A Case of Rotor's Syndrome (로터 증후군(Rotor's syndrome) 1례)

  • Rotor's syndrome is a hereditary disorder characterized by predominantly conjugated hyperbilirubinemia with normal hepatic histology. It resembles Dubin-Johnson syndrome but the main differences are no dark brown pigmentation in the hepatic cells and visualization of the gallbladder in oral cholangiography. We experienced a 14 year-old male patient who had icteric sclerae and predominantly conjugated hyperbilirubinemia when he was hospitalized for varicocelectomy. His liver biopsy specimen showed no dark brown pigmentation and any other pathologic abnormalities in the hepatic cells. Hepatobiliary scan shows no evidence of obstructive lesions. His urinary excretion of total coproporphyrin was markedly increased.

  • Rotor syndrome is a rare disease of hereditary hyperbilirubinemia transmitted with autosomal recessive trait. In general, Rotor syndrome shows direct hyperbilirubinemia and there has been several reports since Sons's report in 1966, in Korea. A 34-year-old female with admitted with the chief complaint of intermittent icteric sclera for 24 years. There was no family history of jaundice. Rotor syndrome was diagnosed by oral cholecystogram, BSP retention test, $^{99m}Tc$-DISIDA scan, liver biopsy and electromiuoscopy study of liver biopsy specimen. We report this case with brief review of the literature.

  • We experienced two cases of Rotor syndrome in brothers who were a 13 year-old boy and an 11 year-old boy, respectively. They presented with icteric scleras for a few months. Their common laboratory characteristics were as follows: Direct bilirubin was more increased than indirect bilirubin, but aminotransferases were normal. Plasma indocyanine green (ICG) test revealed hepatic excretory defect: plasma ICG concentrations 15 minutes after intravenous injection were 80.45% and 78.28%, respectively. 99mTc-DISIDA Hepatobiliary scan showed that severely decreased hepatic extraction with mild cardiac blood pool, markedly delayed biliary excretion in both intra- & extra- hepatic bile ducts, delayed visualization of gall bladder, and markedly delayed intestinal biliary passage. Needle liver biopsy showed normal hepatic histology without pigmentation.

  • This study was conducted about the effect of Shihogesikungang-tang on the central nervous system and cardiovascular system for the investigation of its clinical effect based on the Oriental medicinal references. The results of this study were summerized as follows; Analgesic activity as evaluated by the writhing syndrome in mice was significantly noted. A decrease effect of the spontaneous movement as estimated by wheel cage method, muscle relaxant effect as evaluated by the rotor rod method and the prolonged effect of sleeping time induced by thiopental-Na were significantly shown in mice. A antipyretic activity in febrile rats induced by the endotoxin was recognized. Anti-inflammatory effect in carrageen-induced paw edema in rats was significantly noted. Negative inotropic action on the isolated heart of frogs was noted. A vasodilative action in rabbits peripheral blood vessels and hypotension in anesthetized rabbits were remarkably recognized.

  • Intrathecal drug administration system (ITDAS) can reduce the side effects while increasing the effectiveness of opioids compared to systemic opioid administration. Therefore, the use of ITDAS has increased in the management of cancer pain and chronic intractable pain. Catheter obstruction is a serious complication of ITDAS. Here, we present a case of catheter obstruction by a mass formed at the side hole and in the lumen. A 37-year-old man suffering from failed back surgery syndrome received an ITDAS implantation, and the ITDAS was refilled with morphine every 3 months. When the patient visited the hospital 18 months after ITDAS implantation for a refill, the amount of delivered morphine sulfate was much less than expected. Movement of the pump rotor was examined with fluoroscopy; however, it was normal. CSF aspiration through the catheter access port was impossible. When the intrathecal catheter was removed, we observed that the side hole and lumen of the catheter was plugged.

An Experimental Studies on the Efficacy of Taeumin Handayŏlsot'ang and Gamihandayŏlsot'ang (태음인(太陰人) 한다열소탕(寒多熱少湯)과 가미한다열소탕(加味寒多熱少湯)의 효능(效能)에 대(對)한 실험적(實驗的) 연구(硏究))

  • In order to investigate experimentally the effects of Taeumin Handayolsot'ang and Gamihandayolsot'ang, experimental studies were conducted about antipyretic effect, analgesic effect, antiinflammatory effect of edema, sadative effect, anticonvulsive effect experimental animals (mice and rats). Sample A group was a solid extract of Handayolsot'ang treated group. Sample B group was a solid extract of Gamihandayolsot'ang treated group. The results were summarized as fallows. 1. In antipyretic action by yeast method, sample A group was decreased significantly and sample B group showed decreasing tendency, but showed no significance. 2. In the effect of control for writhing syndrome by the acetic acid stimulating method, sample B group was repressed significantly and sample A group showed repressing tendency, but showed no significance. 3. Antiinflammatory action by carrageenine edema method was showed significant effect at all sample groups. 4. Sadative effect by rotor rod method in rats was not noted at all sample groups. 5. In anticonvulsive action by E C T unit method, time to death in mice was significantly prolonged at all sample groups. According to the above results, it is considered that Taeumin Handayolsot'ang and Gamihandayolsot'ang will be effective on pyretic diseases.
