This study aimed to report the effect of Korean medical treatment including miniscalpel acupuncture on 2 patients who underwen tsurgery for rotator cuff tear. They were treated for almost 4 weeks at the Department of Acupuncture and Moxibustion, Dong-Eui University Korean Medicine Hospital. Visual analog scale (VAS), and range of movement (ROM) were used to evaluate treatment effects. In both patients, shoulder pain and restriction of shoulder joint movement improved after miniscalpel acupuncture treatment. In Case 1, shoulder pain decreased from a VAS score 8 to a VAS score 3, and ROM of the shoulder improved from flexion $100^{\circ}$ to $160^{\circ}$, extension $10^{\circ}$ to $30^{\circ}$, abduction $90^{\circ}$ to 1$30^{\circ}$, adduction $10^{\circ}$ to $40^{\circ}$, internal rotation $10^{\circ}$ to $50^{\circ}$, and external rotation $10^{\circ}$ to $50^{\circ}$. In Case 2, shoulder pain, which was rated a VAS score 8 at first-visit, disappeared, and ROM of the shoulder recovered to normal range. These results suggest miniscalpel acupuncture may contribute to the recovery process after rotator cuff tear surgery.
Purpose : Lymphedema is a common complication in mastectomy patients and is usually characterized by pain, swelling, and limited range of motion (ROM) in the arm. Electromyostimulation (EMS) is widely used for the rehabilitation and recovery of subjects with various neuromusculoskeletal disorders after breast cancer. However, EMS has not yet been used in many Pilates exercises. This study was aimed at comparing the effects of instrument Pilates integrated with EMS on pain, lymphedema, and ROM of the upper extremity (UE) in breast cancer subjects after mastectomy. Methods : Nine female breast cancer subjects who had undergone mastectomy participated in the study. The subjects underwent instrument Pilates with EMS (experimental group) or instrument Pilates only (control group). Pain, lymphedema, and ROM of the UE were measured using the visual analog scale (VAS), the circumference length of the UE, and the ROM of the UE. The Wilcoxon signed-rank test was used to compare the pain, lymphedema, and ROM of the arm before and after the intervention, and the Mann-Whitney U test was used to compare the two groups. The statistical significance level was set to p < .05. Results : In the experimental group, there were significant differences in pain (p<.05) and UE circumference (p<.05) before and after intervention. However, there was no significant difference between the two groups in VAS (p>.05) or circumference length of the UE (p>.05). There was one significant difference between the groups in terms of internal rotation of the ROM of the UE (p<.05). Conclusion : These results show that instrument Pilates exercises combined with EMS may positively affect the internal rotation of the ROM of the UE in breast cancer patients after mastectomy, thus contributing to existing knowledge about instrument Pilates using EMS for the effective management of in breast cancer subjects after mastectomy.

Background: Physical therapy (PT) plays an important role in the recovery of function following anatomic total shoulder arthroplasty (aTSA). While several PT protocols have been published for these patients, there is no standardized protocol for aTSA rehabilitation. This lack of standardization may lead to confusion between patients and physicians, possibly resulting in suboptimal outcomes. This study examines how PT protocols provided by academic orthopedic surgery programs vary regarding therapeutic goals and activities following aTSA. Methods: PT protocols for aTSA available online from the Accreditation Council for Graduate Medical Education accredited orthopedic surgery programs were included for review. Each protocol was analyzed to evaluate it for differences in recommendation of length of immobilization, range of motion (ROM) goals, start time for and progression of therapeutic exercises, and timing for return to functional activity. Results: Of 175 accredited programs, 25 (14.2%) had protocols publicly available, programs (92%) recommended sling immobilization outside of therapy for an average of 4.4±2.0 weeks. Most protocols gave recommendations on starting active forward flexion (24 protocols, range 1-7 weeks), external rotation (22 protocols, range 1-7 weeks), and internal rotation (18 protocols, range 4-7 weeks). Full passive ROM was recommended at 10.8±5.7 weeks, and active ROM was 13.3±3.9 weeks, on average. ROM goals were inconsistent among protocols, with significant variations in recommended ROM and resistance exercise start times. Only 13 protocols (52%) gave recommendations on resuming recreational activities (mean, 17.4±4.4 weeks). Conclusions: Publicly available PT protocols for aTSA rehabilitation are highly variable. Background: Scapular dyskinesis (SD) is closely related to imbalance of the rotator cuff (RC) muscles. However, previous studies have only focused on isometric strength. To our knowledge, there has been no study examining potential differences in concentric and eccentric strength and functional strength ratio (FSR) of the RC muscles based on functional parameters related to throwing in with or without SD. Objects: This study aimed to determine whether there was a difference in the RC muscle strength and FSR between the dominant shoulder with SD and the non-dominant shoulder without SD in adolescent baseball players. Methods: Forty adolescent baseball players participated and classified types of SD based on movement patterns using the SD test by two examiners. The isokinetic concentric and eccentric peak torque of the internal rotation (IR) and external rotation (ER) were measured and quantified as peak torque to body weight (PT/BW). Also, the FSR was obtained by calculating the strength ratios of eccentric IR to concentric ER (IRecc/ERcon) and eccentric ER to concentric IR (ERecc/IRcon), respectively. Results: There was a significant decrease in the IR and ER PT/BW in the dominant shoulder with SD compared to the non-dominant shoulder without SD (p < 0.05), regardless of contraction types. However, no significant difference was observed in the FSR in both IRecc/ERcon and ERecc/IRcon ratios. Conclusion: The findings indicate that the isokinetic concentric and eccentric PT/BW of the IR and ER muscles were significantly lower in the dominant shoulder with SD than in the nonthrowing shoulder without SD. Therefore, when establishing a strategy for addressing RC muscle weakness in adolescent baseball players with SD, it is essential to consider an approach that accounts for scapular kinematic recovery.
본 연구에서는 MTMD의 합성을 통해 빠른 자성회수 및 재사용이 용이한 크롬 흡착제를 개발하였다. 합성을 위해 공침법 및 교대성장을 통한 발산법을 사용하였으며, methyl propionate 및 glutaric acid를 통해 덴드리머를 멀티터미널화 하였다. 합성된 시료의 특성분석은 XRD, FT-IR, TEM, EDS, TGA, Zeta potential analyzer를 통해 이루어졌다. MTMD의 자성코어는 마그네타이트 결정구조를 가졌으며, 4세대 덴드리머의 크기는 15 nm에서 20 nm로 확인되었다. TGA 분석결과 1세대 덴드리머의 유기전도성 가지의 비율은 약 7%였으며 1세대 증가시 3%의 중량 감소가 나타났고 멀티터미널화시 덴드리머의 포텐셜은 -25.26 mV에서 -6.53 mV까지 변화하였다. MTMD 흡착실험 결과 5분 이내에서 80% 이상의 크롬을 흡착하였으며 6.308 mg/g의 흡착능을 나타냈다. 자성 회수시 COOH D와 비교하여 회수시간이 2배 이상 단축되었으며 30분 이내에 100% 회수가 가능하였다. 크롬을 이용한 재생실험의 경우 4회 흡착에도 동일한 흡착능을 유지하였다.

Background: Repair of superior labral anterior to posterior (SLAP) lesion in patients older than 40 years is controversial. The purpose of this study was to evaluate clinical outcomes of arthroscopic repair of SLAP lesions between younger and older patient groups. Methods: We reviewed 50 patients with isolated type II SLAP lesions who underwent arthroscopic repair. Patients were divided into 2 groups: group 1 included 20 patients aged <40 years, and group 2 included 30 patients aged ${\geq}40years$. Functional outcome at the final follow-up was assessed using a visual analog scale for pain and satisfaction, American Shoulder and Elbow Surgeons form, Constant score, University of California at Los Angeles score, and periodic change in range of motion (ROM). Anatomical outcome was evaluated using computed tomography (CT) arthrography at least 1 year after surgery. Results: No significant differences in functional scores or postoperative ROM were observed between the 2 groups. In group 2, later recovery of ROM (forward flexion, p=0.025; internal rotation, p=0.034) and lower satisfaction score (p=0.06) were observed for atraumatic patients (n=16) compared to patients with traumatic injury (n=14). Fifteen patients in group 1 (15/17, 88%) and 21 patients in group 2 (21/26, 81%) demonstrated a healed labrum on postoperative CT arthrography, and this difference was not significant. Conclusions: The results of this study suggest that arthroscopic repair of type II SLAP lesions can yield good functional and anatomical outcomes regardless of age, if patient selection is adequate. However, the delay in ROM recovery and lower satisfaction, particularly in older patients without traumatic injury, should be considered.
Purpose : To evaluate effects of McKenzie exercise on the functional recovery and forward head posture of chronic neck pain patients. Methods : The subjects were consisted of fifteen patients who had abnormal neck posture, mild neck pain (28 males, 17 females; mean aged 21.9) from 19 to 33 years of age(mean age 21.9). All subjects were received McKenzie exercise for 35 minutes with clinical massage per day three times a week during 4 weeks period. Neck disability index was used to measure functional disability level. Visual analogue scale(VAS) was used to measure subjective pain level. craniovetebral angle(CVA), cranial rotation angle(CRA) was used to measure forward head posture with digital camera. All measurements of each patients were measured at pre-treatment and after 2 week, after 4 week, post-treatment on 2 week. Result : The VAS of McKenzie exercise was significantly reduced between pre-treatment and post-treatment (p<.05) and effects of pain reduce was continued follow 2 weeks. The Neck Disability Index of McKenzie exercise was significantly reduced between pre-treatment and post-treatment (p<.05) and effects of pain reduce was continued follow 2 weeks. The CVA of McKenzie exercise was significantly reduced between pre-treatment and post-treatment (p<.05) and effects of pain reduce was continued follow 2 weeks. The CRA of McKenzie exercise was not significantly reduced between pre-treatment and post-treatment (p<.05). Conclusion : McKenzie exercise improved pain and function of Chronic neck pain patients.
Objectives Through this study, we investigated the effects of Korean medicine treatments on range of motion recovery and pain reduction in patients with Frozen shoulder. Methods In this study, the medical records of 26 patients diagnosed with frozen shoulder were retrospectively observed and analyzed through the IBM SPSS Statistics 25 program. We used range of motion to observe the patient's motion recovery and numeric rating scale to evaluate pain reduction. Results The shoulder range of motion increased statistically significantly from 160.0 (123.5~170.0) to 170.0 (160.0~180.0) (flexion), 115.0 (90.0~135.0) to 167.5 (130.0~178.8) (abduction), 40.8±22.1 to 58.3±16.0 (external rotation) (p<0.001). The average of numeric rating scale reduced statistically significantly from 6.7±2.0 to 3.2±1.4 (p<0.001). Depending on the period, the range of motion increased statistically significantly when visited within 3 months, also the pain score decreased 7.0 (5.4~8.0) to 3.0 (1.8~3.6) statistically significantly when visited within 3 months. In the treatment intervention, the increase in the range of motion and the decrease in pain were statistically significant, especially when the bee venom acupuncture and chuna manaul therapy were used together. Conclusions Korean medicine treatments was effective in improving the range of motion and reducing pain in frozen shoulders. In particular, when treated within 3 months of onset and when bee venom acupuncture and chuna manual therapy were used together, it was effective in recovering the range of motion and reducing pain.
목적: 강직이 동반된 충돌 증후군으로 진단된 환자에서 수술적 가료를 시행하고 강직이 없는 충돌 증후군 환자의 치료 결과와의 차이를 알아보고자 한다. 대상 및 방법: 강직을 동반하지 않은 충돌 증후군 76 례와 강직을 동반한 충돌 증후군 환자군 24례를 대상으로 하였으며 수술은 강직이 없는 충돌 증후군은 견봉하 감압술 및 점액낭 부분 절제술을 시행하였으며, 강직이 있는 경우는 수동적 조작술을 추가하였다. 추시기간은 평균 32개월 이었다. 결과: 강직군이 비강직군에 비하여 술 전의 통증이 심하였으며, American Shoulder and Elbow Society의 기능 점수(ASES 점수)도 나빴다(p<0.05). 술 후 통증 및 ASES 기능 평가는 양군에서 모두 향상되었다(p<0.05). 양군의 술 후 결과를 비교할 때 통증과 ASES 점수는 양군간의 차이가 없었으나(p>0.05), 양호이상의 결과는 강직군에서 67%였으며, 비강직군에서80%이었다. 만족도는 강직군은 83%, 비강직군은 93%이었다. 당뇨병이 있었던 환자의 만족군는 강직군이 47%, 비강직군이 81%이었다. 강직군의 환자 7명중 4명(57%)이, 비강직군의 환자 11명중 9명(81%)이 수술에 만족하였다. 술 후 전방거상 및 90도 외전에서의 외회전, 내회전은 강직군과 비강직군에 차이가 없었다. 외회전은 강직군에서 적은 운동 범위를 보였다(p<0.05).결론: 강직을 동반한 충돌 증후군 환자군에서 견봉하 감압술과 수동적 조작술을 이용한 수술을 시행할 때 통증 감소 및 환자의 만족도, 외회전를 제외한 관절 운동 범위 증가는 강직이 없는 충돌 증후군 환자와 유사하게 기대할 수있다. 하지만 당뇨와 강직이 동반된 충돌 증후군은 술후 치료 결과가 기대에 못 미치므로 수술여부에 신중을 기하여야 한다.
The results of the operative treatment of the Grade III acromioclavicular joint injury is defined by the durability of the reduced joint and free of exertional pain. Several surgical techniques have been applied to reduce and stabilize the joints effectively. Resection of clavicular lateral end and subacromial decompression also could be applied to prevent post-operative arthritic change. Biomechanical studies reveals the role of clavicular elevation and rotation to achieve more than 90 degrees of elevation. It also serves as a attachment site of deltoid and trapezius muscle. The stability and mobility of the both acromioclavicular and coracoclavicular joint are important to get full functional recovery. We modified the methods of coracoacromial ligament transfer described by Weaver-Dunn and by Shoji et a!. to prevent pullout of the transferred ligament and to get more improved functional results. Main technical point was harvesting full thickness bone block and fix it through the key-hole to reduce pull out angle.
