  • High temperature materials in service are subjected to mechanical damage due to operating load and metallurgical damage due to operating temperature. Therefore, when designing or assessing life of high temperature components, both factors must be considered. In this paper, the effect of tensile hold time on high temperature fatigue crack growth and long term prior thermal aging heat treatment on creep rupture behavior were investigated using STS 316L and STS 316 austenitic stainless steels, which are widely used for high temperature components like in automotive exhaust and piping systems. In high temperature fatigue crack growth tests using STS 316L, as tensile hold time increased, crack growth rate decreased in relatively short tensile hold time region. In long term aged specimens, cavity type microcracks have been observed at the interface of grain boundary and coarsened carbide.

  • Intraperitoneal metastatic hepatocellular carcinoma (HCC) is uncommon. Although rare, it can spontaneously rupture and cause hemoperitoneum similar to primary HCC in the liver. We present a case of intraperitoneal metastatic HCC that had spontaneously ruptured and appeared as an irregularly margined hemorrhagic mass with T1 high and T2 dark signal intensities on magnetic resonance imaging. Ruptured HCC is a life-threatening emergency with high mortality rate. Spontaneously ruptured intraperitoneal metastatic HCC should be considered if a patient with a history of HCC presents with acute abdomen, although rare.

  • Cardiac tamponade is an acute life-threatening condition that predominantly involves the intra-pericardial space; however, an expanding mediastinal hematoma can also sometimes cause cardiac tamponade. Here we describe the case of a 45-year-old male driver in whom a traffic accident resulted in rupture of the left internal thoracic artery (ITA), extra-pericardial hematoma, and sternal fracture. After resuscitation, he was scheduled to undergo angio-embolization to repair the ruptured left ITA, but he suddenly developed cardiac tamponade that required a decompressive sternotomy. Nevertheless, the patient had an uncomplicated recovery, and this case suggests that extra-pericardial cardiac tamponade should be considered as a possible consequence of retro-sternal hematoma due to traumatic ITA rupture.

  • Traumatic pulmonary artery rupture is a rare, life-threatening injury. Currently, no strict guidelines for its management exist. Herein, we report a successful surgical repair of a right pulmonary artery rupture caused by being stepped on.

  • Background: Traumatic rupture of the aorta is a life-threatening injury that must be diagnosed as rapidly as possible and treated immediately. The chest X-ray is a valuable tool for screening traumatic rupture of the aorta in blunt chest trauma. And various chest radiologic parameters are being used as diagnostic tools for aortic injury. The purpose of this study is to identify chest radiographic parameters that may assist in the detection of traumatic rupture of the aorta and to compare these findings with those of other reports. Methods: This study involved 30 adult patients with traumatic rupture of the aorta seen at the emergency department of the Asan Medical Center from 1997 to 2004. The control subjects were 30 healthy patients with neither lung nor cardiovascular disease. We retrospectively assessed over 14 parameters on chest X-rays. Results: In 11 of the 14 parameters, there were significant differences between the study group and the control group. There was no significant difference in the M/C ratio (mediastinumto-chest width ratio) between the two groups, and neither the left nor the right paraspinal interface was statistically significant (p value>0.05). Our study indicates that new criteria for the MC ratio and for the paraspinal interfaces are needed for screening traumatic aorta injury. The other radiographic parameters for traumatic rupture of the aorta need to be further assessed through a prospective study.

급성 아킬레스 건 파열에서의 초음파 이용 (Ultrasonographic Guidance in Acute Achilles Tendon Rupture)

  • 정형외과적 영역에서 족부 족관절 부의의 손상은 흔한 문제이다. 비록 손상들이 생명을 위협하지는 않지만 정확한 진단이나 치료가 지연됐을 시에는 하지의 기능에 장애를 유발 할 수 있으므로 정확한 진단이 장기간의 합병증을 예방 할 수 있다. 초음파는 비용-효과면에서 우수하고 방사선 피폭의 염려가 없으며 연부 조직의 검사에 유용하고 역동적 검사를 시행할 수 있어서 아킬레스건 파열에서 그 유용성이 증가하고 있다. 초음파 영상은 아킬레스건 파열의 진단적 도구이다. 영상의학적 검사 없이 아킬레스건 파열을 진단하기 위해서는 신체 검사와 환자의 병력에 필요하지만 개인 의원에서는 약 20%에서 파열을 놓지는 것으로 알려져 있다. 초음파 영상 하에서 정상적인 섬유 구조의 단절이 아킬레스건 파열의 진단이 되며 실시간으로 족배 굴곡 및 족저 굴곡을 함으로써 단절이 더 확대되는 것을 관찰 할 수 있다. 그리고 수술 후나 보존적인 치료를 시행하는 경우에도 추시 관찰 기간 동안 지속적으로 초음파를 통해 건의 상태를 관찰할 수 있는 유용한 도구이다.

과실의 충격특성에 관한 연구 (Mechanical Behavior of Fruits under Impact Loading)

  • Impact is one of the major cause of damage to fruits druing varios processes from the production on the farm to the consumer. The tissue of fruits are ruptured in a very short period time less than 10ms by impact loading. Mechanical behavior of fruits under impact loading can be analyzed better with high speed sampling data acquisition system and one of them is a digital storage oscilloscope. A impact test system was developed to test the physical properties of fruits including apple, pear, and peach which may lead to a better understanding of the physical laws. The test system consisted of a digital storage oscilloscope and simple mechanism which can apply impact force to fresh produce. Rupture force, energy, and deffrmation were measured at the five levels of drop heights from 4 to 24cm fur each internal and external tissues. Rupture forces for apple and pear were in the range of 72.9 to 87.7 N and 70.8 to 84.1 N for external and internal tissues, respectively. Rupture forces far peach external tissues were in the range of 43.4 to 65.0 N.

다축응력상태에서의 304 스테인리스강의 고온 파괴수명에 관한 연구 (High temperature rupture lifetime of 304 stainless steel under multiaxial stress states)

  • Specimens of 304 stainless steel were tested to failure at elevated temperatures under multiaxial stress states, uniaxial tension using smooth bar specimens, biaxial shearing using double shear bar specimens, and triaxial tension using notched bar specimens. Rupture times are compared for uniaxial, biaxial, and triaxial stress states with respect to the maximum principal stress, the von Mises effective stress, and the principal facet stress. The results indicate that the principal facet stress gives the best correlation for the material investigated, and this parameter can predict creep life data under multiaxial stress states with rupture data obtained with specimens under uniaxial stresses. The results also suggest that grain boundary cavitation, coupled with localized deformation processes such as grain boudary sliding, controls the lifetimes of the specimens.

Staged Management of a Ruptured Internal Mammary Artery Aneurysm

  • The rupture of an internal mammary artery (IMA) aneurysm in a patient with type 1 neurofibromatosis (NF-1) is a rare but life-threatening complication requiring emergency management. A 50-year-old man with NF-1 was transferred to the emergency department of Kyungpook National University Hospital, where an IMA aneurysmal rupture and hemothorax were diagnosed and drained. The IMA aneurysmal rupture and hemothorax were successfully repaired by staged management combining endovascular treatment and subsequent video-assisted thoracoscopic surgery (VATS). The patient required cardiopulmonary cerebral resuscitation, the staged management of coil embolization, and a subsequent VATS procedure. This staged approach may be an effective therapeutic strategy in cases of IMA aneurysmal rupture.

Ruptured uterus in a 36-week pregnant patient with hemorrhagic shock after blunt trauma in Korea: a case report

  • Traumatic uterine rupture is uncommon but can be fatal and life-threatening for both the mother and infant. In addition to complications caused by trauma itself, such as pelvic fracture, gestational complications such as placental abruption, abortion, premature labor, rupture of membranes, maternal death, and stillbirth can occur. In particular, fetuses have been reported to have a high mortality rate in cases of traumatic uterine rupture. A 35-year-old pregnant female patient fell from the fourth floor and was admitted to our trauma center. We observed large hemoperitoneum, pelvic fractures, and spleen laceration, and the fetus was presumed to be located outside the uterus. The pregnant woman was hemodynamically unstable. Although the fetus was stillborn, angioembolization and surgical treatment were properly performed through collaboration with an interventional radiologist, obstetrician, and trauma surgeons. After two orthopedic operations, the patient was discharged after 34 days. This case report suggests the importance of a multidisciplinary approach in the treatment of pregnant trauma patients.
