척수의 혈관성 질환으로 인한 하지마비 환자에 대한 한방치료 치험 2례 보고 (Case Report on the 2 Cases of Patients with Vascular Disease of Spinal Cord)

  • Objectives : The study aimed to show the clinical effects of oriental medical treatments for vascular disease of spinal cord. Methods : The patients were treated using acupuncture, electroacupuncture, pharmacoacupuncture, herbal medication. And the effects for vascular disease of spinal cord have measured in ASIA scale, SCIM III. Results : Sensory score on the ASIA scale is increased 128 to 156 in case 1, 189 to 196 in case 2. Motor score on the ASIA scale isn't changed in case 1, but is increased 90 to 99 in case 2. SCIM III is improved 25 to 30 in case 2. Conclusions : From the above results, It is considered that oriental medical treatments is effective on vascular disease of spinal cord.

Integrated Effect of Non-Invasive Neuromodulation on Bladder Capacity in Traumatic Spinal Cord Injury Patient: Single Case Report

  • Objective: To evaluate the changes in bladder capacity and storage through non-invasive neuromodulation by application of repetitive Trans magnetic stimulation (rTMS) and genital nerve stimulation (GNS) in traumatic spinal cord survivors. Design: A Single Case Study. Method: The Patient was registered in trail with the clinical trial registry of India (CTRI/2022/05/042431). The Patient was interposed with rTMS on lumbar area, from T11-L4 vertebrae with 1 Hz and the intensity was 20% below that elicited local paraspinal muscular contraction for 13 minutes. GNS was placed over dorsum of the penis with the cathode at the base and anode 2 cm distally at 20 Hz, 200 microseconds, Continuous and biphasic current was delivered and amplitude of stimulation necessary to elicit the genito-anal reflex. For assessment, Neurological examination was done for peri-anal sensation (PAS), voluntary anal contraction (VAC) and bulbocavernous reflex (BCR), deep anal pressure (DAP), and American Spinal Injury Association Impairment Scale (ASIA scale). Outcome assessment was done using Urodynamics, Spinal Cord Independence Measure Scale Version-III (SCIM-III), American Spinal Injury Association Impairment Score (ASIA Score), Beck's Depression Inventory Scale (BDI). The baseline evaluation was taken on Day 0 and on Day 30. Results: The pre-and post-data were collected through ASIA score, SCIM-III, BDI and Urodynamics test which showed significant improvement in bladder capacity and storage outcomes in the urodynamics study across the span of 4 weeks. Conclusion: rTMS along with GNS showed improvement in bladder capacity & storage, on sensory-motor score, in functional independence of individual after SCI.

Effect of Complex Korean Medical Treatment on Spinal Epidural Hematoma: A Case Report

  • This paper presents the clinical outcome of an 82-year-old female who experienced sudden back pain combined with lower limb paresthesia and weakness after epidural steroid injection. The magnetic resonance imaging of the thoracolumbar spine showed a spinal epidural hematoma (SEH) extending from T8 to L4. She was treated non-operatively in the traditional Korean medicine (TKM) hospital. The patient's progress was assessed using the Numerical Rating Scale (NRS), American Spinal Injury Association (ASIS), Spinal Cord Independence Measure version III (SCIM III), and self-reported symptoms. During the 22-day hospitalization period,the NRS score decreased from 7 to 2 points, the motor score on the ASIS scale increased from 65 to 95 points. The subjective sense of lower extremities was felt by 1 increased to 8. The SCIM III score increased from 32 to 69 points. These results suggest that TKM could effectively reduce pain and aid the rehabilitation of patients with SEH.

지역사회 거주 척수장애인의 참여 제약에 영향을 미치는 예측요인 (Predictors of Participation Restriction in Community-dwelling Persons With Spinal Cord Injury)

  • 목적 : 본 연구의 목적은 지역사회 거주 척수장애인의 참여 제약과 관련이 있는 요인간의 상관관계 및 그 영향을 종합적으로 분석하는 것이다. 연구방법 : 연구 대상은 지역사회에 거주하는 미국 척수 손상 협회(American Spinal Injury Assocation; ASIA) 분류법 상 손상정도(Frankel type)가 A,B,C에 해당하는 만 18세 이상 성인 척수장애인 82명이었다. 대상자들을 통하여 참여, 환경적 요인, 기능적 요인, 손상관련 요인, 심리사회적 요인, 인구사회학적 요인을 조사하였다. 참여 수준은 한국형 단축형 크레이그 핸디캡 평가(CHART-K-SF)로, 환경적 요인은 CHIEF-25 한국형으로, 기능적 요인은 척수손상환자 독립성 지수SCIM-III을 사용하였다. 손상관련 요인의 통증 및 통증관련 심리적 고통은 Pain and Distress Scale (PAD)로, 심리사회적 요인 중 자존감은 로젠버그 자아존중감 척도(RSES), 자기효능감은 일반적 자기효능감 척도(GSS)로 평가하였다. 수집된 정보는 SPSS Version 21.0을 통하여 상관관계분석, 로지스틱 회귀분석, 다중회귀분석에 사용되었다. 결과 : 참여 제약의 위험요인은 환경적 요인과 기능적 요인이었으며, 본 회귀모형의 설명력은 약 73.7%였다(R2N=.737). 환경적 장벽이 있는 것이 없는 것에 비해 약 40배 정도 참여 제약을 겪을 위험도가 높았으며, 기능적 수준이 4사분위수(상위 25%)인 것에 비해, 2사분위수일 경우 약 21배, 1사분위수(하위25%)일 경우 약 236배 정도 참여 제약을 겪을 위험도가 높아졌다. 결론 : 본 연구는 지역사회 거주 척수장애인의 참여 제약의 예측요인을 다각적인 측면으로 확인한 것에 의의가 있으며, 이를 통하여 지역사회에서 척수장애인에 대한 작업치료사의 역할을 규명한 것에 중요한 의미를 갖는다.

운동심상이 만성 경수 손상 환자의 근활성도와 일상생활에 미치는 영향 (Effect of a Motor Imagery Program on Upper Extremity Strength and Activities of Daily Living of Chronic Cervical Spinal Cord Injury Patients)

  • Purpose: The purpose of this study is to determine the effect of motor imagery training on residual upper extremity strength and activities of daily living of chronic cervical spinal cord injury patients. Methods: Twelve ASIA A B patients, who had more than a 12-month duration of illness and C5 or 6 motor nerve injury level, were randomly divided into experimental group (n=6) and control group (n=6). Patients in the experimental group performed motor imagery training for five minutes prior to general muscle strengthening training, while those in the control group performed general muscle strengthening training only. The training was performed five times per week, 30 minutes per day, for a period of four weeks. General muscle strengthening training consisted of a progressive resistive exercise for residual upper extremity. Motor imagery training consisted of imagining this task performance. Before and after the training, EMG activity using BTS Pocket Electromyography and Spinal Cord Independent Measure III(SCIM III) were compared and analyzed. Results: The residual upper extremity muscle strengths showed improvement in both groups after training. Comparison of muscle strength improvement between the two groups showed a statistically significant improvement in the experimental group compared to the control group (p<0.05). SCIM III measurements showed significant improvement in the scores for Self-care and Transfer items in the experimental group. Conclusion: Motor imagery training was more effective than general muscle strengthening training in improving the residual upper extremity muscle strength and activities of daily living of patients with chronic cervical spinal cord injury.

후종인대골화증으로 발생한 경추 척수손상 환자에 대한 수술 후 한방치료 증례보고 1례 (A Case Report of a Postoperative State Patient with Cervical Spinal Cord Injury due to Ossification of Posterior Longitudinal Ligament Treated by Korean Medicine)

  • Objectives: The purpose of this study is to report the effectiveness of Korean medicine treatment on postoperative state patients with cervical spinal cord injury due to ossification of the posterior longitudinal ligament. Methods: A 55-year-old Korean male patient diagnosed with cervical spinal cord injury due to ossification of the posterior longitudinal ligament was treated in an inpatient setting with Korean medicine treatment consisting of herbal medicine, acupuncture, and others from May 22 to August 21. The outcome was evaluated with the American Spinal Injury Association Impairment Scale (AIS), Spinal Cord Independence Measure-III (SCIM-III), modified Japanese Orthopaedic Association scale (mJOA), Numeric Rating Scale (NRS) before and after, or during the treatment period. Results: After 92 days of treatment, the patient's scores on all scales improved. Conclusion: The results suggest that Korean medicine treatment may be effective for postoperative patients with cervical spinal cord injury due to ossification of the posterior longitudinal ligament. Further studies should be conducted on a larger number of patients.
