Pulmonary embolism (PE) is a serious complication that can occur after orthopedic surgery. Most instances of PE in the orthopedic field have occurred after hip or knee arthroplasties or after fracture surgeries. The occurrence of PE related to arthroscopic shoulder surgery is very rare. We report a case of PE that developed after arthroscopic rotator cuff repair, in which the patient did not show preoperatively any remarkable risk factors for PE. We also review the current literature related to this topic.
Traumatic anterior shoulder dislocation combined with scapular fracture in elderly patients is relatively rare. In this case, a patient visited Emergency Room of Dong-A University Hospital for shoulder pain after falling off a ladder. Radiographs demonstrated anterior shoulder dislocation with displaced Ideberg type IIb scapula (glenoid fossa) fracture combined with a large rotator cuff tear on magnetic resonance imaging. We performed arthroscopic rotator cuff repair, but a large fragment in the inferior glenoid was left untreated. At the 1 year follow-up visit, the pain visual analogue scale of the patient was 2, the American Shoulder and Elbow Society score was 88 and the patient had gained nearly full range of motion without any apprehension.
Purpose: To evaluate the usefulness of arthroscopic decompression and miniopen repair that was related with large and massive sized full thickness rotator cuff tear and assess clinical result. Materials and Methods: Twenthy-nine cases of miniopen repaired full thickness tear of rotator cuffs that arthroscopically decompressed were studied. From October 1998 to December 2004 we have analysed 29 repairs of large and massive sized FTRCT, the average age 44 ($32{\sim}71$) years old, mean follow-up was 34 ($12{\sim}84$) months. We analyzed the results statistically by paired t-test. Results: Postoperative VAS of pain improved average 7.0 to 1.7, UCLA score improved 13.7 to 31.9, ADL improved 11.3 to 25.3 respectively (all, P=0.000). Twenty five cases(82.8%) of the patients showed excellent & good results at the final follow-up. The satisfied rate was 26 cases(89.7%). Conclusions: Arthroscopic decompression and miniopen repair in large and massive sized full thickness rotator cuff tears are effective surgical methods.
최근 회전근개 질환을 수술적으로 치료하는 경우가 점점 많아지고 있다. 회전근개 파열의 술후 재활 치료는 진단, 수술만큼이나 중요하며 성공적인 치료 결과에 결정적 영향을 미친다. 또한 적절한 재활 치료를 위해서는 견관절의 해부학적, 생역학적 이해를 필요로 한다. 술후 재활은 추가 손상을 방지하고, 염증을 줄이며, 교원질의 회복을 도모하고 약해진 근육을 강화시키며 유착, 구축 등에 의해 감소된 관절 운동을 회복하고, 쉽게 피로한 근육의 지구력을 기르면서 봉합 부위와 삼각근을 보호해야 한다. 이러한 치료 지침은 환자의 조건에 따라서 개별화 되어야만 할 것이다.
관절경적 회전근 개 복원술 시에 시행하게 되는 골 처치에는 견봉 성형술, 원위 쇄골 절제술, 건 부착부(대결절 부위) 처치 및 오구 성형술 등이 있다. 각각의 골 처치는 여러 이론적 근거에 의해 적응증을 가지며, 수술방법 또한 다양하게 알려져 있다. 이에 관절경적 회전근 개 복원술 시에 사용되는 여러 골 처치의 적응증 및 방법에 대해 기술하고자 하였다.
회전근 개 파열의 병리 생태와 증상이 없는 파열이 증상이 있는 파열로의 진행 과정은 아직 불분명하여 많은 논란이 있다. 이는 유전적 소인, 회전근 개를 싸고 있는 조직의 외인성 충돌, 건 자체 내의 내인성 퇴행성 변성과 같은 여러 요소들이 관여한다. 연령의 증가에 따른 회전근 개의 퇴행성 변화는 회전근 개의 해부학 및 역학적 환경과 관련이 깊다. 또한 파열된 회전근 개 표본의 조직 병리 소견으로 골에 부착되는 건의 기시부에서는 퇴행성 변성이 나타나며 이러한 현상은 건의 신장력을 약화시키게 된다. 건의 퇴행성 변성의 진행이 내인성이냐 외인성이냐의 의문에 대해 저자는 내인성에 의해 퇴행성 변성이 진행된다고 생각하며 비록 회전근 개 파열이 여러 요소에 의해 이차적으로 발생되기는 하나, 회전근 개 파열의 일차적 원인은 존재하는 퇴행성 변화일 것으로 사료된다.
목적: 본 연구의 목적은 광범위 회전근 개 파열에서 방사선학적 소견과 임상 소견 간의 관계를 알아보고자 하였다. 대상 및 방법: 광범위 회전근 개 파열로 진단된 45예 (35명)를 대상으로 하였다. Hamada의 분류법을 사용하여 광범위 회전근 개 파열 환자의 방사선학적 소견을 분석하고, 임상적 평가는 UCLA 점수를 이용하였으며, 서로 간의 결과를 비교하였다. 결과: 광범위 회전근 개 파열에서 관절염 등급과 임상 소견 사이에 유의한 연관성은 확인되지 않았다($r_s$=0.220, p=0.151). 우세수 측 침범이 더 높은 수술적 치료의 비율과 더 낮은 UCLA 점수와 관련이 있었다. 결론: 광범위 회전근 개 파열 환자에서 관절염의 방사선학적 소견이 일상 생활에 영향을 미치는 임상적 소견과 항상 관련이 있는 것은 아닌 것으로 사료된다. 따라서, 광범위 회전근 개 파열 환자의 치료 방법의 결정에 있어서는 신중한 고려가 필요할 것으로 사료된다.
Background: Many hyaluronic acid (HA)-based anti-adhesive agents have been commercialized for clinical use in the pharmaceutical market. But their efficacy in arthroscopic rotator cuff repairs remains elusive. To determine their efficacy, we performed a comparative analysis of the effects of two hyaluronate/carboxymethylcellulose (CMC)-based anti-adhesive agents, Protescal and Guardix. Methods: We recruited a total of 256 patients who had received an arthroscopic rotator cuff repair at our hospital between January 2014 and March 2015. Among them, 96 patients fulfilled the study's selection criteria and were enrolled as the final population sample. Thirty patients who had received a postoperative injection of Protescal were allocated into Group A. Another 30 patients who had received a postoperative injection of Guardix were allocated into Group B. As controls, 36 patients who did not receive any injection were allocated into Group C. The patients included in this study were aged between 19 and 75 years. For the clinical assessment, we measured the following clinical parameters-the visual analogue scale for pain (PVAS), the American Shoulder and Elbow Surgeons (ASES) score, and the constant score, as well as passive range of motions (ROMs)-at three time-points (preoperatively, 2-month postoperatively, and 6-month postoperatively). Results: We found that Group A compared to Group B tended to show a swifter recovery in passive anterior elevation and in internal rotation by the 2-month postoperative follow-up, but the differences were not statistically significant. Conclusions: We found that the effects of HA/CMC-based injections were minimal after arthroscopic rotator cuff repairs.
Background: Assessment of the clinical outcomes after rotator cuff repair is essential for their effectiveness on treatment. The Korean Shoulder and Elbow Society devised the Korean Shoulder Scoring System (KSS) for patients with rotator cuff disorder. The purpose of this study was to evaluate the availability of the KSS for assessment of clinical outcomes in patients after arthroscopic rotator cuff repair, and for comparison with other appraisal scoring systems. Methods: A total of 130 patients with partial-thickness or full-thickness rotator cuff tear who underwent arthroscopic repair using a single row or double row suture bridge technique were enrolled. The average follow-up period was 25.9 months. All patients were classified according to various factors. Comparison within corresponding categories was performed, and the correlation between the KSS and other shoulder assessment methods including University of California Los Angeles (UCLA), Constant and American Shoulder and Elbow Surgeons (ASES) score was analyzed. Results: Total score of the KSS response had increased from 59.6 preoperatively to 88.96 at last follow-up. All KSS domains, including function, pain, satisfaction, range of motion, and muscle power had improved up to 24 months postoperatively. Statistical significance was observed mainly in preoperative measurements with number and size of torn tendons, and greater than or equal to grade 3 of fatty infiltration. The KSS was best correlated with the UCLA scoring system in both preoperative (r=0.785) and postoperative (r=0.951) measurements. Conclusions: The KSS was highly reliable and valid as a discriminative instrument, and it showed strong correlation with ASES and UCLA scoring systems.
목적, 대상 및 방법: 봉합이 불가능한 회전근 개 광범위 파열에 대한 치료로는 보존적 치료, 변연절제술, 부분 봉합술, 건 이전술, 인공 건 이식술 및 역행성 견관절 전치환술 등이 있으나 비교적 젊은 연령이거나 활동성이 높은 환자에서는 견관절의 생역학적 재건으로 force couple을 향상시켜서 견관절의 기능을 호전시키는 건 이전술이 보다 효과적인 치료로 사료된다. 결과 및 결론: 따라서 저자들은 봉합이 불가능한 회전근 개 광범위 파열에서 후상방형 결손에 대해서는 광배근 이전술을 그리고 견갑하건 결손에 대해서는 대흉근 이전술을 소개하고자 한다.
