Traumatic Aneurysm of the Pericallosal artery - A Case Report - (뇌량주위동맥에 발생한 외상성 뇌동맥류 - 증례보고 -)

  • Traumatic intracranial aneurysms are rare, compromising less than 1% of intracranial aneurysms. The case of 20-year-old man suffered from delayed frontal intracerebral hematoma, subarachnoid hemorrhage and intraventricular hemorrhage from traumatic pericallosal aneurysm 12 days after head injury is presented. Traumatic pericallosal artery aneurysm is always near the falcine edge, is unrelated to arterial branching point. Sudden movement of brain and artery causes vessel wall injury against the stationary edge of the falx. Because of high mortality rate of ruptured traumatic aneurysm, clinical suspicion must be focused on the prompt diagnostic work-up and early treatment.

Bilateral Vertebral Artery Dissecting Aneurysms : A Long Term Follow-up Results of Microsurgical Trapping and Proximal Occlusion

  • Regarding the bilateral vertebral artery [VA] dissecting aneurysms, treatment strategy remains controversial because there have not been enough cases to reach a conclusion on the best treatment. We present a patient underwent staged microsurgical trapping and endovascular coiling for each dissecting aneurysm of bilateral VA presenting subarachnoid hemorrhage [SAH]. The ruptured side was managed by VA trapping procedure without any neurological deficit. Postoperative cerebral angiography revealed patent right PICA without filling of previous right dissecting aneurysm and spontaneous occlusion of the left dissecting aneurysm one month after trapping procedure. However, follow-up angiography revealed recanalization and growing of the left VA dissecting aneurysm one year after the operation. The patient underwent endovascular embolization using GDC for the proximal occlusion of the left VA and postoperative course was uneventful.

  • The advent of endovascular therapy for intracranial aneurysms and the rapid advances in that field have supplanted microsurgical treatment for many intracranial aneurysms. Applying current outcome data and other parameters, nuances of selecting the modality of treatment for intracranial aneurysms are reviewed. Patient factors, such a age, co-morbidities, vasospasm and other medical conditions, are addressed. A custom-tailored multimodality treatment paradigm for the management of ruptured and unruptured aneurysms will maximize the favorable results seen in this difficult patient population.

An Experimental Study on the Effects of Aebaegeum Applicable to the Treatment of Epistaxis (鼻出血에 應用되는 艾柏飮의 效能에 關한 實驗的 硏究)

  • Aebaegeum has been widely used in the treatment of epistaxis, as based on Oriental Medical 1iteratures. In order to investigate experimentally the clinical effects of Aebaegeum, hemostatic time, plasma recalcification time and prothrombin time of hypoprothrombinemia induced by warfarin injection, action on isolated ileum, action on blood vessel, action on blood pressure and respiration and action on heart were observed. The result of the studies were obtained as follows: 1. Hemostatic time of vein ruptured mice was significantly shortened. 2. Plasma recalification time and prothrombin time of hypoprothrombinemic rats induced by warfarin injection were significantly shortened. 3. Spontaneous mobilities in the isolated ileum of mice were significantly suppressed, and contraction by acetylcholine chloride and barium chloride were inhibited. 4. Dilatation of blood vessel and downing of blood pressure of rabbit were noted. 5. Contraction of heart in the original position and isolated heart of frog were significantly shortened. According to the above result, it is expected that Aebaegeum can be widely applicable to the treatment of epistaxis.

Traumatic Injury of Diaphragm (외상성 횡격막 손상)

  • 13 cases of traumatic diaphragmatic ruptures were treated at the department of thoracic and cardiovascular surgery in Lee-Rha general hospital, Cheong-Ju, Choong Cheong Buk Do, between Oct. 1989 and Feb. 1992. The above 13 cases were reviewed in this study. And the following results were obtained. 1. Sex ratio is 11: 2 with male dominance 2. The 9 cases were due to blunt trauma and other 4 cases were due to penetrating injury. 3. Right side injury was more common than left[7: 5] and there was 1 case of central type which ruptured through subepicardial diaphragm. 4. All of the cases had association injury. 5. Preoperative diagnosis was possible in the 9 cases and others were diagnosed during operation under other indication. 6. Finger exploration was one of effective diagnostic procedure. 7. All of diaphragmatic ruptures was corrected through thoracotomy and exploratory laparotomy was done in 6 cases. 8. A patient died after operation due to associated injuries.

  • The Cox-Maze procedure was developed as a cure for atrial fibrillation. The recovery rate of both atrial contractility is reported low in the atrial fibrillation associated with mitral valvular heart disease than that of loan atrial fibrillation. We performed the Cox-Maze procedure (Maze III) in three cases who suffered from non-mitral heart diseases associated with atrial fibrillation: A ruptured sinus of Valsalva aneurysm, a ventricular septal defect, and an aortic stenoinsufficiency. The Cox-Maze procedure was performed concomitantly with correction of the underlying heart disease. Conversion to sinus rhythm was achieved in all three patients, and both right and left atrial mechanical activities could be identified echocardiographically after three postoperative months.

  • Mediastinal tumor are frequently encountered in clinical practice. Hanten, in 1955, reported-2 adult patients with spontaneous rupture of mediastinal dermoid cysts into the pleural cavity and also, Thompson, in 1963, reported 2 child patients with spontaneous rupture of mediastinal teratoma into the pleural cavity. Mediastinal teratomas have also been reported rupture into other contiguous structures, such as the bronchus, aorta, pericardium, SVC and esophagus. This report presents an instance of spontaneous rupture of an anterior mediastinal teratoma into the right pleural cavity of a 43 year old female. Despite variable diagnostic procedures, the true nature of the lesion was not determined until a thoracotomy and window formation was performed for adequate drainage of empyema thoracis. Removal of the teratoma and mediastinal window formation resulted in complete cure.

Left Ventricular Rupture after Mitral Valve Replacement - 3 cases report - (승모판막 치환술후 합병한 좌심실 파열의 외과적 고찰 - 3례 보고 -)

  • An unusual but often lethal complication of mitral valve replacement is rupture of the left ventricle. From March 1977 through June 1990, 424 mitral valve replacements were performed as isolated or combined procedures. Rupture of the posterior wall of the left ventricle was observed in 3 patients. Their was one type I and two type II rupture. Once the diagnosis was made, all of the patient were connected to the heart-lung machine again and total cardiopulmonary bypass is re-established. Repair was attempted in all of them from the outside of the heart. One of them was successively repaired but two were failed due to myocardial ischemia by circumflex coronary artery injury and failure of adequate closure of the ruptured site. From this results, we concluded that prevention is the best solution. But if we encountered this condition, early diagnosis and rapid treatment may improve the patient`s chances for survival.

Bronchial Rupture following Blunt Chest Trauma - 1 case report - (외상성 기관지 파열)

  • Rupture of a bronchus is an unusual result of blunt trauma to the chest. We experienced a case of bronchial rupture caused by blunt chest trauma without external wound or rib fracture. This case was a 13 years old male who was elementary school student. He was compressed on anterior chest by basket ball goal post being failed down on the afternoon of admission day. After this accident, he was suffered from progressive dyspnea and chest pain. The bronchial rupture was confirmed by bronchogram, The operation was performed through standard posterolateral thoracotomy incision along the 5th rib course. The right upper lobe bronchus and intermediate bronchus was completely transected. The right upper lobectomy was done. The ruptured intermediate bronchus was connected direct simply. Post-operative course was uneventful. Thus we report this case of traumatic bronchial rupture with review of literature.

Infected Abdominal Aortic Aneurysm (감염성 복부 대동맥류 -수술치험 1례 보고-)

  • Mycotic anuerysms are uncommon but it is a fulminant infectious process frequently resulting in rupture and death if not properly treated. Commonly known it as infected aneurysm caused by noncardiogenic bacteremia. We experienced a case of infected aneurysm of the abdominal aorta that ruptured into the retroperitoneum. A 57 year old man was admitted with lower back pain, fever and palpable mass. It was identified as an inf cted abdominal aneurysm with staphylococcal septicemia. He underwent resection of aneurysm and replacement with a prosthetic graft and prolonged postoperative organism-specific antibiotics therapy. He recovered well and discharged without complications postoperatively.
