순서형 프로빗 모형을 이용한 렌터카 사고 심각도 영향요인 분석 (Risk Factors Affecting the Injury Severity of Rental Car Accidents in South Korea : an Application of Ordered Probit Model)

  • 렌터카 차량의 교통사고 발생건수 및 사망자 수는 꾸준히 증가하는 추세를 보이며, 사고로 인한 사망률 또한 일반 승용차 및 영업용 택시보다 높다. 본 연구는 전국을 대상으로 2010년부터 2014년까지 발생한 렌터카 사고 데이터를 분석하여 사고 심각도에 영향을 미치는 요인들을 도출하였다. 사고 심각도와 관련된 17개 변수를 렌터카 사고 데이터로부터 추출하였으며 이를 4가지(인적, 차량, 도로 및 환경) 요인으로 구분한 후 순서형 로짓 모델을 적용하여 통계적으로 분석하였다. 분석 결과, 운전자 연령, 교차로 형태, 사고 발생 요일을 제외한 나머지 14개 변수가 렌터카 사고 심각도에 영향을 미치는 요인으로 통계적 유의성이 있는 것으로 분석되었다. 연구결과는 다음과 같이 요약될 수 있다. 첫째, 거주 지역이 아닌 곳에서 발생한 렌터카 사고의 심각도가 더 큰 것으로 나타났다. 둘째, 일반 도로 대비 고속도로 렌터카 사고, 과속 및 11대 중과실 위반 등 법규 위반 관련 렌터카 사고의 경우 사고 심각도가 더 큰 것으로 나타났다. 셋째, 지리적으로 익숙하지 않은 렌터카 운전자 특성상 도로 기하구조가 복잡하며, 운전자의 시야가 분산되는 커브구간 등에서의 사고 심각도가 높게 나타났다. 넷째, 지역별로 렌터카 사고 심각도에 차이가 있으나, 2010년 이후 사고 심각도는 전반적으로 감소하고 있는 것으로 나타났다. 본 연구의 결과는 향후 렌터카 사고 심각도를 줄이기 위한 기초자료로 활용될 수 있을 것으로 생각한다.

Digital Infrared Thermal Imaging에 따른 수족냉증 중증도와 Heart Rate Variability의 상관관계 연구 (Study on the Correlation between Digital Infrared Thermal Imaging-induced Severity of Cold Hypersensitivity of Hands and Feet and Heart Rate Variability)

  • Objectives: Cold Hypersensitivity of Hands and Feet (CHHF) has been diagnosed objectively by Digital Infrared Thermal Imaging (DITI) and has been known to be associated with autonomic nervous system (ANS), which can be assessed by Heart Rate Variability (HRV). This study evaluated the correlation between severity of CHHF and HRV variables.Methods: We studied 155 non-menopausal women with CHHF who visited Kyung Hee University Hospital at Gangdong from 01 October 2013 to 30 April 2016. We measured DITI and HRV of each patient. We used DITI to calculate the severity of CHHF with thermal difference between upper arm (L4, 俠白穴) and palm (P8, 勞宮穴) of both hands and anterior thigh (ST32, 伏兎穴) and dorsum of foot (LI3, 太衝穴) of both feet. The correlation between severity of CHHF and HRV variables were analyzed.Results: In time domain analysis, there was significantly positive correlation between the severity of CHHF and both SDNN and RMSSD. In frequency domain analysis, there was significantly positive correlation between the severity of CHHF and TP, HF and HF Norm while there was significantly negative correlation between the severity of CHHF and LF Norm as well as between the severity of cold hypersensitivity of both hands and LF/HF ratio.Conclusions: The more severe CHHF is, the more increased the function of parasympathetic nerve system (PNS) and relatively decreased the function of sympathetic nerve system (SNS) is. Also, it has known that cold hypersensitivity could be caused by deficiency syndrome and qi deficiency has the same ANS tendency as CHHF. Therefore, in practical fields, this result can be helpful in planning treatment and deciding prognosis in respect of deficiency syndrome.

순서형 프로빗 모형을 이용한 버스 운송사업 유형 별 사고심각도 영향요인 분석 (Factors Influencing Crash Severity by the Types of Bus Transportation Services Using Ordered Probit Models)

  • 버스는 대표적인 대중교통수단 중 하나로서, 운행 목적, 운영 거리, 관리 기관 등에 따라 운송사업 유형별로 구분되어 다양하게 운영되고 있다. 버스 관련 사고는 높은 재차인원으로 인하여 큰 피해를 야기할 수 있음에도 불구하고, 버스 운송사업 유형에 따른 사고심각도에 관한 연구는 상대적으로 기존 연구에서 많이 다루어지지 않았다. 본 연구는 버스 관련 사고의 심각도에 미치는 영향요인들을 분석하고 버스 운송사업 유형 별 사고심각도를 저감하기 위한 정책 방향을 제시하는 것을 목적으로 한다. 이를 위해 교통사고 분석시스템(TAAS)에서 추출된 5년 동안의 버스 관련 사고 자료가 사용된다. 시내버스, 시외 및 고속버스, 그리고 전세버스의 세 가지 운송사업 유형 별 순서형 프로빗 모형이 사고심각도 영향요인을 분석하기 위해 추정된다. 분석 결과 속도, 노면상태 등 모든 버스 운송사업 유형의 사고심각도에 유의미한 영향을 미치는 변수도 존재하는 반면 버스 운송사업 유형에 따라 상이하게 사고심각도에 유의미하게 영향을 미치는 변수들이 존재함을 확인하였다. 시내버스의 경우 시간대, 도로 선형 및 신호기 설치 여부가 통계적으로 유의한 변수인 것으로 확인되었다. 시외 및 고속버스의 경우 안전벨트 착용여부와 도로 등급이 사고심각도에 유의미한 영향을 미치는 변수였으며, 전세버스의 경우 시간대, 운전자의 운전 경험, 안전벨트 착용여부, 신호기 설치 여부 및 시간대가 통계적으로 유의한 변수인 것으로 분석되었다. 본 연구의 결과를 통해 버스 운송사업 유형 별 사고심각도를 저감하는 데 기여할 수 있을 것으로 사료된다.

교통사고 심각도 예측 모형의 활용방안에 관한 연구 (서해안 고속도로를 중심으로) (A Study on the Application of Accident Severity Prediction Model)

  • 교통사고는 인간의 생명과 직결되는 문제이므로, 교통사고 예방 및 심각도 감소를 위한 연구는 매우 중요하다고 할 수 있다. 따라서 본 연구에서는 교통사고 심각도에 영향을 줄 수 있는 다양한 영향요소들을 고려하여 교통사고로 인한 피해를 최소화할 수 있는 대응책을 수립하고자 하였다. 본 연구에서는 2004~2006년 동안 서해안 고속도로에서 발생한 174개의 구간별 사고자료를 이용하여 사고 심각도 예측모형을 만들었다. 이렇게 분석된 모형을 이용하여 사고심각도에 영향을 미치는 주요요인을 찾고 이를 이용하여 교통사고로 인한 피해를 줄일 수 있는 다양한 방법들을 고려해 보았다. 분석결과 과속으로 인한 사고, 차량결함, 차대차 사고, 차대사람 사고, 교통량, 곡선반경 및 종단경사 변동계수에 의해 사고심각도가 정의되는 것을 알 수 있다. 이와 같은 사고심각도와 밀접한 관련이 있는 독립변수들을 대상으로 구간에 따른 각 영향의 정도를 그래프를 나타내 보았으며, 그 결과 심각한 사고를 유발하는 특정원인이 몇 가지 존재하며 이러한 특정원인 및 사고유형은 서해안 고속도로의 특정구간에서 주로 발생한다는 것을 알 수 있다. 이러한 결과들은 서해안 고속도로의 특정 구간에 속도제한 단속 카메라, 차내/외 경고정보제공등과 같은 선택적 교통정보 및 교통시설을 제공함으로써 사고를 예방하는 방법에 활용될 수 있을 것이다.

원발성(原發性) 월경통(月經痛) 정도에 따른 하복부(下腹部) 온도차이(溫度差異) 연구(硏究) (The study on the abdominal temperature difference according to primary dysmenorrhea severity)

  • Purpose: We intended to research the relations between abdominal temperature and primary dysmenorrhea severity. Methods: We selected the 95 primary dysmenorrhea patients by means of screening test (first screening test-inquiry, second screening test-clinical test, additionally Waist-to-Hip ratio (WHR) by Inbody 2.0). We measured 4 points abdominal temperature (Chon-jung(CV17), Chung-wan(CV12), Kwan-won(CV4), Chung-guk(CV3)) by DITI (DOREX Inc., USA). And then we checked the difference of temperature $({\Delta}T)$ between CV17 and CV4 / CV17 and CV3 / CV12 and CV4 / CV12 and CV3. After that, we compared ${\Delta}T$ with primary dysmenorrhea severity evaluated by multidimensional scales (verbal rating scale modified from the one devised by Bibe roglu & Berhrman(VRS; B&B), multidimensional verbal rating scale by Andersch & Milsom(MVRS)). In dysmenorrhea severity, we standardized scale score and 3-group-severity by score (mild, moderate, severe). For statistics, we used Pearson correlations and Spearman's rho correlations, SPSS 11.0 for windows. Results: In case of MVRS, MVRS score and 3-group-severity were not correlated to ${\Delta}T$. In case of VRS; B&B, VRS; B&B score was correlated to ${\Delta}T$ (CV12 and CV4 / CV12 and CV3) and 3-group-severity was correlated to ${\Delta}T$ (CV12 and CV3). Statistically they showed significant result (p<0.05). So we can consider that ${\Delta}T$ (CV12 and CV3) and the primary dysmenorrhea severity by VRS; B&B are most correlated. Conclusion: The primary dysmenorrhea patients showed that severity by VRS; B&B was connected with ${\Delta}T$ (CV12 and CV3). So we can consider DITI as primary dysmenorrhea evaluation instrument and must further research measurement points for the exact primary dysmenorrhea evaluation by DITI.

Machine Learning-Based Prediction of COVID-19 Severity and Progression to Critical Illness Using CT Imaging and Clinical Data

  • Objective: To develop a machine learning (ML) pipeline based on radiomics to predict Coronavirus Disease 2019 (COVID-19) severity and the future deterioration to critical illness using CT and clinical variables. Materials and Methods: Clinical data were collected from 981 patients from a multi-institutional international cohort with real-time polymerase chain reaction-confirmed COVID-19. Radiomics features were extracted from chest CT of the patients. The data of the cohort were randomly divided into training, validation, and test sets using a 7:1:2 ratio. A ML pipeline consisting of a model to predict severity and time-to-event model to predict progression to critical illness were trained on radiomics features and clinical variables. The receiver operating characteristic area under the curve (ROC-AUC), concordance index (C-index), and time-dependent ROC-AUC were calculated to determine model performance, which was compared with consensus CT severity scores obtained by visual interpretation by radiologists. Results: Among 981 patients with confirmed COVID-19, 274 patients developed critical illness. Radiomics features and clinical variables resulted in the best performance for the prediction of disease severity with a highest test ROC-AUC of 0.76 compared with 0.70 (0.76 vs. 0.70, p = 0.023) for visual CT severity score and clinical variables. The progression prediction model achieved a test C-index of 0.868 when it was based on the combination of CT radiomics and clinical variables compared with 0.767 when based on CT radiomics features alone (p < 0.001), 0.847 when based on clinical variables alone (p = 0.110), and 0.860 when based on the combination of visual CT severity scores and clinical variables (p = 0.549). Furthermore, the model based on the combination of CT radiomics and clinical variables achieved time-dependent ROC-AUCs of 0.897, 0.933, and 0.927 for the prediction of progression risks at 3, 5 and 7 days, respectively. Conclusion: CT radiomics features combined with clinical variables were predictive of COVID-19 severity and progression to critical illness with fairly high accuracy.

Usefulness of Impulse Oscillometry in Predicting the Severity of Bronchiectasis

  • Background: Bronchiectasis is a chronic respiratory disease that leads to airway inflammation, destruction, and airflow limitation, which reflects its severity. Impulse oscillometry (IOS) is a non-invasive method that uses sound waves to estimate lung function and airway resistance. The aim of this study was to assess the usefulness of IOS in predicting the severity of bronchiectasis. Methods: We retrospectively reviewed the IOS parameters and clinical characteristics in 145 patients diagnosed with bronchiectasis between March 2020 and May 2021. Disease severity was evaluated using the FACED score, and patients were divided into mild and moderate/severe groups. Results: Forty-four patients (30.3%) were in the moderate/severe group, and 101 (69.7%) were in the mild group. Patients with moderate/severe bronchiectasis had a higher airway resistance at 5 Hz (R5), a higher difference between the resistance at 5 and 20 Hz (R5-R20), a higher resonant frequency (Fres), and a higher area of reactance (AX) than patients with mild bronchiectasis. R5 ≥0.43, resistance at 20 Hz (R20) ≥0.234, R5-R20 ≥28.3, AX ≥1.02, reactance at 5 Hz (X5) ≤-0.238, and Fres ≥20.88 revealed significant univariable relationships with bronchiectasis severity (p<0.05). Among these, only X5 ≤-0.238 exhibited a significant multivariable relationship with bronchiectasis severity (p=0.039). The receiver operating characteristic curve for predicting moderate-to-severe bronchiectasis of FACED score based on IOS parameters exhibited an area under the curve of 0.809. Conclusion: The IOS assessed by the disease severity of FACED score can effectively reflect airway resistance and elasticity in bronchiectasis patients and serve as valuable tools for predicting bronchiectasis severity.

산림 공간구조 특성과 산불 연소강도와의 관계에 관한 연구 (Linking Spatial Characteristics of Forest Structure and Burn Severity)

  • Because fire has significant impacts on fauna and flora in forest ecosystems, as well as socioeconomic influences to local community, it has been an important field of study for decades. One of the most common ways to reduce fire risk is to enhance fire-resilience of forest through fuel treatments including thinning and prescribed burning. Since fuel treatment can't be practiced over all forested areas, appropriate and effective strategies are needed. The present study aims to look at the relationship between spatial characteristics of forest structure measured with landscape pattern metrics and burn severity to provide guidelines for effective fuel treatments. Samchuck fire was selected for the study, and 232 grids covering the study areas were generated, and the grid size was 1km. The burn severity is measured with dNBR derived from satellite imagery, and spatial characteristics of forest structure were measured using FRAGSTATS for both landscape and class levels for each 1km grid. The results of this study strongly indicated that heterogeneity in composition and configuration of forests may significantly reduce burn severity. By enhancing heterogeneity of forests, fuel treatments for fire-resilience forest could be more effective.

교통사고심각도 영향요인 분석에 관한 연구 - 곡선부가 포함된 국도를 중심으로 - (Analysis on Factors Affecting Traffic Accident Severity - Case Study : Arterial Included Curve Section -)

  • The main causes of traffic accidents can be classified by 3 factors - human error, vehicle deficiency and road environmental problem and most accidents occurs not only 1 factor but combination of 2 or 3-factors. Among these factors, road environmental factor is the most important factor due to influence the behavior of cars and road users and road environmental factor affects 30% of total accidents approximately. The 5 years traffic accidents data analyzed to verify the accidents severity on Korea National Highways. In order to analyze the severity, Ordered Probit Model was used. As a independent variables of this model the number of lane, neighbor road environments, sight distance, vertical grade, lane width, shoulder width and traffic volume were used and as a dependent variables the minor injuries, serious injuries and fatalities were used. Research results shows that sight distance and lane width are identified as significant factors for the traffic accident severity and lesser sight distance and lane width shows greater traffic accident severity.

여성 요실금 환자의 요실금 중증도 및 삶의 질에 대한 영향요인 분석 (Factors Affecting the Severity of Urinary Incontinence and the Quality of Life of Women with Urinary Incontinence)

  • Purpose: This study was conducted to investigate the factors that affect the severity of urinary incontinence and the quality of life of women with urinary incontinence. Methods: An exploratory correlation research design was applied and the sixty women who responded that they currently experience involuntary urine leakage at least once per week were invited to participate in the study. Results: According to the study results, the factors that significantly contribute to the severity of urinary incontinence were age, number of parity, and number of normal deliveries. Other influencing factors, such as obesity, menopausal state, hysterectomy, and number of episiotomies, were not significant to predict the severity of urinary incontinence. Urinary incontinence frequency per week and duration of experiencing urinary incontinence were shown to be factors that significantly affect quality of life. Conclusion: In women above 60 years old, multiparity, and multiple experiences of vaginal delivery tended to present a higher level of severity of urinary incontinence. In addition, women with higher frequencies of urinary incontinence per week and a longer duration of urinary incontinence showed a lower level of quality of life.
