스트레스에 대한 심리 반응 유형과 심박변이도의 관련성 (Relationships of Psychological Factors to Stress and Heart Rate Variability as Stress Responses Induced by Cognitive Stressors)

  • 본 연구에서는 스트레스에 대한 심리 반응 요인과 인지 스트레스원에 의해 유발된 심박변이도의 변화와의 관계를 확인하고자 하였다. 33명의 실험참여자는 지난 2주 동안 경험한 스트레스에 대하여 정신적 신체적 증상들을 스트레스 반응 척도 상에 평가하였다. 또한 실험참여자가 인지 스트레스 과제를 수행하는 동안 심전도 신호가 기록되었다. 심박변이도 지표로서, R-R간격의 표준편차(SDNN), 연속한 R-R 간격 차이 값의 평균제곱근(RMSSD)과 심박변이도의 저주파 성분(LF)과 고주파 성분(HF)의 비율이 추출되었다. 스트레스 반응은 회복기 동안의 평균에서 기저선의 평균을 뺀 차이값으로 분석에 사용되었다. 분석 결과, 스트레스 요인과 심박변이도 지표들과의 유의한 정적 상관을 보였다. 특히 스트레스에 대한 공격 반응의 증가는 모든 심박변이도 지표의 증가와 연관이 있었다. 모든 심박변이도 지표의 증가는 자율신경계가 동시에 활성화됨을 의미한다. 즉, 공격 반응의 증가는 교감 및 부교감신경계의 활성도의 증가와 관련이 있다. 또한 긴장, 우울, 피로와 좌절은 RMSSD와의 정적 상관이 있었다. RMSSD의 증가는 부교감신경계 활성의 증가를 의미하는 것으로, 긴장, 우울, 피로, 좌절의 증가와 부교감 신경의 활성도의 증가가 연관이 있었다. 이러한 자율신경의 공동 활성화는 주의 집중을 요하는 인지 스트레스원에 대한 통합된 반응으로 여겨진다.

중풍 환자의 심박변이도 특성에 대한 임상적 연구 (Clinical Study for Characteristics of Heart Rate Variability in Stroke Patients)

  • Objectives : We designed this study to investigate difference of heart rate variability(HRV) according to sex, age, acute or chronic phase, category of stroke, national institute of health stroke scale(NIHSS), lesion of stroke. Methods : 64 subjects were recruited from the patients admitted to the department of oriental medicine at East-West Neo Medical Center, Kyung Hee University from 1 September 2009 to 31 August 2010. We compared heart rate(HR), standard deviation of all normal P-P intervals(SDNN), low frequency(LF), high frequency(HF), LF/HF ratio. Results and Conclusions : 1. LF/HF ratio is significantly different between over-70 and below-70 of age. 2. SDNN is significantly different between acute and chronic stroke patients. 3. In sex, category of stroke, national institute of health stroke scale(NIHSS), lesion of stroke, there are no significantly different among the any values of heart rate variability(HRV).

실내 온도가 뉴로에너지에 미치는 영향에 관한 연구 (A Study on the Indoor Temperature effects on Neuro-energy)

  • 본 연구는 상대습도 50[RH%], 조도 1,000[Lux] 및 기류 속도 0.02[m/sec]의 동일한 조건의 항온 항습실에서 온도를 18, 23, 25, 28 및 $31[^{\circ}C]$로 변화를 주어 EEG, HRV 및 바이브라이미지를 비교 분석하였다. 실험 결과, 온도 $25[^{\circ}C]$에서 상대 $M{\alpha}$파, 상대 $M{\beta}$파, $\frac{SMR}{\theta}$ 및 SDNN은 활성화되었으며, ${\alpha}$파의 비대칭지수 $A_2$, HRT, 스트레스지수 및 피로도는 감소하였다. 따라서, 온도 $25[^{\circ}C]$가 쾌적성, 생산성 및 집중력 등과 같은 뉴로에너지 향상에 효과적인 것을 알 수 있었다.

Reduced Heart Rate Variability in Somatic Symptom Disorder: Associations with Alexithymia

  • Objectives : We investigated heart rate variability (HRV) patterns in patients with somatic symptom disorder (SSD) and the relationships of these patterns with alexithymia. Methods : In total, 42 patients with SSD and 33 healthy controls were enrolled in this study. Demographic, psychological, and HRV data were assessed at baseline, and 24 patients with SSD were reassessed after 6 months of treatment. The psychological data included somatic symptoms and levels of depression, anxiety, and alexithymia as indicated by the somatic symptom subscale of the Symptom Checklist 90-Revision (SCL-12), Beck Depression Inventory-II (BDI-II), Beck Anxiety Inventory (BAI), and the Toronto Alexithymia Scale 20 (TAS-20), respectively. Results : Patients with SSD had a lower standard deviation of normal-to-normal R-R intervals (SDNN) and lower proportions of adjacent R-R intervals greater than 50 milliseconds (pNN50) compared with controls. These HRV parameters were negatively correlated with alexithymia severity. After treatment, patients exhibited significantly decreased levels of somatic symptoms and reduced anxiety and depression, but there were no significant differences in the HRV parameters. In patients with alexithymia, a high baseline SDNN and pNN50 were associated with a decrease in somatic symptoms. Conclusions : Patients with SSD have different HRV patterns, and several HRV parameters are associated with alexithymia severity. These findings suggest that ANS regulation is involved in the pathophysiology of SSD, mediated by alexithymia. Furthermore, these results suggest that certain HRV parameters may be associated with clinical outcomes of SSD.

HRV 검사를 통한 피부과 내원환자의 자율신경 균형 및 스트레스 단면 연구 (The Cross Sectional Study of Autonomic Nerve Balance And Stress In Patient Who Visted Dept. of Oriental Medical Dermatology)

  • Objective : Etiologically, stress is a important factor to immune system and skin disease. Human body is controled to keep his homeostasis from stress by autonomic nerve system. So we investigated autonomic nerve balance and stress level of patients with skin disease by heart rate variability(HRV) test. Research Methods and Procedures : Fifty two patient(from 15 years old to 35 years old) who visited Dept. of Oriental medical Dermatology, Kyunghee university at first and had an examination by HRV test, were selected as the subject of our study. We checked items of HRV test(such as SDNN, RMSSD, VLF, LF, HF, TP, norm-LF, norm-HF, LF/HF ratio, LnHF and LnHF), and analyzed time and frequency domain by sex, age, onset, color on end of patient's tongue, sweating and sleep condition. Results and Conclusion : Men showed significant higher SDNN, RMSSD, LF, norm-LF, norm-HF, LF/HF ratio, LnLF and LnHF than women(p <0.05). Patients who were attacked by skin disease within 6 months, showed significant higher TP and VLF than over 6 months(p <0.05). Patients with normal color on the end of their tongue showed significant higher TP and VLF than with flare color. We analyzed correlation to color on the end of tongue and RMSSD. Prevalence risk(RMSSD <20ms) in the case of normal color on the end of tongue was 5 times as high as in the case of flare color(p <0.1). And we analyzed correlation to age and LF/HF ratio. Prevalence risk(LF/HF ratio <1) of patients from 15 years old to 25 years old was 1.467 times as high as from 26 years old to 35 years old(p <0.1).

비만 치료제 Phentermine Hydrochloride가 심박동 변이율(Heart Rate Variability)에 미치는 영향 (The influence of phentermine hydrochloride on heart rate variability)

  • Background : Phentermine classified by "sympathomimmetic amie", is a stimulant of sympatheic tone But there has been no concrete study which presents the influence of phentermine on autonomic nervous system. Analysis of Heart rate variability is reliable, non-invasive and very useful for evaluating function of autonomic nervous system. We tried to elucidate the influence of phentermine on autonomic nervovs system by heart rate variability. MethodsAmong the 70 candidates who participated in the double-blind case control study whichwas designed in purpose of approving whether- Adipekⓓ is effective for treatment of obesity, 45persons were folled up. From April, 2005 to May 2005, HRV of the candidates who takes phentermine or placebo for 1month, was recorded using BFM-5000ⓓ(medi-core) for 5 minutes in resting state. HRV measures were assessed by time-domain and by frequency- domain analysis. Time domain parameters contain SDNN(Standard Deviation of NN intervals) and RMSSD(Root-Mean-Square of Successive Differences), etc and frequency domain Parameters contain Total Power(TP), Low frequency(LF'0.04-0.15Hz) power. High Frequency(HF:0.15-0.4Hz) power and LF/HF ratio etc. Results: Intakes of phentermine reduce HRV significant1y. SDNN & RMSSD, the main tine domain parameters of HRV, were decreased significantly(P=0.007. 0.016). PSI(Physical Stress Index of Pressure Index) was increase significantly(P=0.002)The main frequency domain parameters(TP, LF & VLF), also decreased significantly. (P=0.024,0.033, 0.015)Conclusion: The result showed that intakes of phentermine reduce heart rate variability and influence on most parameters of HRV. So phentermine not only accelerates sympathetic tone, but also inhibit the balance and function of autonomic nervous system.

추적관찰을 통해 살펴본 한증 HRV지표 (Heart Rate Variability in Cold Pattern: 3-year Follow-up Study)

  • This study aimed to investigate heart rate variability (HRV) characteristics of cold pattern with repeated measurement data. Participants were taken from a Daejeon University cohort study from 2015 to 2018. Forty-seven of the participants studied displayed cold pattern while 23 showed signs of non-cold pattern. HRV was measured in supine position for 5 minutes at each year, and an 8-item cold pattern questionnaire was used for the diagnosis of cold pattern. SDNN (standard deviation of the NN intervals) and RMSSD (the square root of the mean squared differences of successive NN intervals) were used as time domain analysis, and TP (total power), VLF (power in very low frequency range), LF (power in low frequency range), HF (power in high frequency range), LF norm (LF power in normalized units), HF norm (HF power in normalized units) and LF/HF were used as frequency domain analysis. In the Mann-Whitney U test, LF norm, HF norm, and LF/HF showed differences between the cold pattern group and non-cold pattern group at every measurement, and in the independent t-test, the differences were also observed at three points except for the baseline (2015). In the repeated measures ANOVA, the interaction effects were not observed in all HRV parameters, but the time period effects were observed in SDNN, RMSSD, TP, VLF, LF and HF. There were significant differences between those two groups in LF norm, HF norm and LF/HF. This study suggests that LF norm, HF norm and LF/HF might be a useful indicator of cold pattern properties.

자침수기가 뇌파에 미치는 영향 (The Effect of Manual Acupuncture on the Waves of the Electroencephalogram)

  • Objectives : The aim of this study was to examine the effect of manual acupuncture at the bilateral $LI_4$, $ST_{36}$ and $LR_3$ on normal human every 3 minute time interval by using power spectrum analysis. Methods : 25 healthy volunteers participated in this study. 32 channel EEG(Electroencephalogram) measurements were carried out before, during and after acupuncture treatments. In this study, power spectrum was used as a measure of complexity. Statistical analysis was performed using repeated measure ANOVA and Bonferroni method. Results : 1. In acupuncture group, Mean HRT, PSI, Normalized HF decreased significantly. SDNN, Ln(TP), Ln(LF), Ln(VLF), Normalized LF and LF/HF ratio increased significantly. RMSSD, Ln(HF) were not affected. 2. In sham acupuncture group, all indicators were not affected. 3. In control group, mean HRT, Ln(TP) decreased significantly. SDNN, RMSSD, PSI, Ln(LF), Ln(HF), Ln(VLF), normalized LF, normalized HF and LF/HF ratio were not affected. Conclusions : This results suggest that manipulation needling of 6 acupoints induced increase of delta, theta, beta waves. Both manipulation and simple needling induced decrease of alpha wave during 12~15 time intervals.

음허와 생체신호의 상관성 및 비교 연구 (Correlation and Comparison Between $Yin$-Deficiency Questionnaire Score and Biofunctional signals)

  • Objectives: The purpose of this study was to analyze the relationship between Yin-deficiency questionnaire score and various biofunctional signals in women. Methods: A retrospective chart review was performed on charts of 195 patients who visited Gangdong Kyung Hee Hospital between April 1st and September 30th, 2011. The subjects were categorized into two groups, a low Yin-deficiency group (n=118) and a high Yin-deficiency group (n=77). The authors analyzed the correlation between Yin-deficiency questionnaire score and biofunctional signals by Pearson's correlation coefficient test and the difference in biofunctional signals between the two groups by independent samples t-test using SPSS for windows. Results: 1. Negative correlations were observed between the temperature difference of back-humerus, standard deviation of all R-R intervals (SDNN), total power (TP), low frequency (LF), high frequency (HF) on heart rate variability parameters, and Yin-deficiency questionnaire score. A positive correlation was observed between the temperature difference of knee-humerus and Yin-deficiency questionnaire score. 2. The temperature difference of back-humerus in the high Yin-deficiency group was significantly higher than that in the low Yin-deficiency group. The temperature difference of knee-humerus, height, waist-hip ratio, SDNN, TP, LF, and HF of the high Yin-deficiency group were significantly lower than those of the low Yin-deficiency group. Conclusions: The results of this study suggest that the comprehensive diagnosis of Yin-deficiency and biofunctional signals is useful.

임상 실습 동안 다양한 온도의 족욕이 물리치료학과 학생들의 스트레스에 미치는 영향 (The Immediate Effects of Foot-bath at Diverse Temperatures on the Stress of Students in the Department of Physical Therapy who Experience Clinical Practice)

  • PURPOSE: The purpose of the present study is to apply foot therapy at diverse temperatures to students in the Department of Physical Therapy who are experiencing clinical practice to find the most effective foot-bath temperatures for stress relief. METHODS: Sixty four students in the department of physical therapy who were undergoing the course on clinical practice participated in the present study. SDNN, TP, LF, and HF were measured to compare the control group, cold group, tepid group, and the hop group. The data were analyzed through analysis of covariance and paired t-tests. RESULTS: Although SDNN increased in the cold group and the tepid group, the differences were not statistically significant. TP and LF showed statistically significant increases in the cold group. In comparisons between the groups, the cold group showed statistically significant increases compared to the control group and the hot group. Although the tepid group also showed increases, the differences were not statistically significant. HF statistically decreased in the hot group. In comparisons between the groups, statistically significant differences appeared between the cold group and the hot group. CONCLUSION: Cold foot-bath was the most effective therapy on the stress of students in the department of physical therapy who were experiencing clinical practice. Tepid foot-bath had the same directivity but showed no statistically significant difference. Hot foot-bath was shown to rather increase stress.
