기관지흉막루의 임상적 연구 (A clinical study of bronchopleural fistula)

  • Bronchopleural fistula is the communication between a bronchus or bronchiole and the pleural space, clinically bronchopleural fistula is usually combined with infection. But pneumothorax is the communication between a peripheral air space such as a ruptured bleb, and the pleural space. The author studies on the 62 bronchopleural fistula patients who were admitted to P.N.U.H. from 1974 to October, 1984 according to causative mechanism which was postresectional origin, non resectional surgical origin and spontaneous origin, and its underlying diseases. The following results were obtained. 1.The causes of BPF: 5.16% was spontaneous origin, 30.6% was postresectional origin, 17.7% was other surgical origin. 2.The causes of BPF after resectional operation: 42.1% was tuberculosis, 21.1% was tumor resectional case. 3.The proper operational method for BPF was thoracoplasty with myoplasty. 4.The average operation for BPF was 2.7 times.

외상성 기관지 단절의 수술 치험 -1례- (Bronchial Rupture by Blunt Chest Trauma -a case-)

  • Rupture of the main bronchus followed by blunt chest trauma is comparatively very rare. Early recognition of bronchial rupture and emergency thoracostomy and management is essential for reducing of morbidity and mortality and late complications. This case was 11 years old female who was a primary school student. The patient was sustained a crushing injury to her right hemithorax by traffic accident and had been taken emergency closed thoracostomy at her second intercostal space, midclavicular line at emergency room. In the course of the next 2 hours, the girl`s condition remained critical with tension pneumothorax and abnormal arterial blood gas analysis. Induction of anesthesia started 3 hours after the accident. During the general anesthesia, cardiac arrest was occurred and cardiac resuscitation was performed. Right upper lobectomy and end-to-end anastomosis of ruptured right main bronchus was performed. Postoperative course was satisfactory.

비관통성 흉부 손상에 의한 기도 파열 -1례 보고- (Traumatic Tracheal Rupture by Blunt Chest Injury -Report of a Case-)

  • Tracheal rupture by a blunt trauma is an uncommon injury, and its clinical presentations are variable. It is a kind of the modern hazard. Herewith, we report a successful management of the tracheal rupture. A 22 year-old female was transferred from other hospital 4 hours after a car crash. Physical examination, simple chest X-ray, Chest CT and fiberoptic bronchoscopy revealed rupture of the membranous portion of the trachea about 5cm in length extending to the right main bronchus. Ruptured membraous portion of the trachea was sutured directly with absorbable suture. Her postoperative course was uneventful, and follow-up fiberoptic bronchoscopy revealed intact membranous portion of the trachea.

The Sundt Encircling Clip as a Vascular Rescue : A Case Report and a Review of Repair Methods for Arterial Tearing

  • The Sundt encircling clip was developed to repair defects of the vessel wall. With the advent of microvascular techniques, most parts of the damaged vessel wall during aneurysm surgery can be repaired by primary closure or by the bypass technique. However, these methods are not always successful. Here, we illustrate two cases of surgical clipping with the Sundt encircling clip in the ruptured internal carotid artery trunk aneurysm. The Sundt clip provides prompt control of unexpected tearing of the vessel wall or aneurysm and plays an important role in vascular rescue during aneurysm surgery.

후방 십자 인대 재건술에서 동종 이식건의 역할 (The Role of Allograft for Posterior Cruciate Ligament Reconstruction)

  • The use of autogenous tissues is preferred for knee ligament reconstruction. However allografts play a role in major ligament reconstructive procedures in which multiple substitutions or revisions are required. In the dislocated knee, allografts may offer an advantage in reconstructing the PCL. But allografts in knee ligament surgery must be considered in terms of biomechanical and regenerative properties, disease transmission and immunogenecity, and methods of preservation and sterilization. Also only a few authors have described the use of allograft for reconstruction of a ruptured PCL, either a single procedure, or in combination with ACL repair following knee dislocation. Furthermore, the problems that the clinician faces with use of allografts is the necessity for supervision to ensure that the grafts are correctly processed, secondarily sterilized, and free of transmissible diseases. For these reasons, the routine use of allograft materials in the treatment of ligament deficiencies should be avoid and provide with meaningful outcome studies, including longterm follow-up.

  • Facet joint synovial cysts are usually associated with osteoarthritis of the adjacent facet joint and/or spondylolisthesis. In between the conservative and operative ends of the treatment spectrum lie minimally invasive techniques such as cyst rupture using epiduroscopy. In this report, we describe an 82-year-old male patient presenting with low back pain radiating to his lower left extremity and associated paresthesia. Magnetic resonance imaging of the lumbar spine revealed a synovial cyst at left L4/5 facet joint. Using epiduroscopy, the cyst was mechanically ruptured by popping it with the tip of the scope. The patient remained symptom-free at his successive visits until 12 months after the procedure, and was opened for desired follow up.

  • The purpose of this study was to investigate the social support, understanding of the children's problem in the female-headed single parents, and their child rearing methods. To obtain this purpose, in-depth interview was conducted to 8 female-headed single parents from March 3 to April 5, 2003. The contents of interviews were recorded and analyzed by qualitative method. The results were as follows: First, female-headed single parents were stuck for the social support and the relationships with parents-in-law were ruptured, especially. Secondly, female-headed single parents had well known about their children's daily life. Third, in the early stage of female-headed single parents, they had expressed the stresses in their hardship of life to their children, but it decreased whenever they were accustomed to their life.

Contralateral Intraparenchymal Hemorrhage Following Aneurysmal Clipping

  • Post-clipping intraparenchymal hemorrhage of the contralateral hemisphere is a very unusual phenomenon in a patient with aneurysmal subarachnoid hemorrhage, unless there is an underlying condition. We report a complicated case of 47-year-old man, who underwent uneventful clipping of ruptured aneurysm and experienced vasospasm two weeks later. Vasospasm was treated by intra-arterial nimodipine and systemic hyperdynamic therapy. One week thereafter, he became unconscious due to intraparenchymal hemorrhage on the anterior border-zone of contalateral hemisphere, but intraoperative and pathologic findings failed to disclose any vascular anomaly. We suggest that the anti-spastic regimens cause local hemodynamic redistribution through the vasodilatory effect and in turn, resulted in such an unexpected bleeding.

Early Rupture after Coiling of Unruptured Aneurysm

  • We describe a case of an unruptured basilar top aneurysm that was associated with early rupture after incomplete coiling. A 62-year-old woman with a history of several small infarctions has undergone coiling of unruptured basilar top aneurysm. Two weeks after initial coiling the patient presented with Hunt and Hess grade IV subarachnoid hemorrhage consistent with a ruptured basilar top aneurysm. Repeat angiography revealed a rupture of recanalized basilar top aneurysm. Second embolization with additional coils resulted in complete occlusion. However, her neurological status was not improved afterward and she was transferred to department of rehabilitation one month after hemorrhage with comatous state. To our knowledge, this is the first case of fatal early rupture after coiling of unruptured aneurysm. It has been speculated that coiling could cause injury to aneurysmal wall and facilitate rupture.

Concurrent Ruptured Pseudoaneurysm of the Internal Carotid Artery and Cerebral Infarction as an Initial Manifestation of Polycythemia Vera

  • The most common neurologic manifestations of polycythemia vera (PV) are cerebral infarction and transient ischemic attacks, while cerebral hemorrhage or intracranial dissection has been rarely associated with PV. Here we report the first case of a 59-year-old patient with intracranial supraclinoid internal carotid artery (ICA) dissection causing cerebral infarction and concomitant subarachnoid hemorrhage due to pseudoaneurysm rupture as clinical onset of PV. This case report discusses the possible mechanism and treatment of this extremely rare condition.
