Association of vitamin D deficiency with clinical outcomes in critically ill Korean children

  • BACKGROUND/OBJECTIVES: Vitamin D is a pleiotropic hormone that affects various body organ systems. We evaluated the prevalence of a vitamin D deficiency (VDD) and its potential role in the clinical condition of critically ill Korean children. SUBJECTS/METHODS: Patients under 18 years old with a 25(OH) vitamin D measurement on the first day of PICU admission were included from among the children admitted to the pediatric intensive care unit (PICU) of our tertiary children's hospital between October 2017 and January 2019. RESULTS: A total of 172 pediatric patients were enrolled. The mean 25(OH) vitamin D level was 17.5 ± 12.8 ng/mL. There was a 65.1% prevalence of VDD (25(OH) vitamin D level < 20 ng/mL). VDD was associated with age at PICU admission, gastrointestinal/hepatobiliary disorders, International Society of Thrombosis and Hemostasis disseminated intravascular coagulation (ISTH DIC) score, pediatric multiple organ dysfunction syndrome (pMODS) score and with several laboratory test findings including hemoglobin, platelet, C-reactive protein, serum albumin, total bilirubin, prothrombin time, and anti-thrombin III levels. Most of these parameters also showed significant linear correlations with the 25(OH) vitamin D level (P < 0.05). However, no statistically meaningful association was found between VDD and other clinical conditions such as the need for a mechanical ventilator, requirement for vasoactive drugs, duration of the PICU and hospital stays, or PICU mortality. CONCLUSION: There is a high prevalence of VDD in critically ill Korean children. There were significant associations between the 25(OH) vitamin D level and gastrointestinal/hepatobiliary disorders, the pMODS score and with coagulation related factors. Further large-scale studies with more specific subgroup analyses are required to more precisely assess the clinical implications of VDD in critically ill pediatric patients.

  • The study of nitrogen dilution effect on flame stability was experimentally investigated in non-premixed turbulent lifted hydrogen jet with coaxial air. hydrogen gas was used as a fuel and coaxial air was injected to make flame liftoff. And both of the fuel jet and coaxial air velocity were fixed as $u_F$=200 m/s and $u_A$=16 m/s, while nitrogen diluents mole fraction was varied from 0 to 0.2. For the analysis of flame structure and flame stabilization mechanism, the simultaneous measurement of PIV/OH PLIF had been performed. It was found that the turbulent flame propagation velocity increased as decreasing of nitrogen mole fraction. We concluded that the turbulent flame propagation velocity was expressed as a function of turbulent intensity, even though the mole fraction of nitrogen diluents gas was changed.

Relationship between Serum 25-hydroxyvitamin D Concentration and the Risks of Metabolic Syndrome in Premenopausal and Postmenopausal Women (폐경 전, 폐경 후 여성의 혈청 25-hydroxyvitamin D 수준과대사증후군 위험도의 관계)

  • Metabolic syndrome (MetS) has become a global epidemic. In particular, it is known that there is a dramatic increase in the prevalence of MetS among women during the postmenopausal period. Recently, accumulating studies have suggested that vitamin D deficiency may be inversely associated with the risk factors regarding MetS. However, evidence from postmenopausal women is limited. In this study, we examined the association between the serum 25-hydroxyvitamin D [25(OH)D] and the MetS in Korean adult women aged 20-69 years (n = 2,618) by using the 2007-2008 Korean National Health and Nutrition Examination Survey data. The geometric mean of plasma 25(OH)D were $17.16{\pm}6.28\;ng$/mL and $20.20{\pm}7.69\;ng$/mL for premenopausal and postmenopausal women, respectively. The percentages of vitamin D deficiency [25(OH)D < 12 ng/mL] were 22.5% and 14.4%, respectively. MetS was more prevalent in postmenopausal women (43.0%) compared with premenopausal women (11.2%). When serum concentrations of 25(OH)D were categorized in quintiles, there was no relationship in the prevalence of MetS in both premonopausal and postmenopausal women. However, in premenopausal women, compared with the lowest 25(OH)D quintile, the odds ratio for hypertriglyceridemia in the highest quintile was 0.57 (95% CI 0.34-0.95, $P_{trend}$ = 0.041) and for low serum HDL cholesterol 0.60 (95% CI 0.42-0.85, $P_{trend}$ = 0.014) after adjusting for all potential confounders. On the other hand, we observed the tendency of an inverse relationship for 25(OH)D regarding low serum HDL cholesterol (OR 0.78, 95% CI 0.50-1.22, $P_{trend}$ = 0.029) and a direct relationship with abdominal obesity (OR 1.94, 95% CI 1.01, 3.74, $P_{trend}$ = 0.049) in postmenopausal women. Further studies are needed to confirm these findings in other research settings.

Activation of Intestinal Immune System by an Orally Administered Methanol Extract from Pine Needles (솔잎 메탄올추출물의 마우스 경구투여에 의한 장관면역 활성)

  • When pine (Pinus densiflora) needles were fractionated into cold water (PD-CW), hot water (PD-HW) and methanol extract (PD-M), PD-M showed potent simulating activity (1.19-fold of the saline control) for proliferation of bone marrow cells mediated by Peyer's patch cells in vitro. MeOH extracts were prepared by homogenization, stirring or reflux to identify the method of methanol extraction, and MeOH extract by reflux method showed significantly highest intestinal immune system modulating activity (1.30-fold) in vitro. The intestinal immune system modulating effect of orally administered PD-M fractionated from pine needles also were studied in mice. Analyzing intestinal immune system modulating activity mediated Peyer's patch cells from C3H/He mice which had been fed with PD-M at different doses for 7 days, 1.0 g/kg of BW/day indicated that the bone marrow cells had proliferated (3.65-fold of 3% EtOH administered group). In addition, the amounts of IL-6 in the culture supernatant of Peyer's patch cells at 1.0 g/kg of BW/day were increased (1.13-fold) whereas the production of GM-CSF was not dose dependent. These results indicate that oral administration of PD-M enhances the secretion of hematopoietic growth factors such as GM-CSF and IL-6 from Peyer's patch cells, and these cytokines also act on modulator of bone marrow cell proliferation.

Study on the Manufacturing Technology of Mural Tomb in Goa-dong of Daegaya Period (대가야 고아동 고분벽화 제작기술에 관한 연구)

  • Rigorous analysis was performed to identify the structure and materials of the murals to study techniques used on mural tombs of ancient Daegaya era(6th century). The murals were painted by applying mortar on the walls and the ceiling after building a stone chamber and creating ground layers on mortar layers. Mud was applied on most of the mortar layers on four sides of the walls except the ceiling. Sand was not used in mortar but was made of materials with pure calcium substances. In addition, shells in irregular sizes with incomplete calcination were mixed; and the mortar's white powder was inferred as lime obtained by calcination of oyster shells. Kaolinite($Al_2Si_2O_5(OH)_4$) was used in the ground layer, Cinnabar(HgS) was used for red pigment, Malachite($Cu_2CO_3(OH)_2$) for green and Lead white($PbCO_3{\cdot}Pb(OH)_2$) for white. Mud plaster was applied on the mortar and was composed thinly and densely using clayey of particle size smaller than that of medium sand. It was assumed that the finishing was for repair after long time had passed since the mortar layer came off. Using lime made with oyster shells as mortar is unprecedented in ancient Korean mural tombs and its durability was very poor, suggesting that Gaya's mortar production technique was relatively behind compared to that of Koguryo's in the same era.

Isolation and Identification of an Antibacterial Substance from Sea Mustard, Undaria pinnatifida, for Streptococcus mutans (미역 추출물로부터 충치 원인균, Streptococcus mutans에 대한 항균물질의 분리 및 동정)

  • An antibacterial substance to the Streptococcus mutans, a causative bacterium for decayed teeth, was isolated from the dried sea mustard, Undaria pinnatifida, and identified by GC and GC/MS. Acetone extract from the sea mustard (10.4 kg), was evaporated and partitioned to 4 fractions such as hexane, chloroform, butanol and water. The most active chloroform fraction were further purified through basic alumina, silicic acid and ODS column, successively, and finally, 3 antibacterial substances were isolated on the HPLC attached ODS column by using 95% MeOH and guided with UV detector (254 nm). Antibacterial substances (total 160mg, yield $1.5\times10^{-3}$%) had the same Rf value (0.42) on the TLC developed hexane diethyl ether acetic acid (80:30:1) and those methyl esters moved to 0.95. They were identified as the same unsaturated fatty acid, $C_{18:4,\;n-3}$ (3,6,9,12-octadecatetraenoic acid, stearidonic acid) compared relative retention times (15.5 min) with authentic fatty acid on the GC chromatogram. It was further confirmed unambiguously on the GC/MS giving molecular ion peak at m/z 290 which coincided with its methyl ester.

Isolation and Characterization of High Viscosity Polysaccharide Producing Endophytic Bacteria from Pueraria Root (고점도 다당류를 생산하는 갈근 내생균의 분리 및 특성)

  • Fifty endophytic bacteria, which produced slime around the colonies, were isolated from Pueraria roots. In particular, HDN-14, TDG-3, and TNB-3 strains, which appeared to be high viscosity producers, were selected. These strains produced high levels of polysaccharides in Puerara root medium extract. The purified polysaccharide was digested with 1N HCI and analyzed by HPLC, with glucose ($45.6{\sim}63.1%$), maltose ($14.6{\sim}23.7%$), and fructose ($17.4{\sim}23.7%$) detected as constitutive sugars. When determined by the homology relationship of the 16S rDNA sequence with the relative taxa, the HDN-14 and TNB-3 strains were closely ($99.06{\sim}99.32%$) related to the Pseudomonas $koreensis^T$ and Pseudomonas $jessenii^T$, while TDG-3 were closely ($99.48{\sim}99.74%$) related to Pseudomonas $plecoglossicida^T$, Pseudomonas $mosselii^T$, and Pseudomonas $monteilii^T$. The major cellular Pseudomonas acids are $3OH-C_{10:0}$, $2OH-C_{12:0}$, $3OH-C_{12:0}$, and $3OH-C_{12:1}$, with these strains being further differentiated in species belonging to the genus Pseudomonas.
