  • Background: To evaluate the efficacy of the subclavian portal approach for the arthroscopic repair of isolated subscapularis tendon tear. Methods: We used the subclavian portal to carry out arthroscopic repair of the isolated subscapularis tendon tear. The surgery was carried out in 18 cases (average age of 53) from May 2006 to December 2009 with a mean follow-up period of 35 months. Of these cases, 13 patients had the tear in their dominant arms, 16 were male, and 12 were from traumatic ruptures with on average 7.6 months of symptom period to operation. Additional surgery, acromioplasty and subacromial debridement, were carried out on 4 cases each during the subscapularis repair. The integrity of cuff status was assessed by ultrasonographic examination at 6 months and at 1 year after operation. Results: The initial average range of motion in forward flexion, external rotation, and internal rotation were $160^{\circ}$, $50^{\circ}$ and L4, respectively. At the 1 year follow-up period, these improved to $160^{\circ}$, $52^{\circ}$ and T12, respectively. The initial average functional scores were assessed by KSS, ASES, UCLA, and Constant scoring systems, which were 67, 60, 26, and 65, respectively. These scores improved progressively with time. At 3 months after operation, the scores were 74, 67, 27, and 74; at 6 months, 83, 77, 31, and 75; at 1 year, 88, 86, 32, and 79; and at the final follow-up of 35 months, 84, 92, 34, and 84. Conclusions: In the repair of isolated subscapularis tendon tear, the subclavian portal approach provided a good angle for anchor insertion and sufficient space to manage the upper portion of the tendon tear. In turn, these provisions resulted in satisfactory clinical results.

  • Purpose: This study aimed to investigate the effect of angular joint mobilization (AJM) on the shoulder pain, range of motion, and functional improvement in a patient with shoulder adhesive capsulitis. Methods: The patient diagnosed with right shoulder adhesive capsulitis by an orthopedic surgeon was a 60-year-old male, right hand/arm dominant, with a height of 175 cm and weight of 75 kg. The patient received 12 sessions of AJM once or twice per week for eight weeks. AJM was applied for 5 min each of flexion, abduction, external rotation, internal rotation, for a total of 20 min per session. The visual analog scale, the goniometer, and the Oxford shoulder score were used to measure pain, range of motion, and shoulder pain & disability index, respectively. Results: After all the treatments, the pain decreased from 6 to 2 points. The range of motion increased in flexion by $54.3^{\circ}$ from $125^{\circ}$ to $179.3^{\circ}$, abduction by $38^{\circ}$ from $140^{\circ}$ to $178^{\circ}$, external rotation by $54.4^{\circ}$ from $30.3^{\circ}$ to $84.7^{\circ}$, and internal rotation by $25^{\circ}$ from $45^{\circ}$ to $70^{\circ}$. The shoulder disability index decreased from 33 points to 17 points. Conclusion: This study found that AJM has a positive effect on the improvement of shoulder pain, range of motion, and function in a patient with shoulder adhesive capsulitis. Further studies on AJM are needed in the future.

  • This study was carried out to compare the amount of the maxillary bone remodeling and tooth displacement in each three maxillary superimposition methods, Ricketts, Best-fit, Structural method. Forty cases of the lateral cephalometric radiographs from 27 boys and 13 girls who had been treated to correct anterior cross-bite were selected for the study. The initial radiographs were taken at about 8-year-old and the second radiographs were taken in about 3.3 years later. Followings were the results: 1. With the Structural method, backward movement was shown in PNS, while forward movement was observed in ANS and point A. With the Ricketts method, however, all structures were shown significant backward movement comparing with Structural method(P<0.05). With the Best-fit method, the amount of horizontal movement was similar to that of the Structural method(P>0.05). 2. The palate seemed to be moved downward with Structural method, but there was no measured downward remodeling on nasal floor with Ricketts and Best-fit method(P<0.05). 3. Comparing with Structural method, Ricketts and Best-fit method significantly underestimated the eruption of the teeth by 20% to 30% (P<0.05). 4. The Structural method showed the anteroinferior rotation (43%) and posteroinferior rotation(57%) of the palatal plane, while the Best-fit method showed mostly anterosuperior rotation(87%), but no change was found in the Ricketts method. 5. With the Structural method, there was a statistically significant correlation between the amount of the rotation of the palatal plane and that of N-S line(r=0.86). 6. The measured angles of the long axis of the incisors and molars showed no significant difference in each 3 methods(P>0.05).

  • Since the mid of the 20th century in the world, it has been observed that the number of minor or moderate earthquake motions tend to be increased year by year. Owing to the topographical condition, moreover, large numbers of skew bridges have been constructed for the requirements of more than DB18 ton bridge in Korea. It has been also observed from foreign countries that lots of superstructures collapse in bridge were occurred in previous earthquakes, inclusive of 1995 Kobe earthquake. This is caused by a relative displacement between the upper and lower structure of bridge by the earthquake and the rotation with respect to the vertical axis of skew bridges, which were subjected to and earthquake motion. In this study, the probabilistic analysis of unseating failure of skew bridges under scenario earthquake has been carried out by evaluating the longitudinal displacement of skew bridges.

  • The Early Life Problems Scale consisting of 5-point rating for each item was prepared from the results of a questionnaire of 163 items with two sub scales constructed on the basis of Diagnostic Classification: 0-3. Respondents were 361 parents with newborn to 3-year-old children (197 boys and 164 girls). Hypothetical constructs of mental and developmental problems were identified by exploratory factor analysis. The resulting 22 factors explained 58.5% and 72.9% of the common variance for scales I and II, respectively. The rotation procedure yielded 134 items of which 118 had factor loadings of more than .30. Face validity was obtained on the basis of 18 items. The coefficients of correlation between and within factors suggest that there are interrelationships. The reliability of total scales by Cronbach's Alpha was relatively high.

  • This paper aims to point out the problems and solutions of soil fertility enhancement in Korean organic farming which does not harmonize with internationally recognized standard of organic agriculture such as Basic Standard of IFOAM, EU regulations, and guidelines of FAO/WHO codex alimentarius. The necessity of rotation, legume, green manure to enhance the soil fertility or organic farming system was discussed, and the reasons why salt accumulation including nitrate and phosphate in the rooted soil profiles was frequently occurred in Korean organic farmer's field also discussed. Soil testing for correct handling of organic fertilization was introduced to void the salt accumulation just applying the huge amount of organic fertilizer year to year to reach a maximum crop yield. In the conclusion it was suggested for Korean organic agriculture to make an effort to coincide it's regulation with internationally recognized standards for organic farming.

  • Background: The research examined the effect of a scapular stabilizing exercise on shoulder pain, active range of motion (AROM) and changes in the position of the shoulder blade of patients with frozen shoulder. Methods: General physical therapy and a scapular stabilizing exercise were applied to a 53-year-old female patient with frozen shoulder three times a week for four weeks. Before and after the therapy, measurements were made on the visual analog scale (VAS) and of the AROM, and a lateral scapular slide test was conducted. Results: Following the therapy, VAS fell from 7 to 3. Before therapy, flexion, abduction, internal rotation, and external rotation recorded $133^{\circ}$, $102^{\circ}$, $45^{\circ}$, and $53^{\circ}$ respectively; after the therapy they increased to $150^{\circ}$, $123^{\circ}$, $55^{\circ}$, and $65^{\circ}$ respectively. The lateral scapular slide test showed sliding distances before therapy of 8.5cm at $0^{\circ}$, 9.2cm at $45^{\circ}$, and 11.3cm at $90^{\circ}$; after therapy, the distances decreased to 6.8 cm at $0^{\circ}$, 8.2 cm at $45^{\circ}$, and 9cm at $90^{\circ}$. Conclusion: The scapular stabilizing exercise was effective in alleviating shoulder pain, increasing AROM, and reducing changes in the position of the shoulder blade of frozen shoulder patients.
