Periodic analysis of local epidemiologic data of prevalent pathogens of blood culture can provide clinicians with relevant information to guide empirical antibiotic therapy. In this study, we analyzed a pattern of change of causative microorganisms and antimicrobial resistance at a tertiary medical center in Chungcheong province from 2003 to 2012, retrospectively. Of 70,258 blood specimens cultured, 6,063 (8.6%) were positive. Among the positive isolates, 95.9% were aerobic or facultative anaerobic bacteria, 0.1% were anaerobes, and 3.9% were fungi. Coagulase-negative Staphylococci (CoNS) (32.9%), Escherichia coli (16.7%), Staphylococcus aureus (9.1%), Klebsiella pneumoniae (6.4%), and ${\alpha}$-hemolytic Streptococcus (5.9%) were commonly isolated bacteria, and Candida albicans (1.4%) was the most commonly isolated fungi. Enterococcus faecium progressively increased but Streptococcus pneumoniae, Acinetobacter baumannii and Proteus species gradually decreased over a period of 10 years. The multidrug-resistant microorganisms such as methicillin-resistant S. aureus (MRSA), vancomycin-resistant enterococci (VRE), cefotaxime-resistant E. coli, imipenem-resistant Pseudomonas aeruginosa (IRPA) and imipenem-resistant A. baumannii (IRAB), were significantly increased. Therefore, there is a need for a more strict control of antibiotics and a more updated guideline for the treatment of bloodstream infection.

Purpose: Mycoplasma pneumoniae (MP) pneumonia epidemics have occurred in 3- to 4-year cycles in Korea. We evaluated the epidemiologic characteristics of MP pneumonia in Daejeon, Korea, from 2003 to 2012. Methods: We retrospectively analyzed 779 medical records of children (0-15 years of old) with MP pneumonia admitted to our institution and compared the data from 3 recent epidemics. Results: In 779 patients, the mean age and male-to-female ratio were $5.0{\pm}2.2$ years and 1:1, and most cases were observed in autumn. There were three epidemics during the study period, in 2003, 2006-2007, and 2011. In our comparison of the three epidemics, we found no differences in mean age, the male-to-female ratio, hospital stay, or the rate of seroconverters during hospitalization. All three epidemics began in early summer and peaked in September 2003 and 2011 and in October 2006 and then gradually decreased until the next year's spring season, although the 2006 epidemic extended further into 2007. The peak age groups in the children in 2003 and 2006 were 3-6 year-olds (57.5% and 56%, respectively), but in the 2011 epidemic, the peak group was 1-4 year-olds (46.5%). The proportion of the <2 years of age group was 20%, 15.7% and 28.8%, and >10 years of age group was 5.2%, 13.8%, and 14.8% of total patients, respectively. Conclusion: MP pneumonia outbreaks occurred every 3-4 years. The pattern of 3 recent epidemics was similar in demographic characteristics and seasonality with some variations in each outbreak.

We studied the dyeability and functionality of silks dyed singly with Caesalpinia sappan, Dactylopius coccus, combination-dyed with Caesalpinia sappan and Phellodendron Amurense Rupr., and with Dactylopius coccus and Phellodendron Amurense Rupr.. The color of silk dyed singly with Caesalpinia sappan was Red with Hue of 2.2 R, the color of silk dyed singly with Dactylopius coccus was Reddish Purple with Hue of 3.0 RP, The color of silk combination-dyed with Caesalpinia sappan and Phellodendron Amurense Rupr. was 2.7YR being in the Orange family, and the color of silk combination-dyed with Dactylopius coccus and Phellodendron Amurense Rupr. was Red with Hue of 4.3R. In the case of antimicrobial activity, except for silk dyed only with Dactylopius coccus, the other three types of dyed silk showed excellent antimicrobial activity against Staphylococcus aureus achieving higher than 91.1% of bacteria reduction rate. Against Klebsiella pneumoniae, silk combination-dyed with Dactylopius coccus and Phellodendron Amurense Rupr., and with Caesalpinia sappan and Phellodendron Amurense Rupr. presented very excellent antimicrobial property achieving bacteria reduction rate of 99.9%.
Community-acquired pneumonia (CAP) is a major risk factor of mortality in chronic obstructive pulmonary disease (COPD). Streptococcus pneumoniae (colloquially known as pneumococcus) is one of important pathogens of CAP in patients with COPD. Preventive interventions for pneumonia include pneumococcal and influenza vaccinations. A prospective, cohort study has been performed to investigate the protective effects of pneumococcal and influenza vaccinations on the severity of community-acquired pneumonia requring hospital admission in patients with COPD. Seven university-affiliated hospitals in Korea have participated in the study. The aim of this study was to construct an online registration system for the multi-institutional researchers that facilitates efficient collection and management of COPD patient data. This study has presented three basic strategies-accurate data input, convenient data completion, and real-time data management-to supplement the demerits of existing offline clinical study. The proposed online registration system has already been applied to a multi-institutional clinical study and was acknowledged for its high performance.
Pyogenic liver abscess is a rare condition in healthy children in developed countries, but it can have severe consequences if diagnosis and treatment are delayed. Staphylococcus aureus is the most commonly identified cause of pyogenic liver abscess worldwide, while Klebsiella pneumoniae is the predominant causative agent in Korea. However, cases of pyogenic liver abscess caused by anaerobic bacteria have also been rarely reported. A 14-year-old boy presented to the emergency room with fever and pain in the upper right abdomen. He exhibited tenderness in the right upper quadrant, elevated white blood cell count, anemia, and elevated liver enzyme levels. Abdominal computed tomography with contrast revealed pyogenic liver abscess. The patient underwent percutaneous catheter drainage for two weeks and received a 4-week course of antibiotic therapy. Prevotella intermedia, an anaerobic bacterium commonly found in the oral flora, was isolated from the drained pus. However, no evidence was found suggesting that the infection originated from a dental source. This case highlights the importance of considering the possibility of pyogenic liver abscess even in otherwise healthy children.
This study was observed to the effect of the feeding Platycodon grandiflorum A. DC (3 years) extract on the bronchus diseases bacteria ( C. diphtheriae, S. aureus, Mycobacterium sp., F. nucleatum, S. pygogenes, K. pneumoniae and N. gonorrhoeae) and fungi(A. fumicatus). Platycodon grandiflorum A. DC was extracted ethanon, water, ethyl ether and petroleum ether. The extraction rate of Platycodon grandiflorum A. DC to the extract solution was identified 71.8%, 100%, 15.4% and 14.1%. Each extract solution was injected culture media into several concentrations and then investigated the bacteria cell growth during 32 hours. As a result antimicrobial activity was excellent an extract by ethyl ether and petroleum ether. Among several concentrations, bacteria cell growth inhibition was observed from 0.06% to 0.14%. The rate of antimicrobial activity was over 70%. The cell growth inhibition rate of each bacteria was appeared in order of ethyl ether > petroleum ether > water > ethanol.
Nonwoven fabric was treated with chitosan solution to impart antimicrobial activities by pad-dry method. Antimicrobial activity was measured by Shake Flask Method. Two chitosans of different molecular weight(Mw) with similar degree of deacetylation(DDA) were used : ca. 1,800(chitosan oligomer : DDA 84%) and 180,000(DDA 86%). Chitosan oligomer displayed high antimicrobial activity against P. vulgaris at 0.01%, S. aureus and E. coli at 0.05% treatment concentration, shelving above 90% of reduction rate. Chitosan of Mw 180,000 was effective against S. aureus, E. coil and P. vulgaris at 0.05% treatment concentration, showing almost 100% reduction rate. While chitosan of Mw 180,000 shelved reduction rate above 75% against K. pneumoniae and P. aeruginosa at 0.5% treatment concentration, chitosan oligomer was not effective against them. Fabrics become stiffer and less air permeable as treatment concentration increases. Liquid strike-through time of the sample treated with 0.5% chitosan oligomer solution (3.0 sec) was comparable with a hydrophilic finished sample commercially available(2.6 sec).

Melaleuca alternifolia contains terpineol-4, cineol, sesquiterpenes etc., and has a germicidal effect and skin moisturizing effect. It also has the characteristics of relieving acne inflammation, treating dandruff, relieving pain, and relieving depression. In this study, an antimicrobial substance extracted from tea tree using an organic solvent (methanol, dichloromethane, ethyl acetate) and hydrothermal extraction method. And confirmed the antimicrobial activity of each extract. In order to verify the antimicrobial activity, nine pathogenic bacteria (Bacillus cereus, Bacillus subtilis, Listeria monocytogenes, Enterococcus faecalis, Staphylococcus aureus, Klebsiella pneumoniae, Escherichia coli, Salmonella typhimurium, Pseudomonas aeruginosa and Vibrio parahaemolyticus) were used. The antimicrobial activity of each extracts were confirmed by the commonly used disc diffusion method. The results showed that the fraction extracts of ethyl acetate and methanol had antimicrobial effects against V. parahaemolyticus and S. aureus. Using these results, we confirmed the antimicrobial activity of each fraction extracts and hot water extracts against V. parahaemolyticus. After the treat of samples, we confirmed at over 99.9 % of antimicrobial activity. In case of antifungal activities, we confirmed of preservation effect during over 45 hours. Based on the results of this research, further studies will be conducted to confirm the possibility of development as a new antimicrobial agent.
