and pain level (r=0.016). The results of this study showed that health-promoting behaviors are significant predictors of subjective well-being in physical therapists (p<0.01). CONCLUSION: Physical therapists have to maintain their health by avoiding infections. Regular exercise is the most important factor among all health-promoting behaviors.

Objective : Degenerative changes in the cervical spine are commonly accompanied by cervical kyphosis which can cause neck pain. This study examined the relationship between neck pain and cervical alignment. Methods : A total of 323 female nursing staff from our hospital were enrolled. Sagittal radiographs of the cervical spine, Body Mass Index (BMI), Visual Analogue Scale (VAS) measures of neck and arm pain, Neck Disability Index (NDI) and the Short Form (36) Health Survey (SF-36 scores) were obtained and reviewed retrospectively. Global lordosis (GL) of the cervical spine was measured on radiograph images. Correlations between GL and questionnaire scores were investigated using the following three methods : 1) correlation between GL and questionnaire scores among the entire sample; 2) subgroup analysis of patients with "kyphosis (KYP) : GL scores<0" vs. those with "lordosis (LOR) : GL scores>0" on questionnaire measures; and 3) subgroup analysis of patients with pain vs. those without pain, on GL and questionnaire measures. Results : There was no significant correlation between GL and any questionnaire measure. There was a significant difference between the mean GLs of the KYP and LOR groups, but there were no group differences in BMI, age or any questionnaire measures. There was no difference between the pain (n=92) and pain-free (n=231) groups in age, BMI or GL, but there were differences in neck, and arm pain, and physical function and NDI scores. Conclusions : Our data suggest that kyphotic deformity was not associated with neck pain.
Objective : To test the hypotheses that individualized traditional Korean acupuncture improves pain and disability in patients with osteoarthritis of the knee and that benefits remain after stopping treatment more so than is the case for standardized minimal acupuncture. Design : Randomized single blind controlled trial with two intervention arms (individualized traditional Korean acupuncture, standardized minimal acupuncture) of six weeks' duration and three months follow-up. Setting : Acupuncture interventions were applied by two training doctors in the Department of Acupuncture and Moxibustion in a 1000-bed hospital. Assessment of the result was performed in a university-based laboratory. Participants : 50 patients with symptoms of knee osteoarthritis as diagnosed by an orthopedist. Intervention : Individualized traditional Korean acupuncture or standardized minimal acupuncture for six weeks. Main outcome measures: Primary outcome measure was pain as measured by the visual analogue scale. Secondary measures of pain and disability included the Western Ontario and McMaster Universities (WOMAC) index, Short Form-36 (SF-36), Lequesne Functional Index (LFI) score and Korean version of Health Assessment Questionnaire (KHAQ). Discussion : This paper presents detail on the rationale, design, methods and operational aspects of the trial.

Purpose: The objective of this study was to define the predictors of health related quality of life(HRQoL) of women caregivers of the demented elderly patients. Methods: The subjects of this study were 140 pairs of patients with dementia and their caregivers. The characteristics of dementia patients and caregivers, Barthel index and SF-36 were measured in this study. T-test, ANOVA, Pearson correlation coefficient, and stepwise multiple regression were used for data analysis. Results: The health related quality of life(HRQoL) score of women caregivers was $288.35{\pm}66.10$ for norm based scoring. HRQoL of women caregivers was correlated with patients' ADL, severity of dementia, caregivers' age, burden, and family support. The major factors that affected the physical components of women caregivers of patients with dementia was the age of the caregiver, burden, and ADL which explained 36.0% of HRQoL. The main predictors of women caregivers' emotional state was the caregiver's burden. Conclusion: Patients' support systems must be implemented to improve the physical HRQoL of caregivers. A caregivers' burden relief program needs to be prepared to increase their emotional HRQoL and further studies and efforts will be needed to evaluate those effects.

Measurement of quality of life among prostate cancer patients helps the health care providers to understand the impact of the disease in the patients' own perspective. The main aim of this study is to measure the quality of life among prostate cancer patients at University Malaya Medical Center (UMMC) and Universiti Kebangsaan Malaysia Medical Centre (UKMMC) and to ascertain the association factors for physical coefficient summary (PCS) and mental coefficient summary (MCS). A hospital based, cross sectional study using the Short Form-36 (SF-36) questionnaire was conducted over a period of 6 months. A total of 193 respondents were recruited. Their total quality of life score was $70.1{\pm}14.7$ and the PCS score was lower compared to MCS. The factors associated for PCS were: age, living partner, renal problem, urinary problem of intermittency, dysuria and hematuria. Factors associated for MCS were: age, living partner, renal problem, presenting prostatic specific antigen and urinary problem of intermittency and dysuria. Our prostate cancer patients had moderate quality of life in the physical health components but their mental health was less affected.
Purpose: The purpose of this study was to examine the sensation changes in upper extremity and the quality of life for post-operative mastectomy patients. Methods: This study used a descriptive study design. The participants were 132 women who had mastectomies after being diagnosed with breast cancer and were participating in breast cancer self-help groups. The sensation changes in the arm of the surgery site was measured by the arm symptoms which were subjective uncomfortable feelings of the affected arm, and the quality of life was measured by Medical Outcomes Study Short Form-36. Collected data were analyzed by ANOVA, t-test, ${\chi}^2$-test and multiple regression with SPSS WIN 12.0 program. Results: Patients with stage IV breast cancer had severe changes in sensation of the arm. Participants with lymphedema had statistically significant changes in sensation compared to participants without lymphedema, but the quality of life was not different between the two groups. Pain by the SF 36 was statistically different between the two groups, with- and without-lymphedema. The factors in upper extremity's sensation changes which influence on quality of life were pain and heaviness. Conclusion: Nursing intervention for relieving pain and heaviness of the affected arm needs to be developed in order to improve QOL of the breast cancer survivors.
